Radiographic Features of Traumatically Caused Retroflexion of Urinary Bladder in Two Dogs

  • The urinary bladder retroflexion, flipped over backwards into the pelvic canal, by trauma has been rarely reported in dogs. This paper describes clinical and radiological features of urinary bladder retroflexion in two dogs with a history of traffic accident. The main clinical signs were hind limb lameness, severe pain and dysuria. Radiography and ultrasonography were performed to evaluate the patient's damages. No remarkable findings were observed except pelvic fracture on survey radiograph. On cystourethrography, caudal displacement and retroflexion of the bladders were identified. Cystopexy was performed and the bladders were repositioned. After surgery one dog with severe azotemia and bilateral hydroureteronephrosis was deteriorated noticeably and euthanized by request of the owner, while the other dog was recovered no recurrence of the problem by 12 months post-surgery. It should be considered that contrast study and serial assessment in urinary tract be important to demonstrate the evidence of bladder retroflexion and evaluate the prognosis in dogs with severe trauma.

An acoustic study of fricated vowels in Nuosu Yi: an exploratory study

  • Fricated nuclei in Nuosu Yi were found to be more correctly described as fricated vowels, rather than syllabic fricatives due to the presence of clear formant structures typical of front vowels. In this exploratory study, two types of fricated nuclei were examined: retroflex "yr" and non-retroflex "y". The retroflex nucleus "yr" had higher F1 and lower F3 than non-retroflex "y", indicating a lower tongue height. On the other hand, F2 was found to correlate not with nucleus retroflexion, but instead with onset consonant retroflexion: F2 was higher following retroflex onsets, in both vowels. This effect was persistent through the entire vowel, suggesting a phonological effect, rather than a coarticulatory one. Interpretation of the F2 results require accompanying articulatory data since the usual coupling of F2 and tongue backness does not always hold for retroflex vowels. Examining the articulation of the fricated nuclei in Nuosu Yi is a direction for future research.

Articulatory and Acoustic Correlates of Korean /1/

  • This study investigated the articulatory and acoustic correlates of Korean /1/. In particular, direct comparison between Korean /1/ and English /1/ was made to evaluate the current assumption about Korean /1/ such that Korean /1/ is phonetically similar to English clear /1/. The present study revealed that Korea /1/ is different from English /1/ in several properties. First, F2 for Korean /1/ is around 600-700 Hz higher than F2 for English /1/. The overall higher F2 for Korean /1/ is attributed to the fact that Korean /1/ involves tongue body raising while it lacks a dorsal gesture. Second, F3 value for Korean /1/ becomes significantly lower when the preceeding vowel is a back vowel. This kind of variable F3 pattern was not observed in English /1/. The current study relates the F3 lowering to the retroflexion of Korean /1/ in the back vowel context.

자궁경부암의 방사선치료후 대장 및 직장합병증에 대한 분석 (Bowel Complication after Radiotherpy of Uterine Cervix Carcinoma)

  • 1979년부터 1986년까지 자궁경부암으로 진단되어 외부방사선 및 강내 방사선 치료를 함께 받은 550명의 환자를 대상으로 직장 합병증과 방사선량에 대한 후향적 분석을 시행하였다. 전체 환자550명 중 468명은 근치적 목적으로 방사선 치료를 받았으며, 82명은 수술후에 추가적인 방사선 치료를 받았다. 이들 82명 중 43명은 수술절제연 양성으로, 31명은 원발질환의 재발로, 8명은 stump cancer로 방사선 치료를 받았다. Grade 2와 3를 포함하는 직장 합병증의 발생률은 5년에 $6.7였다. 합병증이 생긴 환자군의 직장에 조사된 방사선량은 $7424{\pm}834$ cGy이었으며, 이는 합병증이 발생하지 않은 환자군의 $6946{\pm}717$ cGy보다 많았고 통계학적 유의성이 있었다(p<0.05) 직장합병증의 정도에 따른 방사선량의 차이는 통계학적 유의성이 없었다(p>0.05). 전체 합병증의 발생률은 직장에 조사된 방사선량에 따라 증가하였는데, 6,500 cGy 이하에서는 5년 합병증 발생률이 15.6이었으며 8,000 cGy 이 상 조사된 환자군에서는 71.2이었다. 직장 합병증에 영향을 줄 수 있는 요인들을 Cox의 방법에 의해 다변량분석한 결과 당뇨가 있는 경우 합병증 발생률이 증가하였으며, 강내 방사선치료는 한번 시행한 경우보다 두번 시행한 환자군에서 합병증 발생률이 더 적었다. 또한 자궁의 후방만곡은 ovoid의 크기와 함께 분석된 경우에 통계적으로 중요한 의미를 나타내었고, ovoid의 크기는 이번 분석에서 합병증 발생에 가장 중요한 인자로 나타났다. 직장에 조사된 방사선량도 중요한 요소로서 방사선량이 많아질수록 합병증 발생률은 증가하였다(p<0.05). TDF와 CRE단위로 분석하였으며 선량과 합병증의 관계는 cGy 단위의 결과와 같았다.

근접치료 시 외부 방사선 치료 전후의 MR 영상 비교분석 (Comparison Analysis of MR Images Before and After External Beam Radiotherapy in Brachytherapy)

  • 목 적: 자궁경부암 환자의 방사선 치료는 일반적으로 외부 방사선 치료 후 근접치료를 시행하는데, 외부 방사선 치료 후 종양의 위치나 부피가 변화하므로 근접치료 시 이전의 MR 영상만을 참고하여 applicator 삽입을 할 경우 많은 오류가 발생할 수 있다. 이에 본원에서는 외부 방사선 치료 전후의 MR 영상 비교를 통해 근접치료 시 발생할 수 있는 applicator 삽입 오류를 분석하여 외부 방사선 치료 후 MR 영상의 유용성에 대해 알아보고자 하였다. 대상 및 방법: 2009년 1월부터 2010년 12월까지 Fletcher CT/MR applicator를 사용한 자궁경부암 환자 35명을 대상으로 외부 방사선 치료 전후에 촬영한 MR 영상을 비교하였다. 결 과: MR 영상 분석 결과 외부 방사선 치료 후 종양이 줄어들면서 전굴이었던 자궁이 후굴로 바뀌게 된 경우가 17.1%, tandem 삽입시 방향이나 길이 등이 잘못된 경우가 14.3%로 나타났다. 결 론: 외부 방사선 치료 후 MRI를 촬영하면 줄어든 종양과 함께 정확한 자궁의 위치와 길이를 확인할 수 있어 근접치료 시행 시 applicator 삽입으로 인해 발생할 수 있는 오류를 줄여 환자에게 최적의 치료를 시행할 수 있었다.

초기 구강설암 환자에서 부분 설절제술 후 조음변화 (Articulation Changes after Partial Glossectomy in Patients with Early Tongue Cancer)

  • Backgrounds and Objects: Carcinoma of the tongue is the most common cancer of the oral cavity. A primary treatment strategy includes surgery and/or radiotherapy. Resection of the tongue often results in speech dysfunction, which depends on the site and extent of resection, type of reconstruction, and the mobility of remaining tongue. This study aimed to evaluate the characteristics of articulation errors that were resulted from the partial glossectomy without free flap reconstruction. Materials & Method : Articulation evaluations including speech intelligibility and percent of correct consonants (PCC) were performed for 24 patients who underwent partial glossectomy for their T1 or T2 tongue cancer. Mobility of the tongue, size of the resected tongue, and the history of adjuvant radiotherapy were analyzed for their relationship with the results of articulation evaluation. Results: Speech intelligibility score was $6.4{\pm}0.9$ (on 7-point scale) and overall PCC was 96.9%. There were close relationships between the size of resection and limitations in the tongue mobility, especially in "protrusion and elevation (r=-0.687)" and "retroflexion (r=-0.775)". Errors in "alveolar fricatives" and "palatal affricates" were also closely related with the size of resection (r=-0.537 and -0.538, respectively). PCC for "liquid sound" /r/ was 83.2%, which was closely related with the history of radiation therapy. Conclusion : Overall articulatory function was satisfactory in cases of early tongue cancer after partial glossectomy of a limited volume without flap reconstruction. However, the size of resection and the history of radiation therapy were closely related to the limitations in some types of tongue mobility and the resultant articulation errors.
