회전식 nickel-titanium file을 이용한 유치의 근관치료 (ROOT CANAL TREATMENT ON PRIMARY TEETH USING NICKEL-TITANIUM NOTARY FILES)

  • 유치의 근관치료에서 치수절제술은 실활된 유치를 치열궁내에 유지하기 위해 치관과 근관부분의 괴사된 치수조직을 완전하게 제거하는 것을 말한다. 이 술식은 감염된 치수의 세균을감소시키는 것을 목적으로 하며, 다양하고 불규칙한 유치 근관의 형태가 주된 장애 요소로 작용한다. 더욱이 비협조적인 어린이의 경우에는 오랜 시간동안 정교한 시술을 하기가 곤란하다. 최근 들어 근관치료시에 회전식 기구를 이용하여 근관형성을 하는 장비들이 많이 개발되었으며 저속전기 모터에 회전식 NiTi file을 이용한 술식이 많이 사용되고 있다. 본 증례에서는 회전식 NiTi file과 전기모터를 이용하여 유구치의 근관치료를 시도하였다. 치수강을 개방하고 치관부 치수를 제거한 후 잠정적 작업장을 측정하였으며 적절한 NiTi file을 선택하여 잠정적 작업장까지 crown-down방법으로 점차적으로 형성하였다. 수용 file로 근단부 도달 및 성형을 하고 전체적인 마무리를 하여 Vitapex로 충전하여 양호한 결과를 얻었기에 보고하는 바이다.

The effect of root canal preparation on the surface roughness of WaveOne and WaveOne Gold files: atomic force microscopy study

  • Objectives: To examine the surface topography of intact WaveOne (WO; Dentsply Sirona Endodontics) and WaveOne Gold (WOG; Dentsply Sirona Endodontics) nickel-titanium rotary files and to evaluate the presence of alterations to the surface topography after root canal preparations of severely curved root canals in molar teeth. Materials and Methods: Forty-eight severely curved canals of extracted molar teeth were divided into 2 groups (n = 24/each group). In group 1, the canals were prepared using WO and in group 2, the canals were prepared using WOG files. After the preparation of 3 root canals, instruments were subjected to atomic force microscopy analysis. Average roughness and root mean square values were chosen to investigate the surface features of endodontic files. The data was analyzed using one-way analysis of variance and post hoc Tamhane's tests at 5% significant level. Results: The surface roughness values of WO and WOG files significantly changed after use in root canals (p < 0.05). The used WOG files exhibited higher surface roughness change when compared with the used WO files (p < 0.05). Conclusions: Using WO and WOG Primary files in 3 root canals affected the surface topography of the files. After being used in root canals, the WOG files showed a higher level of surface porosity value than the WO files.

Dentinal defects induced by 6 different endodontic files when used for oval root canals: an in vitro comparative study

  • Objectives: To compare the formation of dentinal defects using stainless-steel hand K-files (HFs), rotary files, reciprocating files, and Self-Adjusting File (SAF), when used for oval root canals. Materials and Methods: One hundred and forty extracted human mandibular premolar with single root and oval canal were selected for this study. Oval canals were confirmed by exposing to mesio-distal and bucco-lingual radiographs. Teeth with open apices or anatomic irregularities were excluded. All selected teeth were de-coronated perpendicular to the long axis of the tooth, leaving roots segments approximately of 16 mm in length. Twenty teeth were left unprepared (control), and the remaining 120 teeth were divided into 6 groups (n = 20) and instrumented using HF (size 40/0.02), Revo-S (RS; size 40/0.06), ProTaper NEXT (PTN; size 40/0.06), WaveOne (WO; size 40/0.09), RECIPROC (RC; size 40/0.06), and the SAF (2 mm). Roots were then sectioned 3, 6, and 9 mm from the apex, and observed under stereomicroscope, for presence of dentinal defects. "No defect" was defined as root dentin that presented with no visible microcracks or fractures. "Defect" was defined by microcracks or fractures in the root dentin. Results: The control, HF, and SAF did not exhibit any dentinal defects. In roots instrumented by RS, PTN, WO, and RC files exhibited microcracks (incomplete or complete) in 40%, 30%, 55%, and 50%, respectively. Conclusions: The motor-driven root canal instrumentation with rotary and reciprocating files may create microcracks in radicular dentine, whereas the stainless-steel hand file instrumentation, and the SAF produce minimal or less cracks.

다양한 엔진 구동형 파일의 근관 성형 능력 비교 (SHAPING ABILITY OF NICKEL-TITANIUMROTARY FILES)

  • This study compared the shaping ability of nickel-titanium rotary files with different rake angle and radial land. The nickel-titanium files used in this study were Profile(Dentsply, Maillefer, Ballaigues, Switzerland), Hero 642(Micromega, Besancon, France) and K3 (SybronEndo, Glendora, Ca, USA) file. Resin blocks substituted for root canals. 36 resin blocks were divided into 3 groups with 12 canals each The time for canal preparation was recorded. The images of pre- and postoperative resin canal were scanned and those were superimposed Amounts of canal deviation, total canal widths, inner canal widths, and outer canal widths were measured at apical 1, 2 ,3, 4, 5, 6, and 7mm levels. The amount of canal deviation was the smallest in Profile group , and the time far canal preparation was the shortest in Hero 642 group. K3 group resulted in competent characteristics in both measurements. Positive rake angle seemed to result in fast shaping of root canal and radial land guide the instrument in center of the canals and around curvatures. Radial land also tended to reduce the sense of screwing into the root canal. The proper selection of the nickel-titanium fie based on the knowledge about file design is needed for the safer, simpler and faster root canal therapy.

Effect of adaptive motion on cyclic fatigue resistance of a nickel titanium instrument designed for retreatment

  • Objectives: The aim of this study was to evaluate the cyclic fatigue resistance of the ProTaper Universal D1 file (Dentsply Maillefer) under continuous and adaptive motion. Materials and Methods: Forty ProTaper Universal D1 files were included in this study. The cyclic fatigue tests were performed using a dynamic cyclic fatigue testing device, which had an artificial stainless steel canal with a $60^{\circ}$ angle of curvature and a 5 mm radius of curvature. The files were randomly divided into two groups (Group 1, Rotary motion; Group 2, Adaptive motion). The time to failure of the files were recorded in seconds. The number of cycles to failure (NCF) was calculated for each group. The data were statistically analyzed using Student's t-test. The statistical significant level was set at p < 0.05. Results: The cyclic fatigue resistance of the adaptive motion group was significantly higher than the rotary motion group (p < 0.05). Conclusion: Within the limitations of the present study, the 'Adaptive motion' significantly increased the resistance of the ProTaper Universal D1 file to cyclic facture.

Cyclic fatigue resistance tests of Nickel-Titanium rotary files using simulated canal and weight loading conditions

  • Objectives: This study compared the cyclic fatigue resistance of nickel-titanium (NiTi) files obtained in a conventional test using a simulated canal with a newly developed method that allows the application of constant fatigue load conditions. Materials and Methods: ProFile and K3 files of #25/.06, #30/.06, and #40/.04 were selected. Two types of testing devices were built to test their fatigue performance. The first (conventional) device prescribed curvature inside a simulated canal (C-test), the second new device exerted a constant load (L-test) whilst allowing any resulting curvature. Ten new instruments of each size and brand were tested with each device. The files were rotated until fracture and the number of cycles to failure (NCF) was determined. The NCF were subjected to one-way ANOVA and Duncan's post-hoc test for each method. Spearman's rank correlation coefficient was computed to examine any association between methods. Results: Spearman's rank correlation coefficient (${\rho}$ = -0.905) showed a significant negative correlation between methods. Groups with significant difference after the L-test divided into 4 clusters, whilst the C-test gave just 2 clusters. From the L-test, considering the negative correlation of NCF, K3 gave a significantly lower fatigue resistance than ProFile as in the C-test. K3 #30/.06 showed a lower fatigue resistance than K3 #25/.06, which was not found by the C-test. Variation in fatigue test methodology resulted in different cyclic fatigue resistance rankings for various NiTi files. Conclusions: The new methodology standardized the load during fatigue testing, allowing determination fatigue behavior under constant load conditions.

  • The aim of this study was to compare shaping ability of nickel-titanium rotary files with different taper in simulated resin canals. The instruments used in this study were GT with .04 taper ProFile with ISO-sized tip, .04 taper ProFile with ISO-sized tip alone, and .02 taper Naviflex file and all canals were prepared by crown-down technique. A total of 30 composite images were made from pre- and post-canal scanned images using Scanjet 4C scanner and Corel photopaint 8.0 and then, prevalence of canal aberrations was measured. The amount of coronal substance the instruments removed was also calculated two-dimensionally on digitized images with the Brain C software to compare the relative enlarging efficiency. Finally, the prepared internal canal form was evaluated three-dimensionally with impression technique. The results were as follows; 1. The incidence of zipping in Naviflex group was significantly high compared to the other two groups(P<.05). 2. The amount of coronal substance the GT combined group removed was significantly larger than the other two groups(P<.05). 3. The GT combined group produced canals with good taper and flow. Under the conditionas of this study, the combined use of larger tapered nickel-titanium rotary files prepared simulated canals safely, efficiently and with good three-dimensional form.

Comparison of apical extrusion of intracanal bacteria by various glide-path establishing systems: an in vitro study

  • Objectives: This study compared the amount of apically extruded bacteria during the glide-path preparation by using multi-file and single-file glide-path establishing nickel-titanium (NiTi) rotary systems. Materials and Methods: Sixty mandibular first molar teeth were used to prepare the test apparatus. They were decoronated, blocked into glass vials, sterilized in ethylene oxide gas, infected with a pure culture of Enterococcus faecalis, randomly assigned to 5 experimental groups, and then prepared using manual stainless-steel files (group KF) and glide-path establishing NiTi rotary files (group PF with PathFiles, group GF with G-Files, group PG with ProGlider, and group OG with One G). At the end of canal preparation, 0.01 mL NaCl solution was taken from the experimental vials. The suspension was plated on brain heart infusion agar and colonies of bacteria were counted, and the results were given as number of colony-forming units (CFU). Results: The manual instrumentation technique tested in group KF extruded the highest number of bacteria compared to the other 4 groups (p < 0.05). The 4 groups using rotary glide-path establishing instruments extruded similar amounts of bacteria. Conclusions: All glide-path establishment instrument systems tested caused a measurable apical extrusion of bacteria. The manual glide-path preparation showed the highest number of bacteria extruded compared to the other NiTi glide-path establishing instruments.
