의료기관의 위험도 분석 조사 - 지역공공의료원을 중심으로 (A Study on the Hazard and Risk Analysis of Hospital in Korea - Focused on Local Medical Centers)

  • The purpose of this study is to analyse the hazard risk by examining the magnitude and severity of each type of hazard in order to mitigate and prepare for disasters in medical facilities. Methods: The hazard risk analysis for hazard types was surveyed for team leaders of medical facilities. The questionnaire analyzed data from 27 facilities, which were returned from 41 Local Medical Centers. Results: When looking at the 'Risk' by category type of hazard, the influence of health safety and fire/energy safety comes first, followed by natural disaster, facility safety, and crime safety. On the other hand, as for 'Magnitude', facility safety and crime safety come first, followed by health safety, fire/energy safety, and natural disasters. Most of the top types of disaster judged to have high hazard in medical facilities are health types. The top five priorities of hazard in medical facilities, they are affected by the geographical and industrial conditions of the treatment area. In the case of cities, the hazard was found to be high in the order of infectious disease, patient surge, and wind and flood damage. On the other hand, in rural areas, livestock diseases and infectious diseases showed the highest hazard. In the case of forest areas, the hazard was high in the order of wildfire, fire accident, lightning, tide, earthquake, and landslide, whereas in coastal areas of industrial complexes, the hazard was high due to fire, landslide, water pollution, marine pollution, and chemical spill accident. Implications: Through the research, standards will be established for the design of hospitals with disaster preparedness, and will contribute to the preparation of preemptive measures in terms of maintenance.

대학생 문제도박의 성별 차이와 건강위험행동과의 관련성 (Gender Differences in Problem Gambling of University Students and their Relationship with Health Risk Behaviors)

  • Objectives: This study aims to identify differences in problem gambling among Korean university students by gender and to analyze the relationship between problem gambling and health risk behaviors. Methods: With a sample of 2,026 4-year university students, a questionnaire included CPGI (Canadian Problem Gambling Index) scale and health risk behavior items was administered. Descriptive statistics, t-test, and ANOVA were performed on the data. Results: The prevalence of gambling addiction of male students(14.6%) was two times higher than that of female students(6.6%). The severity of problem gambling was higher in: smokers, those with drug use experience, heavy drinkers, and those with frequently recurring suicidal thoughts, respectively. Conclusions: This study suggests that the problem gambling of university students is a complicated and comprehensive public health problem that is related with health risk behaviors such as alcohol drinking, smoking, drug use, and suicidal thoughts. Prevention strategies and policies are suggested based on the study results.

소방용품의 강제인증을 위한 위험도평가 및 품목분류에 관한 연구 (A Study on Risk Evaluation and Classification of Fire Equipments for Certification)

  • This study focuses on the classification of fire equipments for certification based on the risk evaluation. In general, known statistics on fire equipment-related accidents needs to be used for risk evaluation. When statistics is not available, however, expected frequency and severity of accident for individual equipment can be taken into account in evaluating the related risks. Based on the level of inherent risks, each equipment is then classified into three categories for certification. For equipments that risk evaluation is not possible, characteristics of those products such as reliability are considered for classification. Once classified, each equipment is assigned an appropriate certification module.

혈우병 A의 발병에 관여하는 유전적 요인 (Genetic Risk Factors of Hemophilia A)

  • Hemophilia A is a sex-linked recessive coagulation disorder associated with diverse mutations of the factor VIII gene and a variety of phenotypes. The type of mutation involved dictates the activity of factor VIII, and in turn the severity of bleeding episodes and development of alloantibodies against factor VIII (inhibitors). Missense mutations are the most common genetic risk factors for hemophilia A, especially mild to moderate cases, but carry the lowest risk for inhibitor development. On the other hand, intron 22 inversion is the most common mutation associated with severe hemophilia A and is associated with high risk of inhibitor formation. Large deletions and nonsense mutations are also associated with high risk of inhibitor development. Additional mutations associated with hemophilia A include frameshift and splice site mutations. It is therefore valuable to assess the mutational backgrounds of hemophilia A patients in order to to interpret their symptoms and manage their health problems.

  • Risk-based security impact evaluation may be affected by various factors according to numerous combinations of explosive devices, cutting devices, impact vehicles, and specific attack location to consider. Presently, in planning and design phases, designers are still often uncertain of their responsibility, lack of information and training of security. Therefore, designers are still failing to exploit the potential to reduce threats on site. In this study, the concept of security impact assessment is introduced in order to derive the performing design for safety in design phase. For this purpose, a framework for security impact assessment model using risk-based approach for bridge structures is suggested. The suggested model includes of information survey, classification of terror threats, and quantitative estimation of severity and occurrence.

제조물 위험성 평가에 의한 제조물 책임 및 제조물 안전대책 (An Effective Plan for Product Liability and Product Safety by the Hazard Analysis)

  • Product liability(PL) and product safety(PS) is one of the most costly and confusing problems facing manufacturing today. Accordingly, this paper presents an integral process for product safety and product liability resulting from a defective product and proposes countermeasures of PL and PS for the four types of risk based on severity and frequency. Finally, we have presented a framework and practical procedure to provide the manufacturer with useful way of analyzing the risk potential of consumer products.

A Survey on the Implementation of Musculoskeletal Disorders Prevention Program in Shipbuilding Industry

  • Objective: In 2010, the rate of injuries and illnesses in the shipbuilding industry was 1.20, about 1.74 times higher than the average of overall industries(0.69). Musculoskeletal disorders(MSDs) became a major problem since 2000 in shipbuilding industry. The purpose of this study was to provide efficient implementation management through evaluating the economic effectiveness of the Musculoskeletal Disorders Prevention Program(MDPP). Successfully implementing the MDPP in the workplace may decrease the risk of MSDs and therefore reduce MSDs related costs. Methods: Occupational accident index (frequency rate, severity rate, and frequency severity indicator), estimated direct costs, and total occupational accident occurrence rate were provided by analysis of MSDs, statistical analysis of MDPP decreed companies, and data analysis of implementation. Results: The study shows the number of MSDs for MDPP decreed companies decreased 76.2% from 2,319 in 2004 to 551 in 2008. Above all, the number of MSDs decreased 78.7% in Shipbuilding industries. Forty companies (85.1%) showed a decreasing tendency and 27.7% of those had an $R^2$ > 0.7. Similar results were found in severity rate and frequency severity indicator. From these results, it appears there are a number of advantages in implementing the MDPP. Conclusion: MDPP effectiveness was demonstrated by analysing implementation and occupational accident index. A more effective and efficient implementation would take into account business sector, size, policy, and organization of the company.

C-Reactive Protein a Promising Biomarker of COVID-19 Severity

  • The 2019 coronavirus outbreak poses a threat to scientific, societal, financial, and health resources. The complex pathogenesis of severe acute respiratory syndrome coronavirus centers on the unpredictable clinical progression of the disease, which may evolve abruptly and result in critical and life-threatening clinical complications. Effective clinical laboratory biomarkers that can classify patients according to risk are essential for ensuring timely treatment, and an analysis of recently published studies found cytokine storm and coagulation disorders were leading factors of severe COVID-19 complications. The following inflammatory, biochemical, and hematology biomarkers markers have been identified in COVID-19 patients; neutrophil to lymphocyte ratio, c-reactive protein, procalcitonin, urea, liver enzymes, lactate dehydrogenase, serum amyloid A, cytokines, d-dimer, fibrinogen, ferritin, troponin, creatinine kinase, and lymphocyte, leukocyte, and platelet counts. These factors are predictors of disease severity and some are involved in the pathogenesis of COVID-19. CRP is an acute-phase, non-specific serological biomarker of inflammation and infection and is related to disease severities and outcomes. In the present study, CRP levels were found to rise dramatically among COVID-19 patients, and our findings suggest CRP could be utilized clinically to predict COVID-19 prognosis and severity even before disease progression and the manifestation of clinical symptoms.

건설 보통인부의 안전재해 영향요인 및 재해강도 분석 (An Analysis on the Accident Influence Factor and Severity of Construction General Workers)

  • General workers who assist various technicians in different fields with their work across the whole construction sites without having a particular skill are at risk of the highest accident rate and their accident form becomes varied. Accordingly, this study was conducted to identify the relationship between form of safety accident and influence factor in general workers and analyze accident severity by influence factor. The followings are the results from this study. First, as a result of analyzing major form of accident and influence factors in general workers with network analysis methodology, nine forms of accident and seventeen influence factors were drawn. Second, it was found that in accident severity among general workers, collapsing, among various forms of accident, appeared the highest, followed by fall, electric shock, fire, hit by an object, bumped against, trip, scission getting cut chopped in order. Third, main points of special, concentrated, and permanent management were presented in order to reduce the safety accident in general workers effectively.

ICD-10을 이용한 ICISS의 타당도 평가 (Validation of the International Classification of Diseases 10th Edition Based Injury Severity Score(ICISS))

  • Objective : To compare the predictive power of International Classification of Diseases 10th Edition(ICD-10) based International Classification of Diseases based Injury Severity Score(ICISS) with Trauma and Injury Severity Score(TRISS) and International Classification of Diseases 9th Edition Clinical Modification(ICD-9CM) based ICISS in the injury severity measure. Methods : ICD-10 version of Survival Risk Ratios(SRRs) was derived from 47,750 trauma patients from 35 Emergency Centers for 1 year. The predictive power of TRISS, the ICD-9CM based ICISS and ICD-10 based ICISS were compared in a group of 367 severely injured patients admitted to two university hospitals. The predictive power was compared by using the measures of discrimination(disparity, sensitivity, specificity, misclassification rates, and ROC curve analysis) and calibration(Hosmer-Lemeshow goodness-of-fit statistics), all calculated by logistic regression procedure. Results : ICD-10 based ICISS showed a lower performance than TRISS and ICD-9CM based ICISS. When age and Revised Trauma Score(RTS) were incorporated into the survival probability model, however, ICD-10 based ICISS full model showed a similar predictive power compared with TRISS and ICD-9CM based ICISS full model. ICD-10 based ICISS had some disadvantages in predicting outcomes among patients with intracranial injuries. However, such weakness was largely compensated by incorporating age and RTS in the model. Conclusions : The ICISS methodology can be extended to ICD-10 horizon as a standard injury severity measure in the place of TRISS, especially when age and RTS were incorporated in the model. In patients with intracranial injuries, the predictive power of ICD-10 based ICISS was relatively low because of differences in the classifying system between ICD-10 and ICD-9CM.
