Clinical Study on the Efficacy and Safety of Chiljehyangbuhwan in the Treatment of Dysmenorrhea (칠제향부환제제(七製香附丸製劑)의 원발성(原發性) 월경통(月經痛)에 대한 임상적 효과)

  • Objective : The purpose of this study is to identify the clinical effects of Chiljehyangbuhwan in the treatment of dysmenorrhea patients. Methods : We studied fifty patients who visited Kyung Hee University Oriental Medical Center from July, 2004 to August, 2004. Women with organic disease such as ut. myoma, ov. cyst and pelvic inflammatory disease were excluded from this study. We treated them with Chiljehyangbuhwan for one menstrual cycle. The severity of dysmenorrhea were measured by MVRS (Multidimensional Verbal Rating scale), VRS (Verbal Rating scale) and VAS (Visual Analog Scale). Results : Chiljehyangbuhwan significantly decreased the severity of dysmenorrhea. Furthermore Chiljehyangbuhwan had an effect on dysmenorrhea for next menstrual cycle without taking. Chiljehyangbuhwan did not show hepatic and renal virulence. Conclusion : This study shows that Chiljehyangbuhwan has remarkable effects on dysmenorrhea patients and that effects continue to next menstrual cycle without taking medicine.

Clinical Study on the Effect of Gyebongnyeong capsule in the Treatment of Primary Dysmenorrhea (계지복령환(桂枝茯笭丸) 제제의 원발성 월경통에 대한 임상적 효과)

  • The purpose of this study is to identify the clinical effects of Gyebongnyeong capsule in the treatment of primary dysmenorrhea patients. We studied ten patients visit Se-Myung University Oriental Medical Center from February, 2005 to March, 2005. Women with organic disease such as ut. myoma, ov. cyst and pelvic inflammatory disease were excluded from this study. We treated them with Gyebongnyeong capsule for two menstrual cycles. The severity of dysmenorrhea were measured by VAS (Visual Analog Scale) and Patient's Global Assessment. Gyebongnyeong capsule significantly decreased the severity of dysmenorrhea. Gyebongnyeong capsule did not show hepatic and renal virulence. This study shows that Gyebongnyeong capsule has remarkable effects on dysmenorrhea patients.

Video-assisted Thoracic Surgery [VATS] in Diagnosis and Treatment of Thoracic Diaseas; Report of 90 Cases (비디오 흉강경: 흉부질환의 진단과 치료;90례 보고)

  • 90 patients[75 men and 15 women] with the thoracic disease underwent video-assisted thoracic surgery[VATS] during the period March 1992 to February 1993. The thoracic diseases were classified into two groups of spontaneous pneumothorax and general thoracic patients and they were 66 and 24, respectively.The mean size of the tumor resected was 4.3 $\pm$ 2.0 cm x 3.3 $\pm$ 1.1 cm x 2.7 $\pm$ 1.0 cm. The mean time of anesthesia and operation were 90.0 $\pm$ 19.9 min and 43.7 $\pm$ 13.1 min in spontaneous pneumothorax group and 123.3 $\pm$ 40.3 min and 62.8 $\pm$ 32.2 min in general thoracic group. The mean period of postoperative chest tube drainage and hospital stay were 5.0$\pm$ 5.5 days and 6.6 $\pm$ 7.4 days in spontaneous pneumothorax group and 3.5$\pm$ 1.6 days and 9.5 $\pm$ 6.1 days in general thoracic group. The indications of VATS were 71 pleural disease[78.9%: 66 spontaneous pneumothorax; 3 pleural effusions ; 1 pleural paragonimus westermanii cyst; 1 malignant pleural tumor with metastasis to the lung], 9 mediastinal disease[10.0%: 5 benign neurogenic tumor; 2 pericardial cyst; 1 benign cystic teratoma; 1 undifferentiated carcinoma], 8 pulmonary parenchymal disease[8.9%: 3 infectious disease ; 3 interstitial disease ; 2 malignant tumor ], and 2 traumatic cases of exploration and removal of hematoma[2.2%]. The applicated objectives of VATS were diagnostic[ 7 ], therapeutic[ 67 ] and both[ 16 ] and the performed procedures were pleurodesis[ 66 ], wedge resection of lung[ 59 ], parietal pleurectomy[ 11 ], removal of benign tumor[ 9 ], excision and/or biopsy of tumor[ 4 ], pleural biopsy and aspiration of pleural fluid[ 3 ] and exploration of hemothorax and removal of hematoma in traumatic 2 patients. The complication rate was 24.2%[ 16/66 ] in the spontaneous pneumothorax group and 8.3%[ 2/24 ] in the general thoracic group and so overally 20.0%[ 18/90 ]. The mortality within postoperative 30 days was 2.2%[ 2/90 ], including 1 acute renal failure and 1 respiratory failure due to rapid progression of pneumonia. The conversion rate to open thoracotomy during VATS was 5.6%[ 5/90 ], including 2 immediate postoperative massive air leakage, 1 giant bullae, 1 malignant pleural tumor with metastasis to lung and 1 pulmonary malignancy. The successful cure rate of VATS was 75.8%[ 50/66 ] in the spontaneous pneumothorax group and 76.5%[ 13/17 ] in the general thoracic group and the successful diagnostic rate was 100%[ 7/7 ]. In conclusion, although prospective trials should be progressed to define the precise role of VATS, the VATS carries a low morbidity and mortality and high diagnostic and therapeutic success rate and now can be effectively applicated to the surgical treatment of the extensive thoracic disease.

Clinical Outcomes of 48 Pet Ferrets with Adrenal Disease (부신질환을 지닌 애완 페렛 48마리의 임상적 결과)

  • Medical records of 48 pet ferrets that underwent adrenalectomy were retrospectively reviewed to investigate the frequency and clinical outcomes of adrenal diseases in pet ferrets. These diseases were most commonly diagnosed in ferrets aged 3 to 5 years and in neutered females (58.3%). Adrenal disease occurred most frequently in the left adrenal gland (72.9%), followed by involvement of both adrenal glands (16.7%) and the right adrenal gland (10.4%). The mean sizes (length * thickness) of the adrenal glands as determined by ultrasonography were 8.96 * 5.08 mm and 12.91 * 8.26 mm for the left and right adrenal glands, respectively. In the ferrets with adrenal disease, alopecia (82.2%) was the main presenting clinical signs in both sexes, and vulvar swelling was seen in 32.1% of the females with adrenal disease. The common incidental findings included renal cysts (29.2%) and splenomegaly (25.0%). Histological findings showed pheochromocytoma, adenoma, and hyperplasia in 44.7%, 14.9%, and 12.8% of cases, respectively. The survival rates at 1- and 2- years after surgery were 87.5% and 74.0%, respectively. Alopecia and vulvar swelling improved within an average of 3.4 months and 12 days after surgery.

Analysis of Urinary Mass Screening for Second Grade of Elemantary School Children in Paju City (파주 지역 초등학교 2학년생에게 실시된 집단 뇨검사 분석)

  • Purpose We performed urinary mass screening(UMS) program for 2,804 children of second grade elemantary school 8 years of age in Paju city with cooperation of Paju City Health Center to determine the prevalence of asymptomatic proteinuria and hematuria, and to estimate the risk of incipient renal diseases. Also we attempted to evaluate the significance of hematuria in UMS in addidtion to proteinuria. Methods : 2,804 children of the 2nd grade of elementary school who lived in Paju city were included to our UMS program in 2000. They were constituted with 1,428 boys and 1,376 girls. The screening program was carried out in 3 steps The 1st screenig test was performed at schools and then students with abnormal results were examined repeatedly at Paju City Health Center and our hospital. Those students who showed proteinuria and/or hematuria in the 1st and 2nd test were referred to our hospital to undertake the 3rd close examination including physical examination, laboratory tests and radiologic tests. Results : (I) The prevalence of urinary abnormality in the 1st screening test was $8.3\%$(233 students), comprised of $5.9\%$ of boys, $10.8\%$ of girls. (2) Among 2,804 children tested in the first screening, prevalences of asymptomatic proteinuria and isolated hematuria were 64($2.3\%$), 163($5.8\%$) respectively, and the prevalence of proteinuria with hematuria was 6($0.2\%$). (3) Among 233 students with urinary abnormalities at the 1st screening test, 102 students applied to the 2nd test. 32 children, about one third of them, were also found to have abnormal urinary findings ; isolated hematuria 30, proteinuria with hematuria 2. (4) Those findings of clinical evaluation for children with isolated hematuria at the hospital showed as follows: idiopathic isolated microscopic hematuria 21, normal 6, urinary tract infection 1, idiopathic hypercalciuria 1 and simple renal cyst 1. Those 2 students with proteinuria and hematuria seemed to have chronic glomerulonephritis. Conclusion : (1) The clinical evaluation for children who showed positive results at the 1st screening test should be done judiciously. Because of high false positive rate, almost who showed positive results was normal, only a few of them had pathologic conditions. In this study, actual incidence of incipient renal diseases in children of 8 year old was calculated to be $0.4\%$. (2) The definite conclusion whether a urinary mass screening test can alter the prognosis of incipient renal diseases could not be drawn with this study. Further study must be necessary (3) We could acknowledge the significance of hematuria in UMS, but it is necessary that one should be judicious in managing and follow-up those that show abnormal results.   • Branchio-otic syndrome(BOS) is a relatively uncommon genetic malformation associated with dysmorphogenesis of the first and second branchial arches and is characterized by branchial fistulae, congenital preauricular fistulae, and anomalies of the pinnae, external, middle, and inner ears, accompanied by hearing loss. Recently, we experienced a case of BOS in a 10 years old female patient and report this case with a review of literature. 10-year-old girl presented with hearing impairment, bilateral preauricular fistula and cervical fistula. The pure tone audiometry revealed that she had 60dB sensorineural hearing loss on right side and 90dB mixed hearing loss on left. Bilateral branchial fistula was found on the neck CT scan and bilateral ossicular and cochlear abnormality combined with enlarged internal auditory canal was noted on the temporal bone CT scan. To investigate the association with EYA1 gene, we performed DNA sequncing with peripheral white blood cell and found the point mutations on Exon 7, 12 and 16 of EYA1 gene. The preauricular fistula and branchial fistula was excised surgically and hearing aid was applied on her left side. There was no sign of fistula recurrence for seven years after the surgery.

Classification of Congenital Urinary Tract Anomalies Diagnosed by Antenatal Ultrasonogram (산전 초음파로 발견된 선천성 요로계 기형의 분류)

  • Purpose : A study was done to assess the incidence and classification of congenital urinary tract anomalies detected by antenatal ultrasonogram. Methods : We reviewed 558 cases of urinary tract anomaly which were detected by antenatal ultrasonogram and postnatally confirmed between June 1989 and May 2002. We investigated the incidence and classified congenital urinary tract anomalies by review of medical records, antenatal and postnatal radiologic studies retrospectively. Results : In 558 cases of congenital urinary tract anomalies, 292 cases of hydronephrosis were found and the most common. Another anomalies were composed of 65 cases of multicystic dysplastic kidney, 32 cases of hydroureteronephrosis, 31 cases of duplication of kidney, 25 cases of renal agenesis, 21 cases of simple renal cyst, 20 cases of polycystic disease, 13 cases of ureterocele, 11 cases of renal hypoplasia, 10 cases of horseshoe kidney, 9 cases of vesicoureteral reflux, 8 cases of posterior urethral valve, 7 cases of bladder diverticulum, 6 casts of megaureter, 5 cases of ectopia, 2 cases of megacystis, and 1 case of medullary cystic disease. In 82 of the 558 cases, there were two or more combined urinary tract anomalies. Associated diseases other than urinary tract were observed in 13 cases, of which the congenital heart disease was the most common. Conclusion : The congenital urinary tract anomaly is frequently found and diverse during the antenatal ultrasonography. The multicenter study is needed to investigate precise incidence and distribution of each anomalies in general population.

Intraperitoneal Cisplatin Chemotherapy in A Canine Ovarian Cancer with Peritoneal Metastasis (복강전이가 동반된 개 악성 난소암의 cisplatin 치료)

  • A 4-year-old female Shih-tzu dog weighing 5.1 kg was referred with abdominal distension and diagnosed as cystic papillary adenocarcinoma with metastasis to serosal surface of abdomen by biopsy. Intraperitoneal chemotherapy with cisplatin (50 $mg/m^2$) diluted in normal saline (250 $ml/m^2$) was initiated following removal of ascites one month after ovariohysterectomy (OHE). Clinical response was great and had no side effects during the chemotherapy period (3 relapses, 33 months after diagnosis). However, pleural effusion occurred with ascites and renal failure at the end of treatment. Eleven treatments of cisplatin were performed during 35 months after diagnosis. Intraperitoneal cisplatin instillation has been effective to control malignant ascites and pleural effusion in ovarian cancers and could be a reasonable palliative treatment. This is the first case report describing intraperitoneal cisplatin chemotherapy in metastatic ovarian cyst papillary adenocarcinoma in Korea.

Radiologic Evaluation of Intraabdomenal Masses in Childhood (소아복부 종괴의 방사선학적 고찰)

  • The abdominal tumors in children are different from those of adult. These tumors are the third most common one, preceded by leukemia and brain tumors, in children under 15 years. X-ray examination is the most important method among diagnostic approaches. The role of diagnostic imaging is to identify the precise anatomic location and extent of pathologic process with the minimal number of imaging procedures. 23 cases of abdominal tumors were reviewed in respect of age incidence, site of origin, radiologic findings. The results are briefly summarized as follows : 1. Neuroblastoma was the most common(6 cases) and wi1m's tumor(5 cases), choledocal cyst(4 cases), ovarian mass(3 cases), hydronephrosis(2 cases), were descending order in frequency. 2. The most common site was retroperitoneum(60%). Kidney was the single most common site of origin. 3. Radiologic findings. The most common findings of plain radiography was ill defined soft tissue mass and this method was helpful in the presence of calcification especially in neuroblastoma. Ultrasonographic pattern was anechoic(cystic), echoic or mixed pattern, but this method provide less precise anatomical details, nevertheless ultrasonography wes paticullary useful imaging modality for the pediatric abdominal tumors. IVP findings were renal displacement, caliceopelvic system distortion or nonvisualization of kidney, these information was helpful in determining the location of tumors. CT scan showed homogenous or inhomogenous, cystic or solid, mass with their anatomic location. 4. Ultrasonography was the most widely used specific diagnostic method, but had limited value in detecting the anatomic location of tumors. CT scan was superior to ultrasound for determining the extent of tumors.

Diagnosis, Treatment and Prognosis of Low Grade Central Osteosarcoma (저등급 중심부 골육종의 진단, 치료 및 예후)

  • Purpose: We analyzed the diagnosis and the treatment outcomes of patients with central low grade osteosarcoma. Materials and Methods: We retrospectively reviewed 16 patients with central low grade osteosarcoma were treated at out institution between 1994 and 2011. Results: There were 4 men and 12 women with mean age of 26 years. Eleven patients were correctly diagnosed but 5 patients were misdiagnosed as osteoid osteoma, non ossifying fibroma, aneurysmal bone cyst, desmoplastic fibroma. 15 patients finally received wide margin en bloc excision and one of them treated under neoadjuvant chemotherapy. Final survival status was continuous disease free in 14 and 1 patient died of renal cell cancer. Remaining 1 with multifocal lesions is alive with disease for 7 years only treated radiation therapy on residual tumors. Nine (56%) of 16 tumors showed extra-osseous extension of tumor (56%) and 1 of them showed extra-compartmental tumors. Conclusion: The diagnosis of central low grade osteosarcoma is challenging, however, considering of the clinical suspicion, the typical findings of radiologic and pathologic features, proper diagnosis is needed. This tumor should be treated with wide excision, even after an intralesional excision, to avoid local recurrence or transformation to higher histologic grade.
