Detection of laser doppler blood flow signal from human teeth

  • Laser doppler flowmeter (LDF) has been applied to the measurement of pulpal blood flow (PBF) in human teeth. As far as we searched, the detection area of the pulp in the blood flow measurement has not been clarified, yet. Therefore, the purpose of this study was to obtain information of the detection area in PBF measurement using LDF. The experiments were performed on the artificial blood circulation in extracted human upper central incisors. The apical portions of examined teeth (n=6) were severed and root canals were enlarged from the apical end to the 2mm incisal to the level of enamel-cement junction. An individual resin cap of each tooth was prepared and a hole was drilled 2mm incisal to enamel-cement junction of the labial side of the cap. The measurement probe of LDF (MBF3D, Moor Instrument, UK) was plugged into the hole of the cap. Heparinized human peripheral blood, which was in advance collected and diluted 3 times with physiological saline, was pumped through the apical foramen of the teeth via a silicone tube and a disposable needle (o.d. 0.7mm) and blood flow signals were monitored. The flux signal significantly increased with the enlargement of the root canal to incisal direction (p<0.01, Friedman analysis). The result indicates that the performance of LDF in PBF with human teeth is limited.

Evaluation of the relationship between sleep bruxism and pulpal calcifications in young women: A clinico-radiological study

  • Purpose: This study was performed to investigate the relationship between sleep bruxism(SB) and pulpal calcifications in young women. Materials and Methods: A total of 100 female participants between 20 and 31 years of age who were referred to our radiology clinic for a dental check-up, including 59 SB and 41 non-SB patients, were sampled for the analysis. SB was diagnosed based on the American Academy of Sleep Medicine criteria. All teeth were evaluated on digital panoramic radiographs to detect pulpal calcifications, except third molars, teeth with root canal treatment, and teeth with root resorption. Binary logistic regression analysis was used to determine the risk factors for pulpal calcifications. The Spearman correlation coefficient was applied and the Pearson chi-square test was used for categorical variables. To test intra-examiner reproducibility, Cohen kappa analysis was applied. P values <.05 were considered to indicate statistical significance. Results: A total of 2800 teeth were evaluated (1652 teeth from SB patients and 1148 from non-SB patients), and 61% of patients had at least 1 dental pulpal calcification. No statistically significant relationship was found between SB and pulpal calcifications (P>0.05). In SB patients, the total number of pulpal calcifications was 129, while in non-SB patients, it was 84. Binary logistic analysis showed that SB was not a risk factor for the presence of pulpal calcifications(odds ratio, 1.19; 95% CI, 0.52-2.69, P>.05). Conclusion: No relationship was found between SB and pulpal calcifications.

Recurrent Herpetic Stomatitis Mimicking Post-Root Resection Complication (치근 절제술의 합병증으로 오인 가능한 재발성 구내 헤르페스)

  • This case report describes about recurrent herpetic stomatitis mimicking post-root resection complication. A 49 year-old male patient was diagnosed vertical root fracture of the mesiobuccal root of his left maxillary first molar (#26). The mesiobuccal root was resected following root canal treatment of the same tooth. 19 months later, the patient presented with pain on left hard palate after a barbecue party. Intra oral examination revealed a gum boil-like blister at the hard palate corresponding to the apex of the palatal root of #26. On clinical examination, there was bleeding on probing and the periodontal pocket depth was measured less than 5 mm with no tooth mobility. On a periapical radiograph, periodontal ligament space widening was observed. Tracing the sinus tract with gutta percha cone was attempted, however, it was impossible. Extending the field of vision, small multiple round ulcerations were observed at the palate front which caused pain to the patient. Therefore, the pain was considered a non odontogenic and the patient was referred to the department of oral medicine. The patient was diagnosed recurrent herpetic stomatitis and after 3 days of antiviral medication, the pain and ulceration were subsided.

THE STUDY OF EVALUATION TO RELATIONSHIP BETWEEN THE INFERIOR ALVEOLAR NERVE AND THE MANDIBULAR THIRD MOLAR BY USING RADIOGRAPHIC IMAGE (방사선 사진을 이용한 하악 제3대구치와 하치조신경의 관계에 대한 연구)

  • Purpose: This study was performed to evaluate relationship between the inferior alveolar nerve injury and the findings of panoramic and tomographic images for preventing inferior alveolar nerve injury after the 3rd molar extraction. Material and Method: From April, 2005 to June, 2005, The 190 patients who visited in the Department of Oral and Maxillofacia Surgery, Chonnam National University Hospital and the panoramic radiographies were taken for extraction of the mandibular third molar, was selected. Among 215 mandibular third molars, Scanora tomographic imagings were taken in the 90 teeth which were overlaped to the mandibular canal in the panoramic imagies. In panoramic radiographies, the angulation, the level, the root morphology, and the superimposition sign of the mandibular third molars with the mandibular canal were evaluated. In the tomographic radiographies, the location and distance of the mandibular third molar from the canal were also evaluated. The relationships between these findings and the inferior alveolar nerve injury were examined. Results: In the panoramic findings, the inferior alveolar nerve injuries were occurred in the darkened roots (5 molars, 7%), the uncontinuous radiopaque image (3 molars, 7%), and the depositioned mandibular canal (2 molars, 10%). In the tomographic findings of 90 molars, 20 molars also had the superimposition imagies. Five molars in those molars (25%) had the inferior alveolar nerve injury after extraction. There were 10 patients who had the inferior alveolar nerve injury. The sensory was began to be recovered in 9 patients, except 1 patient, within 2 weeks, then fully recovered within 3 months. Conclusion: These results indicate that the depth mandibular third molar and the superimposition sign may be related with the risk of the inferior alveolar nerve injury after extraction.

Step by Step Analysis of Root Canal Instrumentation with ProTaper® (ProTaper를 이용한 근관 형성의 단계별 분석)

  • The purpose of this study was to investigate influence of each file step of $ProTaper^{(R)}$ system on canal transportation. Twenty simulated canals were prepared with either engine-driven $ProTaper^{(R)}$ or manual $ProTaper^{(R)}$, Group R-resin blocks were instrumented with rotary $ProTaper^{(R)}$ and group M-resin blocks were instrumented with manual $ProTaper^{(R)}$. Pre-operative resin blocks and post-operative resin blocks after each file step preparation were scanned. Original canal image and the image after using each file step were superimposed for calculation of centering ratio The image after using each file step alld image after using previous file step were superimposed for calculation of the amount of deviation. Measurements were taken horizontally at five different levels (1 2, 3, 4 and 5 mm) from the level of apical foramen. In rotary $ProTaper^{(R)}$ instrumentation group, centering ratio and the amount of deviation of each step at all levels were not significantly different (p>0.05). In manual $ProTaper^{(R)}$ instrumentation group, centering ratio and the amount of deviation of each step at all levels except of 1 mm were not significantly different (p>0.05). At the level of 1 mn, F2 file step had significantly large centering ratio and the amount of deviation (p<0.05). Under the condition of this study, F2 file step of manual ProTaper tended to transport the apical part of the canals than that of rotary $ProTaper^{(R)}$.

REGENERATIVE ENDODONTIC TREATMENT OF IMMATURE PERMANENT TEETH BY USING PLATELET-RICH FIBRIN (치근단 병변이 있는 미성숙 영구치에서 Platelet-rich fibrin(PRF)를 이용한 보존적 근관치료)

  • In case of an immature tooth with necrotic pulp, regeneration of pulp tissue into a canal would be the ideal outcome. It may be capable of promoting the continuation of normal root development. Platelet-rich fibrin has been suggested as a potentially ideal scaffold for regenerative endodontic treatment. Immature permanent teeth of young children were diagnosed with pulp necrosis and apical abscess as the result of clinical and radiographic examination. After removal of necrotic pulp, canal was irrigated with 5.25% NaOCl and dried with paper point. A triple antibiotic mixture was placed in canal space in 3 weeks. After removal of the antibiotic mixture, the platelet-rich fibrin was injected into the canal space with MTA placed directly over the platelet-rich fibrin clot. The coronal region was restored by composite resin. On the basis of short-term results of the present 3 cases, regeneration of vital tissues appears to be possible in a tooth with necrotic pulp and a periapical lesion. Also, platelet-rich fibrin proves to be potentially an ideal scaffold for this procedure. Therefore, long-term clinical observation and examination about this treatment using platelet-rich fibrin in immature permanent teeth of young children are considered to be necessary.

Characteristics of teeth referred to a dental university hospital for endodontic reason (근관치료적 이유로 치과대학병원으로 의뢰된 치아들의 특성)

  • Purpose: The aim of this study is to investigate the characteristics of patients and teeth referred to a university dental hospital for endodontic problem. Materials and Methods: From January, 2017 to December, 2018, patients who were referred to a university dental hospital for endodontic problem were collected from clinical records. A total of 1171 patient records were analyzed. The status of the referred teeth was divided into three groups according to whether they were treated endodontically based on radiographs and clinical records at the time of referred visit. Results: 69.9% of the referred teeth were maxillary and mandibular first and second molars. The average time from referral to actual visit is 9.03 days and 65.6% of the case referred with referring letter. The most primary reasons of referral were persistent clinical symptom (pain, swelling, and sinus tract) (37.9%), diagnosis difficulty (16.7%), blockage of canal space (13.8%) and difficult tooth anatomy (11.4%). In the case of referral before endodontic treatment, the most primary reason of referral was failure to make a proper diagnosis. If the teeth were referred in the middle of endodontic treatment, the most primary reason of referral was persistent clinical symptom and blockage of canal space. In the case of referral after root canal filling, the most primary reason of referral was persistent clinical symptom. Conclusion: In the case of molars, the rate of persistent clinical symptom and blockage of canal space were the most primary reason of referral, and the rate of apical surgery and management of trauma was high in the case of anterior teeth.

PULPAL AND PERIAPICAL REACT10N TO FORMOCRESOL AND DEPULPIN® IN THE RAT TEETH (백서에서 Depulpin®과 Formocresol에 대한 치수와 치근단 조직의 반응)

  • One fifth dilution of formocresol is usually used for pulpotomy of the primary teeth and emergency pulpotomy of the permanent teeth. However the use of formaldehyde has been subjected to criticism because it may be absorbed into the blood stream and become distributed systemically, it nay also alter the pulp tissue rendering it immunologically active, and have carcinogenic potential. Recently Depulpin$^{\circledR}$(VoCo., Germany) gains popularity as a devitalizing agent during root canal therapy in spite of high concentration of 49 % paraformaldehyde because it facilitate devitalization of pulp and make root canal therapy easier But there have been not enough publications about the reaction of pulp and periapical tissue caused by Depulpin. This study was performed to evaluate the histological changes in pulp and periapical tissue of rats after pulpotomy using formocresol and Depulpin and to elucidate the toxic effects of these agents. Thirty six Sprague-Dawley rats were anesthetized by intraperitoneal injection of ketamine Maxillary first molar teeth were used for pulpotomy with formocresol and Depulpin. Rats were sacrificed after 2 days, 4 days, 1 week, 2 weeks, 3 weeks and 4 weeks respectively. Specimens were histologically observed by light microscope changes in pulp and periapical tissue. The obtained results were as follows. 1. Formocresol group A zone of fixed tissue. in which odontoblasts could clearly be defined, was present directly underneath the pulpotomy dressing in almost all teeth of this group. This was followed by an area of necrotic tissue which resembled dried out fibrous tissue with no cellular detail except some pyknotic nuclei. In the specimens of after 2 days, 4 days, 1 week, 2 weeks in which vital tissue was present, it was separated from the fibrous area by a zone of inflammation. In the specimens of after 3 weeks and after 4 weeks, inflammatory infiltrate was in the periodontal ligament adjacent to the apical foramina of the teeth. 2. Depulpin$^{\circledR}$ group The area of necrotic tissue which had no cells and fibers, was present adjacent to the dressing. This was followed by dried out fibrous tissue with no cellular details except some pyknotic nuclei, A short stump of vital pulp with odontoblasts was present at the end of the canal after 2 days. Inflammatory infiltrate was in the periodontal ligament after 4 days and after 1week. Severe root resolution and necrosis of periapical tissue opposite the root resorption site were defined after 2 weeks and after 3 weeks. Periapical lesion which consist of necrotic tissue surrounded by a fibrous connective wall, was found after 4 weeks. The results indicated that Depulpin can cause more adverse reaction to the dental pulp and periapical tissue than formocresol, and further studies are needed for its clinical use with safety.

A STUDY ON STRESS DISTRIBUTION OF ENDODONTICALLY TREATED TOOTH ACCORDING TO THE VARIOUS POST LENGTH USINGTHREE-DIMENSIONAL FINITE ELEMENT METHOD (포스트 길이가 치근내 응력분산에 미치는 영향에 관한 삼차원 유한요소법적 연구)

  • The endodontically treated tooth is generally restored with post & core, owing to the brittleness and the loss of large amount of tooth structure. Although there have been lots of studies about the endodontically treated teeth, the three-dimensional quantitative studies about the strees distribution of them are in rare cases. In this study, it was assumed that the coronal portion of the upper incisou had severely damaged. After the root canal therapy it was post cored, and restored with PFM crown, for this experiment nine types of model were constructed : 1); long, 2); medium, 3); short gold post for the roots supported with a narmal alveolar bone, 4); long, 5); medium, 6); short gold post for the roots supported with an alveolar bone resorbed to its 1/3 of root length, 7); long, 8); medium, 9); short base metal post for the roots supported with an alveolar bone resorbed to its 1/3 of root length. Force was applied from two directions. One was functional maximum bite force(300N) applied to the spot just lingual to the incisal edge with the angle of 45 degrees to the long axis of the tooth, and the other one was horizontal force(300N) applied to the labial surface. The results analyzed with three-dimensional finite element method were as follows : 1. Stress was concentrated on the middle portion of the labial side dentin of the root and the lingual portion of the apical dentin of the root. Stress in the post showed maximum value at 2 mm above the post apex. 2. In case of the long post and base metal post, strees was concentrated on the apex of the root and the post. 3. In case of the longer post, the displacement on the post-cement interface was lessened. The gold post was more displaceable than the base metal post. 4. In case of the alveolar bone resorption, stress concentrated on the root and the post and displacement on the post-cement interface were increased.

Effect of Calcium Hydroxide on the Microhardness of Root Dentin of Primary Tooth (수산화칼슘 근관충전제의 적용이 유치 치근상아질의 미세경도에 미치는 영향)

  • Calcium hydroxide mixture medicaments can nearly be considered to be the ideal primary tooth filling material. However, long-term application of calcium hydroxide combinations as an intra canal medicament softens dentin. The aim of this study was to evaluate the effect of calcium hydroxide on the microhardness of root dentin of primary tooth. For the study, 60 extractedprimary incisors were divided into 3 groups (no medicament, calcium hydroxide/iodorform mixture, and calcium hydroxide/distilled water mixture). After the cleansing and shaping of canals, calcium hydroxide medicaments were applied and stored for different periods of time (1, 7, 30, 90 days). The root was horizontally sectioned into 2 mm thick specimens and the microhardness was measured using Vickers microhardness tester. The results were as follows : Root dentin microhardness of primary teeth decreased with long term exposure to calcium hydroxide medicaments according to the experimental period and showed statistically significance (p < 0.05). Root dentin microhardness of primary tooth filled with calcium hydroxide mixed with distilled water showed more decrease than filled with Vitapex and showed statistically significance (p < 0.05). Root dentin microhardness of a control group without exposure to calcium hydroxide decreased according to the experimental period and showed statistically significance (p < 0.05).
