A Study on the Skin Diseases of the Kings during the First Period of Joseon Dynasty (조선전기(朝鮮前期) 임금들의 피부병(皮膚病)에 관한 고찰 - 『조선왕조실록(朝鮮王朝實錄)』을 중심으로 -)

  • The "Annals of the Joseon Dynasty (朝鮮王朝實錄)" is the precious historical material which contains royal culture of Joseon dynasty as an official document. It kept a record of the diseases and treatments relating to the 27 Kings for a period of 518 years, who had a variety of different symptoms. Among them the most frequent disease was a skin disorder such as a boil of a painful infected swelling. Dermatosis became the direct cause of death of several Kings. In this article we tried to conduct research using the "Annals of the Joseon Dynasty" into the skin diseases of the Kings during the first period of Joseon dynasty from the first King Taejo (太祖) to the 12th King Injong (仁宗). Among the 12 Kings, the 5th King Munjong (文宗), the 7th King Sejo (世祖), the 9th King Seongjong(成宗), the 10th King Yeonsangun (燕山君), and the 11th King Jungjong suffered from dermatosis. The King Munjong died at the age of 38 and suffered from severe boils before his death. The cause of death is thought to be septicaemia. The King Sejo does not have any specific record of skin disease, however, the recently discovered relics showed the indications of serious skin trouble of boils. The King Seongjong suffered from skin diseases at the age of 20, 27, 28 and 38. Nevertheless, the direct cause of death was not dermatosis. The King Yeonsangun had skin trouble of boils on his face when he was 20. He lost the throne and died of an infectious disease at 31. The King Jungjong had a record of suffering from dermatosis at the beginning of twenties, at the middle of forties, and at the age of 57 when he died. The skin trouble affected the whole of the body. He was treated with acupuncture therapy and medication for both internal and external uses among which folk remedies were included.

A Study on the Intake Patterns of Health Food of the Elderly Aged over 60 Years in the Chuneheon Area (춘천지역 60세 이상 노년층의 건강식품 섭취실태 조사)

  • This study was carried out to investigate the intake patterns of health food according to the kind of disease of the elderly aged over 60 years living in the Chuncheon area. This study was conducted by using a questionnaire from December 1999 to the March 2000. The data was analyzed using a SPSS program. The results were summarized as follows : The poorer the self- perceived health condition, the lower the level of activity of daily living(ADL) and instrumental activities of daily living(IADL) were the higher. the frequency score of health food intake. The incidence of disease, such as arthritis(44.7%), hypertension(18.8%), digestive diseases(17.1%), diabetes(10.5%) were found to be higher. than other chronic diseases among the elderly. The frequency score of health food intake was not significantly different according to self-perceived health status, disease status, or the level of ADL and IADl.. The must important source of information for health food intake was the recommendation of friends and relatives(68.7%). Twenty eight point two percent of the elderly considered health foods to be effective, however 8.3% of the elderly considered them to be ineffective. The elderly with chronic diseases had taken health floods in the descending order of neuralgia-arthritis, hypertension, digestive disease, diabetes. The elderly with digestive diseases, diabetes, renal disease, neuralgia- arthritis and respiratory disease were inclined to habitually take health floods once or twice per year. Intake of Korean traditional folk food was as high as intake of registered health food and Chinese tonic medicine. The most frequently taken health food was tonic medicine, which is fo11owed by pumpkin, ginseng products, herbal tea formula with black goat, deer antler and Kye-so-ju. Intakes of health food such as tonic medicine, blood of the deer, royal jelly, aloe were found to be higher among the elderly with chronic diseases such as diabetes, hypertension, and digestive disease. However, intakes of ginseng products, deer antler, pumpkin, herbal tea formula with black goat and Kye-so-ju were found to be higher among the healthy elderly. The elderly with neuralgia-arthritis had taken sixteen kinds of health foods. From these results, a wide consumer education program should be developed to convince people of the importance of well-balanced diet and to choose proper health foods according to the elderly's health conditions. Also, comprehensive and scientific research into Korean traditional folk foods are needed for the correct use.

Hemodynamic Evaluation of Acute Mitral Valve Insufficiency Model induced by Chordae Tendinae Rupture in Normal Dogs (개에서 건삭파열로 유발한 급성 이첨판 폐쇄부전 모델의 혈류역학적 평가)

  • The study was to observe hemodynamic alterations of cardiac function to design a model of canine mitral valve insufficiency (MVI) based on chordae tendinae rupture (CTR). Ten healthy beagles with normal heart function were used in this study. To measure hemodynamics, the patient monitor was equipped for invasive blood pressure and a Swan-Ganz catheter. Hemodynamic alterations were checked promptly during CTR procedures. MVI model was made by transection of the chordae tendinae with small arthroscopy hook knife through $5^{th}$ intercostal open chest. Color Doppler at the level of the mitral valve showed high-velocity regurgitant flow immediately after CTR at intraoperative echocardiography. In hemodynamic measurements, pulmonary capillary wedge pressure (PCWP) was significantly increased, while mean arterial pressure (MAP), venous pressure (VP), pulmonary arterial pressure (PAP), cardiac output (CO) and cardiac index (CI) were significantly decreased after CTR. It was known that the left atrium was overloaded by regurgitant volume from the left ventricle. In conclusion, the MVI model induced by CTR technique in this study should be used as suitable one for the effective research of canine mitral valve disease. Further study should be needed to measure the chronic alternation of mitral valve in the model.

Outcomes into Adulthood of Survivors Born Either Extremely Low Birthweight or Extremely Preterm

  • We need to understand the outcomes into adulthood for survivors born either extremely low birthweight (ELBW; <1,000 g) or extremely preterm (EP; <28 weeks' gestational age), particularly their blood pressure and cardiovascular metabolic status,respiratory function, growth, psychological and mental health performance, and functional outcomes. Blood pressure is higher in late adolescence and early adulthood in ELBW/EP survivors compared with controls. In some studies, expreterm survivors have higher insulin and blood lipid concentrations than controls, which may also increase their risk for later cardiovascular disease. ELBW/EP survivors have more expiratory airflow obstruction than do controls. Those who had bronchopulmonary dysplasia (BPD) in the newborn period have even worse lung function than those who did not have BPD. As a group, they are unlikely to achieve their full lung growth potential, which means that more of them are likely to develop chronic obstructive airway disease in later life. Although they are smaller than term born controls, their weight gradually rises and ultimately reaches a mean z-score close to zero in late adolescence, and they ultimately attain a height z-score close to their mid-parental height z-score. On average, ex-preterm survivors have intelligence quotient (IQ) scores and performance on tests of academic achievement approximately 2/3 SD lower than do controls, and they also perform less well on tests of attention and executive function. They have similar high rates of anxiety and depression symptoms in late adolescence as do controls. They are, however, over-represented in population registries for rarer disorders such as schizophrenia and Autism Spectrum Disorder. In cohort studies, ex-preterm survivors mostly report good quality of life and participation in daily activities, and they report good levels of self-esteem. In population studies, they require higher levels of economic assistance, such as disability pensions, they do not achieve education levels as high as controls, fewer are married, and their rates of reproduction are lower, at least in early adulthood. Survivors born ELBW/EP will present more and more to health carers in adulthood, as they survive in larger numbers.

  • The community of women into society in various fields is expanding, and married late, and accordingly, the first child birth is increasing age. Accordingly, dystocia has also increased. Listed in the literature that the traditional method of birth seemed to oriental medical interpretation of one of the ancient wisdom of the predecessors to explore. The result of this study is summarized as follows. 1. Child birth custom divide into childbirth preparation, childbirth and postpartum care. Postpartum care divide into preparation of the birthplace, food for pregnant woman and birth preparation of the necessary things. 2. Preparation of the birthplace of the court case and that qeen is three months, one month before that in the case of concubines were installed. The difference in manners, and actual number but the configuration of the same item was prepared. In the private, birthplace is installed husband's home or parent's home. 3. For the safe of a paturient woman and a baby, magical prescription was practiced in so-guk-so(昭格署) in early chosun dynasty. After so-guk-so(昭格署) abolished magical practice was stopped. Then Wore old clothes based on dong-ui-bo-gam(東醫寶鑑) 5. Im-san-ye-zi-bub(臨产豫智法) specifically to instruct the royal birth is the birth of guidelines. Im-san-ye-zi-bub(臨产豫智法) cosist of birth place, abdominal pain Precautions, food, care after childbirth, remove the plecenta posion, method of cut navel, bath, protection. Through various court records of the situation, according to the guidelines are properly applied based on dong-ui-bo-gam(東醫寶鑑).

Soft Tissue Reconstruction of Complete Circumferential Defects of the Upper Extremity

  • Background Upper extremity soft tissue defects with complete circumferential involvement are not common. Coupled with the unique anatomy of the upper extremity, the underlying etiology of such circumferential soft tissue defects represent additional reconstructive challenges that require treatment to be tailored to both the patient and the wound. The aim of this study is to review the various options for soft tissue reconstruction of complete circumferential defects in the upper extremity. Methods A literature review of PubMed and MEDLINE up to December 2016 was performed. The current study focuses on forearm and arm defects from the level at or proximal to the wrist and were assessed based on Tajima's classification (J Trauma 1974). Data reviewed for analysis included patient demographics, causality, defect size, reconstructive technique(s) employed, and postoperative follow-up and functional outcomes (when available). Results In accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, 14 unique articles were identified for a total of 50 patients (mean=28.1 years). Underlying etiologies varied from extensive thermal or electrical burns to high impact trauma leading to degloving or avulsion, crush injuries, or even occur iatrogenically after tumor extirpation or extensive debridement. Treatment options ranged from the application of negative pressure wound dressings to the opposite end of the spectrum in hand transplantation. Conclusions With the evolution of reconstructive techniques over time, the extent of functional and aesthetic rehabilitation of these complex upper extremity injuries has also improved. The proposed management algorithm comprehensively addresses the inherent challenges associated with these complex cases.

Analysis of Hereditary Nonpolyposis Colorectal Cancer in Malay Cohorts using Immunohistochemical Screening

  • Background: Lynch syndrome (LS) is an inherited predisposition to colorectal, endometrial (uterine) and other cancers. Although most cancers are not inherited, about 5 percent (%) of people who have colorectal or endometrial cancer have the Lynch syndrome. It involves the alteration of mismatch repair (MMR) genes; MLH1, MSH2, MSH6 or PMS2. In this study, we analyzed the expression of MMR proteins in colorectal cancer in a Malay cohort by immunohistochemistry. Materials and Methods: A total of 17 patients were selected fulfilling one of the Bethesda criteria: colorectal cancer diagnosed in a patient aged less than 50 years old, having synchronous and metachronous colorectal cancer or with a strong family history. Immunohistochemical staining was performed on paraffin embedded tumour tissue samples using four antibodies: MLH1, MSH2, MSH6 and PMS2. Results: Twelve out of 17 patients (70.6%) were noted to have a family history. A total of 41% (n=7) of the patients had abnormal immunohistochemical staining with one or more of the four antibodies. Loss of expression were noted in 13 tumour tissues with a negative staining score <4. Of 13 tumour tissues, four showed loss expression of MLH1. For PMS2, loss of expression were noted in five cases. Both MSH2 and MSH6 showed loss of expression in two tumour tissues respectively. Conclusions: Revised Bethesda criteria and immunohistochemical analysis constituted a convenient approach and is recommended to be a first-line screening for Lynch syndrome in Malay cohorts.

Genetic Characterization of Indigenous Goats of Sub-saharan Africa Using Microsatellite DNA Markers

  • Genetic diversity of sub-Saharan African goats was assessed using 19 microsatellite markers. Breeds were sampled from eastern Africa (Maasai, Kigezi, Mubende, North West Highland, Arsi-Bale), southern Africa (Ndebele, Pafuri) and West Africa (West African Dwarf, Maure, Djallonke). European breeds (Grisons Striped, Toggenburg), Asian breeds (Mongolian Cashmere, Bandipur) and a Middle East breed (Arab) were also included. The mean number of alleles per locus and average gene diversity ranged from 5.26$\pm$0.464 (Djallonke) to 7.05$\pm$0.516 (Mubende) and from 0.542$\pm$0.036 (Pafuri) to 0.672$\pm$0.031 (Ndebele), respectively. The between breeds variation evaluated using $$G_{ST}$$ and $\theta$ were found to account for 14.6% ($\theta$) and 15.7% ($$G_{ST}$$) of the total genetic variation. The $D_{A}$ measure of genetic distance between pairs of breeds indicated that the largest genetic distance was between Pafuri and Djallonke while the lowest genetic distance was between Arsi-Bale and North West Highland. A neighbour-joining tree of breed relationships revealed that the breeds were grouped according to their geographic origins. Principal component analysis supported the grouping of the breeds according to their geographic origins. It was concluded that the relationships of sub-Saharan African goat breeds were according to their geographical locations implying that the goats of eastern Africa, West Africa and southern Africa are genetically distinct. Within each sub-region, goat populations could be differentiated according to morphological characteristics.

Anterior Cervical Discectomy and Fusion YouTube Videos as a Source of Patient Education

  • Study Design: Cross sectional study. Purpose: To assess the quality of anterior cervical discectomy and fusion (ACDF) videos available on YouTube and identify factors associated with video quality. Overview of Literature: Patients commonly use the internet as a source of information regarding their surgeries. However, there is currently limited information regarding the quality of online videos about ACDF. Methods: A search was performed on YouTube using the phrase 'anterior cervical discectomy and fusion.' The Journal of the American Medical Association (JAMA), DISCERN, and Health on the Net (HON) systems were used to rate the first 50 videos obtained. Information about each video was collected, including number of views, duration since the video was posted, percentage positivity (defined as number of likes the video received, divided by the total number of likes or dislikes of that video), number of comments, and the author of the video. Relationships between video quality and these factors were investigated. Results: The average number of views for each video was 96,239. The most common videos were those published by surgeons and those containing patient testimonies. Overall, the video quality was poor, with mean scores of 1.78/5 using the DISCERN criteria, 1.63/4 using the JAMA criteria, and 1.96/8 using the HON criteria. Surgeon authors' videos scored higher than patient testimony videos when reviewed using the HON or JAMA systems. However, no other factors were found to be associated with video quality. Conclusions: The quality of ACDF videos on YouTube is low, with the majority of videos produced by unreliable sources. Therefore, these YouTube videos should not be recommended as patient education tools for ACDF.

Effects of Farinelli Breating Exercise on Respiratory Function and Symptoms in Patients with Chronic Obstructive Pulmonary Disease

  • Background: Farinelli breathing (FB) exercise is a typical breathing exercise used by singers. This study aimed to compare effects of FB exercise and diaphragmatic breathing (DB) exercise on respiratory function and symptoms in patients with chronic obstructive pulmonary disease (COPD). Methods: Sixteen patients aged 51-80 years with mild or moderate COPD were recruited for this study. They were divided into two groups: DB group (n=8) and FB group (n=8). Both groups received complete breathing exercise training five times per week for 8 weeks. Their respiratory functions, COPD symptoms, cytokine levels, and oxidative stress variables were analyzed during pre- and post-tests. Dependent variables were compared between pre- and post-tests using paired t-tests. An independent t-test was used to compare variables between the groups. Differences were considered significant at p<0.05. Results: The maximal expiratory pressure (MEP), maximum oxygen consumption (VO2max), and COPD Assessment Test (CAT) scores changed significantly in the DB group after the 8-week intervention compared to those at pre-test, whereas force vital capacity, forced expiratory volume in the first second, maximum voluntary ventilation, maximal inspiratory pressure (MIP), MEP, VO2max, CAT score, tumor necrosis factor-α, and malondialdehyde level changed significantly in the FB group at post-test compared to those at pre-test. Moreover, both MIP and MEP in the FB group were significantly higher than those in the DB group. Conclusion: FB exercise improved respiratory functions and COPD symptoms of patients with COPD. It might be an alternative breathing exercise in pulmonary rehabilitation programs for patients with COPD.
