서울지역 유통한약재의 통계적 품질관리 - 회분, 산불용성회분, 건조감량, 유해중금속을 중심으로 - (Statistical Quality Control of Herbal Drugstuffs Distributed in Seoul Area - Centering around Ash, Acid-insoluble Ash, Loss on Drying and Hazardous Heavy Metals -)

  • To survey the status of quality control of major ingredients of 'Ssanghwatang (Oriental medicine used for fatigue)', we analyzed 1,024 samples (9 kinds) of medical herbs to determine amounts of ashes, acid-insoluble ashes, loss on drying that are major elements to ensure basic quality of herbal drugstuffs. After ash analysis, Paeoniae Radix (from China) failed to meet the herbal standard criterion. After yielding Z-score(indicating the probability of exceeding its criterion) Zizyphi Fructus, Zingiberis Rhizoma, Paeoniae Radix, Rehmanniae Radix Preparata became objects of intense quality control (=IQC). Analysis on loss on drying shows Cinnamomi Cortex was unsuitable and was put under the IQC. In case of respective heavy metals, Angelicae Gigantis Radix, Cnidii Rhizoma, Cinnamomi Cortex exceeded the maximum permissible range for Cd. The 3 kinds of sub-standards were put to the IQC. Statistic figures showed a significant correlation (t-test, p<0.01) between country-of-origin and sub-quality rates. Compared to domestic one, imported medicinal herbs were less suitable in regard of acid-insoluble ash, loss on drying and Pb/Hg content. Meanwhile, amounts of Acid-insoluble ash showed positive relation with amounts of Pb (r=0.202) and As (r=0.243) among heavy metals leading to an inference that herbs of which root/rhizome is used for medical purposes attribute its high heavy metal content to the fine earth/sand being stuck to its root stalk.

두드러기 증례 연구에서 사용된 한약 처방 분석 (Analysis of Korean Herbal Medicine Used in Case Studies of Treating Urticaria)

  • Objectives : The purpose of this study is to analyze the use of herbal medicine for the treatment of urticaria in case studies and to find the general tendency of herbal medicine treatment and to establish the primary treatment direction of urticaria. Methods : In the domestic databases, Oriental medicine Advanced Searching Integrated System (OASIS), Korean Traditional Knowledge Portal (KTKP), National Discovery for Science Leader (NDSL), Research Information Sharing Service (RISS), we selected among the case study papers published in the last 20 years using search terms related to "urticaria". Reports related to Sasang Constitutional medicine were excluded and total of 15 papers were finally selected. Results : 23 prescriptions were retrieved from 15 papers. The most commonly used prescription was Seungmagalgeun-tang (升麻葛根湯), and Bangpoongtongsung-san (防風通聖散) and Pyeongwi-san (平胃散) were presented twice. Commonly used herbs are Glycyrrhizae Radix et Rhizoma (甘草), Zingiberis Rhizoma Recens (生薑), and Scutellariae Radix (黃芩). 18 out of 78 herbs are belonging to heat-clearing medicinal (淸熱藥) and 16 out of 78 herbs are belonging to exterior-releasing medicinal (解表藥). Concurrent therapies such as acupuncture, pharmacoacupuncture, moxibustion, cupping therapy, ice pack applying therapy were performed and several evaluation methods were used, patient's subjective remission rate was the most used. Conclusions : Through this study, we were able to discover the tendency of herbal medicine prescription for the treatment of urticaria.

  • Two isoforms of cyclooxygenase (COX) have been identified - COX-1, which is constitlitively expressed in most tissues, and the inducible form, COX-2, of which expression is induced by inflammatory signals and mitogens. It has been considered that the beneficial effects of NSAIDs are due to the inhibition of COX-2 activity and the side effects are from the inhibition of COX-1 activity. Therefore, it is essential to develop selective COX-2 inhibitor for developing new GI-tolerable NSAIDS. To discover new leads for developing selective COX-2 inhibitors, three-hundred extracts of natural products were primarily screened with the system of prostaglandin accumulation in LPS-stimulated mouse peritoneal macrophages. To identify whether these inhibitory activities of crude extracts on the accumulation of Prostaglandins were derived from direct action against COX-2, the effects of selected extracts on exogenous arachidonic acid-derived production of prostaglandins by LPS-stimulated macrophages were determined. Among them, 5 methanol extracts of natural products, such as Zingiberis Rhizoma, Alpinae Officinarum Rhizoma, Caryophilli Flos, Scutellariae Radix, Dalbergia ordorifera. inhibited more than 70% of the prostaglandin production in LPS-stimulated mouse peritoneal macrophages at a con-centration of 1${\mu}$g/ml.

한의원에 내원한 틱장애 아동 환자의 한의복합치료 후 개선 효과: 후향적 관찰 연구 (Effect of Complex Korean Medical Treatment on Tic Disorder in Children Who Visited the Neuropsychiatric Clinic of Korean Medicine: A Retrospective Chart Review)

  • Objectives: To examine the effect of complex Korean medical treatment on tic disorder in children who visited the neuropsychiatric clinic of Korean medicine. Methods: Medical records of 35 patients diagnosed with tic disorder based on DSM-V who received complex Korean medical treatment (herbal medicine, acupuncture and Korean psychotherapy) were reviewed. Tic disorder was then evaluated by Yale Global Tic Severity Scale (YGTSS) before and at 4, 8, and 12 weeks after treatment to determine whether their symptoms were improved. Results: 1) After the treatment, mean YGTSS was reduced from 35.54±14.77 to 23.20±12.65. There were statistically significant changes between scores according to the time of treatment (p< 0.05). 2) At the time of visit, symptoms of motor tics were in the order of eyes, neck and shoulders, mouth, head and arms & hands, nose, face, upper body and other parts, and legs and symptoms of vocal tics were in the order of sniffing, dry coughing and MMM, AA and whistling, and other sounds. 3) For 35 patients, herbal medicine was prescribed every two weeks. The frequency of prescriptions used was in the order of Shihogayonggolmoryo-tang, Gami-ondam-tang, and others. 4) The frequency of herbs used in 35 patients was in the order of Poria, Zingiberis Rhizoma Recens, Pinelliae Rhizoma and Scutellariae Radix, Bupleuri Radix, Ostreae Concha and Bovis Ossis Fossilia, and others. Conclusions: Complex Korean medical treatment including herbal medicine, acupuncture, and Korean psychotherapy is effective for reducing tic symptoms in children.

천연물의 위식도역류질환 예방, 치료 효과에 대한 실험연구 현황 – Pubmed를 중심으로 (Experimental study trends on the prevention and treatment effects of herbal medicine for gastroesophageal reflux disease (GERD) - based on Pubmed)

  • Objectives : This study aimed to review the current trends in experimental studies on the use of natural products for treatment of gastroesophageal reflux disease (GERD). Methods : Experimental studies assessing the efficacy of natural products against GERD were searched on PubMed. Articles were selected based on predefined inclusion and exclusion criteria and then analyzed for experimental methods, interventions, and result analysis techniques. Results : A total 37 studies were included in this review. Predominantly, in vivo experiments were conducted to induce GERD through surgery, involving the ligation of the pylorus and the transitional junction between the corpus and the forestomach using 7-week-old male Sprague-Dawley rats. The acute induction model, sacrificing animals after a single administration following GERD induction, was mainly used.The utilization of cell experiments was relatively infrequent, with a focus on assessing antioxidant and anti-inflammatory effects via the treatment of the RAW 264.7 cell line with lipopolysaccharides treatment. Glycyrrhizae Radix et Rhizoma, Pinelliae Tuber, Ginseng Radix and Zingiberis Rhizoma were used as single ingredients, and herbal formula, STW-5 (iberogast), Rikkunshito (六君子湯), Banhasasim-tang (半夏瀉心湯), and Hewei Jiangni granule (和胃降逆湯) were used. Outcome analysis methods encompassed Macroscopic evaluation, esophageal function assessment, blood biomarker analysis, histological examination, protein analysis, gene expression analysis, and gastric juice analysis. Proton pump inhibitors were predominantly employed as positive controls. Conclusions : This study revealed the current trends in non-clinical research evaluating natural products for GERD. Based on the results of this study, we expect that non-clinical research on clinically effective natural products will be revitalized.

흰쥐 신경세포에서 억제성 및 흥분성 한약재가 유발한 이온전류의 특성 (Characterization of ion current induced by inhibitory and excitatory herbs in rat periaqueductal gray neuron)

  • To research the characteristics of ion currents induced by inhibitory and excitatory herbs of oriental medicine, we used nystatin-perforated patch clamp technique under voltage clamp condition in periaqueductal gray neuron dissociated from Sprauge-Dawley rat, 10-15 days old. The results are as follows. 1. Ion current induced by $10mg/m{\ell}$ of Bupleuri Radix was inhibited $59.50{\pm}4.29%$ by $10^{-4}M$ bicuculline(p>0.01) but inhibition of $10.75{\pm}4.77%$ by $10^{-4}M$ tubocurarine and $4.75{\pm}4.23%$ by $10^{-4}M$ verapamil had no statistical significance(p>0.05). So ion current induced by Bupleuri Radix revealed only GABA induced $Cl^-$ current, not acetylcholine and $Ca^{2+}$ current. 2. Ion current induced by $20mg/m{\ell}$ of Coptidis Rhizoma was inhibited $47.20{\pm}7.88%$ by $10^{-4}M$ bicuculline(p<0.01) but $3.20{\pm}2.33%$ inhibition by $10^{-4}M$ tubocurarine and $1.00{\pm}1.00%$ inhibition by $10^{-4}M$ verapamil had no significance(p>0.05). So ion current induced by Coptidis Rhizoma revealed only GABA induced $Cl^-$ current, not acetylcholine and $Ca^{2+}$ current. 3. Ion current induced by $20mg/m{\ell}$ of Ecliptae Herba was inhibited $55.00{\pm}4.92%$ by $10^{-4}M$ bicuculline (p<0.01), and also inhibited $15.00{\pm}4.26%$ by $10^{-4}M$ tubocurarine(p<0.05), but inhibition of $6.00{\pm}3.03%$ by $10^{-4}M$ verapamil had no significance(p>0.05). So ion current induced by Ecliptae Herba showed GABA activated $Cl^-$ current and acetylcholine activated cation current, not $Ca^{2+}$ current 4. Ion current induced by $5mg/m{\ell}$ of Liriopis Tuber was inhibited $15.20{\pm}4.57%$ by $10^{-4}M$ bicuculline<0.05) and also inhibited $14.00{\pm}3.00%$ by $10^{-4}M$ tubocurarine(p<0.05), but inhibition of $5.20{\pm}4.80%$ by $10^{-4}M$ verapamil had no significance(p>0.05). So ion current induced by Liriopis Tuber showed GABA. activated $Cl^-$ current and acetylcholine activated cation current, not $Ca^{2+}$ current. 5. Ion current induced by $5mg/m{\ell}$ of Aconiti Tuber was inhibited $97.00{\pm}1.34%$ by $10^{-4}M$ bicuculline(p<0.01), $80.00{\pm}9.83%$ by $10^{-4}M$ tubocurarine(p<0.01), and $24.00{\pm}6.18%$ by $10^{-4}M$ verapamil(p<0.05). So ion current induced by Aconiti Tuber revealed GABA activated $Cl^-$ current and acetylcholine activated cation current and $Ca^{2+}$ current. 6. Ion current induced by $10mg/m{\ell}$ of Zingiberis Rhizoma was inhibited $33.00{\pm}7.43%$ by $10^{-4}$ bicuculline(p<0.05), $10.20{\pm}1.83%$ by $10-^{-4}M$ tubocurarine(p<0.01), and $14.00{\pm}2.16%$ by $10^{-4}M$ verapamil(p<0.01) So ion current induced by Zingiberis Rhizoma revealed GABA activated $Cl^-$ current and acetylcholine activated cation outtent and $Ca^{2+}$ current. 7. Ion current induced by $10mg/m{\ell}$ of Boshniakiae Herba was inhibited $65.00{\pm}13.75%$ by $10^{-4}M$ bicuculline(p<0.05), $38.00{\pm}9.24%$ by $10^{-4}M$ tubocurarine(p<0.05), and $33.25{\pm}7.42%$ by $10^{-4}M$ verapamiHp<0.05). So ion current induced by Bpshniakiae Herba revealed GABA activated $Cl^-$ current and acetylcholine activated cation current and $Ca^{2+}$ current. These results suggest that a point of difference between inhibitory and excitatory herbs is existence of$Ca^{2+}$ current.

가미청간탕(加味淸肝場)이 Rat의 알콜성 지방간에 미치는 영향 (The Effects of Ka-Mi-Chung-Gan-Tang on Rat with Alcoholic Fatty Liver)

  • Chronical intake of alcohol can cause alcoholic fatty liver. Fatty liver is caused by fat infiltration: the state of high rate of fat in liver cells and by losing the balance between the synthesis and the secretion of fatty acid. It could be developed into liver necrosis and cirrhosis. Ka-Mi-Chung-Gan-Tang (KMCGT) is a decoction used for fatty liver as oriental medicines in China. The prescription is composed of Ginseng Radix, Bupleuri Radix, Scutellariae Radix, Pinelliae Tuber, Artemisiae capillaris Herba, Gardeniae Fructus, Zingiberis Rhizoma, Zizyphi Fructus and Glycyrrhizae Radix etc. We have induced alcoholic fatty liver by ethanol administration (6 g/kg, single dose/day, for a week) on rats and observed changes of triglyceride, cholesterol and lipid peroxidation in liver tissues of them. Also we checked the activities of GOT and GPT in blood of rats. KMCGT inhibited significantly the increase of triglyceride, cholesterol, lipid peroxidation level and effectively the increase of malondialdehyde (MDA).

생약(生藥) 복합제제(複合製劑)의 약효(藥效) 연구(硏究)(제22보)(第22報) -이진탕(二陳湯)이 소화기계(消化器系) 및 적출자궁(摘出子宮)에 미치는 영향(影響)- (Studies on the Efficacy of Combined Preparation of Crude Drugs (XXII) -Effects of Yijin-Tang on the Digestive System and Isolated Uterus-)

  • Experimental studies were undertaken to investigate for the effect of Yijin-tang on the digestive system and isolated uterus of rats. Yijin-tang was composed of five crude drugs; Pinelliae Tuber, Aurantii nobilis Pericarpium, Hoelen, Glycyrrhizae Radix and Zingiberis Rhizoma. The results obtained from experiments with water soluble fraction extracted from Yijin-tang were as follows; Spontaneous motilities of isolated ileum of mice and rabbits were suppressed, and contraction of isolated ileum of mice and guinea-pigs induced acetylcholine, barium chloride and histamine were inhibited. The anticathartic action of mice induced by castor oil were significantly observed. The spontaneous motility of isolated uterus of rats was suppressed. Inhibitory effect of gastric juice secretion and antiulcerogenic action were significantly shown in Shay rats.

한약재 수치에 관하여(I) -수치 전.후의 한약재의 표준 색도표에 의한 색상변화- (Studies on the processing of herbal medicines(I) -The change of a color tone of processed and unprocessed herbal medicines by korean standard color table-)

  • To obtain a specific pharamcological effect, herbal medicines are processed based on the principle of traditional korean medicines. The problem in drug processing is the variety of methods, technics, and drug quality. In order to establish the base of standardization of processing methods, some herbal medicines(Evodiae Fructus, Scutellariae Radix, Astragali Radix, Zingiberis Rhizoma, Dioscoreae Radix, Aconiti Radix) were processed and studied the change a color tone of processed and unprocessed herbal medicines. The color tone(value and chroma) of these herbal medicines were changed after processing.

멜라노마 세포에서 가미소요산(加味逍遙散)의 MITF, TRP-1, TRP-2, Tyrosinase mRNA 발현 억제 효과 (Inhibition Effect of Gamisoyo-san on MITF, TRP-1, TRP-2, Tyrosinase mRNA Expression in Melanoma Cells (B16F10))

  • Objectives : Gamisoyo-san complex prescription were made with Angelicae Gigantis Radix, Paeoniae Radix, Atractylodes rhizome white, Hoelen, Bupleuri Radix, Moutan Cortex Radicis, Gardeniae Fructus, Zingiberis Rhizoma Crudus, Menthae Herba. The purpose of this study was to research the whitening effect of the extract from Gamisoyo-san, which is one of the used herbal complex prescription. Methods : This study investigated inhibitory effect of Gamisoyo-san in tyrosinase activity. Cell viability were performed using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) assay. Then, Gamisoyo-san measured reversed-transcription-PCR for mRNA expression using B16F10 mouse melanoma cells. Results : For whitening effects, the tyrosinase inhibition effect of extract was shown to 52.4% at $5,000{\mu}g/m{\ell}$ concentration. The cell viability on B16F10 melanoma cells of Gamisoyo-san extract showed higher than 75% at $1,000{\mu}g/m{\ell}$ concentration. In this study, an experiment was performed by setting the non-toxic concentration range of 50, 150, $250{\mu}g/m{\ell}$. The Glyceraldehyde-3-phosphate dehydrogenase (GAPDH) was used as a positive control. The microphthalmia-associated transcription factor (MITF), tyrosinase related protein-1 (TRP-1), tyrosinase related protein-2 (TRP-2), tyrosinase mRNA expression inhibitory by reverse transcription-PCR of Gamisoyo-san extract were decreased by 95.3%, 98.8%, 96.3% and 49.5% at $250{\mu}g/m{\ell}$ which the highest concentration. Conclusions : All these findings could verify that whitening effects of Gamisoyo-san extract by tyrosinase inhibitory activity and mRNA expression. The Gamisoyo-san could be used as material for functional cosmetics, such as skin whitening products.
