Robot-Assisted Thoracic Surgery in Non-small Cell Lung Cancer

  • Lobectomy is the standard treatment for early non-small cell lung cancer. Various surgical techniques for lobectomy have been developed, and minimally invasive thoracic surgery, such as video-assisted thoracic surgery or robot-assisted thoracic surgery, has been considered as an alternative to conventional open thoracotomy. The recently robotic lobectomy technique has developed since the first case series was published in 2002. Several studies have reported that robotic lobectomy has comparable oncologic and perioperative outcomes to those of video-assisted thoracic surgery lobectomy and open lobectomy. However, robotic lobectomy remains a challenge for surgeons because of the steep learning curve, reduced tactile sensation, difficulty in port placement, and challenges in cooperation between the surgeon and assistant. Many studies have reported on robotic lobectomy, but few have presented surgical techniques for robotic lobectomy. In this article, the surgical techniques and optimal performance of robotic lobectomy are described in detail for all 5 types of lobectomy for surgeons beginning with robotic lobectomy.

Robot-Assisted Cardiac Surgery Using the Da Vinci Surgical System: A Single Center Experience

  • Background: We report our initial experiences of robot-assisted cardiac surgery using the da Vinci Surgical System. Methods: Between February 2010 and March 2014, 50 consecutive patients underwent minimally invasive robot-assisted cardiac surgery. Results: Robot-assisted cardiac surgery was employed in two cases of minimally invasive direct coronary artery bypass, 17 cases of mitral valve repair, 10 cases of cardiac myxoma removal, 20 cases of atrial septal defect repair, and one isolated CryoMaze procedure. Average cardiopulmonary bypass time and average aorta cross-clamping time were $194.8{\pm}48.6$ minutes and $126.1{\pm}22.6$ minutes in mitral valve repair operations and $132.0{\pm}32.0$ minutes and $76.1{\pm}23.1$ minutes in myxoma removal operations, respectively. During atrial septal defect closure operations, the average cardiopulmonary bypass time was $128.3{\pm}43.1$ minutes. The median length of stay was between five and seven days. The only complication was that one patient needed reoperation to address bleeding. There were no hospital mortalities. Conclusion: Robot-assisted cardiac surgery is safe and effective for mitral valve repair, atrial septal defect closure, and cardiac myxoma removal surgery. Reducing operative time depends heavily on the experience of the entire robotic surgical team.

폐암에서 로봇을 이용한 폐절제술 (Robot-Assisted Pulmonary Resection For Lung Cancer)

  • MMinimally invasive surgery (MIS) for early stage lung cancer has been an important treatment modality. However, the ergonomic discomfort and counterintuitive instruments hindered the application of video-assisted thoracic surgery (VATS) to more advanced procedures. To improve the compliance with MIS, robotic surgery was adopted. This advance aimed to alleviate the shortcomings of VATS by maximizing the comfort of the surgeon while providing instruments that enabled technically demanding operations and three-dimensional views with increased freedom for intrathoracic movement owing to EndoWrist$^{(R)}$. In this session, we introduced the clinical applications and its results of robot-assisted thoracic surgery in the field of lung cancer surgery. In conclusion, robot-assisted pulmonary resection with lymph node dissection for lung cancer is safe as well as feasible, and it results in a satisfying postoperative outcome. Robot-assisted surgery may provide a good alternative to conventional open or thoracoscopic surgery for lung cancer, provided that the cost effectiveness and long-term prognosis are confirmed.

Robot-Assisted Thoracoscopic Esophagectomy with Total Mediastinal Lymphadenectomy: A Guide to a Systematic Approach Using the Concept of Fascial Plane Dissection

  • Recent case series and meta-analyses have suggested that robot-assisted minimally invasive esophagectomy (RAMIE) could be a useful alternative to video-assisted thoracic surgery esophagectomy. The advantages of RAMIE are a 3-dimensional view, 7 degrees of freedom, and tremor filtering, which enable more meticulous lymph node dissection with a lower incidence of complications. However, in radical esophagectomy, understanding the concepts of the fascia and compartment is crucial for successful and reliable dissection. The first RAMIE in Korea was performed by our team in July 2006, and since then, we have developed related techniques to achieve better short- and long-term outcomes. The key step in RAMIE for esophageal squamous cell carcinoma is dissection of the upper mediastinum due to the difficulty of lymph node dissection and the high incidence of nodal metastasis in the area. Herein, we describe the technique of fascial plane dissection with esophageal suspension during RAMIE.

Robot-Assisted Repair of Atrial Septal Defect: A Comparison of Beating and Non-Beating Heart Surgery

  • Background: Robot-assisted repair of atrial septal defect (ASD) can be performed under either beating-heart or non-beating-heart conditions. However, the risk of cerebral air embolism (i.e., stroke) is a concern in the beating-heart approach. This study aimed to compare the outcomes of beating- and non-beating-heart approaches in robot-assisted ASD repair. Methods: From 2010 to 2019, a total of 45 patients (mean age, 43.4±14.6 years; range, 19-79 years) underwent ASD repair using the da Vinci robotic surgical system. Twenty-seven of these cases were performed on a beating heart (beating-heart group, n=27) and the other cases were performed on an arrested or fibrillating heart (non-beating-heart group, n=18). Cardiopulmonary bypass (CPB) was achieved via cannulation of the femoral vessels and the right internal jugular vein in all patients. Results: Complete ASD closure was verified using intraoperative transesophageal echocardiography in all patients. Conversion to open surgery was not performed in any cases, and there were no major complications. All patients recovered from anesthesia without any immediate postoperative neurologic symptoms. In a subgroup analysis of isolated ASD patch repair (beating-heart group: n=22 vs. non-beating-heart group: n=5), the operation time and CPB time were shorter in the beating-heart group (234±38 vs. 253±29 minutes, p=0.133 and 113±28 vs. 143±29 minutes, p=0.034, respectively). Conclusion: Robot-assisted ASD repair can be safely performed with the beating-heart approach. No additional risk in terms of cerebral embolism was found in the beating-heart group.

  • 흉강경을 이용한 폐절제술이 1990년대 초반 처음으로 소개된 이후로 조기 폐암 환자의 수술에 있어 안정성과 적합성이 발표되면서 그 시행 횟수가 서서히 증가되고 있다. 그러나, 흉강경을 이용한 수술은 2차원 영상과 불안정한 카메라의 움직임, 기구의 움직임이 제한되는 등의 여러 가지 단점들이 있다. 이러한 단점을 극복하고 최근에 개발된 다빈치 로봇 시스템은 고해상도의 3차원 입체영상과 사람의 팔목관절과 유사한 움직임을 구사하는 기구들로 인하여 더욱 안정되고 편리하게 수술을 시행할 수 있게 하였다. 본원에서는 다빈치 로봇 시스템을 이용하여 3명의 조기 폐암환자에서 폐엽 절제술을 시행하였기에 보고하는 바이다.

  • The postoperative respiratory complications such as pneumonia and ARDS, are poor prognostic factors after esophagectomy in patients with esophageal cancer. To avoid these complications, there have been attempts to use minimally invasive approach. Recently introduced daVinciTM S surgical system is used in esophagectomy because of its advantages of minimal invasiveness, clear 3-dimensional imaging and precise handling of robotic arms. We report a 83-year-old esophageal cancer patient who underwent daVinciTMS robot-assisted esophagectomy, laparoscopic stomach mobilization followed by cervical esophagogastrostomy.

Robot-Assisted Thoracic Surgery Thymectomy

  • Robotic thymectomy has been adopted recently and has been shown to be safe and feasible in treating thymic tumors and myasthenia gravis. The surgical indications of robotic technology are expanding, with advantages including an excellent surgical view and sophisticated manipulation. Herein, we describe technical aspects, considerations, and outcomes of robotic thymectomy.

  • 중증 근무력증의 치료법으로서 흉선절제술은 효과가 입증된 표준 치료법 중 하나로 받아들여지고 있다. 흉선절제 수술법은 종래에 시행되던 흉골절개나 혹은 다른 개흉 수술법에 반해 최근 최소 침습 수술법이 시도되고 있다. 증례는 28세 남자로, 양손의 위약감 및 피로감을 호소하여 검사 결과 흉선 과형성을 동반한 중증 근무력증으로 진단하였고, da Vinci 로봇을 이용하여 흉선절제술을 시행하였다. 수술은 좌우 흉강을 통해 양측으로 접근하여, 우측에서 흉선의 2/3를 절제하고 좌측에서 나머지를 절제하였으며, 소요시간은 각각 1시간, 30분이었다. 환자는 술후 8일째 합병증 없이 퇴원하였다. 최소 침습 흉선절제술로서 da Vinci 로봇을 이용한 수술법이 국외에서 시도되고 있지만 국내에는 아직 보고된 바가 없어, 본원에서 수행한 성공적인 경험을 보고하는 바이다.

식도암 환자에서 da Vinci 로봇을 이용한 식도암 수술 (흉골하 통로를 통한 식도-위 문합술) - 1예 보고 - (da Vinci Robot-Assisted Esophagectomy for Esophageal Cancer: A Case of Esophago-gastrostomy through the Retrosternal Route - A case report -)

  • 다빈치 로봇수술은 현재 다양한 외과영역에서 시행되고 있으나 폐, 식도 외과영역에서는 아직까지 널리 이용되지 않고 있다. 본원에서는 74세 남자환자가 딸꾹질과 연하곤란을 주소로 내원하여 시행한 검사에서 식도암으로 진단되어 da Vinci 로봇을 이용하여 식도절제술을 시행한 후, 복강경으로 위장튜브를 만들어 흉골하 통로를 통해 경부에서 위장-식도 단단 문합을 시행하였다. 환자는 수술 후 특별한 합병증 없이 완쾌 퇴원하였다. 향후 다빈치로봇을 이용한 최소 침습수술이 식도암을 비롯한 폐, 식도외과 영역에의 활용에 있어서 좋은 결과가 기대되므로 보고하는 바이다.
