Complications of the retromandibular transparotid approach for low condylar neck and subcondylar fractures: a retrospective study

  • Objectives: The goal of this study was to evaluate the rates of complications, morbidity, and safety with the transparotid approach. Materials and Methods: A retrospective study was conducted and consisted of 53 surgically treated patients in the past five years for low condylar neck and subcondylar fractures. Only patients with malocclusion and who underwent open reduction with internal fixation with the retromandibular transparotid approach were included. The examined parameters were postoperative suboptimal occlusion, deflection, saliva fistula, and facial nerve weakness. Results: Fifty-three patients had an open reduction with internal fixation on 55 sides (41 males, 77.4%; mean age, 42 years [range, 18-72 years]). Four patients (7.5%) experienced transient facial nerve weakness of the marginal mandibular branch, but none was permanent. Four patients had a salivary fistula, and 5 patients showed postoperative malocclusion, where one needed repeat surgery after one year. One patient showed long-term deflection. No other complications were observed. Conclusion: The retromandibular transparotid approach is a safe procedure for open reduction and internal fixation of low condylar neck and subcondylar fractures, and it has minimal complications.

자기공명영상에서 나타난 만성 족근관절 외측 불안정성 동반 병변 (Associated Lesions of Magnetic Resonance Image in the Chronic Lateral Ankle Instability)

  • Purpose: This retrospective study was designed to determine the type and frequency of associated lesions in patients with chronic lateral ankle instability who had modified Brostrom lateral ankle ligament reconstruction. Materials and Methods: Between 2004 and 2007, 60 cases of 60 patients were enrolled in this study. A retrospective review of the magnetic resonance images of the affected ankle was conducted by two orthopedic surgeons who did not get any information about intraoperative findings and the lesions were admitted when two doctors were coincident. Results: The overall incidence of associated lesions found in this study was about 83%. Peroneal tenosynovitis was the highest frequency (32%), followed by osteochondral lesion of talus (28%), anterolateral impingement (15%), Os subfibula (13%), Os trigonum (12%), ankle synovitis (12%), anterior tibiofibular ligament tear (15%), anterior bony spur (7%). Another findings were loose bodies (5%), flexor tendon tenosynovitis (5%), medial osteophyte (3%). Conclusion: Identifying these associated lesions will be helpful in treating chronic lateral ankle instability especially when the surgeon have a plan to operate the instability. We suggest that the better results can be obtained when the associated lesions are corrected simultaneously.

경항통으로 한방병원에 입원치료한 환자 459명에 대한 후향적 연구 (The Retrospective Medical Charts Review of 459 Neck Pain Inpatients in Korean Medicine Hospital)

  • Objectives The purpose of this study was to investigate the characteristics of inpatients who were admitted to Korean medicine hospital due to neck pain. Methods Four hundred fifty nine neck pain patients who were admitted to Cheonan Korean Medicine Hospital, Dae-jeon University from 1st, January, 2013 to 31th, December, 2013 were retrospectively reviewed according to medical charts. Results In this study, people in their 30s, females, within 0~2 weeks after onset, motivated by traffic accident, diagnosed with neck sprain, with no medical history related to neck pain, underwent outpatient treatment at medical institutions, took the highest percentage from each category of patients who admitted to Korean Medicine Hospital due to neck pain. In most (78.77%) of the patients, symptoms were more than improved. Conclusions Neck pain inpatients in Korean medicine hospital, it is difficult to find tendency. This study suggests the characteristics of neck pain inpatients who were hospitalized in Koean medicine hospital.

Clinical and Clinicopathological Findings of Naturally Occurring Hyperadrenocorticism in Dogs: A Retrospective Study of 17 Cases

  • A retrospective study of 17 dogs with hyperadrenocorticism (HAC) was performed to evaluate the clinical and clinicopathological outcomes in dogs with pituitary-dependent HAC or adrenal gland-dependent HAC. Sixteen dogs were monitored at day 10, 30 and 90 then every 3 month after first presentation. In each examination, physical examination and ACTH stimulation test and client questionnaire were performed. In 17 cases, eight dogs were successfully treated with mitotane therapy (median dose 16.7 mg/kg; range 5.3 to 50 mg/kg, SID); another one dog showed favorable response, but the clients declined to further treatment before the full control of the disease had been achieved; four dogs showed poor response to treatment. One dog was euthanized due to septicemia. These results showed canine HAC can be medically managed successfully without side effects at the doses used in this study.

A retrospective study of mandibular fractures in children

  • Objectives: The incidence and patterns of mandibular fractures vary by country and population age. This retrospective study evaluated the etiologies and patterns of mandibular fractures in children. Materials and Methods: The clinical records of 89 children (45 males and 44 females) aged 0 to 12 years who presented with mandibular fractures from July 2012 to June 2016 were retrospectively reviewed. The sex, patient age, site of fracture, etiology of trauma, and monthly variations of the fractures were recorded. Descriptive statistics, the z-test and chi-square test were used for statistical analysis and the P-value was set at <0.05. Results: Eighty-nine children (male-to-female ratio 1.02:1) sustained 131 mandibular fractures. Within the study sample, the 6 to 9 year age group most frequently experienced fractures (47.3%). Falls and road traffic accidents (RTA) were the two most common etiological factors that accounted for 44.9% and 24.7% of cases. The condylar fracture was the most common anatomical location (38.9%) followed by the angle (20.6%), parasymphysis (18.3%), body (15.3%), and symphysis (5.3%). A single fracture (51.7%) was more common than multiple fractures (48.3%). The month-wise distribution of mandibular fractures was fairly constant. Conclusion: The condylar region was the most common anatomic site for fractures; in addition, a fall and RTA were the major etiological factors for mandibular fractures. A single fracture was observed in 51.7% of patients while multiple fracture lines accounted for 48.3% of cases.

혈액투석 환자의 도관 기능부전 관리 프로토콜 적용이 도관개존 성공률 및 출혈 합병증에 미치는 영향 (Effect of Evidence-Based Practice Protocol for Catheter Dysfunction in Hemodialysis Patients: Focus on Patency Rates and Bleeding Complications)

  • Purpose: This study was a retrospective cohort study to evaluate the effect of evidence-based guidelines for catheter dysfunction among hemodialysis patients, Success rate and bleeding complications in catheterization were examined. Methods: We performed a retrospective cohort study, including 94 patients with catheter dysfunction who were receiving hemodialysis at a university hospital; 55 in the control group and 39 in the protocol group. This protocol was composed of the catheter dysfunction assessment, conservative management of catheter dysfunction, drug management of catheter dysfunction, catheter function test and maintenance management. Data were analyzed with a Chi-square test and t-test using SPSS/WIN 23.0 program. Results: The patency rate of the protocol group was significantly higher than that of the control group. The rate of bleeding complications was not statistically significant for either the control group or the protocol group before or after application. Conclusion: The application of the guidelines effectively increased the patency rate of the catheter without bleeding complications. It is hoped that this guideline can be disseminated to nurses nationwide to improve the efficiency of catheter function for hemodialysis patients.

슬관절 반월상 연골 파열 후 수술 환자의 통증 감소에 대한 한의학적 치료 효과: 후향적 관찰연구 (Effects of Korean Medicine Treatments on Pain Reduction of Postoperative Meniscal Injury Patients : A Retrospective Observational Study)

  • The objective of this study is to observe the effectiveness of complex Korean Medicine treatments on pain reduction of patients after meniscal surgery. We analyzed 16 patients who have been diagnosed as Meniscal injury on Magnetic Resonance Imaging(MRI) scan and had admitted to Dunsan Korean Medical hospital from January 30, 2014 to April 30, 2019. The analysis was conducted as retrospective study which analyzes patient's medical records. Statistical analysis was performed using the IBM SPSS statistics 25 program. We used Numeric Rating Scale to evaluate pain reduction, Five-point likert scale to asses treatment satisfaction. The result showed that there were statistically significant pain reduction on the postoperative meniscal injury patients treated with complex Korean Medicine treatments. In conclusion, we found that the complex Korean Medicine treatments showed a positive effect on pain reduction of postoperative meniscal injury patients. Further research is required to confirm the effectiveness of Korean Medicine treatment.

A retrospective comparison of 3 approaches of vestibuloplasty around mandibular molar implants: apically positioned flap versus free gingival graft versus modified periosteal fenestration

  • Purpose: This study aimed to clinically evaluate the efficacy of vestibuloplasty around lower molar implants using 3 different modalities: apically positioned flap alone (APF), APF with a free gingival graft (FGG), and APF with modified periosteal fenestration (mPF). Methods: Three different vestibuloplasty procedures during second-stage implant surgery were performed at the mandibular molar area in 61 patients with a shallow vestibule and insufficient keratinized tissue (KT). The clinical measurements of KT width were recorded at baseline, immediately after surgery (T0), 6 months after surgery (T6), and 12 months after surgery (T12). Soft tissue esthetic scores were measured. Results: An additional KT width gain from baseline to T12 of approximately 2 mm was obtained with FGG and mPF compared to that with APF. Shrinkage of the re-established tissue was lower with mPF and FGG than with APF, whereas the esthetic profile was better with APF and mPF than with FGG. Conclusions: Within the limitations of this study, mPF showed potential as a promising approach for vestibuloplasty around the lower molar implants compared to the traditional APF and FGG.

Analysis of factors related systemic recurrence after breast conserving surgery in stage I breast cancer

  • Objectives: In these days, patients with stage I breast cancer have increased by regular health examination and diagnostic tool development. The aim of this retrospective study is to identify systemic recurrence related factors after breast conserving surgery (BCS) for stage I breast cancer. Methods: In this study, we analyzed the correlation between systemic recurrence and pathologic factors. We reviewed 223 patients who underwent BCS for stage I breast cancer. Postoperative pathologic factors, recurrent rates and sites were studied. In addition, preoperative patients'data were also collected. Statistical analysis was done by using PASW 16.0 (SPSS Inc., Chicago, IL, USA). Results: Systemic recurrence was found in 16 patients (7.17%) within 5 years after primary surgery. 5 patients had lymphatic invasion and 6 patients had vascular invasion. Lymphatic and vascular invasion had statistical correlation with systemic recurrence (P = 0.004, P = 0.001). Conclusions: In this retrospective study, we can conclude that vascular invasion and lymphatic invasion are related systemic recurrence after BCS for stage I patients. Further studies with large cohort will be required to fully understand the risk factors of systemic recurrence for stage I breast cancer patients.

Effectiveness of dental implantation with the partial split-flap technique on vertical guided bone regeneration: a retrospective study

  • Purpose: This study aimed to evaluate the effectiveness of the partial split-flap technique with a K-incision on vertical guided bone regeneration (vGBR) and to retrospectively analyze the clinical and radiographic outcomes of dental implantation using this approach. Methods: In total, 78 patients who received 104 dental implants with vGBR, categorized as (1) pre-GBR and post-implantation and (2) simultaneous GBR and implantation, were enrolled. Data analysis was based on periapical radiographs, clinical photos, and dental records. The 2-sample t-test was used to compare the 2 surgical procedures. Results: The baseline vertical bone level, augmented bone height (ABH), and treatment duration were significantly higher in the pre-GBR procedure group. The survival rates of the implants were 96.1% and 94.8% in implant- and patient-based analyses, respectively. In Cox regression analysis, high rates of implant failure were found in the presence of ABH of ≥4 mm, smoking, and diabetes. Conclusions: Within the limitations of this retrospective study, the partial split-flap technique using a K-incision for vGBR showed stable clinical outcomes and favorable dental implant survival.
