Objective: To compare the magnitude of external apical root resorption (EARR) 6 months after starting orthodontic treatment using orthodontic aligners (OAs) and fixed appliances (FAs). Methods: This parallel randomized clinical trial included 40 patients randomized into two groups: OA group (n = 20, 160 incisors) and FA group (n = 20, 160 incisors). For evaluation of the tooth length, periapical radiographs and standardized linear measurements of the maxillary and mandibular incisors were acquired before (T0) and 6 months after treatment initiation (T1). EARR was calculated through the difference in length between the two time points (T1-T0). Statistical comparisons were performed by means of using t-tests, chi-squared test and covariance analysis (a = 5%). Results: Rounding of the root apex was observed in both groups; the resorption involved 2.88% of the root length, so 97.12% of the tooth length remained intact. Intragroup comparisons between the two time points revealed a significant difference, with (T1-T0) ranging from -0.52 to -0.88 mm in the FA group and from -0.52 to -0.85 mm in the OA group. In the intergroup comparisons, only tooth #21 presented a statistically significant difference (OA: -0.52 ± 0.57 mm, FA: -0.86 ± 0.60 mm); however, the overall differences between groups were not clinically relevant, ranging from 0.03 to 0.35 mm. Conclusions: OA and FA treatment resulted in a similar degree of EARR in the maxillary and mandibular incisors at 6 months after treatment initiation. However, the amount of resorption was small and does not impair tooth longevity.

Purpose: Prior to cone-beam computed tomography (CBCT), orthodontic competence included radiological interpretation. Nevertheless, maxillary impacted canines (MICs), because of their position and adjacent complex anatomy, have been challenging to interpret, particularly with regard to root resorption. Although CBCT cross-sectional reconstructions of MICs yielded clearer insights into its diagnosis and treatment planning, the value of simultaneously using 2 different cross-sectional or multiplanar reconstructions of the CBCT datasets - orthogonal and curved/panoramic multiplanar reconstructions - has hitherto not been considered. Materials and Methods: Both orthogonal and curved/panoramic multiplanar reconstruction series of 5 screenshots were each reconstructed from the 5 cm×5 cm CBCT datasets of 15 separate MICs. Fifteen credentialled and experienced orthodontist volunteers reviewed 2 separate PowerPoints of 15 randomized series each, 1 week apart. Their review considered 6 factors that could affect treatment: the position and level of the MIC, the presence or absence of root resorption, ankylosis, cysts, and dilaceration. Results: All 15 orthodontists were statistically similar regarding overall years of experience and of CBCT use. Although either reconstruction alone allowed the orthodontists to determine whether ankylosis and, to a lesser extent, most of the other features were present or absent in the MIC, reviewing both reconstructions together was necessary to determine whether root resorption was present or absent in the adjacent tooth. Conclusion: Reviewing both orthogonal and curved/panoramic multiplanar reconstructions was necessary to evaluate the presence or absence of root resorption in the teeth adjacent to MICs and that of many other features.
유치의 치근 흡수의 시기에 대한 정보는 소아치과 또는 교정과에서 진단 및 치료 계획의 수립에 중요한 역할을 제공하며, 치아의 발육 상태는 유전, 환경, 인종, 성별, 영양, 사회 경제적 상태 및 시대 변화에 따라 영향을 받는다. 본 연구의 목적은 현대 한국 아동의 유치 치근 흡수의 평균 연령을 측정하고, 1990년대 초와 2000년대 초의 유치 치근 흡수의 평균 연령을 비교하는 것이다. 경북대학교병원 소아치과에 내원한 $1990{\sim}1992$년의 1037명(여아 528명, 남아 509명) 및 $2001{\sim}2003$년의 1065명(여아 394명, 남아 671명)의 파노라마 사진을 대상으로 하여 횡적인 방법으로 조사하였다. 상악 치아 및 하악 전치부의 상의 선예도의 문제로 하악 유견치, 하악 제 1 유구치, 하악 제 2 유구치를 대상으로 하였다. 결과는 다음과 같다. 1. 유치 치근의 흡수는 2000년대 초 아동이 1990년대 초 아동에 비해 더 이른 시기에 흡수되는 경향을 보였으며 Res c 단계에서 평균 연령의 차이는 0.4년이었다. 2. Res c 단계에서 1990년대 초와 2000년대 초의 평균 연령의 차이는 여아에서 하악 유견치, 하악 제 1 유구치, 하악 제 2 유구치의 순으로 증가하였으며, 남아에서 하악 제 2 유구치. 하악 제 1 유구치, 하악 유견치의 순으로 증가하였다. 즉 평균 차이는 여아에서 나이 증가에 따라 증가하였으며, 남아에서는 나이 증가에 따라 감소하였다. 3. 1990년대 초와 2000년대 초 모두에서 여아가 남아에 비해 평균 0.3년 더 이른 시기에 흡수되는 경향을 보였다. 4. 하악 유견치의 치근 흡수의 시작은 하악 제 1 유구치 보다 늦었지만, 하악 유견치의 치근 흡수의 완료는 하악 제 1 유구치 보다 빨랐으며, 하악 유견치에서 치근 흡수에 소요된 총 기간은 가장 짧았다. 5. 치근 흡수의 속도는 흡수 말기가 초기에 비해 빠른 경향을 나타냈다. 정확한 유치 치근의 흡수시기에 대해 알기 위해서는 정기적이면서 종적인 연구가 필요하리라 생각한다.
본 연구는 급속상악확대술 시행 후 보정기간중에 지대치의 압박측 치근흡수에 불화나트륨의 경구 투여가 영향을 미치는지를 알아보기 위하여 시행되었다. 이차성징을 아직 나타내지 않은 10마리 소아기 암캐에 견치를 주지대치로 사용하는 주조형 상악 확대장치를 제작, 장착하였다 상악확대장치는 20일간에 걸쳐 5mm 확장시켰으며, 교합면 방사선사진을 이용하여 정중구개봉합의 이개를 확인하였다. 실험동물은 실험군 5 마리와 대조군 5 마리로 나뉘었으며, 보정기간 중 실험군에는 2.2 mg/kg/day의 불화나트륨 (불소로서 1.0mg/kg/day)을 캅셀제의 형태로 1 일 2 회로 나누어 단독 경구 투여하였다. 실험동물들을 보정 후 0 일, 15 일, 30 일, 45 일, 및 60 일째에 각 군당 한 마리씩 배정하여 충분한 마취로 희생시킨 후 상악골을 절취하였다. 이후 실험동물의 좌측 상악 견치의 압박측 치근표면과 압박측 견치 협면을 덮고 있던 골 조직의 치아측 표면을 주사전자현미경을 이용하여 관찰하였다. 본 실험에서 압박측 치근표면을 관찰한 결과 45일 보정군과 60일 보정군의 대조군에서 흡수소와가 발현되는 빈도가 높았다. 지대치 압박측 치근을 덮는 골면에 나타나는 흡수소와의 발현은 30일의 보정기간 이후부터 실험군에서 적게 나타남을 알 수 있었다. 차후 급속상악확대술을 시행하는 경우 효과적인 NaF의 투여시기와 용량에 대한 검증이 필요하리라 생각된다.
본 연구는 탈구치에 대한 alendronate(ALN)도포가 지연재식모델에 있어 염증반응의 억제에 미치는 효과를 관찰하고자 태생 6주된 54마리 SD Rat의 상악 제 1 대구치를 재식하였다. 실험군은 15분 자연건조 후에 즉시 재식군(I군), 60분 자연건조 후 HBSS 도포군(II군) 그리고 60분 자연건조 후 ALN 도포군(III군)으로 분류하였으며 각 군 간의 치근의 염증성 흡수상태, 치근의 대치성 흡수정도, 염증의 정도, 그리고 치주인대의 치유정도를 관찰하여 다음과 같은 결론을 얻었다. 1. I군은 II, III군에 비하여 염증의 정도나 치근의 염증성 흡수정도 등 전반적인 염증반응이 적었고, 치주조직의 재부착 및 재생의 정토가 가장 높았다. 2. II군에서는 전반적으로 염증이 가장 심하였으며, 치근흡수 또한 광범위하였다. 3. III군은 II군에 비하여 치근면에 국소적인 염증반응이 나타났으며 치근의 염증성 흡수 또한 국소적으로 발생하였다(p<0.001). I군에 비하여 치주조직의 염증반응은 증가하였으나(p<0.05) 염증성흡수와 대치성흡수에 있어서는 유의성이 없었다. 4. 각 군에서 대치성흡수의 양상은 크게 차이가 나지 않았으며, 시간의 경과에 따라 약간의 증가가 있었으나 유의성은 없었다. 이상의 결과로부터 탈구치가 구강 외에서 장시간 건조되었을 경우 ALN의 사용은 치근흡수를 억제하고 치주조직의 재생에 유용하다는 것을 알 수 있었다. ALN은 치아의 탈구시에 사용되는 여러 약재들의 대체품으로 사용할 수 있으며, 향후 추가적인 연구를 통하여 탈구치의 재식시 ALN 사용을 위한 적절한 용량과 도포용제재의 개발을 통하여 치아의 재식시 쉽게 사용할 수 있도록 연구 개발이 필요할 것으로 생각된다.
유치의 정상적인 치근 흡수는 계승 영구치의 맹출 압력으로 단핵구 세포가 방출되어 파골세포로의 분화가 촉진되어 일어나며, 계승 영구치의 맹출 경로를 따라 유치의 치근이 흡수되어서 유치가 탈락하게 된다. 계승 영구치가 선천적으로 결손되었을 때에도 유치의 치근은 흡수될 수 있는데, 유치의 비정상적 치근흡수는 외상이나 염증, 교합압의 증가, 지지조직의 약화 등으로 발생할 수 있다. 치아의 선천적 결손은 인간에서 가장 흔한 발육이상이며, 영구치가 결손되었을 때 유치는 만기 잔존할 수 있다. 영구치의 선천적 결손은 전신질환이 있는 경우뿐만 아니라 국소적 원인이나 인류 진화에 따른 영향 등에 의해서도 발생한다. 이 증례들은 계승 영구치가 선천적으로 결손된 어린이에서 유치의 치근이 비정상적으로 흡수된 경우로 계승 영구치가 결손 된 유전치와 유견치, 유구치의 치근이 1/2 ~ 3/4정도까지 흡수 되었으며 흡수 부위는 불규칙적인 형태를 보였다. 원인으로는 흡수에 감수성이 높은 유치의 치주인대, 그리고 성장에 따라 증가되는 저작근육과 저작력, 혼합치열기에 유발되는 비정상적 교합 등을 생각할 수 있다. 계승 영구치가 결손된 유치가 비정상적인 치근 흡수를 보이는 경우에 유치는 유지될 수도 있지만, 여러 요인에 의하여 탈락될 경우에는 공간을 유지 할 것인지 판단해야 하며, 향후 보철치료나 교정치료를 요하므로 장기적인 치료계획을 수립해야 한다.
Objective: Root mobility due to reciprocating movement of the tooth (jiggling) may exacerbate orthodontic root resorption (ORR). "Jiggling" describes mesiodistal or buccolingual movement of the roots of the teeth during orthodontic treatment. In the present study, buccolingual movement is described as "jiggling." We aimed to investigate the relationship between ORR and jiggling and to test for positive cell expression in odontoclasts in resorbed roots during experimental tooth movement (jiggling) in vivo. Methods: Male Wistar rats were divided into control, heavy force (HF), optimal force (OF), and jiggling force (JF) groups. The expression levels of cathepsin K, matrix metalloproteinase (MMP)-9 protein, interleukin (IL)-6, cytokine-induced neutrophil chemoattractant 1 (CINC-1; an IL-8-related protein in rodents), receptor activator of nuclear factor ${\kappa}B$ ligand (RANKL), and osteoprotegerin protein in the dental root were determined using immunohistochemistry. Results: On day 21, a greater number of root resorption lacunae, which contained multinucleated odontoclasts, were observed in the palatal roots of rats in the JF group than in rats from other groups. Furthermore, there was a significant increase in the numbers of cathepsin K-positive and MMP-9-positive odontoclasts in the JF group on day 21. Immunoreactivities for IL-6, CINC-1, and RANKL were stronger in resorbed roots exposed to jiggling than in the other groups on day 21. Negative reactivity was observed in the controls. Conclusions: These results suggest that jiggling may induce ORR via inflammatory cytokine production during orthodontic tooth movement, and that jiggling may be a risk factor for ORR.
A traumatically intruded tooth is one that is forcefully and abruptly dispaced from its position into the surrounding alveolar bone. Although intrusion of permanent teeth is infrequent, the sequelae compromise the longevity of the tooth and often include pulp necrosis, internal and external root resorption, rupture of periodontal ligament and loss of marginal bone. The purpose of this study was to examine three common management techniques for traumatic intrusion, observation for re-eruption, surgical repositioning & fixation and orthodontic extrusion. In the recent, the accepted treatment was to allow the permanent teeth to reerupt spontaneously for 6-8 weeks. If this did not occur, orthodontic traction was applied. The pulpal status of the teeth was monitored and either calcium hydroxide therapy or conventional endodontics was instituted following pulpal necrosis depending on the maturity of the root end. Pulpectomy and a calcium hydroxide filling were also the treatment of choice if there was evidence of internal or external root resorption. This will reduce the chance of root resorption and provide a period of monitoring prior to a definitive root canal filling.
The purpose of this study was to investigate the root resorption pattern in incisors in dog under intrusive orthodontic loadings of various magnitude and duration. Intrusive forces were generated by closed coil springs. Force magnitudes were 15-30gm, 50-60gm and 80-110gm. Durations were continuous and intermittent. Intermittent duration was applied at intervals of 12 hours. The readjustment of the force was done every 4 days. The forces were maintained for 30 days. All specimens were decalfied, embedded in paraffin and stained with hematoxylin-eosin stain. Observations were made with light microscope. The following results were obtained; 1. The continuity of root surface was ceased in all, except intermittent forced teeth with 25gm. 2. The root resorptions, cementoclasts and cemental lacunae, were increased around periapical regions and the destructive scope of bone was deeply extended as the exerted orthodontic forces increased. 3. It was inspected that, under the same forces, root resorption and bone destruction were more deeply appeared in the continuously forced teeth than the intermittently forced ones. 4. All of the alveolar bone, showed direct and undermining bone resorptions.

A material of 48 patients with 60 avulsed and replanted permanent teeth were followed retrospectively in the period of 1996. 1 to 1998. 12 (mean observation period=1year 7months). The age of the patients at the time of replantation ranged from 9 to 63 years (mean=24 years). Clinical records of patients were reviewed to obtain valid data concerning the extent of injury and treatment provided. Pulpal and periodontal healing states were examined with periapical x-rays and clinical examination procedures (i.e. percussion test and mobility test) at their recall visit. Root ankylosis was found in fifty-two teeth(87%) and root resorption in twenty-four(40%). Only two of the replanted teeth(3%) showed partial regeneration of the periodontal ligament. Six teeth(10%) resulted in tooth loss, but the remaining fifty-four were clinically well functioning. Most of teeth have mild marginal bone loss accompanied by gingival retraction without pathological periodontal pockets. The incidence of root resorption was much higher in younger age group. However, it was not affected by the interval between avulsion and replantation, the condition of supporting tissues, the degree of root formation and the type of splinting, indicating that multiple factors involved in determining the prognosis of replanted teeth. Based on these findings, avulsed teeth in unfavorable conditions (i.e. long extra-alveolar periods, etc.) should be preserved if possible.
