Massive Rotator Cuff Tear Repair (광범위 회전근 개 파열의 봉합술)

  • Purpose: Anatomical repair of massive rotator cuff tear has been technically challenging because of medial retraction, muscle atrophy and fatty degeneration. Among several treatment options for massive rotator cuff tear, we reviewed rotator cuff repairs and investigated modalities for improvement of clinical outcomes, decreasing the re-tear rate, and increasing healing. Materials and Methods: Patient-related factors and rotator cuff-related factors were the two major groups of factors we considered when choosing a treatment plan. Results: Mobilization of a massive rotator cuff tear was increased by soft tissue release and by the interval slide technique. After meticulous soft tissue release, anatomical repair could be achieved. If the injury was not amenable to anatomical repair, alternative treatment options such as partial repair, the margin convergence technique and augmentation with a tenotomized biceps tendon were considered. Many reports of massive rotator cuff repair demonstrated satisfactory clinical outcomes, decreased pain, recovery of shoulder functions, and increases in muscle strength. However, the re-tear rate had been reported to be relatively high in long-term follow-up. Conclusion: Despite a high re-tear rate after massive rotator cuff repair, a better understanding of the pathogenesis, progression and clinical symptoms of massive rotator cuff tear and improved surgical materials and techniques will lead to satisfactory clinical outcomes.

  • The maintenance techniques of reinforced concrete structures, which include diagnosis, repair and rehabilit¬ation, are not systematically establishtxL This study develops an expert system for maintenance of reinforced concrete structures based on the related documents and knowledge and experience of experts who work in these field. The analytical results using the developed expert system were compared to the real repair cases in field and they were generally satisfactory.

  • Ear reconstruction is very difficult to perform and often results in a devastating deformity. The use of microsurgical replantation techniques has allowed very favorable anesthetic results. We report a case of a partial ear replantation without venous repair with the use of medicinal leeches to decompress the acute venous congestion during the postoperative care. The medicinal leech therapy can be very useful in a partial ear replantation in cases with no venous repair.

  • Recently, the self-healing technique is being investigated to repair the damaged polymeric composites by the use of microcapsules with the healing agent. This technique can obtains both the damage detection and the damage repair simultaneously over the converntional repairing techniques. In this study, the effects of the catalyst ratio to the healing agent and thermal characteristics to the mixtures of healing agent are investigated through single lap shear tests and DSC. The Healing agents such as DCPD, ENB, and their mixtures are considered and Grubb's catalyst is used as a catalst.

  • As the multidirection and posterior instabilities of the shoulder are not only uncommon but responds well to conservative treatment, the shoulder instabilities which requires surgical treatment are traumatic anterior type in most cases, Although various surgical procedures had been used in the past, Bankart procedure is the standard surgical method as a primary procedure in traumatic anterior instability, Nevertheless there has been changes in the techniques of Bankart procedure in order to minimize decrease of external rotation and effectively address capsular laxilty, Capsular shift might be needed if there remains excessive capsular laxity of the inferior capsule after repair of the Bankart lesion, Large bony Bankart lesion should be fixed if possible and severe glenoid rim erosion requires extracapsular bone block after repair of the capsule. Although a few surgical procedures are described for the management of Hill-Sachs lesion in special circumstances, Hill-Sachs lesion does not usually need to be addressed.

  • Corrosion of reinforced concrete structures in marine environment is one of the most important mechanism of deterioration. However, conventional rehabilitation techniques in tidal zone, which consist of removing delaminated areas of concrete, cleaning affected steel and patching with portland cements mortar, have proven to be ineffective for marine structures. Also, repairs are often repeated every several years. The purpose of this report is to announce appropriate repair method of highway bridge damaged by chloride attack in marine environment (application of cathodic protection) using FRP and antiwashout underwater mortar.

  • The Sundt encircling clip was developed to repair defects of the vessel wall. With the advent of microvascular techniques, most parts of the damaged vessel wall during aneurysm surgery can be repaired by primary closure or by the bypass technique. However, these methods are not always successful. Here, we illustrate two cases of surgical clipping with the Sundt encircling clip in the ruptured internal carotid artery trunk aneurysm. The Sundt clip provides prompt control of unexpected tearing of the vessel wall or aneurysm and plays an important role in vascular rescue during aneurysm surgery.

  • The incidence of foreign body insertion in the anorectum is rare, however, it is increasing, especially in urban populations. Foreign objects in the anorectum can be of different sizes, shapes and materials. Frangible objects like glass or beakers, and sharp foreign bodies that may easily injure the bowel mucosa are particularly dangerous. Physicians have to consider more innovative options on how to extract these foreign bodies without inciting injury; and, if injury would occur, consider different techniques to repair it, whether transanally or transabdominally, and opt for primary repair, or resection with either anastomosis or stoma creation. Here, I introduce our cases with colorectal foreign bodies and present several literatures to help physicians decide when presented with cases like these.

  • Acute subluxation of the peroneal tendon is an uncommon injury which is predominantly associated with a trauma. Traumatic peroneal tendon subluxation in athletes is an uncommon cause of ankle pain. As a result, the diagnosis is often delayed. Numerous surgical techniques have been described for chronic peroneal subluxation or dislocation, however reports in acute cases are rare. Authors experienced two cases of acute subluxation of peroneal tendon in athletes which were treated by simple primary repair of superior peroneal retinaculum with good results.

Partial Tear of Upper Portion of Subscapularis (견갑하근 상 1/3 파열)

  • An isolated tear of the subscapularis is uncommon, and there are a few literatures regarding the treatment of this problem. But, the incidence has increased with development of the arthroscopic techniques. An all-arthroscopic rotator cuff repair is a challenging procedure that can be effectively performed for treatment of subscapularis tendon tears. Often, tears of the subscapularis tendon do not involve entire tendon, and retraction of the torn edge is within to 2 cm of its attachment site. Occasionally, the entire tendon is torn and retracted medially to the glenoid. This article outlines the examination, preoperative planning and details the steps necessary to perform this procedure on upper third of subscapularis tears.
