FRACTURE TOUGHNESS OF SELF-CURING DENTURE BASE RESINS WITH DIFFERENT POLYMERIZING CONDITIONS (의치상용 자가중합레진의 중합조건에 따른 파괴인성)

  • Purpose. The intent of this study was to evaluate the effects of curing conditions on self-curing denture base resins to find out proper condition in self-curing resin polymerization. Materials and methods, In this study, 3 commercial self-curing denture base resins are used Vertex SC, Tokuso Rebase and Jet Denture Repair Acrylic. After mixing the self curing resin, it was placed in a stainless steel mold(3$\times$6$\times$60mm). The mold containing the resin was placed under the following conditions: in air at 23$^{\circ}C$; or in water at 23$^{\circ}C$; or in water at 23$^{\circ}C$ under pressure(20psi); or in water at 37$^{\circ}C$ under pressure(20psi) or in water at 50$^{\circ}C$ under pressure(20psi) , or in water at 65$^{\circ}C$ under pressure(20psi), respectively. Also heat-curing denture base resin is polymerized according to manufactures' instructions as control. Fracture toughness was measured by a single edge notched beam(SENB) method. Notch about 3mm deep was carved at the center of the long axis of the specimen using a dental diamond disk driven by a dental micro engine. The flexural test was carried out at a crosshead speed 0.5mm/min and fracture surface were observed under measuring microscope. Results and conclusion . The results obtained were summarized as follows : 1. The fracture toughness value of self-curing denture base resins were relatively lower than that of heat-curing denture base resin. 2. In Vertex SC and Jet Denture Repair Acrylic, higher fracture toughness value was observed in the curing environment with pressure but in Tokuso Rebase, low fracture toughness value was observed but there was no statistical difference. 3. Higher fracture toughness value was observed in the curing environment with water than air but there was no statistical difference. 4. Raising the temperature in water showed the increase of fracture toughness.

Surgical Treatment of Ruptured Achilles Tendon - End-to-end suture & Paratendinous wrapping (아킬레스건 파열의 수술적치료 -단단 봉합술 및 건외막피복술-)

  • Purpose: To evaluate the results and prognosis of operative repair to acute rupture of achilles tend on associated sports injury. Materials and Methods: 21 cases were surgically treated and average follow-up period was 1 year and eight months. The forth decade was most common with $55\%$ and soccer was most common in sports with 5 cases. End-to-end suture of ruptured achilles tendon was performed, and paratendinous structure was wrapped sufficiently. Postoperatively. ankle was plantarflexed for 6 weeks with longleg cast. And then 2 weeks interval, short leg cast with equinous position was conversed to functional position. About 10 weeks after operation, ankle was recovered to right angle. Hooker scale was used to evaluate the results. Results: Compared to normal side, heel-floor distance of ruptures side was decreased 0.7 cm in average, and 0.8 cm was deceased after 20 times weight loaded dorsiflexion. Mid-calf circumference was deceased 0.3 cm, and active dorsiflexion and plantar flexion of ankle was decreased each 3 and 5degree. 16 cases showed ‘excellent’result and 5 cases showed ‘satisfactory’. There was no complication, such as re-rupture or infection at operation site. Conclusion: After end-to-end operative repair to achilles tendon, sufficient wrapping of paratendinous structure is efficient for healing and prevention of postoperative adhesion. And serial dorsiflex-ion cast change is considered to be a successful treatment for preventing residual equinus deformity.

  • The concrete used most in construction materials. There is an overcrowded iron dimensions use of the concrete at time of the other concrete theory on the reinforcing rod back which did congestion and compares it with this, and there are more few dimensions of the aggregate than concrete, and quantity of aggregate passage is superior in mortar than concrete. If a volume rate of the aggregate writes mortar than concrete against this, therefore, unit amount increases, and quantity of paste increases and quantity of dry shrinkage than increase concrete. However, I let I regulate lay priest distribution of the aggregate, and the results rates increase and reduce unit amount and decrease quantity of dry shrinkage, and separation resistance and the gap passage characteristics are judged because it can be it in a substitute document of very superior concrete. I came to carry out the study that I watched to let I was useful a little more and do the improvement repair of a become building wall body, a basement pillar and repair reinforcement of the assistant in the reinforcing rod back, the old age when I made congestion here. I regulated lay priest distribution of the aggregate in the study and regulated substitution rate of the aggregate (40%, 50%, 60%) and divided W/C 30%, 40% standards and produced mortar and I compared quantity of air by this, slump, compression robbery and showed it this time.

Clinical Analysis of Patch Repair of Ventricular Septal Defect in Infant (영아기 심실중격결손 봉합술의 임상적 고찰)

  • Background: Simple ventricular septal defect(VSD) is the most common congenital heart disease. Although closure of VSD is currently associated with a relatively low risk, experience with younger and smaller infants has been variably less satisfactory. We assessed the results of surgical closure of VSD in infant. Materials and Methods: Between 1996 and 2000, 45 non-restrictive VSD patients underwent patch repair and retrospective analysis was done. Patients were divided into two groups based on weight: group I infants weighed 5kg or less(n=16), and group II infants weighed more than 5kg(n=29). Both groups had similar variation in sex, VSD location, aortic cross clamp time and total bypass time. But combined diseases (ASD, PDA, MR) were more in group I. We closed VSD with patch and used simple continuous suture method in all patients. Results: There were no operative mortality, no reoperation for hemodynamically significant residual shunt and no surgically induced complete heart block. As a complication, pneumonia(group I: 2 cases, group II: 2 cases), transient seizure(group II: 2), wound infection(group I: 1, group II: 1), urinary tract infection(group I: 1) and chylopericardium(group I: 1) developed, and there was no significant difference between two groups(p>0.05). Conclusion: Early primary closure with simple continuous suture method was applicable in all patients with non-restrictive VSD without any serious complications.

Arthroscopic Treatment with Suture Anchor for the Isolated Subscapularis Tear (견갑하근 단독 손상 시 시행한 관절경하 봉합 나사를 이용한 봉합술)

  • Purpose: We wanted to evaluate the results of arthroscopic management of an isolated rupture of the subscapularis tendon using suture anchors. Materials and Methods: Twenty nine patients with unilateral ruptures of the subscapularis tendon and who underwent arthroscopic repair between February 2001 and October 2007 were reviewed prospectively. All the cases were isolated tears of the subscapularis without the involvement of any other rotator cuff tendon. In 19 patients the tear was localized to the superior one third, in 7 cases the tear was localized to the upper two thirds and the entire tendon was involved in 3 cases. The mean follow up period was 12.2 months (range: 6-26 months). The results of the treatment were assessed by evaluating the constant shoulder score and the pain score before surgery and after surgery. Results: The constant shoulder score improved from $49.9\pm7.3$ to $73.8\pm3.0$ points compared to before surgery and the pain score improved from $4.3\pm3.2$ to $11.2\pm3.0$ points postoperatively. Conclusion: Arthroscopic repair of isolated ruptures of the subscapularis tendon using suture anchors is a good option for effectively managing these tears.

A Cephalometric Study of Lateral Morphologic Features in Adult Cleft Lip and Palate Patients (구순 구개열 환자의 성장후 측모형태에 관한 두부계측방사선학적 연구)

  • Background: Cleft lip and palate deformity have unknown patterns of maxillofacial growth and development. The maxillofacial growth can be affected either by congenital or environmental factors such as infection and trauma. Surgical repair of cleft lip and palate may interfere the subsequent growth and development of maxillofacial region. The purpose of this study is to evaluate the characteristics of maxillofacial growth patterns in adult cleft lip and palate patients. Materials and Methods: The material for this study consisted of 17 adult male patients with cleft lip and palate. Cephalometric tracing and measurements were done by one investigator. The relationship between 17 cleft lip and palate patients and Korean norms were evaluated statistically. Results: There were statistically differences in Na. perpendicular to point A, SNA angle, effective maxillary length, maxillofacial differencial. Wit's appraisal and upper incisor to point A(p < 0.01). Pogonion to Na. perpendicular also statistically differed(p < 0.05). Other measurements didn't statistically differ. Conclusion: It was evident that in adult cleft lip and palate patients, maxilla was retruded and short. Careful cleft lip and palate repair and treatment are recommended for facilitating normal growth of maxilla.

Effect of Micro-bolt Reinforcement for Composite Scarf Joint (복합재 스카프 조인트에서의 마이크로 볼트 보강에 대한 타당성 연구)

  • The reinforcement effect of micro-bolt for a bonded scarf joint was investigated. Three scarf ratios of 1/10, 1/20, and 1/30 were considered to examine the effect of scarf patch configuration on joint strength. To maintain the same density of micro-bolt, 16, 32, and 48 bolts were installed in the scarf joint specimens with scarf ratios of 1/10, 1/20, and 1/30, respectively. Tests were also carried out on the joints that are bonded with only adhesive and that are fastened with only micro-bolts to obtain reference values. The average failure loads of the adhesive joints with scarf ratios of 1/10, 1/20, and 1/30 were 29.7, 39.6, and 44.8 kN, respectively. In case of micro-bolt reinforcement, the failure loads at the same scarf ratios were 28.4, 37.2, and 40.1 kN, respectively, which corresponds to 96, 94, and 90% of the pure adhesive joint failure loads. In the case of using only micro-bolts, the failure loads were only 13-25% of the average failure loads of pure adhesive joints. Fatigue test was also conducted for the joints with scarf ratio of 1/10. The results show that the fatigue strength of hybrid joints using both adhesive and microbolts together slightly increased compared to the fatigue strength of adhesive joint, but the rate of increase was small to 2-3%. Through this study, it was confirmed that the reinforcement effect of micro-bolt is negligible in the scarf joints where shear stress is dominating the failure, unlike in the structure where peel stress is dominant.

The Status of Damage and Monitoring of Subterranean Termite (Reticulitermes spp.) (Blattodea: Rhinotermitidae) for Wooden Cultural Heritage in Korea (국내 목조문화재에 대한 지중 흰개미 피해 및 모니터링 현황)

  • In this study, the status of damage by subterranean termites and their management according to the region and type of domestic wooden cultural properties were identified. This was based on the survey reports of agencies conducting regular nationwide and regional monitoring of subterranean termites. In addition, using geographical information system (GIS) based on the survey contents, a map was constructed of termite infestation and its progress on 2,805 wooden cultural properties that were surveyed nationwide. Based on the map produced, a total of 486 cases of termite infestation were confirmed in wooden cultural properties during 2018-2019, of which 143 cases (approximately 29.4%) were confirmed to be owing to the invasion of termites in the ground and infestation of wood materials. A web platform and an application using a mapping application program interface were created to increase accessibility to the investigated damage status data. The methods employed by each institution for investigating and monitoring the invasion of termites in the ground included the use of detection dogs, visual observation, installation of wood specimens made of pine, and microwave equipment. However, it was confirmed that monitoring and survey methods were not applied to determine the territorial range of the subterranean termite colonies. Accordingly, the use of dyeing and mark-release-recapture methods were deemed necessary to understand the current status, such as calculating the scope of the target wooden cultural property, when monitoring subterranean termite colonies.

Comprehensive Study of Microsatellite Instability Testing and Its Comparison With Immunohistochemistry in Gastric Cancers

  • Purpose: In this study, polymerase chain reaction (PCR)-based microsatellite instability (MSI) testing was comprehensively analyzed and compared with immunohistochemistry (IHC) for mismatch repair (MMR) protein expression in patients with gastric cancer (GC). Materials and Methods: In 5,676 GC cases, PCR-based MSI testing using five microsatellites (BAT-26, BAT-25, D5S346, D2S123, and D17S250) and IHC for MLH1 were performed. Reevaluation of MSI testing/MLH1 IHC and additional IHC for MSH2, MSH6, and PMS2 were performed in discordant/indeterminate cases. Results: Of the 5,676 cases, microsatellite stable (MSS)/MSI-low and intact MLH1 were observed in 5,082 cases (89.5%), whereas MSI-high (MSI-H) and loss of MLH1 expression were observed in 502 cases (8.8%). We re-evaluated the remaining 92 cases (1.6%) with a discordant/indeterminate status. Re-evaluation showed 1) 37 concordant cases (0.7%) (18 and 19 cases of MSI-H/MMR-deficient (dMMR) and MSS/MMR-proficient (pMMR), respectively), 2) 6 discordant cases (0.1%) (3 cases each of MSI-H/pMMR and MSS/dMMR), 3) 14 MSI indeterminate cases (0.2%) (1 case of dMMR and 13 cases of pMMR), and 4) 35 IHC indeterminate cases (0.6%) (22 and 13 cases of MSI-H and MSS, respectively). Finally, MSI-H or dMMR was observed in 549 cases (9.7%), of which 47 (0.8%) were additionally confirmed as MSI-H or dMMR by reevaluation. Sensitivity was 99.3% for MSI testing and 95.4% for MMR IHC. Conclusions: Considering the low incidence of MSI-H or dMMR, discordant/indeterminate results were occasionally identified in GCs, in which case complementary testing is required. These findings could help improve the accuracy of MSI/MMR testing in daily practice.

Comparison of the Modified Brostrom Repair Technique with and without Augmentation Using Suture Tape for Chronic Ankle Instability (만성 족관절 불안정성을 가진 환자군에서 변형 브로스트롬 술식과 봉합 테이프를 추가한 술식 간의 결과 비교)

  • Purpose: The modified Broström repair (BR) technique has yielded good outcomes in patients with chronic ankle instability. This study compared clinical and radiological outcomes between two groups of patients who underwent modified BR or lateral ligament augmentation using suture tapes (ST). Materials and Methods: Seventy-seven patients (ST group [n=47], BR group [n=30]; body mass index <26.61 kg/m2; mean age, 30.7±11.0 years [range, 17~39 years]; mean follow-up, 34.0±12.0 months [range, 24~59 months]) were retrospectively reviewed between January 2014 and July 2017. The Foot and Ankle Outcome Score (FAOS), American Orthopedic Foot and Ankle Score (AOFAS), Foot and Ankle Ability Measure (FAAM), visual analogue scale (VAS) score, and Sefton grading system were used for clinical assessment. The talar tilt angle and anterior talar translation were measured using the Telos stress device (Telos GmbH, Marburg, Germany) at 150 N for radiological evaluation. Results: FAOS, AOFAS, FAAM, and VAS scores improved in both groups at final follow-up (ST, 91.1±5.2, 93±2, 88.1±4.5, 1.5±0.7 vs. BR, 91.3±5.4, 93±3, 83.3±4.8, 1.2±0.7, respectively; p=0.854, 0.971, <0.001, 0.04, respectively). According to the FAOS, mean sports activity scores for the ST and BR groups at the final follow-up were 90.3±3.2 and 76.6±4.2, respectively, reflecting superior outcomes in the ST group (p<0.001). Sefton grading revealed satisfactory functional outcomes (ST, 91.5% vs. BR, 90.0%). There was significant improvement in the talar tilt angle and anterior talar translation in both the ST and BR groups (7.6°±1.2°, 10.5±1.8 mm vs. 4.9°±1.1°, 7.9±1.5 mm, respectively; p<0.001). Conclusion: The ST group demonstrated comparable clinical but better improvement in mechanical stability and FAOS sports scores than the BR group.
