Respiratory Review of 2014: Tuberculosis and Nontuberculous Mycobacterial Pulmonary Disease

  • Since tuberculosis (TB) remains a major global health concern and the incidence of multi-drug resistant (MDR)-TB is increasing globally, new modalities for the detection of TB and drug resistant TB are needed to improve TB control. The Xpert MTB/RIF test can be a valuable new tool for early detection of TB and rifampicin resistance, with a high sensitivity and specificity. Late-generation fluoroquinolones, levofloxacin, and moxifloxacin, which are the principal drugs for the treatment of MDR-TB, show equally high efficacy and safety. Systemic steroids may reduce the overall TB mortality attributable to all forms of TB across all organ systems, although inhaled corticosteroids can increase the risk of TB development. Although fixed dose combinations were expected to reduce the risk of drug resistance and increase drug compliance, a recent meta-analysis found that they might actually increase the risk of relapse and treatment failure. Regarding treatment duration, patients with cavitation and culture positivity at 2 months of TB treatment may require more than 6 months of standard treatment. New anti-TB drugs, such as linezolid, bedaquiline, and delamanid, could improve the outcomes in drug-resistant TB. Nontuberculous mycobacterial lung disease has typical clinical and immunological phenotypes. Mycobacterial genotyping may predict disease progression, and whole genome sequencing may reveal the transmission of Mycobacterium abscessus. In refractory Mycobacterium avium complex lung disease, a moxifloxacin-containing regimen was expected to improve the treatment outcome.

Effect of Bergamot Essential Oil-Inhalation on Chronic Pain after Surgery for Lumbar Spinal Stenosis (버가못 에센셜 오일 향흡입이 요추관협착증 환자의 수술 후 만성통증에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of a bergamot essential oil-inhalation on chronic pain after surgery for lumbar spinal stenosis. Methods: Fifty-two subjects were randomly assigned to the experimental (bergamot essential oil-inhalation) and control (almond oil-inhalation) group. All patients understood the purpose of this study and a written informed consent for the study was obtained. Results: Bergamot essential oil-inhalation was conducted during twenty minutes. Visual analog scale, blood pressure, heart rate, and respiratory rate were measured before and after treatment in the two groups. The visual analog scale score, heart rate, and respiratory rate in bergamot essential oil-inhalation group were significantly lower than those in almond oil-inhalation group. However, there was no significant difference in systolic and diastolic blood pressure between the two groups. Conclusion: Therefore, bergamot essential oil-inhalation is effective in alleviating pain, heart rate, and respiratory rate. These results suggest that bergamot essential oil-inhalation can be a useful method for the relief of chronic pain during the treatment after spinal surgery.

Analysis of Respiratory Gas by Training on Healthcare Indoor Bicycle (헬스케어용 실내 자전거 운동에 의한 호흡가스 분석)

  • This study was conducted to observe the change of limbs stroke and respiration gas parameters in our new bicycle fitness system. We hypothesized that the variable force of left and right limbs might be effective for sensing stimulation in modified new unequal pedal bicycle system. It has been developed, which can provide visual information and different length of pedal with left and right limbs. Experimental results showed different activities between the left and right limbs where the activity of the left limb increased than that of right limb. Especially, the soleous muscle activity increased both in control and experimental groups by this training method. But oxygen and carbon dioxide partial pressures in respiratory gas increased during training method. These results suggest that acidosis of blood was led by this process. Consequently, this bicycle training is concluded that aerobic training could affect different limb activities. Finally, we expect that our new bicycle system will be effective for healthcare with proper balance between the left and right limbs.

Anesthetic and Cardiovascular Effects Induced by a Combination of Midazolam and Thiopental in Dogs (개에 있어서 Midazolam과 Thiopental 병용 투여시 마취 및 심순환기계에 미치는 영향)

  • Thiopental sodium is known as ultrashort-acting barbiturates and can be employed advantageously for numerous conditions. But thiopental has the side effects of cardiovascular and respiratory systems which has barbiturates and are depend on the dose of thiopental. The side effects are reduced when the thiopental is preceded by a tranquilizer and sedative. In these drugs, benzodiazepines have the minimal effects of cardiovascular and respiratory systems. In this study, the effects of midazolam preanesthetic administration, followed by thiopental anesthetic induction, on cardiovascular system and thiopental induction requirement were studied in 14 mixed breed dogs. Cardiovascular data were recorded baseline, after premedication of saline 0.45 ml/kg or midazolam 0.1, 0.2, 0.4, 0.8 mg/kg, intubation, and 5, 10, 15, 20, 30 minutes after intubation. Extubation, head-up, sternal recombency, standing, and walking recovery times were recorded. The results were summarized as follows; (1) The 0.1, 0.2, 0.4, and 0.8 mg/kg dosages of midazolam insignificantly decreased thiopental dose requirement necessary to accomplish intubation by 6, 20, 21 and 28%. (2) The 0.1, 0.2, 0.4, and 0.8 mg/kg dosages of midazolam insignificantly reduced the times of extubation, head-up, sternal recumbency, standing, and walking recovery. (3) Midazolam was effective in reducing the frequency and duration of arrhythmia after intubation. (4) Heart rates of preanestheic midazolam administraion groups increased after thiopental injection which insignificantly changed smaller than those of control group. (5) Arterial blood pressures did not vary significantly among groups.

Application of Extracorporeal Membranous Oxygenation in Trauma Patient with Possible Transfusion Related Acute Lung Injury (TRALI) (수혈 관련 급성 폐손상이 동반된 외상환자에서 체외막 산화기의 적용 경험)

  • The case of a patient with a transfusion-related acute lung injury (TRALI) to whom extracorporeal membrane oxygenation (ECMO) had been applied is reported. A 55-year-old male injured with liver laceration (grade 3) without chest injury after car accident. He received lots of blood transfusion and underwent damage control abdominal surgery. In the immediate postoperative period, he suffered from severe hypoxia and respiratory acidosis despite of vigorous management such as 100% oxygen with mechanical ventilation, high PEEP and muscle relaxant. Finally, ECMO was applied to the patients as a last resort. Aggressive treatment with ECMO improved the oxygenation and reduced the acidosis. Unfortunately, the patient died of liver failure and infection. TRALI is a part of acute respiratory distress syndrome (ARDS). The use of ECMO for TRALI induced severe hypoxemia might be a useful option for providing time to allow the injured lung to recover.

Differences of Chest and Waist Circumferences in Spastic Diplegic and Hemiplegic Cerebral Palsy

  • Purpose: Circumference of the chest and waist can be one of clinical indicator to reflect respiratory function in children with cerebral palsy. In this study, we compared to differences in the chest/waist circumference and maximal phonation time between children with spastic diplegia and hemiplegia. Methods: Seventeen children with spastic diplegic and hemiplegic cerebral palsy were recruited, who were matched to gender, age, height, weight, and body mass index for control of the known factors affected to respiratory function. The chest/waist circumference and were measured in each group, when children took a breath at rest and at maximal voluntary inspiration/expiration. Results: No significant differences were found in the chest and waist circumference and expansion between the two groups. However, only in the waist expansion, children with diplegic CP were significantly lower extensibility of lung, compared to the other group. In comparison of the maximal phonation time, a significant lower score was shown in children with spastic diplegic CP, compared to children with hemiplegic CP. Conclusion: Our results indicated that children with spastic diplegic CP had smaller chest wall and waist, compared to children with spastic hemiplegic CP. In addition, they showed a shorter time for sustaining phonation than spastic hemiplegic CP did. Therefore, spastic diplegic CP will be required for careful monitor regarding respiratory function in rehabilitation settings.

Epidural Ketamine for Control of Postoperative Pain (수술후 경막외강내에 주입한 케타민의 진통효과)

  • In recent years the use of epidural opiates has increased and although this method of pain relief has shown good results in clinical practice It is still subject to certain drawbacks, the most serious of which appears to be delayed respiratory depression. Since ketamine administered systemically is unlikely to produce respiratory depression it seemed worthwhile to investigate the possibility of exploiting the potent analgesic property to ketamine by its epidural administration. The analgesic effect of ketamine 4 mg, administered epidural space, was evaluated. The duration of pain relief varied from less than 3 hours in 20% to over 24 hours in 30% of the cases. In 62.5% of the cases pain relief exceeded 6 hours. There was no evidence of respiratory depression, and there no postoperative neurologic sequelae. The present results indicated the need for further studies to compare the efficacy and safety of epidural ketamine with the response to epidural opioids for the relief of postoperative pain.

  • Today swine respiratory disease is one of the most important diseases because of its economic losses and severe infection nationwide, and swine society as well as veterinary service are trying to prevent the diseases in Korea. This study would like to obtain some information useful for the control of the diseases. A total of 174 lung specimens with lesion consisted of 3 sorts; 60 were collected from nursey pigs requested for diagnostic service from March of 2006 to October of 2007, 58 finishing pigs and 56 sows were selected from slaughterhouse from September to November 2007. In the detection test of pathogens by PCR, porcine circovirus 2 (PCV2), porcine reproductive and respiratory syndrome virus (PRRSV) and Mycoplasma hyopneumoniae were positive in 95.4%, 31.6%, and 20.1%, respectively. Double infection rate with PCV2 and PRRS was 30.4%, PCV2 and M hyopneumoniae was 19.5%, triple infection with PCV2, PRRS and M hyopneumoniae was 5.7%, respectively.

Biochemical characteristics and antimicrobials susceptibility of pathogenic Streptoccus spp. isolated from respiratory tract of Thoroughbred horses in Jeju, Korea (제주지역에서 사육중인 더러브렛 말 호흡기로부터 분리된 병원성 Streptoccus spp.의 생화학성상 및 약제감수성 양상)

  • This study carried out to investigate the pathogenic Streptococcus spp. isolated from respiratory tract of Thoroughbred in Jeju province. The specimens were collected from nasal mucosa using a culture swab from 113 Thoroughbred horses. Suspected colonies were selected onto blood and MacConkey agar plate, and identified by standard biochemical properties using Vitek 2 system and PCR method. In this study, we isolated S. equi (n=6), S. zooepidemicus (n=31), S. equisimilis (n=5), S. dysgalactiae (n=2), S. agalctiae (n=1), non identified ${\beta}$-hemolytic Streptococcus spp. (n=1) from Thoroughbred horses. In antimicrobial susceptibility test, it showed a high sensibility in the most of antimicrobial except for neomycin, streptomycin, spectinomycin, erythromycin and clindamycin. These results will provide the basic information to establish control measures for the treatment and prevention of respiratory disease by pathogenic Streptococcus spp. in Thoroughbred horses in Korea.

The 6 case reports of chronic upper respiratory infections with high fever in children, treated with Daqinglong-tang (대청룡탕(大靑龍湯)의 투여로 고열이 호전된 소아 만성 상기도감염증 환자 6례(例)에 대한 증례 보고)

  • There are many children suffering from chronic upper respiratory infections with high fever(CURIHF) for years. High fever mainly caused by tonsillitis comes from cold and flu season-related. Observing CURIHF children, they also have a night sweat(盜汗) and a thick voice, a chronic cough, a loss of appetite in common. I found that after administering the Daqinglong-tang(大靑龍湯) to the 6 CURIHF children, high fever was dropping quickly and did not recur for 233~733 days. Also, the symptoms of a night sweat(盜汗) and a thick voice, a chronic cough, a loss of appetite are improved significantly. These case report suggests that in case of CURIHF, Daqinglong-tang could effectively control recur of high fever in children.
