Predictors for Aggressive Behavior of Patients with Mental Illness in a Closed Psychiatric Ward using Zero-Inflated Poisson Regression: A Retrospective Study (영과잉포아송회귀분석을 활용한 안정병동에 입원한 정신질환자의 공격행동 예측요인)

  • Purpose: This study was conducted to identify predictors related to aggressive behavior of patients with mental illness admitted to a closed psychiatric ward. Methods: This study adopted a retrospective design which analyzed the hospital medical records of 363 patients with mental illness admitted to the psychiatric closed ward of a university hospital in Seoul, Korea. The collected data were analyzed using SPSS IBM 20.0 and STATA 12.0 SE. ZIP (Zero-Inflated Poisson) and count data analysis were used for the factor influencing the occurrence and frequency of aggressive behavior. Results: The results of ZIP model showed that the factors influencing non-probability of aggressive behavior were anxiety, non-adherence, and frustration. In addition, the factors influencing frequency of aggressive behavior were bipolar disorder and personality disorder trait. Conclusion: We found that bipolar disorder, frustration, and non-adherence are more likely to increase the likelihood of aggressive behavior in patients with mental illness. In particular, patients diagnosed with bipolar disorder were 1.95 times more likely to engage in repetitive aggressive behavior compared to those without a diagnose. However, since the results were different form previous studies, further studies on the traits of anxiety and personality disorders are needed.

Effectiveness of hemocoagulase, tranexamic acid, and their combination for reducing blood loss in bimaxillary orthognathic surgery: a retrospective study

  • Objectives: Orthognathic surgery is a corrective intervention for maxillofacial deformities. Bleeding is a major concern for oral and maxillofacial surgeons. Various agents, such as hemocoagulase, tranexamic acid, and aprotinin have been developed to reduce intraoperative bleeding and transfusion requirements. Therefore, in this study we aimed to investigate the effects of hemocoagulase and tranexamic acid, as well as their simultaneous use, to reduce bleeding during orthognathic surgery. Patients and Methods: This retrospective study included patients who had undergone simultaneous orthognathic surgery of the maxilla and mandible between January 2013 and September 2022 and were classified into three groups based on drugs administered: hemocoagulase (Botropase), tranexamic acid, and a combination of both drugs. We recorded patient age, sex, weight, blood loss, and duration of surgery. Red blood cell (RBC), hemoglobin, hematocrit, and platelet levels were measured before, immediately after, and one day after surgery. Results: No statistically significant differences were found in blood loss, RBC, hemoglobin, hematocrit, or platelet levels between any of the groups. There were no differences in the drug effects between Le Fort I and bilateral mandibular sagittal split osteotomies, with or without double genioplasty. However, there were significant reductions in RBC, hemoglobin, hematocrit, and platelet levels during genioplasty. Conclusion: Tranexamic acid, hemocoagulase, and their combination had similar efficacy in patients who underwent Le Fort I and bilateral mandibular sagittal split osteotomies with and without genioplasty.

A 3-year retrospective study of clinical durability of bulk-filled resin composite restorations

  • Objectives: This study aimed to assess the clinical longevity of a bulk-fill resin composite in Class II restorations for 3-year. Materials and Methods: Patient record files acquired from the 40 patients who were treated due to needed 2 similar sizes Class II composite restorations were used for this retrospective study. In the experimental cavity, the flowable resin composite SDR was inserted in the dentinal part as a 4 mm intermediate layer. A 2 mm coverage layer with a nano-hybrid resin composite (CeramX) was placed on SDR. The control restoration was performed by an incremental technique of 2 mm using the nano-hybrid resin composite. The restorations were blindly assessed by 2 calibrated examiners using modified United States Public Health Service criteria at baseline and 1, 2, and 3 years. The data were analyzed using non-parametric tests (p = 0.05). Results: Eighty Class II restorations were evaluated. After 3-years, 4 restorations (5%) failed, 1 SDR + CeramX, and 3 CeramX restorations. The annual failure rate (AFR) of the restorations was 1.7%. The SDR + CeramX group revealed an AFR of 0.8%, and the CeramX group an AFR of 2.5% (p > 0.05). Regarding anatomical form and marginal adaptation, significant alterations were observed in the CeramX group after 3-years (p < 0.05). The changes in the color match were observed in each group over time (p < 0.05). Conclusions: The use of SDR demonstrated good clinical durability in deep Class II resin composite restorations.

Correlations of obesity and elevated interleukin-6 levels with length of stay in COVID-19 patients in Korea: a retrospective study (국내 COVID-19 입원 환자의 비만, 상승된 인터루킨-6와 입원기간의 상관관계)

  • Purpose: The purpose of this study was to investigate the associations of obesity and clinical characteristics including interleukin-6 (IL-6) with hospital length of stay (LOS) in coronavirus disease-2019 (COVID-19) patients in Korea. Methods: A retrospective descriptive study design was employed to analyze medical data from a government-designated hospital in a city of Korea. Clinical data were collected from 256 patients with COVID-19 in negative-pressure isolation wards in 2021. The following parameters were analyzed: body mass index (BMI), IL-6 levels, age, sex, comorbidities, healthy habitsat the time ofadmission, and LOS. The statistical package SPSS 26.0 was used for descriptive statistics, the independent t-test, the chi-square test, and partial correlation coefficients. Results: The age of COVID-19 patients was positively correlated with BMI (r = -.16, p = .012), IL-6 levels (r = .14, p = .022) and LOS (r = .26, p < .001). Obesity, non-drinking, hypertension, and older age were associated with longer LOS. Conclusion: These results suggest that age, obesity, and hypertension in COVID-19 patients are related to LOS. Studies investigating other factors that can affect long-term hospitalization in COVID-19 patients are suggested.

Technical Consideration of Endovascular Treatment for Aortoiliac Occlusive Disease Based on a 10-Year Tertiary Hospital Experience: A Retrospective Study (10년간의 3차 의료기관 경험을 바탕으로 한 대동맥장골동맥 폐색 질환 혈관내 치료의 기술적 고찰: 후향적 연구)

  • Purpose To examine the technical considerations of endovascular treatment for aortoiliac occlusive disease (AIOD) based on a 10-year experience in Songklanagarind Hospital. Materials and Methods This retrospective cohort study included 210 patients who underwent endovascular treatment for symptomatic AIOD between January 2010 and December 2020. The patients' clinical and lesion characteristics, including technical considerations of the procedure, were collected, analyzed, and stratified using the Transatlantic Inter-Society Consensus (TASC). Results Most patients (80%) in this study had chronic limb-threatening ischemia lesions, with an occlusion rate of 37%. The technical success rate of TASC C & D was lower than that of TASC A & B, 84.4% vs. 99.2% p ≤ 0.001. A technical success rate of 93.3% (14/15) was found for the femoral and brachial approach, compared with a success rate of 89.0% (57/64) for the unibifemoral approach in TASC C & D, without a statistically significant difference (p = 0.076). However, the puncture site complications in this route were up to 17.6%, which is the highest rate compared with other techniques. These complications could be treated either conservatively or minimally invasively. Conclusion In cases of failed femoral access, simultaneous femoral and brachial approaches improved the technical success rate of endovascular recanalization of TASC C & D aortoiliac occlusions.
