Availability Model For The Remanufacturing System and Performance Index (재 제조시스템의 가용도 분석모델과 평가척도)

  • The remanufacturing system is a series of industrial process in which worn-out products are restored to like-new condition. The remanufacturing system is differ from the repair system not only process characteristics but also product characteristics. So, it is required to design another model for the remanufacturing system which is distinct from the repair system and also performance index is required for the remanufacturing system. Therefore, in this paper we suggest the availability model for remanufacturing system by using Markov Process. This model represents each of the states of the remanufacturing system. Also performance indexes of remanufacturing system are introduced. Performance indexes are consisting of part reuse frequency and time, part disposal frequency and time. As a result, we can have a choice and control the proper part and offer useful information during the remanufacturing by using these availability model and performance indexes.

  • The priority of repair areas are chosen with the probability distribution of 0.3mm wide crack and carbonation induced corrosion. Data is analyzed and evaluated based on the 28 section of Precise Inspection for Safety and Diagnosis (PISD) in seoul. As the crack is distributed in log-normal, the carbonation and cover are in normal distribution. To have rational in repair sections among 503 sheets of underground structure, it is adopted the reliability index as well as the environment factors: strength, sonic speed, $CO_2$ concentration, corrosion, and content of chloride.

Distal Aortic Remodeling after Type A Dissection Repair: An Ongoing Mirage

  • Remodeling is a commonly encountered term in the field of cardiothoracic surgery that is often used to describe various pathophysiological changes in the dimension, structure, and function of various cardiac chambers, including the aorta. Stanford type A or DeBakey type 1 aortic dissection (TAAD) is a perplexing pathologic condition that can present surgical teams with the need to navigate a maze of complex decision-making. Ascending or hemi-arch replacement leaves behind a significant amount of distal diseased aortic tissue, which might have a persistent false lumen or primary or secondary intimal tears (or communications between lumina), which can lead to dilatation of the aortic arch. Unfavorable aortic remodeling is a major cause of distal aortic deterioration after the index surgery. Cardiac surgeons are aware of post-surgical cardiac chamber remodeling, but the concept of distal aortic remodeling is still idealized. The contemporary literature from established aortic centers supports aggressive management of the residual aortic pathology during the index surgery, and with continuing technical advancements, endovascular stenting options are readily available for patients with TAAD or for complicated type B aortic dissection cases. This review discusses the pathophysiology and treatment options for favorable distal aortic remodeling, as well as its impact on mid- to long-term outcomes following TAAD repair.

Personal technique for definite repair of complete unilateral cleft lip: modified Millard technique

  • Background: Millard's rotation-advancement repair, which is used by many surgeons, can make a natural philtral column, but most surgeons use a modification of the rotation-advancement flap. The purpose of this article is to introduce a modification utilized by the authors and to provide detailed surgical procedure. Methods: We retrospectively reviewed 82 patients' medical records and presented surgical technique and outcomes. The main features of the authors' strategy are emphasizing horizontal length of the lip, orbicularis oris muscle duplication for improving the definition of the philtral column, overcorrection of domal portion than the non-cleft side in order to compensate for the recurrence during growth. Two judges rated two times the appearance of the patients' nose and lip using Asher-McDade aesthetic index. Intra- and interobserver reliabilities were determined using Cohen's kappa statistics. Results: All patients recovered eventually after surgery; however, two patients have a minor complications (wound infection in one patient, wound disruption due to trauma in the other patient). The improvement of the aesthetic results can be achieved with this modified Millard technique. Total mean scores of the Asher-McDade index was 2.08, fair to good appearance. The intraobserver reliabilities were substantial to almost perfect agreement and the interobserver reliabilities were moderate to almost perfect agreement. Conclusion: We modified Millard method for repair of complete unilateral cleft lip. The surgical outcomes were favorable in long-term follow-up. We hope our technique will serve as a guide for those new to the procedure.

Comparison of the Rehabilitation Program after Rotator Cuff Repair by Time Closed Chain Exercise (회전근개봉합술 후 닫힌사슬운동 적용 시점에 따른 효과 비교)

  • PURPOSE: This study aimed to examine the pain, range of motion (ROM), upper extremity task performance, and functional levels of patients after rotator cuff repair according to the timing of a closed chain exercise thereby presenting basic data for an effective rehabilitation program. METHODS: The intervention was applied three times per week, one hour per day, for four weeks to 40 participants, 78 of whom had undergone rotator cuff repair. The participants were divided into four groups and assigned to usual general physical therapy and an open chain exercise. Group I consisted of the open chain exercise only. The closed chain exercise was applied to group II after the 4 times, group III after the 7 times, group IV after the 10 times. Measurement were used ROM, visual analogue scale (VAS), box and block test (BBT), and shoulder pain and disability index (SPADI). A one-way analysis of variance was conducted to test differences. RESULTS: There were significant differences in the internal/external rotation between group I and group II. The VAS significantly differed between group II and group I, group III, and group IV. The BBT results of group II and group I were significantly different compared to those of group IV. The SPADI significantly differed between group II and group I and between group II and group IV. CONCLUSION: The closed chain exercise was effective for patients following rotator cuff repair from the second week after active exercise was prescribed, verifying its applicability in rehabilitation programs.

A Rehabilitation protocol for Arthroscopic Bankart repair in Korean Medicine : A Case Report (Bankart 병변의 관절경적 복원술 후 한방 재활치료 프로토콜을 적용하여 호전된 증례 보고)

  • Objectives: The aim of this study was to report the clinical improvement of a patient who underwent arthroscopic Bankart repair using a rehabilitation protocol involving Korean medicine. Methods: The patient was treated with acupuncture, cupping, Korean Medicine, Chuna therapy, and Doin exercise therapy during the admission period. To estimate the patient's status, we used the Numeric Rating Scale (NRS), Pain Disability Index (PDI), assessments of the range of motion (ROM) and Muscle Manual Test (MMT) for the shoulder joint. Results: After administering the abovementioned treatments, we found that the patient's NRS, PDI, and ROM and MMT for the shoulder joint were improved. Conclusions: The rehabilitation protocol involving Korean medicine can be applied to and produce good results for patients who undergo arthroscopic repair for Bankart lesions. The limitation of the study was that the number of cases we studied was insufficient to prove the effectiveness. Hence, further studies are needed for designing the rehabilitation protocol involving Korean medicine.

Significant radiologic factors related to clinical outcomes after arthroscopic rotator cuff retear repair

  • Background: Healing of the tendon itself is not always related to successful clinical outcomes after rotator cuff repair. It was hypothesized that certain radiologic factors affecting clinical outcomes could exist in case of the retear after arthroscopic rotator cuff repair (ARCR) and the radiologic factors could help predict clinical process. The purpose of this study was to identify the radiologic factors associated with clinical outcomes of the retear after ARCR. Methods: Between January 2012 and December 2019, among patients with sufficient footprint coverage for ARCR, 96 patients with Sugaya classification 4 or higher retear on follow-up magnetic resonance imaging were included. The association between clinical outcomes such as American Shoulder and Elbow Surgeons (ASES) score, Constant score and range of motion and radiologic variables such as initial tear dimension, retear dimension, variance of tear dimension, critical shoulder angle, acromial index, and acromiohumeral distance was analyzed. Results: Preoperatively, the ASES and Constant scores were 59.81±17.02 and 64.30±15.27, respectively. And at the last follow-up, they improved to 81.56±16.29 and 78.62±14.16, respectively (p<0.01 and p<0.01). In multiple linear regression analysis, the variance of the mediolateral dimension of tear had statistically significant association with the ASES and Constant scores (p<0.01 and p=0.01). Conclusions: In patients with the retear after ARCR, the variance in the mediolateral dimension of tear had significantly negative association with the clinical outcomes. This could be considered to be reference as relative criteria and needed more sample and mechanical study.

  • The purpose of this study is to give the detailed evaluation index which is considered by reconstruction and repair for the school facilities on the ground of the efficiency evaluation result in each plan through analyzing the school facilities to every case classified into the evaluation index such as the school site plan, the site plan, the floor plan. On this study through the detailed evaluation index each case to the school facilities is to get the result of the efficiency evaluation. Through analysing the result, to make an alternative plan of the school facilities which are fit in both the new educational process and the educational state is to give it as a guide.

  • Barber bogie and Y25 bogie is the main bogie of freight car which is used in the Korean railway. In case of Barber bogie and a mixed formation tile maximum speed limit of empty car is 70 km/h and that of loaded car is 90 km/t and also it runs a speed of 110 km/h in time of an exclusive formation of Y25 bogie. The car named as high speed freight car at present is Y25 bogie which modeled on the Y25 bogie or Europe developed in 1993, Y25 bogie has no problem about the running capacity up to the speed of 110 km/h, but it works as limiting factors to tile increasing cost of maintenance and repair expenses which caused by repair of bogie and the cost of materials. This study is going to describe the contents examined by analysis and tests, aimed at being used as an index of plan in the future through the strength test of high speed freight car which is used now.

  • The measurements were performed to assess the insulation deterioration condition of the stator winding of Dae-Cheong hydro-generator ${\sharp}2$ which had been in service after being constructed 1980. After repair of the stator winding insulation. we conducted insulation diagnostic tests which include resistance, polarization index(P. I). AC current, dissipation factor($Tan{\delta}$) and partial discharges ($Q_{max}$). The results of diagnostic tests were compared to the previous records. On the basis of these test results. this paper tried to present the importance of insulation repair for the generator stator winding.
