생활양식, 체중과 건강수준의 상관성분석 (Correlations Among Body Weight, Life-Style and Health Status in Korean Adults)

  • Correlations among body weight and sociodemographic factors, including life - style were tested as social determinants of health in a sample of 5,201 adults in Korea. The aim of this study was to determine the extent to which sociodemographic variables and life-style associated health behaviors explain body weight distribution. A second aim was to explain the relation of body weight and health status to stress the importance of body weight as an early risk indicator of health status. The canonical correlation between the weight distribution(underweight and overweight) and the independent variables was 0.29, 17% of the total variance was explained. Perceived health level represented the highest contribution(canonical coefficient 0.82) to body weight. Sociodemographic factors such as sex, economic status, and life-style factors such as smoking, exercise, regular meais and sleep showed comparatively high contributions to body weight. The relevance of body weight for health status including the rate of chronic disease and the rate of medical utilization was significant. Especially, underweight was clarified as being mere important than overweight to morbidity level and medical utilization. These findings suggest that perceived body weight is an important indicator of health status and is thus a valuable variable to be considered for nursing intervention and health education related to the promotion of health.

Prevalence and risk indicators of peri-implantitis in Korean patients with a history of periodontal disease: a cross-sectional study

  • Purpose: The aim of this study was to analyze the prevalence and risk indicators of peri-implantitis in Korean patients with history of periodontal disease. Methods: A total of 444 patients with 1,485 implants were selected from patients who had been treated at the Department of Periodontology, Chonbuk National University Dental Hospital between July 2014 and June 2015. A group with a history of peri-implantitis (HP) (370 patients with 1,189 implants) and a group with a current peri-implantitis (CP) (318 patients with 1,004 implants) were created based on the radiographic and clinical assessments of implants. The prevalence of peri-implantitis was calculated at both the patient and implant levels. The influence of risk variables on the occurrence of peri-implantitis was analyzed using generalized estimating equations analysis. Results: The prevalence of peri-implantitis in the HP and CP groups ranged from 6.7% to 19.7%. The cumulative peri-implantitis rate in the HP group estimated with the Kaplan-Meier method was higher than that in the CP group over the follow-up period. Among the patient-related risk variables, supportive periodontal therapy (SPT) was the only significant risk indicator for the occurrence of peri-implantitis in both groups. In the analysis of implant-related variables, implants supporting fixed dental prosthesis (FDP) and implants with subjective discomfort was associated with a higher prevalence of peri-implantitis than single implants and implants without subjective discomfort in the HP group. The presence of subjective discomfort was the only significant implant-related variable predictive of peri-implantitis in the CP group. Conclusions: Within the limitations of this study, the prevalence of peri-implantitis in Korean patients with a history of periodontal disease was similar to that reported in other population samples. Regular SPT was important for preventing peri-implantitis. Single implants were found to be less susceptible to peri-implantitis than those supporting FDP. Patients' subjective discomfort was found to be a strong risk indicator for peri-implantitis.

  • Recently, the concepts of community adaptation and ability to restore have been emphasized in order to evaluate disaster vulnerability and risk exposure level. In particular, the concept of disaster management that voluntarily takes measures prior to disaster situations has been introduced based on participatory disaster management that the community should take responsibility for one's own safety. This study diagnosed the community response capability on four model areas such as Goseong-gun and Gimhae-si, Gyeongsangnam-do and Gimcheon-si and Bonghwa-gun, Gyeongsangbuk-do and represented the Community Preparedness Indicator(CPI) for each region as a result.

Self-rated Health and Its Indicators: A Case of the 2001 Korean National Health and Nutrition Examination Survey

  • There were numerous evidences that subjective health evaluation was a powerful indicator for morbidity and mortality in many countries. Since self-rated health (SRH) was a reasonable health measure, identifying predictors for SRH would be beneficial for assessment of overall health, monitoring health status, and development of health promotion programs. Health risks, health behavior, socioeconomic characteristics and social capital were potential indicators for SRH. We examined association. between SRH and indicators such as health risk factors, subjective living condition, income, education level and dietary variety score. Total 4,262 subjects, aged between 20 and 69 years old, were selected from KNHANES 2001; those who completed health examination, nutrition survey, and provided their socioeconomic information. Results of logistic regression showed that it was likely to have better SRH for those who were younger, male and have higher education, higher income, better living condition, no metabolic syndrome and higher dietary variety.

다중 센서들을 이용한 다양한 화재 상황의 위험도 추정 시스템 개발 (Implementation of Fire Risk Estimation System for various Fire Situations using Multiple Sensors)

  • In this paper, a fire detection system based on quantitative risk estimation is presented. Multiple sensors are used to build a comprehensive indicator that represents the risk of fire quantitatively. The proposed fire risk estimation method consists of two stages which determines the occurrence of fire and estimates the toxicity of the surveillance area. In the first stage, fire is reliably detected under diverse fire scenarios. The risk of fire is estimated in the second stage. Applying Purser's Fractional Effective Dose (FED) model which quantitates harmfulness of toxic gases, the risk of the surveillance area and evacuation time are calculated. A fire experiment conducted using four different types of combustion materials for the verification of the system resulted in a maximum error rate of 12.5%. By using FED calculation and risk estimation methods, the proposed system can detect various signs of fire faster than conventional systems.

일반 질 지표로서의 병원 표준화 사망비에 대한 고찰 (How Can We Use Hospital-Standardized Mortality Ratio as a Quality Indicator of Hospital Care in Korea?)

  • The hospital standardized mortality ratio (HSMR) is a widely used generic measure for assessing quality of hospital care in many countries. However, the validity of HSMR as a quality indicator is still controversial. We critically reviewed characteristics of HSMR and suggested how to use HSMR as a quality indicator in the Korean setting. The association between HSMR and other quality measures of hospital care is inconclusive. In addition current HSMR model has shortcomings in risk adjustment because of the lack of clinical data, accuracy of disease coding, coding variation among hospitals, end-of-life care issues, and so on. Therefore, HSMR should be used as an indicator for improvement, not for judgement such as public reporting and pay-for-performance. More efforts will be needed to tackle practical and methodological weaknesses of HSMR in the Korean setting.

국외 감염병 위험도 평가체계의 비교분석 (Comparative Analysis of Risk Assessment Tools for Infectious Diseases)

  • Background: Emerging infectious diseases, such as Middle East respiratory syndrome or coronavirus disease 2019, pose a continuous threat to public health, making a risk assessment necessary for infectious disease control and prevention. Therefore, we aimed to investigate the risk assessment methods for infectious diseases used by major foreign countries and organizations. Methods: We conducted an investigation and comparative analysis of risk assessment and risk determination methods for infectious diseases. The risk assessment tools included the strategic toolkit for assessing risks, influenza risk assessment tool, pandemic severity assessment framework, and rapid risk assessment methodology. Results: The most frequently reported risk elements were disease severity, antiviral treatment, attack rate, population immunity, and basic productive ratio. The risk evaluation method was evaluated quantitatively and qualitatively by the stakeholders at each institution. Additionally, the final risk level was visualized in a matrix, framework, and x and y-axis. Conclusion: Considering the risk assessment tools, the risk element was classified based on the duplicate of each indicator, and risk evaluation and level of risk assessment were analyzed.

지화학 자료의 확률론적 불확실성 및 위험성 분석을 위한 지시자 지구통계학의 응용 (Application of Indicator Geostatistics for Probabilistic Uncertainty and Risk Analyses of Geochemical Data)

  • 지화학 자료는 환경 관리를 위한 중요한 환경 변수중 하나로 인식되어 왔다. 지화학 자료는 보통 공간적으로 산재되어 수집되기 때문에, 샘플링 되지 않은 지점에서의 속성값 예측과 더불어 부가적인 분석을 위해 예측에 수반되는 불확실성을 추정할 필요가 있다. 이 논문은 지시자 지구통계학이 지화학 자료의 공간적인 분포값의 제시뿐만 아니라 의사결정을 보조할 수 있는 정보를 제공하기 위해 유용하게 사용될 수 있는지를 예시하고자 한다. 카드뮴 자료의 추정사례 연구를 통해 확률론적 불확실성 모델링, 위험성 분석 등 지구통계학적 분석의 틀을 제시하였다. 지시자 크리깅을 통해 조건부 누적 분포 함수를 모델링한 후에, 기대값 추정치와 조건부 분산을 카드뮴의 추정값과 정량적 불확실성 추정을 위해 각각 계산하였다. 그리고 확률 임계치와 속성 임계치의 적용을 통해 오염/비오염 지역을 구분하였다. 또한 조건부 분산과 속성값과 임계치값의 차이를 모두 설명할 수 있는 변동 계수를 통해 추가적인 샘플링 지점을 추출하였다. 이 연구에서 적용한 지시자 지구통계학적 분석 틀은 불확실성을 고려한 의사 결정과 관련하여 지화학 자료를 포함한 환경 변수의 분석에 유용하게 사용될 수 있을 것으로 기대된다.

  • The purpose of this study was to validate knowledge outcomes included Nursing Outcomes Classification(NOC) developed by Johnson and Maas at the University of Iowa. A sample of 71 nurse experts working in university affiliated hospitals participated in this study. They were asked to rate indicators that examplified the outcomes on a scale of 1(indicator is not all characteristic) to 5(indicator is very characteristic). A questionnaire with an adaptation of Fehring's methodology was used to establish the content validity of outcomes. The results were as follow: 1. All indicators were considered to be 'supporting' and no indicators were considered to be 'nonsupporting'. 2. 'Knowledge: Treatment Regimen' attained and OCV score of 0.816 and was the highest OCV score among outcomes. 3. 'Knowledge: Energy Conservation' attained an OCV score of 0.748 and was the lowest OCV score among abuse outcomes. 4. 'Knowledge: Breastfeeding' attained an OCV score of 0.790 and was the highest indicator was 'description of benefits of breastfeeding'. 5. 'Knowledge: Child Safety' attained an OCV score of 0.778 and was the highest indicator was 'demonstration of first aids techniques'. 6. 'Knowledge: Diet' attained an OCV score of 0.779 and was the highest indicator was 'performance of self-monitoring activities'. 7. 'Knowledge: Disease Process' attained an OCV score of 0.815 and was the highest indicator was 'description of signs and symptoms'. 8. 'Knowledge: Health Behaviors' attained an OCV score of 0.800 and was the highest indicator was 'description of safe use of prescription drugs'. 9. 'Knowledge: Health Resources' attained an OCV score of 0.794 and was the highest indicator was 'description of need for follow-up care'. 10. 'Knowledge: Infection Control' attained an OCV score of 0.793 and was the highest indicator was 'description of signs and symptoms'. 11. 'Knowledge: Medication' attained an OCV score of 0.789 and was the highest indicator was 'description of correct administration of medication'. 12. 'Knowledge: Personal Safety' attained an OCV score of 0.804 and was the highest indicator was 'description of measures to reduce risk of accidental injury'. 13. 'Knowledge: Prescribed Activity' attained an OCV score of 0.810 and was the highest indicator was 'proper performance of exercise'. 14. 'Knowledge: Substance Use Control' attained an OCV score of 0.809 and was the highest indicator was 'description of signs of dependence during substance withdrawl'. 15. 'Knowledge: Treatment Procedure(s)' attained an OCV score of 0.795 and was the highest indicator was 'description of appropriate action for complications'. 16. 'Knowledge: Treatment Regimen' attained an OCV score of 0.816 and was the highest indicator was 'description of self-care responsibilities for emergency situations'. More outcomes need to be validated and outcomes sensitive to Korean culture need to be developed.
