Long-term treatment of allogeneic adipose-derived stem cells in a dog with rheumatoid arthritis

  • Background: Although there are growing demands for stem cell-based therapy for companion animals in various diseases, a few clinical trials have been reported. Moreover, most of them are the results from only one or a few times of stem cell injection. Objectives: The aim of this study is to describe a long-term treatment with allogeneic adipose-derived stem cells (ASCs) in a dog with rheumatoid arthritis (RA), which is a rare canine disease. Methods: The dog with RA received intravascular injection of allogeneic ASCs derived from two healthy donors once a month for 11 months. To assess therapeutic effects of ASCs, orthopedic examination and clinical evaluation was performed. Cytokines of tumor necrosis factor-α and interleukin-6 in the plasma were measured using ELISA analysis. Results: Despite this repeated and long-term administration of allogeneic ASCs, there were no side effects such as immunorejection responses or cell toxicity. The orthopedic examination score for the dog decreased after ASCs treatment, and the clinical condition of the dog and owner's satisfaction were very good Conclusions: Although ASCs has been suggested as one of the options for RA treatment because of its anti-inflammatory and immunosuppressive functions, it has never been used to treat RA in dogs. The present report describes a case of canine RA treated with allogeneic ASCs for long-term in which the dog showed clinical improvement without adverse effects.

Response to Bee Venom Acupuncture and Polymorphism of Interleukin 4 Receptor and Interleukin 10 in Rheumatoid Arthritis

  • Objectives : We investigated to find the relationship between single-nucleotide polymorphism (SNP) of IL4R, IL-10 and bee venom therapy efficacy in patients with RA treated with bee venom for 8 weeks. Methods : Korean RA patients (n=114) and healthy subjects (n=109) were included in this prospective study. Korean bee venom was dissolved in saline (diluted 1:3000) and administrated into acupuncture points. Bee venom therapy was applied twice a week and continued for 8 weeks. The clinical response was evaluated using various assessments before and after treatment. Disease severity was measured by determining the number of tender joints and swollen joints. Laboratory studies included ESR, CRP, and rheumatoid factor. Genotyping for IL-4R and IL-10 polymorphism was done by pyrosequencing analysis. Results : 1. In IL4R genotypes, there was significant difference between RA ptitients tind controls group. 2. In IL4R genotypes, there was significant difference among Good, Mild and Bad responders to in RA patients, but in the frequency of alleles and carriers, there were no significant difference. 3. There was no significant difference between RA patients and controls group in IL-10 gene genotypes. 4. In IL-10 genotypes, there was no significant difference among Good, Mild and Bad responders to in RA patients. 5. There was no significant difference in the improvement of ESR, CRP and KHAQ scores after bee venom therapy in RA patients among the IL4R or IL-10 genotypes. Conclusions : In IL-4R genotypes, there was significant difference between RA patients and control group, and among Good, Mild and Bad responders in RA patients. However, in IL-10 genotypes, there was no significant difference between RA patients and controls group and among Good, Mild and Bad responders in RA patients.

류마티스 관절염의 예방과 관리 (Prevention and Management of Rheumatoid Arthritis)

  • Rheumatoid arthritis(RA) is the most common inflammatory joint disease and a major cause of disability, morbidity, and mortality. It occurs worldwide, affecting approximately one percent of adults. Inflammation of the synovial membrane surrounding a joint leads to swollen, tender, and stiff joints RA has no known cure and the diagnosis is made based on clinical criteria and many different options exist for treatment. All of these factors magnify the importance of the patient-physician interaction and place a premium on the art rather than the science of medicine. The major goals of therapy for RA are to relive pain, swelling, and fatigue; improve joint function; stop joint daage, and prevent disability and disease-related morbidity. Some combination of nonsteroidal anti-inflammatory drugs(NSAIDs), steroids, and DMARDs is necessary in almost patients. In many combinations of different DMARDs or DMARDs plus biologicals are necessary for optimal control. Additionaly, all patients with RA should be educated about their disease and the therapies that will be used. Patient education is essentially early in the disease course and on going basis Much research is focused on the further development of biological agent for treatment of RA. Elucidation of the trigger or trigers for RA may allow us to begin to think about prevention of RA.

Inhibitory Effect of Coicis Semen Composition on Inflammatory Responses in the Collagen-induced Arthritis Mouse Model

  • This study was peformed to investigate possible mechanisms underlying possible effect of Coicis Semen composition (CSC) on inflammatory diseases using in vivo model of RA in the mice. Results are summarized as follows. In production of inflammatory cytokines, INF-${\gamma}$ in the spleen and IL-6 in the serum were decreased by CSC treatment. TNF-${\alpha}$ in serum was significantly decreased, IL-4 in the spleen was significantly increased by CSC treatment. In production of rheumatoid factors, IgM and IgG were significantly decreased by CSC treatment. The present data suggest that CSC treatment can improve pathological damage by CIA. So we expect that CSC should be used as a effective drugs for not only rheumatoid arthritis but also another autoimmune disease. Therefore we have to survey continuously in looking for the effective substance and mechanism in the future.

  • Inflammatory mediators has been recognized as an important role in the pathogenesis of rheumatoid arthritis (RA). IL-17 is increasingly recognized as an important regulator of immune and inflammatory responses, including induction of proinflammatory cytokines and osteoclastic bone resorption. Evidence of the expression and proinflammatory activity of IL-17 has been demonstrated in RA synovium and in animal models of RA. However, the signaling pathways that regulate IL-17 production remain unknown. In the present study, we investigated the role of the phosphatidylinositol 3 kinase (PI3K)-Akt pathway in the regulation of IL-17 production in RA. PBMC were separated from RA (n=24) patients, and stimulated with various agents (anti CD3, anti CD28, PHA, ConA, IL-15). IL-17 levels were determined by sandwich ELISA and RT-PCR. The production of IL-17 was significantly increased in cells treated with anti-CD3 antibody, PHA, IL-15 or MCP-1 (P<0.05). ConA also strongly induced IL-17 production (P<0.001), whereas TNF-alpha, IL-1beta, IL-18 or TGF-beta did not. IL-17 was detected in the PBMC of patients with osteoarthritis (OA) but their expression levels were much lower than those of RA PBMC. Anti-CD3 antibody activated the PI3K-Akt pathway and activation of the PI3K-Akt pathway resulted in a pronounced augmentation of nuclear factor kappaB ($NF-{\kappa}B$). IL-17 production by activated PBMC in RA is completely or partially blocked in the presence of $NF-{\kappa}B$ inhibitor PDTC and PI3K-Akt inhibitor, wortmannin and LY294002, respectively. Whereas the inhibition of AP-1 and extracellular signal-regulated kinase (ERK)1/2 did not affect IL-17 production. These results provide new insight into that PI3K/Akt and $NF-{\kappa}B$ dependent signal transduction pathway could be involved in the overproduction of key inflammatory cytokine, IL-17 in rheumatoid arthritis.

봉독약침이 제2형 콜라겐유도 관절염에 미치는 영향 (The effect of Bee Venom on type II Collagen-induced Arthritis)

  • Objective : Bee venom (BV) has traditionally been used in Oriental medicine to relieve pain and to treat inflammatory disease such as rheumatoid arthritis (RA). Autoimmunity to type II collagen (CII) may involve in the pathogenesis of RA. This study was performed to evaluate the effect of BV on type II collagen induced arthritis (CIA) with the naked eye, a immunohistochemical method and the examination of histology. Method : Male mice were immunized by subcutaneously injection of an $200{\mu}g$ emulsion mixed with bovine CII and complete Freund's adjuvant (CFA) twice for two weeks. In the control group, normal saline was injected, and in the experimental group, BV was applied. Result : The incidence of arthritis, the mean arthritis index and the number of the arthritic limbs of the BV group were all significantly lower than those of the control group. Among the pro-inflammatory cytokines, the production of $TNF-{\alpha}$ in the BV group was also suppressed compared with the control group, but $IL-1{\beta}$ was not. The examination on the histopathology of joints of CIA mice showed the effect of Bee Venom Herbal Acupuncture on the arthritis. Conculusion : Treatment with BV resulted in inhibition of development of arthritis and immune responses to CII.

An overlap syndrome of Churg-Strauss syndrome and rheumatoid arthritis

  • Churg-Strauss syndrome (CSS) is a necrotizing vasculitis with extra-, peri-vascular eosinophilic infiltration. Chronic symmetric polyarthritis with the presence of rheumatoid factor (RF) and anti-cyclic citrullinated peptide antibody are the mainstay of rheumatoid arthritis (RA) diagnosis. Mononeuritis multiplex is a peripheral neuropathy involving more than 2 separate nerve areas. A 62-year-old male patient was referred for left foot drop and polyarthritis of both hands and feet for 4 months. During evaluation, mononeuritis multiplex was detected on nerve conduction study and electromyography tests: vasculitis with neutrophil, eosinophil, and lymphocyte infiltration on peroneal nerve biopsy. A positive response to methacholin and bronchodilator was observed on the pulmonary function test. Radiologic tests showed peri-articular soft tissue swelling and osteopenia on both hand and foot. Marked peripheral eosinophilia, high RF, and positive perinuclear anti-neutrophil cytoplasmic antibody were detected on blood tests. Here, we report on a patient with overlap syndrome of CSS and RA with review of the relevant literature, from which a few references to overlap syndrome of CSS and RA were available.

  • Arthritis may be broadly classified as degenerative - related to defects in cartilage and other joint constituents, often age-associated - or inflammatory disease. Inflammatory arthritis called as rheumatoid arthritis (RA) is a chronic inflammatory arthropathy and characterized by a destructive arthritis. RA encompasses infectious arthritis, arthritis caused by intra-articular deposits of crystalline material (gout), syndromes associated with genetic defects (familial Mediterranean fever), and the immune-mediated inflammatory arthropathy. Degenerative arthritis called as osteoarthritis (OA), which is most frequently occurring, causes degenerative figures of knee, waist and knuckle, and accompanies severe pain around the cartilage. Also, it may cause morning stiffness, gelling effect, tenderness, bone swelling, crepitus, and motion disorders.

류마티스 관절염 여성환자의 성생활에 관한 지각된 질병의거변화와 영향요인 (Illness Intrusiveness on Sexuality and Its Influencing Factors in Rheumatoid Arthritis Women)

  • This study was conducted to clarify the effect of physical ability, pain, fatigue, depression, sexual satisfaction, and spouse criticism on the sexual intrusiveness of rheumatoid arthritis(RA)women. The subjects of the study were 89 RA outpatients who visited the department of rheumatology of Dong-A University Hospital in Pusan from March 2000 to April, 2000. Pain & Fatigue via graphic rating scale, depression via CES-D(Center for Epidemiologic Studies -Depression) scale and physical ability via the Health Assessment questionnaire(HAQ) were assessed. Also we used both Sexual satisfaction scale(Kim, et al, 1997) to measure sexual satisfaction and Spouse criticism scale(Kraaimaat et al. 1996) to measure spouse criticism. The Intrusiveness rating scale which was derived from Arthritis Impact Measurement Scale(AIMS) was used to measure illness intrusiveness in sexuality. The relationships between the variables such as pain, depression, fatigue, physical ability, spouse criticism, sexual satisfaction and illness intrusiveness on sexuality were analyzed by Pearson correlation. As a result, pain & depression were significantly positively related to illness intrusiveness and physical ability & sexual satisfaction was significantly negatively related to illness intrusiveness. But these variables were interrelated. So hierarchical regression was performed to examine the relative contribution of pain, depression, physical ability, and sexual satisfaction with regard to intrusiveness of RA on sexuality. It revealed that pain, depression & sexual satisfaction predicted significantly illness intrusiveness on sexuality. Therefore it is very important to reduce pain and depression and to increase sexual satisfaction for reducing illness intrusiveness.

Periodontal pathogens and the association between periodontitis and rheumatoid arthritis in Korean adults

  • Purpose: Periodontitis and rheumatoid arthritis (RA) share a similar inflammatory pathogenesis. Porphyromonas gingivalis (Pg) can induce anticyclic-citrullinated peptide autoantibodies (anti-CCP antibodies), a key factor in the development of RA. This study aimed at evaluating the relationships between the 2 diseases and identifying the clinical implications thereof, with a focus on periodontal pathogens in Korean adults. Methods: A total of 260 RA patients and 86 age- and sex-matched control patients without arthritis were enrolled in this prospective cross-sectional study. Periodontal indices and the prevalence and amount of periodontal pathogens were compared between the groups. Correlations between periodontal and RA indices were examined, as were correlations between 9 periodontal pathogens and RA indices. Results: The RA group had significantly higher values than the control group for all investigated periodontal indices (P<0.05) except the number of teeth. The gingival index (GI) was correlated with the disease activity score 28 (DAS28) (r=0.125, P=0.049), RA disease duration (r=0.253, P<0.001), erythrocyte sedimentation rate (ESR) (r=0.162, P=0.010), and anti-CCP antibody titer (r=0.205, P=0.004). Probing pocket depth (PPD) was correlated with ESR (r=0.139, P=0.027) and anti-Pg antibody titer (r=0.203, P=0.001). Bleeding on probing (BOP) was correlated with DAS28 (r=0.137, P=0.030), RA disease duration (r=0.202, P=0.001), ESR (r=0.136, P=0.030), anti-Pg antibody titer (r=0.177, P=0.005), and anti-CCP antibody titer (r=0.188, P=0.007). Clinical attachment level (CAL) and periodontitis severity were correlated with anti-Pg antibody titer (the former r=0.201, P=0.002; the latter r=0.175, P=0.006). The quantity of Pg was positively correlated with the serum anti-Pg antibody titer (r=0.148, P=0.020). Conclusions: The GI, BOP, and PPD showed positive relationships with several RA indices. The anti-Pg antibody titer had positive relationships with PPD, BOP, CAL, and periodontitis severity. Thus, increasing values of periodontal indices could be used as a risk indicator of disease development in RA patients, and an increasing anti-Pg antibody titer could be considered as a warning sign in RA patients suffering with periodontitis.
