Risk Factors for Lung Cancer Mortality in a Referral Center

  • Background: Lung cancer is one of the most common causes of death that is rising in many countries including Iran. This study aimed to determine the impact of factors on survival of lung cancer patients at a referral center of lung diseases in Tehran, Iran. Materials and Methods: A retrospective study was conducted on adult lung cancer cases admitted to a referral center for lung diseases from 2011 to 2015. Multivariate analysis was performed to determine the risk factors for all-cause mortality. Results: Of a total 933 patients with lung cancer, 53.4% died, 49.3% of them at the hospital. Overall median follow-up time was 7 months. The most common histological type of cancer was adenocarcinoma with a 13 month median survival time. Age ${\geq}55$ and smoking remained significant for all-cause mortality on Cox analysis, whereas gender was not. Conclusions: The survival of lung cancer patients is poor and the patients with history of smoking and age${\geq}55$ are at increased risk of death. Having a large hospital-based registry provides a good measurement of prognostic statistics for lung cancer. Further investigations are necessary to establish reasons for mortality.

  • The single-item question of self-assessed health status has consistently been reported to be associated with mortality in some developed countries, even after controlling for a wide range of health measurements and known risk factors for. mortality. It was intended in this study to find out whether or not such a relationship would also be valid in Korea. This study examined the effect of point of reference year on. the, predictive validity of self-assessed health for mortality in 6-year follow-up period. we need to test the validity of the self-assessed health, as an indicator for assessing health status using Cox's proportional hazard model. For the analysis, we used the data from the 2nd (1999) to the 7th survey of "Korean Labor and Income Panel Study," and assessed relative risk of death based on subjective health state by tracing 11,366 people who replied to the question of self-assessed health state in the 2nd year. According to the result, those who reported poor self assessed health state in the 2nd year showed a relatively high death rate, and their relative risk of death was significantly higher. Such a relationship was accentuated if the predictive value of the 2nd survey result would be replaced by the average of the cumulative data on the past six years. Thus, it can be concluded that self-assessed health state is valid as an index for assessing Korean people's health status.

Impact of Individual and Combined Health Behaviors on All Causes of Premature Mortality Among Middle Aged Men in Korea: The Seoul Male Cohort Study

  • Objectives: The aim of this study was to evaluate and quantify the risk of both individual and combined health behaviors on premature mortality in middle aged men in Korea. Methods: In total, 14 533 male subjects 40 to 59 years of age were recruited. At enrollment, subjects completed a baseline questionnaire, which included information about socio-demographic factors, past medical history, and life style. During the follow-up period from 1993 to 2008, we identified 990 all-cause premature deaths using national death certificates. A Cox proportional hazard regression model was used to estimate the hazard ratio (HR) of each health risk behavior, which included smoking, drinking, physical inactivity, and lack of sleep hours. Using the Cox model, each health behavior was assigned a risk score proportional to its regression coefficient value. Health risk scores were calculated for each patient and the HR of all-cause premature mortality was calculated according to risk score. Results: Current smoking and drinking, high body mass index, less sleep hours, and less education were significantly associated with all-cause premature mortality, while regular exercise was associated with a reduced risk. When combined by health risk score, there was a strong trend for increased mortality risk with increased score (p-trend < 0.01). When compared with the 1-9 score group, HRs of the 10-19 and 20-28 score groups were 2.58 (95% confidence intervals [CIs], 2.19 to 3.03) and 7.09 (95% CIs, 5.21 to 9.66), respectively. Conclusions: Modifiable risk factors, such as smoking, drinking, and regular exercise, have considerable impact on premature mortality and should be assessed in combination.

Meta-analysis on risk stratification of malignant ventricular tachyarrhythmic events in arrhythmogenic right ventricular cardiomyopathy

  • Background: Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a cardiomyopathy characterized by predominant right ventricular fibro-fatty replacement, right ventricular dysfunction and ventricular arrhythmias. It is a rare but important cause of sudden cardiac death in children and young adults. A meta-analysis on risk stratification of major ventricular tachyarrhythmic events indicating the need for implantable cardioverter defibrillator therapy in ARVC was performed. Methods: The pubmed database was searched from its inception to May 2015. Of the 433 citations identified, 12 were included in this meta-analysis. Data regarding major ventricular tachyarrhythmic events were retrieved in 817 subjects from the studies. For the variables, a combined odds ratio (OR) was calculated using a fixed-effects meta-analysis. Results: Extensive right ventricular dysfunction (OR, 2.44), ventricular late potential (OR, 1.66), inducible ventricular tachyarrhythmia during electrophysiology study (OR, 3.67), non-sustained ventricular tachycardia (OR, 3.78), and history of fatal event/sustained VT (OR, 5.66) identified as significant risk factors (p<0.0001). Conclusion: This meta-analysis shows that extensive right ventricular dysfunction, ventricular late potential, inducible ventricular tachyarrhythmia during electrophysiological study, non-sustained ventricular tachycardia, and history of sustained ventricular tachycardia/fibrillation are consistently reported risk factors of major ventricular tachyarrhythmic events indicating implantable cardioverter defibrillator therapy in patients with ARVC.

Risk Factors for Displacement of the Abomasum in Dairy Cows and its Relationship with Postpartum Disorders, Milk Yield, and Reproductive Performance

  • We determined the risk factors for displacement of the abomasum (DA), and the relationships between DA and postpartum disorders, milk yield, and reproductive performance in dairy cows. Initially, we identified the risk factors for DA using data regarding cow health and calving season from 2,208 lactations. Then, we compared the incidence of postpartum disorders, culling, death, and reproductive performance between cows with DA and their control herdmates (each n = 57). In addition, serum metabolites concentrations and milk yield were compared between cows with DA and controls (each n = 33). Ketosis (odds ratio [OR] = 9.27, p < 0.0001) and twin calves (p = 0.06) increased the risk of DA. Cows with a parity of three had a higher risk (OR = 5.23, p < 0.01) of DA than primiparous cows. Serum total cholesterol concentration was lower but non-esterified fatty acid, ${\beta}-hydroxybutyrate$, and alanine aminotransferase concentrations were higher after calving in cows with DA than in controls (p < 0.05). The removal rate from the herd by 2 months after calving was higher (p < 0.05) but milk yield 1 and 2 months after calving (p < 0.01) and the rate of first insemination by 150 days postpartum were lower (hazard ratio = 0.49, p < 0.05) in cows with DA than controls. In conclusion, higher parity, twin calves, and ketosis are risk factors for DA in dairy cows, which is associated with a higher removal rate from the herd, lower milk yield, a longer calving to first insemination interval, and unfavorable levels of metabolites related to energy and liver function.

Under-use of Radiotherapy in Stage III Bronchioaveolar Lung Cancer and Socio-economic Disparities in Cause Specific Survival: a Population Study

  • Background: This study used the receiver operating characteristic curve (ROC) to analyze Surveillance, Epidemiology and End Results (SEER) bronchioaveolar carcinoma data to identify predictive models and potential disparity in outcomes. Materials and Methods: Socio-economic, staging and treatment factors were assessed. For the risk modeling, each factor was fitted by a Generalized Linear Model to predict cause specific survival. The area under the ROC was computed. Similar strata were combined to construct the most parsimonious models. A random sampling algorithm was used to estimate modeling errors. Risk of cause specific death was computed for the predictors for comparison. Results: There were 7,309 patients included in this study. The mean follow up time (S.D.) was 24.2 (20) months. Female patients outnumbered male ones 3:2. The mean (S.D.) age was 70.1 (10.6) years. Stage was the most predictive factor of outcome (ROC area of 0.76). After optimization, several strata were fused, with a comparable ROC area of 0.75. There was a 4% additional risk of death associated with lower county family income, African American race, rural residency and lower than 25% county college graduate. Radiotherapy had not been used in 2/3 of patients with stage III disease. Conclusions: There are socio-economic disparities in cause specific survival. Under-use of radiotherapy may have contributed to poor outcome. Improving education, access and rates of radiotherapy use may improve outcome.

위험 지각이 소비자 태도 및 식품 구매 행동에 미치는 영향 - 멜라민 함유 식품군을 중심으로- (The Influence of Risk Perception upon Consumer"s Attitude and Purchase Behavior - Focusing on Melamine-Contained Food-)

  • The shock of melamine-contained food has been known with the incident of hospitalization or death of infants at the age of 11-month old in China. The purpose of this study was to examine consumers' risk perception, attitude and purchase behavior of melamine-contained food which recently came to the fore as a social issue, focusing on the relationship between consumers' attitude and purchase behavior based on physical risk, social risk, psychological risk, financial risk, consumers' attitude and purchase behavior. Study findings are as follows. First, the four factors of risk perception of melamine-contained food were significantly different by consumers. Consumers regarded social risk as most significant and it was followed by financial risk and psychological risk. And they regarded physical risk as most insignificant. Second, as for the difference of risk perception by demographic features, such factors as gender, age, marital status and schooling had no statistically significant influence. Third, the potential risk of melamine-contained food had a negative influence upon consumers' attitude and purchase behavior. In addition, consumers' attitude and purchase behavior were decided by perceived risks. This finding implies that consumers' attitude can accompany with negative or positive behavior and consumers' behavior can be related to social, financial and physical risks. Because the risk perception of melamine-contained food can cause consumers' distrust about all kinds of food in general, domestic manufacturers of milk product-contained confectionery and food can be harmed. In order to overcome this problem and make consumers purchase all the milk products and milk-contained food without fear, governmental agencies must reestablish systems with which the safety of imported raw materials and its processing can be insured and manufactures must improve the quality of products in a diverse and discriminative manner. This study seems to be meaningful in that it examined consumers' risk perception of melamine-contained food, a current social issue, and then looked into the influence of risk perception upon consumers" attitude and behavior, thus presenting the strategy of reestablishing the system of relationship between consumers and business entities in a desirable manner.

한국인 자궁경부암의 복합위험도 추정 (Estimation of Joint Risks for Developing Uterine Cervix Cancer in Korea)

  • Objective : This study was aiming at estimating the joint effects of various risk factors associated with uterine cervix cancer in Korea. Methods : Data obtained from a case-control study were analyzed with a multiplicative model. Results : After adjustment for age and husband's educational attainments, the family history of cervical cancer (OR=2.1, 95% CI=1.2-3.9), unstable marital status due to separation, by death or divorce, etc. (OR=2.8, 95% CI=1.7-4.6), and a large number of deliveries ($\geq$3 vs. nulliparous OR=6.5, 55% CI=1.4-29.0) increased the risk of uterine cervix cancer, Conversely, first sexual intercourse at an older age ($\geq$25 years vs. <19 years OR=0.4, 95% CI=0.2-0.6) and husband's circumcision (OR=0.7, 95% CI=0.5-1.0) decreased the risk. In the multiplicative model, the highest joint risk (OR=39.2, 95% CI 5.9-258.9) was observed in women with a family history of uterine cervical cancer, an unstable marital status, where the ex-husband was not circumcised, with 3 or more delivery experiences, and having her first sexual intercourse when younger than 19 years of age. However, women without a family history of uterine cervix cancer, married to a circumcised husband, having had her first sexual intercourse at 25 years or older, and nulliparous, showed the lowest joint effect (OR=0.3, 95% CI=0.1-0.5). Conclusion : As carcinogenesis is a complex action involving various factors, we consider a joint effects approach to be appropriate in an epidemiological study on risk factors for uterine cervix neoplasms cervix neoplasm.

노인 낙상 - 넘어짐 그리고 인체손상의 과학, 넘어지면 다 죽는가?: 문헌 고찰 (Science of Falling and Injury in Older Adults - Do All Falls Lead to Death?: Literature Review)

  • Understanding sciences behind fall-related hip fractures in older adults is important to develop effective interventions for prevention. The aim of this review is to provide biomechanical understanding and prevention strategies of falls and related hip fractures in older adults, in order to guide future research directions from biomechanical perspectives. While most hip fractures are due to a fall, a few of falls are injurious causing hip fractures, and most falls are non-injurious. Fall mechanics are important in determining injurious versus non-injurious falls. Many different biomechanical factors contribute to the risk of hip fracture, and effects of each individual factors are known well. However, combining effects, and correlation and causation among the factors are poorly understood. While fall prevention interventions include exercise, vision correction, vitamin D intake and environment modification, injury prevention strategies include use of hip protectors, compliant flooring and safe landing strategies, vitamin D intake and exercise. While fall risk assessments have well been established, limited efforts have been made for injury risk assessments. Better understanding is necessary on the correlation and causation among factors affecting the risk of falls and related hip fractures in older adults. Development of the hip fracture risk assessment technique is required to establish more efficient intervention models for fall-related hip fractures in older adults.

건설 프로젝트의 재해영향요인 기반 위험성 평가방법 (A Risk Assessment Method using Disaster Influence Factors on Construction Project)

  • Current risk assessment methods typically determine accident risks embedded in construction projects by combining severity and frequency; however, they do not reflect the characteristics of construction projects. To solve the problem, this study aims to develop a novel risk assessment method that combines severity, frequency, and disaster influence factors (i.e., weather conditions and worker's characteristics) for assessing risks of activities occurring on a construction site actually. In this study, a severity was estimated by death against victims, and a frequency was estimated by the victim rate. The frequency was then converted to probability taking disaster influence factors into account. Thus, instead of considering severity and frequency for assessing the original risks (RO), the proposed method uses severity and probability to yield adjusted risks (RA) for each activity. A case study was conducted to determine if the proposed method works as intended in a real setting. The results show that RA is more sensitive to disaster influence factors than RO and, therefore, is able to assess the actual risks reflecting the working environment and conditions of a construction site. This study contributes to risk management of construction projects by offering a risk assessment method that measures a possibility of potential disasters from the probabilistic perspective. This method would help project managers assess accident risks in a more systematic and quantitative manner.
