A respiration measurement system for vital signs was developed. Respiration signals were measured, processed, and analyzed. Four electrodes, attached on the surface of the skin, were used to monitor respiration signals by impedance pneumography. The measured signals were amplified, detrended, filtered, and transferred toan embedded module. The Kalman filter was used to remove motion artifact from the respiration signals. Experiments were conducted at stable condition and walking condition to evaluate the performance of the system. Respiration rates of five males and five females were measured and analyzed at each condition. The referenced respiration signal was determined by temperature of nose surroundings. The results showed that the respiration rates at the walking condition had more motion artifacts than the stable condition. The accuracies of the respiration measurement system with Kalman filter were found as 96% at the stable condition and 95% at the walking condition. The results showed that the Kalman filter was an effective tool to remove the motion artifact from the respiration signal.
호흡 움직임은 종양에 대한 방사선치료의 부정확한 방사선조사가 유도되도록 복부 및 흉부에서 움직임을 야기한다. 그러므로 치료시 이러한 움직임에 대한 정확한 계산은 정상조직에는 저 선량을 조사되도록 CTV의 마진을 줄일 수 있고 방사선치료에 발생되는 부작용을 줄일 수 있다. Intrafraction motion을 고려하는 기술로는 호흡 멈춤, 호흡동조, 4차원 또는 종양추적 기술이 있다. 호흡동조 방법은 환자의 호흡 신호가 호흡주기의 일정한 범위에 위치할 때 주기적으로 빔을 조사하고 그 범위를 벗어날 때는 빔을 조사하지 않는 방법이다. 이러한 기술은 내부장기 움직임과 상관관계가 있는 호흡 움직임 신호의 획득이 요구된다. 호흡동조 방사선치료를 위한 예비연구로 본 연구자들은 호흡 움직임 신호 측정을 수행하였다. 환자의 호흡 움직임 신호 측정을 위해서, 호흡측정시스템을 3 가지 센서, 4 채널 데이터 획득 시스템, 신호처리용 컴퓨터로 구성하였다. 2명의 환자를 대상으로 본 연구자들은 호흡측정시스템을 가지고 호흡주기 및 파형을 평가하였다. 그 결과 호흡주기는 시간의 함수에 따라 일반적으로 정확한 대칭 형태는 아니지만 주기적인 형태로 측정되었다. 호흡측정 시스템은 기대했던 만큼 환자의 호흡을 잘 추적하였으며 실험에 적용하기 위해 쉽게 컨트롤 할 수 있었다.
This study proposes a method to improve the sleep stage and efficiency estimation of sleep apnea patients using a UWB (Ultra-Wideband) radar. Motion and respiration extracted from the radar signal were used. Respiratory signal disturbances by motion artifacts and irregular respiration patterns of sleep apnea patients are compensated for in the preprocessing stage. Preprocessing calculates the standard deviation of the respiration signal for a shift window of 15 seconds to estimate thresholds for compensation and applies it to the breathing signal. The method for estimating the sleep stage is based on the difference in amplitude of two kinds of smoothed respirations signals. In smoothing, the window size is set to 10 seconds and 34 seconds, respectively. The estimated feature was processed by the k-nearest neighbor classifier and the feature filtering model to discriminate between the sleep periods of the rapid eye movement (REM) and non-rapid eye movement (NREM). The feature filtering model reflects the characteristics of the REM sleep that occur continuously and the characteristics that mainly occur in the latter part of this stage. The sleep efficiency is estimated by using the sleep onset time and motion events. Sleep onset time uses estimated features from the gradient changes of the breathing signal. A motion event was applied based on the estimated energy change in the UWB signal. Sleep efficiency and sleep stage accuracy were assessed with polysomnography. The average sleep efficiency and sleep stage accuracy were estimated respectively to be about 96.3% and 88.8% in 18 sleep apnea subjects.
Pulse oximetry, which monitors non-invasively the oxygen saturation in blood, is influenced by patient's respiration, movement or a factor of an environment. Specially, it's difficult to measure a PPG (Photoplethsmography) signal from the moving patient because of the motion artifact. Accordingly, it is required to extract the pure PPG signal from the PPG signal to measure oxygen saturation. In this paper, we propose an adaptive noise canceller to improve the performance of motion artifact removal. Then we design a hardware system for real time monitoring of the oxygen saturation. The proposed algorithm estimates the slope of transition rate between two different wavelength signals.
IR-UWB radar has been regarded as the most promising technology for non-contact respiration and heartbeat monitoring because of its ability of detecting slight motion even in submillimeter range. Measuring heart rate is most challenging since the chest movement by heartbeat is quite subtle and easily interfered with by a random body motion or background noise. Additionally, periodic sampling can be limited by the performance of computer that handles the radar signals. In this paper, we deploy Lomb-Scargle periodogram method that estimates heart rate even with irregularly sampled data and uneven signal amplitude. Lomb-Scargle periodogram is known as a method for finding periodicity in irregularly-sampled and noisy data set. We also implement a motion detection scheme in order to make the heart rate estimation pause when a random motion is detected. Our scheme is implemented using Novelda's X4M03 radar development kit and its corresponding drivers and Python packages. Experimental results show that the estimation with Lomb-Scargle periodogram yield more accurate heart rate than the method of measuring peak-to-peak distance.
최근 IT+BT 융합 기술로 건강관리 및 응급의료 등을 위하여 U-헬스케어 단말 장치가 개발되고 상품화가 되고 있다. 이 단말기에서 측정하는 생체 신호들은 심전도, 체온, 산소포화도, 심박수, 호흡 등이며, 특히 이들 신호 중에 광용적맥파(PPG) 신호는 산소포화도와 심박수, 말초혈관 탄성도 등을 측정함에 있어 매우 중요한 신호이다. 그러나 이 PPG 신호는 환자 또는 사용자의 움직임에 따라 발생하는 동잡음의 영향에 의해 그 정밀도가 저하된다. 따라서 본 연구에서는 이러한 동잡음을 제거하기 위한 광변조 기법과 ICA 기법을 제안한다. 제안된 기법의 성능을 분석하기 위해 다양한 잡음을 인위적으로 가하여 실험하였으며, 실험 결과를 분석한 결과에서 제안된 기법이 기존의 적응 필터법 보다 우수한 성능을 보였다.
This study focusses on the development of a motion-sensing smart leggings prototype with the help of a module that monitors motion using a fiber-type stretch sensor. Additionally, it acquires data on Electrocardiogram (ECG), respiration, and body temperature signals, for the development of smart clothing used in online exercise coaching and customized healthcare systems. The research process was conducted in the following order: 1) Fabrication of a fiber-type elastic strain sensor for motion monitoring, 2) Positioning and attaching the sensor, 3) Pattern development and three-dimensional (3D) design, 4) Prototyping 5) Wearability test, and 6) Expert evaluation. The 3D design method was used to develop an aesthetic design, and for sensing accurate signal acquisition functions, wearability tests, and expert evaluation. As a result, first, the selection or manufacturing of an appropriate sensor for the function is of utmost importance. Second, the selection and attachment method of a location that can maximize the function of the sensor without interfering with any activity should be studied. Third, the signal line selection and connection method should be considered, and fourth, the aesthetic design should be reflected along with functional verification. In addition, the selection of an appropriate material is important, and tests for washability and durability must be made. This study presented a manufacturing method to improve the functionality and design of smart clothing, through the process of developing a prototype of motion-sensing smart leggings.
To appropriately control or compensate breathing motion of targets in thorax or abdomen during radiotherapy is still demanding. Our idea is that a visual signal may help regulate patient's breathing pattern, by controlling its amplitude and cycle. The system involving breathing control with a visual signal for aperture maneuver with controlled breath (AMC) has been developed. A thermocouple is used to detect the temperature change due to patient's breathing. The system also consists of a mask, in which the thermocouple is installed, an operational amplifier, a converter, etc. Patients were instructed to control their respiration by breathing following the visuals signal, as watching a display that shows both patients' current breathing pattern and the signal. The patterns of patients' controlled breathing and the signals coincided well. Therefore, when AMC technique is applied, a target moves in the range that is 60 % less than the range of free breathing motion with the help of the system and so target margins can be reduced significantly. This study reveals that a visual signal is not only useful to control patient's breathing but also clinically effective.
본 논문에서는 소의 사료 취식량(Feed Intake), 반추(Rumination), 발정기(In Heat) 모니터링 기술을 이용하여, RFI(Residual Feed Intake) 모니터링 및 신진대상 이상을 예측하는 웨어러블 디바이스 및 PC용 웹과 스마트폰 어플리케이션을 이용한 모니터링 시스템을 설계하고 구현하였다. 본 시스템의 개발로 농장주는 경제적 효율성의 증가가 기대된다. 사료 섭취량을 분석하면, 소의 체중에 근거한 추천 사료량과 소가 섭취하는 사료량과의 차이를 확인할 수 있으며, Metabolic disorder(신진대사 이상)에 대한 조기 발견이 가능할 것으로 예상된다. 본 논문의 결과물을 사용하는 농장주는 가장 효율적인 성과를 나타내는 소를 구별할 수 있으며, 소의 표피(목)에 부착하는 웨어러블 장치로부터 입력되는 6축 모션 센서 신호와 웨어러블 장치에 부착된 마이크를 통해 입력되는 소의 목넘김 소리를 통해서 소의 반추와 사료섭취량을 측정할 수 있다. 향후에는 심박, 호흡 등의 추가적인 생체신호를 측정할 수 있도록 개선 작업을 진행할 예정이다.
목 적 : 폐암환자의 호흡동조 방사선 치료 계획 시 호흡 훈련 전후 RPM 신호와 횡격막 위치 변화를 분석하여 호흡 훈련의 유용성을 평가하고자 한다. 대상 및 방법 : 2016년 4월부터 8월까지 호흡 동조 방사선 치료를 받는 환자 11명을 대상으로 호흡 훈련을 시행하였고 동시에 RPM 신호 및 횡격막 영상을 획득하였다. 호흡 훈련은 총 3단계로 1단계 자유 호흡 상태의 신호 획득, 2단계 호흡 신호 가이드를 통한 1차 호흡 신호 획득, 3단계 설명과 반복 훈련으로 규칙성과 안정을 유도한 최종 호흡 신호를 획득 하였다. 각 단계의 흡기와 호기시 RPM 신호와 투시 영상의 횡격막 위치의 평균값, 표준편차, 최대값, 최소값을 구하고, 이를 1단계 값으로 표준화 하여 2, 3단계를 상대분포 백분율(%)로 변환하여 환자의 호흡 변화와 내부 움직임을 분석 함으로써 각 환자의 호흡훈련 유용성을 평가 하였다. 결 과 : RPM 신호와 횡격막 진폭을 측정한 뒤, 1단계를 100%으로 표준화하여 각 단계의 평균값과 표준편차의 오차 평균을 구하였다. 그 결과, 3단계 최종호흡 획득 시 진폭의 상대평균 및 표준편차 모두 감소가36.4%, 표준편차만 감소가 18.2%, 진폭만 감소가 36.4%로 나타났으며, 횡격막 영상의 위치 측정 시 3단계에서 전체 81.8%의 환자에게서 상대평균 진폭 값이 30% 감소함을 보였다. 그러나 모든 환자들에게서 2단계 대비 3단계의 RPM 신호와 횡격막 진폭이 각각 평균 52.6%, 42.1% 감소함을 보였다. 또한, RPM 신호와 횡격막 영상 진폭 차이의 연관성은 2번 10번 환자를 제외하고 각각 1, 2, 3단계 움직임의 패턴이 상관관계를 보였다. 결 론 : 호흡 동조 방사선치료에서 호흡 훈련을 시행하였을 때 최적화된 호흡 주기를 유도할 수 있었으며, 모의 호흡 훈련을 치료 전 시행함으로써 불규칙적인 호흡에 의한 환자의 호흡을 제어해 폐의 움직임을 예측 가능 하게 해주는 효과를 기대할 수 있었다. 궁극적으로는 방사선 치료의 체계적 오류를 최소화해 보다 정확한 치료를 기대할 수 있어 호흡 훈련이 유용하다고 할 수 있겠다.그러나 본 연구는 치료 전 호흡 훈련을 시행한 자료를 바탕으로 분석한 연구로 제한되어 있으며 추후 실제 CT 계획과 치료 시 획득한 자료를 가지고 검증하는 것도 필요할 것으로 사료된다.
