디지털 유방 토모신테시스에 대한 국내 사용 현황과 인식에 관한 설문조사 연구 (Patterns in the Use and Perception of Digital Breast Tomosynthesis: A Survey of Korean Breast Radiologists)

  • 목적 국내 디지털 유방 토모신테시스(digital breast tomosynthesis; 이하 DBT) 사용 현황 및 영상의학과 의사들의 DBT에 대한 인식 및 사용자 요구조건에 관해 알아보고자 하였다. 대상과 방법 2021년 3월 대한 유방영상의학회 회원들을 대상으로 DBT와 관련한 26개의 객관식 문항과 1개의 주관식 문항이 포함된 온라인 설문조사를 시행하였다. 설문 문항은 기본 정보, 유방촬영술 및 DBT에 대한 진료 현황, DBT 인식, 관련 연구 경험 및 관심도를 포함하였다. 결과는 로지스틱 회귀분석을 이용하여 분석하였다. 결과 총 257명의 회원 중 120명이 응답하여, 응답률은 46.7%였다. 응답자 중 67명(55.8%)이 현재 DBT를 사용하고 있는 것으로 나타났다. DBT의 전반적인 만족도는 5점 척도를 사용하여 평균 3.31점이었다. DBT 검사의 장점으로 가장 많이 응답한 것은 소환율 감소(55.8%), 병변의 명확도 향상(48.3%), 유방암 발견율 향상(45.8%)이었고, 단점으로 많이 응답한 것은 환자의 추가비용 부담46.7%), 불충분한 미세석회화 검출(43.3%), 불충분한 진단능 향상 효과(39.2%), 방사선량 증가(35.8%)였다. DBT가 더 널리 사용되지 못하는 원인으로는 영상의 저장 용량, 판독 시간 증가를 주요 요인으로 응답하였다. 결론 DBT 사용이 보다 보편화되기 위해서는 사용자 불편사항과 피드백을 반영한 향후 기술개발이 필요하겠다.

자동노출제어를 적용한 흉부 방사선 검사 시 인체 이식형 의료기기가 선량과 화질에 미치는 영향 (Effect of Human Implantable Medical Devices on Dose and Image Quality during Chest Radiography using Automatic Exposure Control)

  • 본 연구는 자동노출제어 흉부 방사선 검사 시 신체 기능 일부를 대체 보조하거나 의약품 등을 주입하는 인체 이식형 의료기기가 흉부 영상의 선량과 화질에 영향을 미치는지 알아보고자 하였다. 제조회사와 모델이 다른 3대의 디지털 X선 발생장치와 인체모형 팬텀을 사용하여 흉부 검사와 동일한 방법으로 위치잡이를 선정 후 선행 연구에서 선량의 변화가 관찰된 인공심장박동기(Pacemaker), 심장 재동기화 치료기(CRT), 케모포트(Chemoport) 3개의 HIMD(Human Implantable Medical Device)를 상단 이온전리조센서에 부착한 후 Monte Carlo 방법론 기반의 프로그램 PCXMC 2.0을 사용하여 실험에서 도출된 DAP(Dose Area Product) 값을 입력하여 유효선량을 측정하였다. 또한 흉부 영상의 화질 평가를 하기 위해 가슴 부위에 관심 영역 3곳과 잡음영역 1곳을 설정하고 신호대잡음비(SNR; Signal to Noise Ratio), 대조도대잡음비(CNR; Contrast to Noise Ratio)를 측정하였다. 연구 결과는 유효선량의 유의미한 차이를 보여주었으며 AEC 적용과 미적용 그룹을 비교하였을 때 Pacemaker와 CRT는 유의한 차이가 있었다. (p<0.05) AEC 적용 시 Pacemaker에서 37%, CRT에서 52% 유효선량이 증가하였다. Chemoport는 유효선량의 10% 차이는 있었지만, 유의미한 차이를 보이지 않았다. (p>0.05) 영상 품질 평가에서는 모든 HIMD 삽입과 AEC 적용 유무에 따른 SNR, CNR의 유의미한 차이를 보이지 않았다. (p>0.05) 최종 결론은 AEC 적용 후 HIMD가 삽입된 환자의 흉부 X-ray 검사 시 유효선량이 증가하였으며 AEC 적용 유무에 따른 흉부 영상의 품질 차이는 없음을 알 수 있었다. 이는 HIMD가 삽입된 환자의 흉부 검사 시 AEC 미사용이 환자의 선량을 낮추는 타당성을 확인한 것이며 과피폭을 주의하고 피폭 저감화를 위한 다양한 방법을 모색하여야 한다.

Tongue Cancer 환자에서 PET/CT 검사 시 Open Mouth 촬영법의 유용성 평가 (Usefulness Evaluation of Open Mouth View when PET/CT scan In Tongue Cancer Patients)

  • Tongue cancer는 혀에 생기는 악성종양으로 전체 암의 1.8%, oral cancer의 75%를 차지할 정도로 발생 빈도가 높은 것으로 알려져 있으며 초기 진단, 전이유무, 병기설정 등을 위해 PET/CT 검사를 실시하고 있다. Tongue cancer 환자의 PET/CT 검사 시 close mouth 상태로 Torso를 촬영하고 있으며 그에 따른 겹침이나 인공물의 영향으로 병소부위가 명확히 관찰되지 않는 경우가 발생되었다. 이에 open mouth로 PET/CT 검사 시 발생되는 변화와 그에 따른 유용성을 알아 보고자 한다. 20명의 환자(남 15명, 여 5명)를 대상으로 시행하였으며 먼저 close mouth 상태로 Torso 촬영 후 open mouth 상태로 1bed 추가 촬영하였다. Tumor(T), Normal Tongue(NT), Lymph Node(LN)에 관심영역을 설정하여 표준화섭취계수(standardized uptake value, SUV) $SUV_{mean}$, $SUV_{max}$를 측정하여 Background(Carotid artery) SUV를 빼주었다. Tumor size 변화를 측정하였으며 육안적 영상 해상도 평가는 blind test를 진행하였다. 비모수검정(wilcoxon signed rank test)을 통해 통계 분석하였다. Tumor에서 SUV는 open mouth 검사 시 증가하는 변화를 보였으며, normal tongue과 lymph node에서는 소폭의 변화가 있었으나 유의한 차이는 없었다. tumor size 변화와 blind test를 통해 전체적으로 비슷하거나 높은 영상 해상도를 보여주는 것을 확인할 수 있었다. Open mouth PET/CT 검사가 close mouth시 발생하는 문제점에 대한 완벽한 대안법이 될 수는 없지만 open mouth를 통해 구강 내 다른 구조물과의 겹침이나 tumor의 경계면을 조금 더 명확하게 관찰할 수 있을 것으로 사료된다.

자기공명 견관절조영검사 시 고함유량 가돌리늄 조영제의 유용성 (The usefulness of the contrast agent high in gadolinium for the magnetic resonance shoulder arthrography)

  • 본 연구는 자기공명 견관절조영검사 시 단위 부피당 가돌리늄 함유량이 2배 많은 1mmol/mL 조영제를 이용하여 T1 단축효과를 높임으로서 진단적 가치가 높은 영상을 얻을 수 있는 방법을 증명하고자 하였다. 2012년 1월부터 2013년 8월까지 0.5mmol/mL 함유량을 가진 기존의 조영제를 사용한 20명과 1mmol/mL 함유량을 가진 새로운 조영제를 사용한 21명 등 총 41명의 환자를 연구대상으로 하였다. 연구방법은 가돌리늄 함유량에 따른 대조도 차이를 알아보기 위하여, 견관절의 관절공간과 인접한 근육조직인 극상근, 그리고 상완골두의 신호강도를 각각 측정한 후 신호대잡음비와 대조도대잡음비를 비교하였다. 연구 결과, 가돌리늄 함유량이 높은 1mmol/mL 조영제가 0.5mmol/mL 조영제에 비해 SNR이 모두 높았으며(관절공간 38.01%, 극상근 8.40%, 상완골두12.78%), CNR도 가돌리늄 함유량이 높은 1mmol/mL 조영제가 높았다(관절공간과 극상근 48.96%, 관절공간과 상완골두 42.00%). 결론적으로 T1 단축효과를 높이는 다양한 방법 중 현실적으로 구현하기 용이한 가돌리늄 함유량이 높은 1mmol/mL 조영제를 이용하면 효율적으로 T1 단축효과를 높여 진단적 가치가 높은 영상을 얻을 수 있을 것으로 판단된다.

입체조준장치를 이용한 중추신경계의 방사선 입체조형치료 계획 (3-D Conformal Radiotherapy for CNS Using CT Simulation)

  • 목적 : 척수에 전이가 가능한 뇌종양 치료를 위한 두개척수 방사선치료를 전산화단층촬영장치(volumetric spiral CT)와 입체조준장치(CT simulator) 및 3차원 조형치료계획장치(3D conformal planning system)를 이용한 두개척수 방사선치료계획 방법을 개발하고 기하학적 검증을 통하여 유용성과 정확성을 평가한다. 방법 : 연세암센터 방사선종양학과에서 두개척수 방사선치료를 받은 환자 11명을 대상으로 전산화 단층촬영을 이용한 입체 모의치료계획과 3차원 방사선조형치료를 시행하였다. 중증의 뇌종양 환자의 두경부는 두부고정틀(thermoplastic mask, $Aquaplast^{R}$)로 고정시키고, 전신은 $Vac-Loc$ $Aquaplast^{R}$ (전성물산, 한국)으로 고정한 후 전산화단층촬영장치(Spiral CT)를 이용하여 전신체적영상(volumetric image)을 얻었다. 환자자세의 재현성 확인 및 검증을 위해 두부에 세 개의 점과 전신에 기준선 및 기준점 등을 표시하였다. 이후 입체조준장치(CT-simulator)의 가상현실영상(virtual fluoroscopy)에서 인체의 크기와 방향에 제약이 없고 치료 침대와 고정기구에 대한 시각장애를 제거함으로써 자유롭게 모의치료를 할 수 있었으며, 조사면과 선속을 결정하고 디지털화재구성사진(digitally reconstructed radiography, DRR)과 디지털화합성사진(digitally composited radiography, DCR)을 통하여 분해능이 좋은 화질의 투시 및 모의치료영상을 획득하였다. 기하학적 검증은 치료중심점 이동시 얻은 모의치료영상과 첫 치료 시에 얻은 조사면 검증 사진(port verification film) 등을 전산화단층촬영영상으로부터 재구성한 DRR 영상과 시각적, 정량적으로 비교, 분석 였다. 결론 : 입체조준장치와 3차원 방사선치료계획 장치 등을 이용하여 두개척수 방사선 치료계획을 신속하고 정확하게 원활히 수행할 수 있었다. 가상현실영상에서 대부분의 설계작업이 이루어지므로 환자의 자세고정을 요하는 시간은 전신체적 영상을 얻는 10분 이내이므로 환자의 불편을 줄일 수 있을 뿐 아니라 모의치료과정 중의 체위 변동 변수를 제거할 수 있었다. 또한 전산화단층촬영영상을 얻음으로써 중요정상조직인 안구, 척수 등을 정확하게 설정할 수 있었고, 조사면 결정과 차폐의 정확성을 증진시킬 수 있었다. 환자자세오차는 디지털화재구성사진과 치료 시마다 얻은 포트필름에서 치료중심점과 척수 사이의 거리를 측정하여 3 mm 이내의 정확성을 얻을 수 있었다. CT조준장치를 이용한 중추신경계의 방사선 입체조형치료는 가상현실모의치료계획으로 두개척수의 방사선치료를 정확하고 용이하게 실현할 수 있었다.

Dose calibrator 측정 깊이와 용량의 변화에 따른 선량 값의 성향에 대한 고찰 (A Study on the Tendency of Dose value According to Dose calibrator Measurement Depth and Volume)

  • 핵의학이란 방사성동위원소 추적자를 인체에 투여하여 관심장기에 대한 형태학적인 정보와 생물학적이고 기능적인 정보를 얻고 평가한다. Dose calibrator는 의료기관에서 단일 핵종의 방사능을 측정하기 위해 사용하는 장비이며, 방사성동위원소의 정확한 용량의 투여는 진단과 치료에 중요한 요인이다. 최근 정량분석을 위한 여러 시스템이 개발되고 있고 ERPF (Effective Renal Plasma Flow), GFR (Glomerular Filtration Rate) 등 정량분석이 필요한 검사에 있어서 정확한 투여 용량 및 재현성 있는 측정이 중요해지고 있다. 따라서 본 논문을 통해 Dose calibrator의 측정 깊이와 용량에 따른 선량 값의 성향을 알아보고 적절한 측정 깊이 및 용량에 대해 알아 보고자 한다. 실험에 사용한 Dose calibrator는 CRC-15R과 CRC-15βeta (Capintec, New Jersey, USA)를 사용하였다. 선원으로는 표준선원(Standard source) 137Cs, 133Ba, 57Co를 사용하였고, 방사성 의약품은 99mTc-pertechnetate를 사용하였다. 표준선원은 측정 깊이만 변화하여 0 cm부터 15 cm까지 1 cm 깊이마다 15회씩 측정했고, 99mTc-pertechnetate의 경우에는 1 mL 주사기로 표준선원과 동일한 깊이로 실험을 진행했고, 용량의 변화를 고려하여 0.1 mL, 0.3 mL, 0.5 mL, 0.7 mL, 0.9 mL로 각 깊이마다 15회씩 측정했다. 표준선원인 137Cs, 133Ba, 57Co의 깊이의 변화에서는 모두 깊이에 따른 선량 값의 변화가 통계적으로 유의한 차이가 있었다(p<0.05). 99mTc-pertechnetate도 깊이에 따른 용량의 변화에서 선량 값이 모두 통계적으로 유의한 차이가 있었다(p<0.05). CRC-15R의 표준선원 비교 그래프에서 137Cs, 133Ba, 57Co는 Plateau를 그리기 시작한 2 cm에서 9 cm까지는 각 선원마다 비교시 통계적으로 유의한 차이가 없었다(p>0.05). CRC-15βeta의 표준선원 비교 그래프에서는 Plateau를 그리기 시작한 영역은 3 cm에서 9 cm로 통계적으로 유의한 차이가 없었다(p>0.05). 99mTc-pertechnetate의 그래프는 0.1 mL에서 1 cm에서 7 cm, 0.3 mL와 0.5 mL에서는 0 cm에서 7 cm, 0.7 mL에서는 0 cm에서 6 cm, 0.9 mL에서는 0 cm에서 5 cm에서 통계적으로 유의한 차이가 나타나지 않았다(p>0.05). 본 연구를 통해 방사성동위원소를 환자에게 주입하거나 정도관리를 진행 시 편차가 적고 재현성을 위해 신뢰할 수 있는 구간을 확인 할 수 있었다. 높이에 따른 변화가 가장 적은 Plateau의 구간이 0.3 mL 혹은 0.5 mL의 용량으로 0 cm 에서 7 cm의 깊이에서 측정한다면, 편차를 최소한으로 줄일 수 있을 것으로 사료된다. 표준선원의 경우에는 본 연구의 결과에 따라 장비마다 Plateau의 변화가 가장 적은 구간의 깊이에서 재현성 있게 측정하는 것이 중요하다고 생각된다. 적절한 구간을 찾아서 재현성 있는 검사를 시행하면 검사의 품질향상 및 피폭선량저감 그리고 진단능을 높이는데 이바지 할 것이라고 생각된다.

의-한의 동시 진료 및 통합의료서비스에 대한 인식조사 - 3차 상급종합병원 이용자를 대상으로 - (A Study on the Perception of Concurrent Use of Western and Korean Medicine Care and Integrated Medical Service in Korea - Targeting tertiary hospital users -)

  • Objectives: Currently, Korea's medical services are divided into Western medicine and Korean medicine, and people who are not satisfied with the existing treatments are looking for complementary and alternative medicine(CAM). Therefore, this study attempted to confirm patients' perception of the ongoing collaborative treatment and integrated medical service that added CAM to collaborative treatment based on tertiary hospital users. So that we can confirm the expected advantage and disadvantage of integrated medical service system and the necessity of supporting medical expenses for it. Methods: The survey was conducted on 100 people who experienced tertiary hospital treatment and other 100 people who experienced both tertiary hospital treatment and Korean medicine treatment at the same period. The survey was conducted until the number of respondents in both group reached 100. The survey was conducted through e-mail and was conducted from September 27, 2021 to October 8, 2021. Results: For the advantages of collaborative treatment 'increased in psychological stability,' and for disadvantages 'longer time spent for treatment' were the most common. If integrated medical services are implemented in the future, expected advantages include 'consideration of various treatments.' and expected disadvantages include 'increased medical cost.' The needs to expand support for health insurance for integrated medical services were 75.5% among responders. Conclusions: We were able to find out the (expected) advantages and disadvantages of the collaborative medical care and the integrated medical system that medical users experienced or expected, also confirmed positive answers to the expansion of health insurance support for the integrated medical system.

High Resolution HC$_3$N Observations toward the Central Region of Sagittarius B2

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500,550,570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500,550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100,300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. On this base, we composed of the radial pulse diagnosis system in korean traditional medicine. The system composed of a radial pulse wave detection system and a radial pulse diagnosis system. With a detection system, we detected Inyoung and Cheongu radial pulse wave and processed it. Then, we have got the characteristic parameters of radial pulse wave and also quantified that according to the method of Inyoung-Cheongu Comparison Radial Pulse Diagnosis. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
