광중합 복합레진 수복시 여러 광조사 방법에 따른 미세변연누출에 관한 연구 (MICROLEAKAGE OF CL V COMPOSITE RESTORATIONS USING VARIOUS LIGHT CURING METHODS)

  • The purpose of this in vitro study was to evaluate the microleakage of 5 curing methods in class V composite restorations which are composed of two-step light curing, pulse-delay cure, low curing-light intensity, moderate curing-light intensity and high curing-light intensity. In this study, class V cavities were prepared on buccal and lingual surfaces of 50 extracted human upper or lower molars on cementum margin. Single Bond adhesive and Z-100 shade A2 were applied for each group following the manufacture's instruction. The experimental teeth were randomly divided into 5 groups of 10 samples (20 surfaces) each. Group 1: two-step light curing; Group 2: pulse-delay cure; Group 3: low curing-light intensity; Group 4: moderate curing-light intensity; Group 5: high curing-light intensity. After 500 thermocycling between $5^{\circ}C$ and $55^{\circ}C$, the 60 teeth were placed in 2% methylene blue dye for 24 hours, then rinsed with tab water. The specimens were embedded in clear resin, then sectioned buccolingually through the center of restoration with a low speed diamond saw. The dye penetration on each of the specimen was then observed with a stereomicroscope at ${\times}20$. The composite resin/tooth interfaces were examined under Scanning Electron Microscopy. The results were statistically analyzed using the Kruskal-Wallis One Way ANOVA and Dunn's Method. The results of this study were as follows. 1. In all groups, the leakage values seen at the enamel margin were significantly lower than those seen at the dentin margin(P<0.05). 2. No group in this study showed significant differences in leakage values at both the enamel and the dentin margins(P<0.05). 3. In all groups, the gaps seen at the enamel margin were significantly lower than those seen at the dentin margin(P<0.05). 4. The gaps in this study showed significant differences and two-step light-curing and low curing-light intensity produced significant less gap than high curing-light intensity(P<0.05).

구치부 레진 수복 재료의 가수분해 (HYDROLYTIC DEGRADATION OF POSTERIOR RESIN RESTORATIVE MATERIALS)

  • 우수한 심미성 수복재료로서 복합레진은 그 사용 빈도가 증가하고 있다. 이런 증가 추세에도 불구하고 복합레진의 부적절한 마모저항성 때문에 구치부 수복에서의 사용이 제한되어왔다. 이와 관련된 인자로 수복물의 표면하 분해가 고려되고 있다. 본 연구에서는 복합레진의 마모에 미치는 환경적 분해의 효과를 알기 위해 알카리성 용액(0.1N NaOH)에 현재 많이 사용되는 Definite($Degussa-H\ddot{u}ls$ AG, Germany), Prodigy(Kerr, USA), Pyramid(Bisco, USA) 및 Synergy(Coltene, Swiss) 등 4종의 복합레진을 보관하였을 때 각 제품의 분해과정을 평가하고자 하였다. 각 제품 당 3개의 시편을 제작한 후 0.1N NaOH용액에 저장하여 $60^{\circ}C$에서 보관하였다. 2주 후 제거하여 HCl로 중화, 세척 후 $60^{\circ}C$에서 건조하였다. 무게 손실, 분해층 깊이, Si농도 등을 기준으로 분해저항성을 평가하여 다음과 같은 결과를 얻었다. 1. 무게 손실은 Synergy에서 $1.24{\pm}0.002%$로 가장 높은 값을 보였으나 각 제품간 유의한 차이는 보이지 않았다. 2. 분해층 깊이는 Synergy에서 $107.83{\pm}2.52{\mu}m$로 가장 높은 값을 보였고, Synergy를 제외한 다른 제품에서는 유의한 차이를 보이지 않았다. 3. Si 용출량에 있어서는 4가지 제품 모두 차이를 보이지 않았다. 4. 무게 손실과 분해층 깊이 사이에는 높은 상관 관계를 보였다(r=0.6127, p<0.05). 5. 무게 손실과 Si 용출량, 분해층 깊이와 Si 용출량 사이에 상관 관계는 없는 것으로 나타났다. 6. 주사전자현미경 관찰시 NaOH 용액에 보관한 후 레진 기질과 필러 사이의 결합 파괴를 관찰할 수 있었다.

Micro-CT를 이용한 복합 레진 수복물 미세 누출도의 정량 분석 (Quantitative Micro-CT Evaluation of Microleakage in Composite Resin Restorations)

  • 치과 수복 재료 검사 시 가장 중요하고 기본적인 것은 치아와 수복물 사이의 미세누출에 대한 평가이다. 미세 누출을 평가하는 방법은 여러 가지가 있지만 이들 대부분은 많은 단점들을 가지고 있다. 최근에 개발된 미세 단층촬영법(micro-CT)을 이용하면 시편을 비파괴적으로 처리하여 특정 밀도에 해당하는 부분의 3차원적 영상을 얻을 수 있으므로, 이를 이용하면 정확하고 정량적인 미세누출 평가도 가능할 것이다. 이 연구의 목적은 micro-CT를 사용하여, 레진 수복물의 미세 누출도를 정량적이고 비파괴적으로 측정할 수 있는 새로운 방법을 찾아내고, 이 새로운 방법을 기존의 색소 침투법과 비교해보는 것이다. 이를 위해, 위의 두 가지 방법을 사용하여 두 종류의 상아질 접착 시스템의 미세누출도가 평가되었다. 사람의 건전 소구치 40개를 임의로 20개씩 두 군으로 나누고 다음과 같이 처리하였다. Group 1 : $Adper^{TM}$ Singe Bond 사용 후 제 V급 와동 레진 수복, Gourp 2 : $Adper^{TM}\;Promp^{TM}$ L-pop 사용 후 제 V급 와동 레진 수복 모든 치아의 5급 와동은 $Filtek^{TM}$ Supreme으로 수복하였다. 그 후 각 군 중 10개의 치아는 micro-CT를 사용하여 미세 누출도를 평가하고, 나머지 10개는 기존의 색소 침투법으로 평가하였다. 이 연구의 결과는 다음과 같았다. 1. Micro-CT를 사용한 경우, 1군은 2군보다 통계적으로 유의성 있게 적은 양의 미세 누출을 보였다(p<0.01). 2. 기존의 색소 침투법을 사용한 경우에서 1군은 2군보다 적은 미세누출을 보였으며 이는 통계적으로 유의성이 있었다. (p<0.01). 3. 두 군간의 미세 누출도는 micro-CT를 사용한 방법에서 더 현저한 차이를 보였다. 4. 두 가지 방법 모두에서 법랑질 변연 부분보다 상아질 변연 부분에서 더 많은 미세 누출을 보였다.

Tricalcium Silicate-Based 치수복조재에 대한 수 종 수복재의 전단결합강도 비교 (Comparison of Shear Bond Strength of Different Restorative Materials to Tricalcium Silicate-Based Pulp Capping Materials)

  • 본 연구의 목적은 Tricalcium silicate를 기반으로 한 3종의 치수복조재(Theracal $LC^{TM}$, $Biodentine^{TM}$, $ProRoot^{TM}$ white MTA)와 임상에서 통상적으로 사용되는 수복재인 복합레진, 레진강화형 글래스아이오노머 시멘트, 그리고 전통적인 글래스아이오노머 시멘트 간의 전단결합강도를 비교 평가하는 것이다. 이를 위해 중심구를 가진 아크릴 레진 블록 90개를 제작하고 각각 30개씩 3종의 치수복조재로 중심구 안에 채운 다음, 다시 무작위로 10개씩 하위 군을 나누어 치수복조재 상방에 복합레진, 레진강화형 글래스아이오노머 시멘트, 전통적인 글래스아이오노머 시멘트를 적용하여 총 9개 군으로 이루어진 시편을 제작하였다. Universal testing machine을 이용해 전단결합강도를 측정한 뒤, 입체현미경을 사용해 파절양상을 분석하였다. 연구 결과, Theracal $LC^{TM}$-복합레진 군이 가장 높은 전단결합강도를, Theracal $LC^{TM}$-전통적인 글래스아이오노머 시멘트 군이 가장 낮은 전단결합강도를 나타냈다. 복합레진이 수복재로 선택된 군들이 다른 수복재 군에 비해 높은 전단결합강도를 보였다. 전통적인 글래스아이오노머 시멘트가 수복재인 경우 $Biodentine^{TM}$이 다른 치수복 조재 군들에 비해 높은 전단결합강도를 나타냈다.

  • 전치부에 발생한 치간이개는 심미적으로 문제를 일으키는 흔한 원인이 된다. "Black triangle"을 생성시키지 않고 치간이 개를 폐쇄시키는 것은 심미치과학에서 도전적 과제 중 하나이다. 전치부 수복치료의 성공 여부는 연조직과 경조직간의 심미적인 연합에 달려있다. 이번 증례에서는 치은-치아 계면에서 자연스러운 외형을 가지는 출현윤곽(emergence profile)을 형성하여 줌으로써 치은의 재생 과정이 일어나 심미적으로 만족스러운 치간이개 폐쇄를 이루어내었기에 이를 보고하는 바이다.

Stress distribution in premolars restored with inlays or onlays: 3D finite element analysis

  • PURPOSE. To analyze stress distribution in premolars restored with inlays or onlays using various materials. MATERIALS AND METHODS. Three-dimensional maxillary premolar models of abutments were designed to include the following: 1) inlay with O cavity (O group), 2) inlay with MO cavity (MO group), 3) inlay with MOD cavity (MOD group), and 4) onlay (ONLAY group). A restoration of each inlay or onlay cavity was simulated using gold alloy, e.max ceramic, or composite resin for restoration. To simulate masticatory forces, a total of 140 N static axial force was applied onto the tooth at the occlusal contact areas. A finite element analysis was performed to predict the magnitude and pattern of stresses generated by occlusal loading. RESULTS. Maximum von Mises stress values generated in the abutment teeth of the ONLAY group were ranged from 26.1 to 26.8 MPa, which were significantly lower than those of inlay groups (O group: 260.3-260.7 MPa; MO group: 252.1-262.4 MPa; MOD group: 281.4-298.8 MPa). Maximum von Mises stresses generated with ceramic, gold, and composite restorations were 280.1, 269.9, and 286.6 MPa, respectively, in the MOD group. They were 252.2, 248.0, 255.1 MPa, respectively, in the ONLAY group. CONCLUSION. The onlay design (ONLAY group) protected tooth structures more effectively than inlay designs (O, MO, and MOD groups). However, stress magnitudes in restorations with various dental materials exhibited no significant difference among groups (O, MO, MOD, ONLAY).

불소도포가 상아질 접착에 미치는 영향 (EFFECT OF FLUORIDE APPLICATION ON DENTIN BONDING)

  • The purpose of this study was to investigate the effect of stannous fluoride on the dentin bonding with three kinds of commercially available dentin bonding systems containing different adhesive monomers. Dentin specimens with exposed labial dentin prepared from freshly extracted bovine mandibular anterior teeth were divided into experimental and control groups. The specimens of experimental groups were bonded with dentin bonding systems and composite resins including All bond 2 ㅡ& Bisfil, Scotchbond Multi-Purpose & Z100, and Denthesive II Charisma after 2 % stannous& fluorided application for S minutes and washing for 1 minute. The specimens of control groups were bonded with the same dentin bonding systems and composite resins as used in the experimental groups. After bonded specimens were stored in $37^{\circ}C$ distilled water for 24 hours, the tensile bond strength and cohesive failure rate were measured, and then the pretreated dentin surfaces and the fractured dentin surfaces were examined under scanning electron microscope. The results were as follows : Mean bond strength of stannous fluoride applied groups of All bond 2, Scotchbond MP, and Denthesive II were 2.5MPa, 1.1MPa, and 1.1MPa respectively, and those of control groups were 7.5MPa, 8.1MPa, and 4.6MPa. Bond strength values of stannous fluoride applied groups were significantly lower than those of the control groups(p<0.05). SEM findings of dentin surfaces after stannous fluoride application demonstrated an appearance of partially remained smear layer and smear plugs inspite of pretreatment with 10 % phosphoric aicd or maleic acid solution, and an appearance of smear layer covered surface under Denthesive II priming. But those of control groups commonly showed clean dentin surfaces without smear layer and smear plugs. On SEM observation of the fractured dentin-resin interface, while most of the specimens of stannous fluoride applied groups showed adhesive failure mode, those of All bond 2 and Scotchbond MP control groups showed mainly adhesive-cohesive mixed failure mode, and mainly adhesive failure mode in Denthesive II control group.

  • The purpose of present study was to evaluate the polymerization shrinkage stress and amount of linear shrinkage of composites and compomers for posterior restoration. For this purpose, linear polymerization shrinkage and polymerization stress were measured. For linear polymerization shrinklage and polymerization stress measurement, custom made Linometer (R&B, Daejon, Korea) and Stress measuring machine was used (R&B, Daejon, Korea). Compositers and compomers were evaluated: Dyract AP (Dentsply Detrey, Gumbh. German) Z100 (3M Dental Products, St. Paul. USA) Surefil (Dentsply Caulk, Milford, USA) Pyramid (Bisco, Schaumburg, USA) Synergy Compact (Coltene, Altstatten, Switzerland), Heliomolar (Vivadent/Ivoclar, Liechtenstein), and Compoglass (Vivadent Ivoclar/Liechtenstein) were used. 15 measurements were made for each material. Micro-CT evaluation of internal adaptation in resin fillings with different dentin adhesives

  • Objectives: The purpose of present study was to evaluate the internal adaptation of composite restorations using different adhesive systems. Materials and Methods: Typical class I cavities were prepared in 32 human third molars. The teeth were divided into the following four groups: 3-step etch-and-rinse, 2-step etch-and-rinse, 2-step self-etch and 1-step self-etch system were used. After the dentin adhesives were applied, composite resins were filled and light-cured in two layers. Then, silver nitrate solution was infiltrated, and all of the samples were scanned by micro-CT before and after thermo-mechanical load cycling. For each image, the length to which silver nitrate infiltrated, as a percentage of the whole pulpal floor length, was calculated (%SP). To evaluate the internal adaptation using conventional method, the samples were cut into 3 pieces by two sectioning at an interval of 1 mm in the middle of the cavity and they were dyed with Rhodamine-B. The cross sections of the specimens were examined by stereomicroscope. The lengths of the parts where actual leakage was shown were measured and calculated as a percentage of real leakage (%RP). The values for %SP and %RP were compared. Results: After thermo-mechanical loading, all specimens showed significantly increased %SP compared to before thermo-mechanical loading and 1-step self-etch system had the highest %SP (p < 0.05). There was a tendency for %SP and %RP to show similar microleakage percentage depending on its sectioning. Conclusions: After thermo-mechanical load cycling, there were differences in internal adaptation among the groups using different adhesive systems.

필러의 실란처리농도가 복합레진의 특성에 미치는 영향 (INFLUENCES OF SILANE CONCENTRATION FOR FILLER SILANIZATION ON THE PROPERTIES OF COMPOSITES)

  • The purpose of this study was to search the optimal silane concentrations for filler- silanization of seven experimental composites. Silica filer was a 25micron crushed type. 0.0%, 0.5%, 1.0%, 1.5%, 2.0%, 2.5%, and 3.0% silane($\gamma$-methacrylooxypropyltrimethoxysilane)were added into silica-filler with weight percentage (wt%). Mixtures(silica filler/silane)were reacted at 6$0^{\circ}C$ for 72hours, and crushed into fine particles those were used as fillers for 7 experimental composites. Monomer was a 3 : 1 mixture of Bis-GMA and TEGDMA containing 0.2% tertiary amine and 0.4% camphoroquinone for light curability. A ratio for mixing the monomer and filler was 75% and 25% respectively. Seven experimental composites was classified with the concentration of silane treated, and the specimen number for each test was 10. Specimens with 6mm diameter and 3mm height dimension for measuring the diametral tensile strength were destroyed with 1mm/min cross-head speed on Instron universal testing machine (No. 4467, USA). Shear bond strength was measured on the specimens bonded to bovine enamel etched with 37% phosphoric acid solution for 1 minute Fractured surfaces were observed by SEM (Hitachi S-3200, Japan) among that of the highest values measured from each groups. Following results were obtained: 1. Experimental composites containing silanized filter showed the significantly higher diametral tensile strength and shear bond strength than the composites containing un-silanized fillers(Group1) (p<0.05). 2. In silanized filler composite resins(Group 2~7), Diametral tensile strength of Group 3 showed the significantly higher than that of Group 2 and Group 6(p<0.05). 3. Shear bond strength was higher in Group 3 than that of Group 7 (p<0.05)in silanized fillers composite resins. 4. Fracture surface was formed in resin matrixes on the specimens from composites containing the fillers treated with 0.5% 1.0%, and 1.5% silane. These results mean that the optimal silane concentrations are exist for each fillet with its size and surface area, and that 1.0% is a optimal value for concentration to coat the 25$\mu\textrm{m}$ filler with silane.
