Rotator cuff repair with or without proximal end detachment for long head of the biceps tendon tenodesis

  • Background: Rotator cuff tears cause pathologies of the long head of the biceps tendon (LHBT). One of the surgical treatments for such a tear is LHBT tenodesis to the humerus. This study aims to compare simultaneous rotator cuff repair and LHBT tenodesis with or without detachment of the proximal end of the LHBT (PELHBT) from its site of adhesion to the glenoid. Methods: This retrospective study involved patients affected by LHBT pathology with rotator cuff tear. The patients were divided into two groups, with or without PELHBT detachment from the glenoid. Therapeutic outcomes were investigated by evaluation of patient satisfaction, pain based on visual analog scale, shoulder function based on Constant score and simple shoulder test, and biceps muscle strength based on the manual muscle testing grading system before surgery, at 6 months, and at the final visit after surgery. Results: Groups 1 and 2 comprised 23 and 26 patients, respectively, who showed no significant differences in demographic characteristics (p>0.05). Shoulder function, biceps muscle strength, pain, and satisfaction rate improved over time (p<0.05) but were not significantly different between the two groups (p>0.05). No post-surgical complication was found in either group. Conclusions: There was no difference in final outcomes of tenodesis with or without detachment of the PELHBT from the supraglenoid tubercle. Such tendon detachment is not necessary.

Comparative Analysis of Platelet-rich Plasma Effect on Tenocytes from Normal Human Rotator Cuff Tendon and Human Rotator Cuff Tendon with Degenerative Tears

  • Background: Platelet-rich plasma (PRP) stimulates cell proliferation and enhances matrix gene expression and synthesis. However, there have been no comparative study of the PRP effect on the normal and degenerative tenocytes. The purpose of this study was to compare the effect of PRP on tenocytes from normal and degenerative tendon. Methods: Tendon tissues were obtained from patients undergoing arthroscopic repair (n=9) and from healthy donors (n=3). Tenocytes were cultured with 10% (vol/vol) platelet-poor plasma, PRP activated with calcium, and PRP activated with calcium and thrombin. The total cell number was assessed at days 7 and 14. The expressions of type I and III collagen, decorin, tenascin-C, and scleraxis were evaluated by quantitative real-time reverse transcriptase polymerase chain reaction. The total collagen and glycosaminoglycan (GAG) synthesis was evaluated at days 7 and 14. Results: No differences were observed between the groups at day 7, but cell proliferation was remarkably increased in tenocytes from the degenerative tendon at day 14. In both tenocyte groups, the gene expressions of type I and III collagen were up-regulated. GAG synthesis was greater in the normal tendon, whereas the expressions of decorin and tenascin-C were increased in tenocytes from the degenerative tendon. Tenocytes from the degenerative tendon had higher fold-change of GAG synthesis and a lower collagen III/I ratio than normal tenocytes. Conclusions: PRP promoted the cell proliferation and enhanced the synthesis of tendon matrix in both groups. PRP has a greater positive effect on cell proliferation, matrix gene expression and synthesis in tenocytes from degenerative tendon.

회전근 개 파열과 동반된 상완 이두 건 장두의 병변 (Long Head of the Biceps Tendon Lesion Associated with Rotator Cuff Tear)

  • 목적: 회전근 개 파열과 동반되어진 상완 이두 건의 병변 및 치료 결과를 알아보고자 하였다. 대상 및 방법: 회전근 개 파열로 수술적 치료를 받고 후향적으로 상완 이두 건의 병변 여부를 확인할 수 있었던 92예를 대상으로 2년 이상 추시하였다. 상완 이두 건 병변은 건막염, 건의 마모 또는 비대, 파열, 불안정성의 4형태로 구분하였다. 회전근 개는 광범위 파열 중 4예를 제외하고는 모두 봉합하였으며 상완 이두 건 병변에 대해서는 변연 절제 30예, 건 절단 10예, 건 고정 8예, 재배치 4예를 시행하였다. 결과는 UCLA 평가 지수를 이용하였다. 결과: 상완 이두 건 병변을 보인 예가 70예 (76%)로 건막염은 19예, 건의 마모 또는 비대 22예, 파열 21예, 불안정성 8예의 병변을 보였다. 회전근 개 파열의 크기에 따라서 중범위 이하에서는 63%, 대범위 이상에서는 88%의 상완 이두 건 병변을 보였다. 치료 결과는 상완 이두 건 병변이 없었던 22예에서는 29.6점, 병변이 있었던 예 중 회전근 개 봉합을 시행하였던 66예에서는 28.3점이었다. 동반된 상완 이두 건을 절단하거나 재고정을 시행한 예에서는 28.2점이었다. 결론: 회전근 개 파열의 크기가 클수록 상완 이두 건 병변이 더 많이 발생하였고 형태도 보다 심하게 나타났다. 따라서 상완 이두 건 병변의 원인이 회전근 개 파열을 유발시키는 원인과 연관될 수 있다고 추측되었다. 동반된 상완 이두 건 병변의 치료로 건 절단이나 건 고정이 동통 완화에 효과적일 수 있다고 사료되었다.

Anterior capsular reconstruction with acellular dermal allograft for subscapularis deficiency: a report of two cases

  • Anterior glenohumeral instability with an irreparable subscapularis tear is a challenging problem for the orthopedic shoulder surgeon. Current techniques, including tendon transfers, yield inconsistent results with high rates of recurrent instability. Acellular dermal allografting has been used in young patients with massive superior rotator cuff tears with early success, but acellular dermal allografting is comparatively unstudied in anterior deficiency. We present two cases of anterior capsular reconstruction with an acellular dermal allograft in patients ages 66 and 58 years with irreparable subscapularis tendon tears. Follow-up for both patients exceeded 4 years, with forward flexion >140°, external rotation exceeding 60°, a Single Assessment Numeric Evaluation score >90 points, a visual analog scale score of 0 points, and an American Shoulder and Elbow Score of 98 points. In conclusion, acellular dermal allografting can be used to reconstruct the anterior capsule in patients with massive irreparable subscapularis tears, similar to its use in superior capsular reconstruction in patients with massive posterosuperior rotator cuff tears.

Clinical outcome of ultrasound-guided atelocollagen injection for patients with partial rotator cuff tear in an outpatient clinic: a preliminary study

  • Background: Atelocollagen has been studied for restoration of rotator cuff tendon. In this study, we attempted to evaluate the clinical outcome of ultrasound-guided atelocollagen injection in an outpatient clinic for patients with partial rotator cuff tear. Methods: We recruited 42 outpatients who visited our hospital from May 2019 to September 2019. Atelocollagen injection was performed in patients with partial rotator cuff tear diagnosed by magnetic resonance imaging and ultrasound. American Shoulder and Elbow Surgeons (ASES), Constant, Korean Shoulder Score (KSS) and Simple Shoulder Test (SST) scores, and range of motion were assessed before injection and after 2 months. Statistically, we analyzed the clinical results using the Wilcoxon signed-rank test. Results: Finally, 15 patients were enrolled for analysis. There was no significant difference between pre- and post-injection in terms of range of motion, ASES (57.0 vs. 60.4), Constant (56.4 vs. 58.9), KSS (64.6 vs. 68.5), and pain-visual analog scale (4.2 vs. 3.7), except function-visual analog scale (F-VAS; 6.3 vs. 7.1) and SST (6.6 vs. 6.9). A significant difference was found in SST (P=0.046) and F-VAS (P=0.009). According to the ultrasound results at 2 months, we found hyperechoic materials in three of seven patients. The most common complication of atelocollagen injection was post-injection pain (53%, 8/15). Conclusions: Ultrasound-guided atelocollagen injection for partial rotator cuff tear showed no significant change in terms of clinical outcomes, except for F-vas and SST score. Tendon regeneration was not clear due to the remnants of atelocollagen present at 2-month follow-up ultrasound. There seems to be alarming post-injection pain for 2 to 3 days in the patients who received atelocollagen injection in an outpatient clinic.

관절경적 회전근 개 봉합술: 일열 봉합 수기 (Arthroscopic Rotator Cuff Repair: Single Row Technique)

  • 관절경적 일열 봉합법은 잘 정립된 회전근 개 봉합술로 우수한 임상성적을 보여왔다. 하지만, 수술 후 재파열의 빈도가 높은 것으로 알려져 있어 다양한 방법들이 술 후 재파열을 줄이기 위하여 시도되어왔다. 일부 연구들에서는 해부학적 회전근 개 부착부를 재건하면 봉합한 회전근 개의 치유 및 초기 역학적 강도를 증가시킬 수 있을 것이라고 보고하였고, 이열 봉합법이 해부학적 회전근 개 부착부 재건과 봉합부의 강도를 증가 시키고, 간격형성을 감소 시킬 목적으로 소개되었다. 하지만, 재부착된 회전근 개 건의 장력, 봉합된 건의 혈관 형성 등의 생물학적 치유환경이 일렬 봉합법에 비하여 우수함이 아직 입증되지 않았고 수술 후 기능적 개선도 측면에서도 두 봉합술간 차이가 없으므로, 수술 수기가 상대적으로 쉽고, 요구되는 기구 및 내고정물의 추가적 비용이 적어 경제적인 측면에 장점이 있는 일렬 봉합법은 여전히 추천되는 수술수기이다.

광범위 회전근개 파열의 수술적 치료 후 발생한 단기 합병증 (Early Complications after Repair of Massive Rotator Cuff Tear)

  • Purpose: To investigate early complications after repair of massive rotator cuff tears and to find out factors that compromise the results. Materials and Methods: Fourteen patients who had two or more cuff tendons involved were included. All patients were operated by open acromioplasty and rotator cuff repair. At 3 months after operation, we investigated whether there were any early complications or not. We used ASES scoring system for preoperative and follow up evaluation. In addition, various preoperative factors, such as duration of symptom, degree of tendon retraction, degree of fatty degeneration, and acromio-humeral distance, were compared between the complicated patients and non-complicated patients. Results: At 3 months after operation, the ASES score and pain were improved in any degree in all patients. But 5 patients complained persisting pain, and three of them showed major complications such as re-rupture of rotator cuff or deltoid rupture. But no preoperative factors in complicated patients were significantly different from those in non-complicated patients. Conclusion: None of the preoperative factors were related to the complications. There was a tendency of overestimation of fatty degeneration in MRI. Some factors in surgical technique and rehabilitation were highly suspected to be related to the complications.

견갑하근 상 1/3 파열 (Partial Tear of Upper Portion of Subscapularis)

  • An isolated tear of the subscapularis is uncommon, and there are a few literatures regarding the treatment of this problem. But, the incidence has increased with development of the arthroscopic techniques. An all-arthroscopic rotator cuff repair is a challenging procedure that can be effectively performed for treatment of subscapularis tendon tears. Often, tears of the subscapularis tendon do not involve entire tendon, and retraction of the torn edge is within to 2 cm of its attachment site. Occasionally, the entire tendon is torn and retracted medially to the glenoid. This article outlines the examination, preoperative planning and details the steps necessary to perform this procedure on upper third of subscapularis tears.

Isokinetic Muscle Strength and Muscle Endurance by the Types and Size of Rotator Cuff Tear in Men

  • Background: Our study was to determine the effect on shoulder isokinetic muscle strength and muscle endurance in isolated full-thickness supraspinatus tendon tear and combined other rotator cuff tear. Methods: Total of 81 male patients (mean age $57.8{\pm}7.4$ years) who were diagnosed as a full-thickness supraspinatus tendon tear were included. They were classified into isolated or combined tear. The isokinetic muscle strength and muscle endurance were measured using the Biodex multi-joint system $PRO^{(R)}$ (Biodex Medical Systems, Shirley, NY, USA) in following movements: shoulder abduction, adduction, flexion, extension, external rotation, and internal rotation. Then, the difference in muscle function according to the type of tears were assessed. Fifty-seven patients had isolated supraspinatus tendon (mean age $56.9{\pm}7.3$ years). They were classified into either anteroposterior tear or modified mediolateral tear. The size were measured using T2-weighted magnetic resonance imaging scans in sagittal plane. Results: Between subjects categorized into the type of tear, we found significant inter-categorical differences in isokinetic muscle strength during abduction, adduction, flexion, extension, and internal rotation, and in muscle endurance during flexion, extension, and internal rotation. Anteroposterior diameter tear, we did not show significant differences in either isokinetic muscle strength or muscle endurance during any movements. However, with modified mediolateral diameter, we found significant differences with isokinetic muscle strength during adduction, and in muscle endurance the external rotation and internal rotation. Conclusions: We found that a supraspinatus tendon tear associated with more numbers of rotator cuff tears has lower isokinetic muscle strength and muscle endurance than a tear found alone.
