OrthoMTA, ProRoot MTA 그리고 Portland cement의 경화 팽창과 경화 시간 비교 (COMPARISON OF SETTING EXPANSION AND TIME OF ORTHOMTA, PROROOT MTA AND PORTLAND CEMENT)

  • 현재 시판되고 있는 ProRoot white MTA(Dentsply Tulsa, U.S.A)는 portland cement와 비슷한 성분으로 수분의 첨가로 인하여 경화 팽창하는 성질이 있고, 긴 경화 시간을 갖는다. 본 연구는 최근 개발된 OrthoMTA(BioMTA, Korea), 기존의 ProRoot white MTA(Dentsply Tulsa, U.S.A) 그리고 White portland cement(Union, Korea)를 대상으로 2008년에 개정된 치근관 충전재(KS P ISO 6876)의 요구 사항 중에서 경화 팽창과 경화 시간에 대한 연구를 하였다. 연구 결과, OrthoMTA의 경화 팽창률은 $0.08{\pm}0.02%$이었고, ProRoot white MTA, White portland cement는 각각 $0.28{\pm}0.06$, $0.80{\pm}0.25%$였다(p<0.05). 경화 시간은 각각 $307.78{\pm}3.83$분, $150.44{\pm}2.35$분, $235.33{\pm}9.07$분으로, ProRoot white MTA가 가장 짧았고, OrthoMTA가 가장 길었다(p<0.05).

엔진의 토크 조절 능력과 니켈-티타늄 파일의 삭제 성향이 근관 성형에 미치는 영향 (THE EFFECT OF DIFFERENT FLUTE DESIGN AND TORQUE-CONTROLLED MOTOR ON THE SHAPING ABILITY OF SIMULATED RESIN ROOT CANALS)

  • 이 연구의 목적은 니켈-니타늄 파일의 삭제 성향과 엔진의 토크 조절 능력이 근관 성형에 미치는 영향을 평가하는 것이다 레진 모형 40개를 사용하였으며, Hero Shaper(R)와 Endo-Mate2을 이용한 HE군, Hero Shaper(R)와 Tecnika(R)를 이용한 HT군, Profile(R)과 Endo-Mate2(R)를 이용한 PE군, Profile(R)과 Tecnika(R)를 이용한 PT군으로 구분하여 근관성형 하였다. 각 군별 근관 성형 시간을 기록하였다. 근단공으로부터의 수직 이동 거리 1, 3 그리고 5mm 위치에서의 근관 삭제량, 중심이 동률을 산출하여 다음의 결론을 얻었다. 1. 근관 성형 시간은 HT군이 가장 빨랐다 (p<0.05). 2. 삭제량은 1mm지점에서 HE군이 PT군보다 컸고 (p<0.05), 3mm지점에서는 PT군이 다른 군들보다 작았다 (p<0.05). 5mm에서는 PE군이 PT군보다 컸다 (p<0.05). 3. 중심 이동률은 1mm 지점에서 HE군이 다른 군들보다 컸으며 (p<0.05), 5mm에서는 HT군이 PE군과 PT군보다 컸다 (p<0.05). 따라서 토크 조절이 되는 엔진을 사용하는 것이 근관의 변위를 적게 유발하고 과도한 삭제 량을 예방 할 수 있으며 근관 성형시간도 단축 할 수 있는 것으로 보이며 따라서 양호한 근관 성형 결과에 영향을 준다고 볼 수 있다.

하악제 3대구치와 하치조관의 위치에 관한 X선학적 연구 (A RADIOGRAPHIC STUDY OF LOCALIZATION OF THE INFERIOR ALVEOLAR CANALS IN RELATION TO THE APICES OF THE MANDIBULAR THIRD MOLARS)

  • The purpose of this study was to localize the inferior alveolar canals in relation to the root apices of the mandibular third molars, according to their positions and degrees of impaction using vertical tube shift technique. One hundred three mandibular third molars, from 95 persons consisted of 57 males and 38 females, were clinically and radiographically investigated. The mandibular third molars had no pericoronitis and periapical lesions, and showed an evidence of complete root formation. The obtained results were as follows: 1. In localiztion of the inferior alveolar canals in relation to the root apices of the mandibular third molars, the inferior alveolar canal was located at the buccal side of the root apices of mandibular third molar in 77.7%, below the root apices in 16.5%, and the lingual side of root apices in 5.8%. 2. The positions of the mandibular third molars according to the Winter's Classification were as follows; 36.9% in Class Ⅰ, 21.3% in Class Ⅱ, 14.7% in Class Ⅲ, 4.8% in Class Ⅳ, 1.9% in Class Ⅴ, 17.5% in Class Ⅵ, 2.9% in Class Ⅶ. In localization of the inferior alveolar canals in relation to the root apices of the mandibular third molars according to the Winter's Classification, 92.1 % of Class Ⅰ, 86.4% of Class Ⅱ, 80.0% of Class Ⅲ, and 100.0% of Class Ⅳ and Ⅴ were located at the buccal side. In Class Ⅵ, however, 33.3% was located at the buccal side, 44.5% below the root apices, and 22.2% at the lingual side. 3. The degree of impaction was revealed to be 53.4% in Degree Ⅰ, 36.9% in Degree Ⅱ, and 9.7% in Degree Ⅲ. In localization of the inferior alveolar canals in relation to the root apices of mandibular third molars according to degree of impaction, 98.2% of Degree Ⅰ was located at the buccal side. In Degree Ⅱ, 60.5% was located at the bucal side, 31.6% below the root apices, and 7.9% at the lingual side. In Degree Ⅲ, 30.0% was located at the buccal side, 40.0% below the root apices, and 30.0% at the lingual side.

Effect of passive ultrasonic agitation during final irrigation on cleaning capacity of hybrid instrumentation

  • Objectives: To evaluate the effect of passive ultrasonic agitation on the cleaning capacity of a hybrid instrumentation technique. Materials and Methods: Twenty mandibular incisors with mesiodistal-flattened root shape had their crowns sectioned at 1 mm from the cementoenamel junction. Instrumentation was initiated by catheterization with K-type files (Denstply Maillefer) #10, #15, and #20 at 3 mm from the working length. Cervical preparation was performed with Largo bur #1 (Dentsply Maillefer) followed by apical instrumentation with K-type files #15, #20 and #25, and finishing with ProTaper F2 file (Denstply Maillefer). All files were used up to the working length under irrigation with 1 mL of 2.5% sodium hypochlorite (Biodyn$\hat{a}$mica) at each instrument change. At the end of instrumentation, the roots were randomly separated into 2 groups (n = 10). All specimens received final irrigation with 1 mL of 2.5% sodium hypochlorite. The solution remained in the root canals in Group 1 for one minute; and ultrasonic agitation was performed in Group 2 for one minute using a straight tip inserted at 1 mm from working length. The specimens were processed histologically and the sections were analyzed under optic microscope (x64) to quantify debris present in the root canal. Results: The samples submitted to ultrasonic agitation (Group 2) presented significant decrease in the amount of debris in comparison with those of Group 1 (p < 0.05). Conclusions: The hybrid instrumentation technique associated with passive ultrasonic agitation promoted greater debris removal in the apical third of the root canals.

Pulp revascularization with and without platelet-rich plasma in two anterior teeth with horizontal radicular fractures: a case report

  • Pulp revascularization is an alternative treatment in immature traumatized teeth with necrotic pulp. However, this procedure has not been reported in horizontal root fractures. This is a case report of a 9-year-old patient with multiple horizontal root fractures in 2 upper central incisors that were successfully treated with pulp revascularization. The patient presented for treatment 2 years after the initial trauma, and revascularization was attempted after the initial treatment with calcium hydroxide had failed. Prior to pulp revascularization, cone-beam computed tomography and autoradiograms demonstrated multiple horizontal fractures in the middle and apical thirds of the roots of the 2 affected teeth. Revascularization was performed in both teeth; platelet-rich plasma (PRP) was used in one tooth (#11) and the conventional method (blood clot) was used in the other tooth (#21). Clinical and radiographic follow-up over 4 years demonstrated pulp calcification in the PRP-treated tooth. Neither of the 2 teeth were lost, and the root canal calcification of tooth #11 was greater than that of tooth #21. This case suggests that PRP-based pulp revascularization may be an alternative for horizontal root fractures.

상악 전치부에 발생한 여러 유형의 치아 외상 (DIVERSE DENIAL TRAUMA OF MAXILLARY ANTERIORS : CASE REPORT)

  • 외상으로 인해 완전 탈구와 함입성 및 정출성 탈구가 병합되어 나타나는 경우는 드물다. 2003년 7월에 10세 2개월의 여아가 상악우측 측절치의 완전탈구와 상악우측 중절치의 함입성 탈구, 상악 좌측 중절치의 정출성 탈구로 전남대학교병원 소아치과에 내원하였다. 내원 당일 외상치아를 외과적으로 재위치시켰으며 레진 강선 고정을 시행하였다. 외상 2주 후 완전탈구된 상악우측 측절치는 근관치료를 시행하였다. 외상 3개월 후 방사선사진 상에 상악 좌우측 중절치의 치근 외흡수가 관찰되었으며 수산화칼슘을 이용한 근관치료를 시행한 후 치근 외흡수가 정지되었다. 1년 4개월 동안의 관찰기간 동안 병적인 변화 없이 양호한 결과를 보였으며 추후 장기적인 추적관찰이 필요하리라 사료된다.

Isthmuses, accessory canals, and the direction of root curvature in permanent mandibular first molars: an in vivo computed tomography study

  • Objectives: This study was performed to assess the anatomy of mandibular first molars. Materials and Methods: In this in vivo study, cone-beam computed tomography (CBCT) volumes of 312 bilateral intact first mandibular molars from 156 patients (79 men and 77 women; average age, 35.6 ± 11.2 years) were investigated in terms of the direction of each canal's curvature in the buccolingual and mesiodistal dimensions (direction of the position of the apex in relation to the longitudinal axis of the root), the presence of an isthmus (a narrow, ribbon-shaped communication between 2 root canals) in 3 segments (0-2, 2-4, and 4-6 mm) from the apex), and the presence and number of accessory canals (smaller canals besides the main root canals, connecting the pulp to the periodontium). Data were analyzed statistically (α = 0.05). Results: Mesiolingual canals were mostly buccally and distally inclined, while mesiobuccal and distolingual canals were mostly distally curved. Isthmuses were more common in younger patients (χ2 test, p < 0.05). The average numbers of accessory canals in the apical, middle, and coronal segments were 9.9 ± 4.2, 6.9 ± 2.9, and 9.3 ± 3.0 canals per segment, respectively (analysis of variance, p < 0.001). Age and sex were not associated with the number of accessory canals (p > 0.05). Conclusions: The complex anatomy of these teeth deserves attention during non-surgical or surgical endodontic treatment. Around the apex, isthmuses might be more prevalent in younger and female individuals.

Comparative analysis of physicochemical properties of root perforation sealer materials

  • Objectives: This study evaluated the solubility, dimensional alteration, pH, electrical conductivity, and radiopacity of root perforation sealer materials. Materials and Methods: For the pH test, the samples were immersed in distilled water for different periods of time. Then, the samples were retained in plastic recipients, and the electrical conductivity of the solution was measured. The solubility, dimensional alteration, and radiopacity properties were evaluated according to Specification No. 57 of the American National Standards Institute/American Dental Association (ANSI/ADA). Statistical analyses were carried out using analysis of variance (ANOVA) and Tukey's test at a significance level of 5%. When the sample distribution was not normal, a nonparametric ANOVA was performed with a Kruskal-Wallis test (${\alpha}$ = 0.05). Results: The results showed that white structural Portland cement (PC) had the highest solubility, while mineral trioxide aggregate (MTA)-based cements, ProRoot MTA (Dentsply-Tulsa Dental) and MTA BIO ($\hat{A}$ngelus Ind. Prod.), had the lowest values. MTA BIO showed the lowest dimensional alteration values and white PC presented the highest values. No differences among the tested materials were observed in the the pH and electrical conductivity analyses. Only the MTA-based cements met the ANSI/ADA recommendations regarding radiopacity, overcoming the three steps of the aluminum step wedge. Conclusions: On the basis of these results, we concluded that the values of solubility and dimensional alteration of the materials were in accordance with the ANSI/ADA specifications. PCs did not fulfill the ANSI/ADA requirements regarding radiopacity. No differences were observed among the materials with respect to the pH and electrical conductivity analyses.

Cone-beam computed tomography-based diagnosis and treatment simulation for a patient with a protrusive profile and a gummy smile

  • For patients with bimaxillary protrusion, significant retraction and intrusion of the anterior teeth are sometimes essential to improve the facial profile. However, severe root resorption of the maxillary incisors occasionally occurs after treatment because of various factors. For instance, it has been reported that approximation or invasion of the incisive canal by the anterior tooth roots during retraction may cause apical root damage. Thus, determination of the position of the maxillary incisors is key for orthodontic diagnosis and treatment planning in such cases. Cone-beam computed tomography (CBCT) may be useful for simulating the post-treatment position of the maxillary incisors and surrounding structures in order to ensure safe teeth movement. Here, we present a case of Class II malocclusion with bimaxillary protrusion, wherein apical root damage due to treatment was minimized by pretreatment evaluation of the anatomical structures and simulation of the maxillary central incisor movement using CBCT. Considerable retraction and intrusion of the maxillary incisors, which resulted in a significant improvement in the facial profile and smile, were achieved without severe root resorption. Our findings suggest that CBCT-based diagnosis and treatment simulation may facilitate safe and dynamic orthodontic tooth movement, particularly in patients requiring maximum anterior tooth retraction.

감염 근관에서 분리된 연쇄구균의 16S Ribosomal DNA 중합효소 연쇄반응과 제한효소 절단길이 다형성에 관한 연구 (POLYMERASE CHAIN REACTION AND RESTRICTION FRAGMENT LENGTH POLYMORPHISM OF 16S RIBOSOMAL DNA OF STREPTOCOCCI ISOLATED FROM INFECTED ROOT CANALS)

  • Bacteria have been regarded as one of the most important factors in pulpal and periapical diseases. Streptococci are frequently isolated facultative anaerobes in infected root canals. Recently molecular biological techniques have been rapidly progressed. This study was designed to apply the molecular biological tools to the identification and classification of streptococci in the endodontic microbiology. Streptococci isolated from infected root canals were identified with both Vitek Systems and API 20 STREP. Identification results were somewhat different in several strains of streptococci. Eighteen streptococci and enterococcal was difficult so to digest plasmid DNA using Hind III and EcoRI to differentiate strains by restriction enzyme analysis of plasmid DNA. 16S rDNA of chromosome was amplified by polymerase chain reaction(PCR) and then restricition fragment length polymorphism(RFLP) using several restriction enzymes was observed. The molecular mass of 16S rDNA of chromosomal DNA was approximately 1.4kb. There were three to five RFLP patterns using eight restriction enzymes. RFLP patterns digested with CfoI which recognizes four base sequences were identical in all stains. Hind III which recognizes six base sequences could not digest the 16S rDNA. Restriction enzymes which recognize five base sequences were suitable for RFLP pattern analysis. At least three different restriction enzymes were needed to compare each strains. 16S rDNA PCR-RFLP was simple and rapid to differentiate and classify strains and could be used in the epidemiological study of root canal infections.
