중증외상환자에서 알코올 섭취에 따른 중증도와 예후에 대한 연구 (The Study of the Severity and Prognosis in Severe Traumatic Patients according to Alcohol Ingestion)

  • Purpose: Alcohol ingestion is a significant risk factor for injuries. However, the influence of high blood alcohol concentration about the severe traumatic injury is controversial. The aim of study was to analyze the injury severity, prognosis in severe traumatic patients according to alcohol ingestion. Methods: This study was performed retrospectively with severe traumatic patients (Injury Severity $Score{\geq}16$) who visited the emergency department at Pusan National University Hospital from January 2013 to December 2013. Results: In total 98 severe traumatic patients, blood alcohol concentration (BAC) positive group (BAC>30 mg/dl) is 42 (42.90%) patients and BAC negative group ($BAC{\leq}30mg/dl$) is 56 (57.10% )patients. Head and neck injury is significantly high in BAC positive group (35 patients, 83.3%) compared to BAC negative group (33 patients, 58.9%). Comparison of injury severity, outcome and mortality is not significantly different between two groups. Conclusion: In severe traumatic patients, head and neck injury occurred high in BAC positive group. Alcohol ingestion did not influence injury severity, outcome in severe traumatic patients. However, effort to decreasing injury related to alcohol ingestion and prospective multi-center study is needed.

상선 운항 사고의 양적 위기평가기법 개발 (Development of Quantitative Risk Assessment Methodology for the Maritime Transportation Accident of Merchant Ship)

  • 본 논문에서는 상선의 운항 사고에 관한 양적 위기평가에 관한 실험적인 접근방법들을 기술했다. 이 연구의 목적은 국제해사기구의 공식 안전성 평가(FSA)를 기반으로 운항 사고에 크게 기여하는 요소들을 분석하고, 양적 위기평가기법에 기반을 둔 운항 사고의 확률적인 위기수준을 평가한 후, 선박 안전을 저해할 수 있는 운항 사고 위기를 예측하는 것이다. 확률지수(PI)와 심각성지수(SI) 구성된 위기지수(RI)에 대한 운항 사고의 확률적인 위기수준은 베이지안 이론을 적용한 베이지안 네트워크를 기반으로 본 연구에서 제안한 운항사고 위기 모델을 이용해서 예측했다. 그리고 355건의 핵심 손상 사고기록으로 구성된 시나리오 그룹을 이용하여 제안한 모델의 적용 가능성을 평가하였다. 평가결과, 예측한 PI의 정답률 $r_{Acc}$은 82.8%로 나타났고, $S_p{\gg}1.0$$S_p{\ll}1.0$에 포함되는 PI 변수들의 민감도 초과비율은 10% 이내로 나타났으며, 예측한 SI의 평균 오차 $\bar{d_{SI}}$는 0.0195로 나타났고, 예측한 RI의 정답률은 91.8%로 나타났다. 이러한 결과는 제안한 모델과 방법이 실제 해상운송 현장에 적용 가능함을 나타낸다.

A Policy Intervention Study to Identify High-Risk Groups to Prevent Industrial Accidents in Republic of Korea

  • Background: The objective of this study is to identify high-risk groups for industrial accidents by setting up 2003 as the base year and conducting an in-depth analysis of the trends of major industrial accident indexes the index of industrial accident rate, the index of occupational injury rate, the index of occupational illness and disease rate per 10,000 people, and the index of occupational injury fatality rate per 10,000 people for the past 10 years. Methods: This study selected industrial accident victims, who died or received more than 4 days of medical care benefits, due to occupational accidents and diseases occurring at workplaces, subject to the Industrial Accident Compensation Insurance Act, as the study population. Results: According to the trends of four major indexes by workplace characteristics, the whole industry has shown a decreasing tendency in all four major indexes since the base year (2003); as of 2012, the index of industrial accident rate was 67, while the index of occupational injury fatality rate per 10,000 people was 59. Conclusion: The manufacturing industry, age over 50 years and workplaces with more than 50 employees showed a high severity level of occupational accidents. Male workers showed a higher severity level of occupational accidents than female workers. The employment period of < 3 years and newly hired workers with a relatively shorter working period are likely to have more occupational accidents than others. Overall, an industrial accident prevention policy must be established by concentrating all available resources and capacities of these high-risk groups.

Clinical Phenotype of a First Unprovoked Acute Pulmonary Embolism Associated with Antiphospholipid Antibody Syndrome

  • Background: Antiphospholipid antibody syndrome (APS), an important cause of acquired thrombophilia, is diagnosed when vascular thrombosis or pregnancy morbidity occurs with persistently positive antiphospholipid antibodies (aPL). APS is a risk factor for unprovoked recurrence of pulmonary embolism (PE). Performing laboratory testing for aPL after a first unprovoked acute PE is controversial. We investigated if a specific phenotype existed in patients with unprovoked with acute PE, suggesting the need to evaluate them for APS. Methods: We retrospectively reviewed patients with PE and APS (n=24) and those with unprovoked PE with aPL negative (n=44), evaluated 2006-2016 at the Asan Medical Center. We compared patient demographics, clinical manifestations, laboratory findings, and radiological findings between the groups. Results: On multivariate logistic regression analysis, two models of independent risk factors for APS-PE were suggested. Model I included hemoptysis (odds ratio [OR], 12.897; 95% confidence interval [CI], 1.025-162.343), low PE severity index (OR, 0.948; 95% CI, 0.917-0.979), and activated partial thromboplastin time (aPTT; OR, 1.166; 95% CI, 1.040-1.307). Model II included age (OR, 0.930; 95% CI, 0.893-0.969) and aPTT (OR, 1.104; 95% CI, 1.000-1.217). Conclusion: We conclude that patients with first unprovoked PE with hemoptysis and are age <40; have a low pulmonary embolism severity index, especially in risk class I-II; and/or prolonged aPTT (above 75th percentile of the reference interval), should be suspected of having APS, and undergo laboratory testing for aPL.

인플루엔자 고 위험군의 예방접종 관련요인 연구 (Factors Associated with Influenza Vaccination Behavior Among High-Risk Adults)

  • Objective: This prospective survey assessed factors associated with influenza vaccination behaviors among high-risk adults. Methods: 106 patients aged 65 or high risk for complications of influenza were interviewed to identify influencing factors to vaccination. Six potential consequences of Influenza infection and nine factors of vaccination were analysed between compliance and non-compliance groups. Results: Among the 106 patients, the vaccination rate was 62.3%. The rate of the group under the sixties was 37.0010, but the rate over the sixties was 88.5%. Factors in dependently associated with both influenza vaccination behaviors included older age, chronic disease, and especially, related to factors in older age were having positive attitudes toward immunization, perceived severity of infection and willingness to comply with the provider's recommendation. Conclusions: Emphasis on provider recommendations and the knowledge and attitudes of influenza infection and vaccination may enhance influenza vaccination rates in the organized vaccination programs.

Location Analysis for Emergency Medical Service Vehicle in Sub District Area

  • This research aims to formulate a mathematical model and develop an algorithm for solving a location problem in emergency medical service vehicle parking. To find an optimal parking location which has the least risk score or risk priority number calculated from severity, occurrence, detection, and distance from parking location for emergency patients, data were collected from Pratoom sub-district Disaster Prevention and Mitigation Center from October 2010 to April 2011. The criteria of risk evaluation were modified from Automotive Industry Action Group's criteria. An adaptive simulated annealing algorithm with multiple cooling schedules called multi-agent simulated quenching (MASQ) is proposed for solving the problem in two schemes of algorithms including dual agent and triple agent quenching. The result showed that the solution obtained from both scheme of MASQ was better than the traditional solution. The best locations obtained from MASQ-dual agent quenching scheme was nodes #5 and #133. The risk score was reduced 61% from 6,022 to 2,371 points.

충격흡수 세이프티롤러 가드레일의 탑승자 위험도 해석 (Occupant Risk Analysis of Safety Roller Guardrail with Energy Absorption Capability)

  • PURPOSES: This paper presents the results of computer simulations of roadside safety barrier, called by safety roller guardrail, consisting of rotational roller, rotation control plate, post and subsidiary members. The rotation roller and rotation control plate are made by EVA(ethylene vinyl acetate), and PE(polyester), respectively. METHODS: The occupant risk analysis has been carried out under vehicle crash condition for high containment level of SB-4 for the purpose of local road. Simulations are performed with the finite element code LS/DYNA-3D. RESULTS: The numerical results obtained by LS/DYNA-3D software from the viewpoints of vehicle stability, vehicle trajectory, occupant risk, etc. CONCLUSIONS: It is noted that not only impact severity is drastically reduced but also vehicle trajectory is improved due to the characteristics of energy absorption and rotation pattern of EVA rollers connected by control plates.

An integrated risk-informed safety classification for unique research reactors

  • Safety classification of systems, structures, and components (SSC) is an essential activity for nuclear reactor design and operation. The current regulatory trend is to require risk-informed safety classification that considers first, the severity, but also the frequency of SSC failures. While safety classification for nuclear power plants is covered in many regulatory and scientific publications, research reactors received less attention. Research reactors are typically of lower power but, at the same time, are less standardized i.e., have more variability in the design, operational modes, and operating conditions. This makes them more challenging when considering safety classification. This work presents the Integrated Risk-Informed Safety Classification (IRISC) procedure which is a novel extension of the IAEA recommended process with dedicated probabilistic treatment of research reactor designs. The article provides the details of probabilistic analysis performed within safety classification process to a degree that is often missing in most literature on the topic. The article presents insight from the implementation of the procedure in the safety classification for the MARIA Research Reactor operated by the National Center for Nuclear Research in Poland.

회복탄력성을 통한 주관적 불면의 심각도와 자살사고와의 관계 (The Relationship between Insomnia and Suicidal Idea Through Resilience)

  • 연구목적 불면은 자살사고의 위험요인으로 알려져 있으나 아직 어떻게 자살의 위험에 영향을 미치는지 그 기전은 명확하지 않다. 이에 불면 증상이 회복탄력성과 자살사고와 상관이 있는 지를 살펴보고, 회복탄력성이 불면증상과 자살사고의 매개요소가 될 수 있을지 알아보고자 한다. 방 법 432명의 대학생을 대상으로 자기보고식 설문지를 통하여 일반적 특성 및 관련 검사를 수행하였다. 불면증 심각성 척도, 한국판 코너-데이비드슨 회복탄력성척도, 벡 자살사고 척도 및 벡 무망감 척도를 활용하여 불면증상, 자살사고, 회복탄력성, 무망감에 대해 조사하였으며, 각 요소간의 상관분석 및 경로분석을 수행하였다. 결 과 불면증상이 심할수록 회복탄력성이 낮았고, 자살사고가 증가되는 것을 알 수 있었다. 이는 무망감, 연령, 성별, 동거가족, 가정 내 월수입을 통제하여도 유의하였다. 더불어 입면의 어려움과 유지의 어려움을 겪는 불면증상이 자살사고와 연관됨을 확인하였다. 경로분석을 통하여 불면증상이 자살사고에 직접적, 간접적으로 영향을 미침을 확인할 수 있었으며, 회복탄력성도 유의하게 자살사고에 영향을 주는 것으로 평가되었다. 이에 회복탄력성이 불면과 자살사고와의 관계에 유의미한 중재자가 됨을 확인하였다. 결 론 수면상태와 회복탄력성에 대한 평가 및 개입이 자살사고를 낮추어 자살을 예방하는데 기여할 수 있을 것으로 기대한다.

타이어 제조회사에서의 참여적 근골격계질환 예방관리프로그램 적용효과 (The Effectiveness of Participatory Ergonomics Programs for Prevention of Musculoskeletal Disorders in the Tire Manufacturing Company)

  • In the field of ergonomics, it is evident that participatory action research methodology could contribute to reduce risk factors of work related musculoskeletal disorders(WMSDs). The objective of this study was to evaluate the effectiveness of a participatory ergonomics in reducing the incidence rate and severity of WMSDs in the tire manufacturing company. In this study, an action committee comprising 13 members was organized in the tire manufacturing company. The action group participated at every research process, education and training, assessment and intervention of risk factors, and early detection and management of WMSDs during the 3 years(2004-2006). The prevention programs contributed to an overall reduction in incidence rate by approximately 37.2%, and lost workdays and workers' compensation costs by approximately 55.1%. But, the incidence rate increased 29.0%, and compensation costs increased 59.0% in the first-year setting programs, and both the incidence rate and compensation costs decreased after the second-year managing programs. And, the reduction rates(63.2%) of insurance cases is higher than that(9.7%) of non-insurance cases. The results showed that participatory ergonomics programs may be effective in reducing the incidence and severity rate of WMSDs.
