Risk Factors for Displacement of the Abomasum in Dairy Cows and its Relationship with Postpartum Disorders, Milk Yield, and Reproductive Performance

  • We determined the risk factors for displacement of the abomasum (DA), and the relationships between DA and postpartum disorders, milk yield, and reproductive performance in dairy cows. Initially, we identified the risk factors for DA using data regarding cow health and calving season from 2,208 lactations. Then, we compared the incidence of postpartum disorders, culling, death, and reproductive performance between cows with DA and their control herdmates (each n = 57). In addition, serum metabolites concentrations and milk yield were compared between cows with DA and controls (each n = 33). Ketosis (odds ratio [OR] = 9.27, p < 0.0001) and twin calves (p = 0.06) increased the risk of DA. Cows with a parity of three had a higher risk (OR = 5.23, p < 0.01) of DA than primiparous cows. Serum total cholesterol concentration was lower but non-esterified fatty acid, ${\beta}-hydroxybutyrate$, and alanine aminotransferase concentrations were higher after calving in cows with DA than in controls (p < 0.05). The removal rate from the herd by 2 months after calving was higher (p < 0.05) but milk yield 1 and 2 months after calving (p < 0.01) and the rate of first insemination by 150 days postpartum were lower (hazard ratio = 0.49, p < 0.05) in cows with DA than controls. In conclusion, higher parity, twin calves, and ketosis are risk factors for DA in dairy cows, which is associated with a higher removal rate from the herd, lower milk yield, a longer calving to first insemination interval, and unfavorable levels of metabolites related to energy and liver function.

  • 목적: 최근 의료기관평가제도는 의료의 질에 대한 병원들의 관심을 높게 하고 그것을 향상시키기 위한 활동을 활발히 전개하고 있다. 또한 추가된 항목인 환자의 위험관리에 대해서도 많은 관심을 보이고 있다. 의정부성모병원 핵의학과는 2008년도 PET/CT도입으로 시작된 부서내 리모델링은 과내 구조 및 업무 프로세스에 많은 변화를 가져왔다. 따라서 변경된 구조와 기존의 업무 처리방식은 미처 알지 못하는 위험요인을 내포하게 되었다. 그러므로 영상 검사 과정에서 발생할 수 있는 위험요인을 다각도로 찾고 분석하여, 부서의 특수한 환경에 맞는 위험관리 프로세스와 위험요인 제거업무를 포함한 개선 활동을 목적으로 한다. 실험재료 및 방법: 핵의학과의 새로워진 프로세스를 다각도로 분석하여 첫째, 위험관리 응대메뉴얼을 제작하여 교육후 실제 업무에 적용한다. 결과분석은 전년도 위험사고 발생건수와 개선활동 후 위험사고 발생건수를 비교하였다. 둘째, 위험관리 홍보동영상은 제작 후 검사전 해당 환자에게 상영하였고 그 후 자체 제작된 설문지로 해당 환자 100명에게 만족도 조사를 하였다. 셋째, 위험요소를 개선할 수 있는 시설물 참여형 개선활동을 통해서 위험요소를 제거하였다. 결과: 안전사고 발생건수는 개선활동이후 PET/CT와 감마카메라 영상 검사 모두 검사건수대비 투약오류, 낙상, 충돌 등의 안전사고가 0건으로 조사되었다. 설문조사 결과는 동영상 상영 후 검사진행과정 이해 74%, 불안감해소 81%로 "만족한다"는 결과가 조사되었다. "위험요소에 대해 주의했습니까?" 라는 물음의 경우 PET/CT는 94%, 감마카메라는 89%가 주의했다는 결과로 조사되었다. 개선활동을 통해서 핵의학과내에서의 고객위험관리를 효과적으로 수행할 수 있었다. 결론: 본 개선활동을 통하여 영상 검사 시 산재된 위험요소를 체계적으로 나열하고 분석하는 계기가 되었다. 또한 안전사고에 대처하는 방사선사의 업무방법에서도 기준이 되는 응대메뉴얼을 적용시킨다면 안전사고의 최소화와 안전사고시 체계적인 피드백이 가능함을 알 수 있었다. 위험요소를 알려주는 방법론적인 부분에서도 일상생활에서 쉽게 접할 수 있는 시청각자료로 제공 되었을 때 보다 효과적이였다. 따라서 이런 활동이 지속적이고 세부화 된 업그레이드 작업을 통해서 환자들에게 제작되어 제공된다면 핵의학 검사 이용의 안전성과 편리성에 도움을 줄 수 있으리라 생각된다.

Risk Factors of Postoperative Hematomas after Surgery for Intracranial Meningiomas

  • Objective : Meningioma is a benign tumor which has a high occurrence rate of postoperative hematomas. The purpose of this study is to analyze risk factors for postoperative hemorrhages after meningioma surgery. Methods : One hundred and fifty three patients with intracranial meningiomas, operated at the Department of Neurosurgery, National Medical Center, between January 1995 and December 2003 were included in this retrospective study. Risk factors considered to be related with postoperative hematomas were age, sex, preoperative pharmacological anticoagulants for medical co-morbidity, tumor location, histological type of the meningioma, infiltration of dural sinus and arachnoid, removal range of tumors, and the perioperative coagulation status including prothrombin time, partial thromboplastin time, and platelet count. Results : Patients' aged more than 70 years with a platelet count of less than $150{\times}10^9{\ell}^{-1}$ after surgery had statistically significant relations to the occurrence rate of postoperative hematomas. The other factors had no statistical significance. Conclusion : Various and intensive preoperative examinations for coagulation factors of patients, especially of older age, and proper transfusion before meningioma surgery are necessary for preventing postoperative hematoma.

Influence of mesiodens on adjacent teeth and the timing of its safe removal

  • Purpose: To focus on the effects of the presence of mesiodens on adjacent teeth and to investigate the timing of its safe removal. Materials and Methods: Cone-beam computed tomography examinations, obtained at Okayama University Hospital over a three-year period, were inspected. Data were recorded including the number of mesiodens; associated abnormalities; and the relationship with neighboring structures. Depending on multiple factors, the risk of developing complications due to early extraction of a mesiodens was divided into three categories: high, medium, and low risk. Results: A total of 5,958 cone-beam computed tomography exams were obtained, 460 patients aged 3-85 years were diagnosed with a total of 568 mesiodens, 382 (67.3%) of which were discovered in young patients (age <10 years), and 333 (87.2%) of these were associated with abnormalities. Regarding the risk categories, 11 (1.9%) were considered to be in the high-risk, five (0.9%) in the medium-risk and 552 (97.2%) in the low-risk categories. Moreover, eight out of 11 high-risk mesiodens were extracted and no post-operative complications have been seen. Conclusion: As the results showed that no postoperative complications were seen in all the extracted cases of high-risk mesiodens, this indicates the possibility of safe extraction at an early age which could reduce related future complications.

BIM 기반 위험요소 도출을 통한 정량적 위험성 평가 모델 개발 - 떨어짐 사고를 중심으로 - (Development of A Quantitative Risk Assessment Model by BIM-based Risk Factor Extraction - Focusing on Falling Accidents -)

  • 건설산업의 중대재해 발생률 및 사망률이 가장 높아짐에 따라 이를 감축하기 위해 국내에서도 다양한 노력을 기울이고 있다. 그 중 위험성 평가는 시공단계 위험요소를 평가하고 재해 감소대책을 위한 자료로 활용되고 있다. 그러나 기존의 위험성 평가는 수행자의 주관이 개입되며 국내 건설현장에 취약한 측면이 있다. 본 연구는 위험성 평가 분야에 BIM을 활용하여 정량적으로 위험요소를 도출함으로써 리스크를 조기 식별하고 사전제거하는 것을 목표로 위험성 평가를 위한 DB 분류체계를 구축하였으며 BIM을 활용한 위험성 평가의 방법론을 제시한다. 이를 통해 리스크의 사전 제거로 시공 작업자의 안전성을 증대시키고 안전관리 분야의 추가비용을 절감한다. 또한, 신규 공법에도 적용 가능하므로 프로젝트 참여자들의 이해도를 높이며 의사소통의 도구가 된다. 본 연구는 BIM을 기반으로 정량적인 위험도를 도출하는 프레임워크를 제안하는 연구로써, 향후 BIM을 활용한 위험성 평가 분야에 기반 기술로 활용될 것이다.

Poor Knowledge of Colorectal Cancer in Brunei Darussalam

  • Background: Colorectal cancer (CRC) is the most common gastrointestinal cancer and the incidence is increasing in many developing countries. While it can be detected early and even prevented through screening and removal of premalignant lesions, there are barriers to screening which include low level of knowledge and awareness of CRC. This study assessed the level of knowledge of CRC in Brunei Darussalam. Materials and Methods: A total of 431 (262 male and 161 female) subjects participated in this questionnaire study. Subjects were scored on their knowledge of signs/symptoms (maximum 10 correct answers) and known risk factors for CRC (maximum 10 correct answers) and were categorised into poor (0-2), moderate (3-4) and satisfactory (5-10). Comparisons were made between the various patient factors. Results: Overall, 54.1% could not name any CRC signs/symptoms or associated risk factors. Most were not aware of any screening modalities. The overall scores for CRC signs/symptoms and risk factors were $1.3{\pm}1.39$ (range 0-6) and $0.6{\pm}1.05$ (range 0-5) respectively. Overall, the breakdown of scores was: poor (78.1%), moderate (20.3%) and satisfactory (6.2%) for signs/symptoms and poor (93.2%), moderate (6.2%) and satisfactory (0.7%) for risk factors. Higher level of education, female gender and non-Malay race were associated with higher scores for both signs/symptoms and knowledge of screening modality; however the overall scores were low. Conclusions: Our study showed that the general knowledge of CRC in Brunei Darussalam is poor. Being female, with higher levels of education and non-Malay race were associated with higher scores, but they were still generally poor. More needs to be done to increase the public knowledge and awareness of CRC.

Removal of miniplates following facial trauma and orthognathic surgery: a 3-year study

  • The purpose of this study was to determine the cause and risk factors of removing bone plateby investigating and analyzing 359 patients who underwent reduction of fracture or orthognathic surgery with bone plate insertion over the past 3 years. Patients were evaluated with respect to age, smoking status, reason for insertion of plates, the numbers of inserted plates, sites of insertion, time between insertion and removal, reasons for removal of plates. The removal rate of bone plates was 33.1%. Of these, 17.0% of patients had clinical symptoms which led to remove plates. The removal rate of men was 29.9% and the rate of women was 39.2%. There were high removal rates from less than 20s (45.8%) and 20s (34.4%) those who are relatively young age group. On the other hand people in their 50s had a removal rate of 27.8% which was higher than those in their 60s with a rate of 20.7%. The removal rate of bone plate inserted into the mandible was 33.5%, and the removal rate of bone plate inserted into the maxilla was 34.7%. The mean period between the insertion and removal of bone plate was 10.9 months. The main reason for the removal of bone plate was the patient's requirement (44.5%). The most common cause of clinical symptoms was infection (22.7%). Infection was manifested within about a year and led to the plate being removed. Therefore, lowering the possibility of infection after surgery could decrease the removal rate of bone plate.

Traumatic Brain Injury in Children under Age 24 Months : Analysis of Demographic Data, Risk Factors, and Outcomes of Post-traumatic Seizure

  • Objective : Traumatic brain injury (TBI) in children under age 24 months has characteristic features because the brain at this age is rapidly growing and sutures are opened. Moreover, children this age are completely dependent on their parents. We analyzed the demographic data and risk factors for outcomes in TBI patients in this age group to elucidate their clinical characteristics. Methods : We retrospectively reviewed the medical records and radiological films of children under 24 months who were admitted to Kyungpook National University Hospital from January 2004 to December 2013 for TBI. Specifically, we analyzed age, cause of injury, initial Glasgow coma scale (GCS) score, radiological diagnosis, seizure, hydrocephalus, subdural hygroma, and Glasgow outcome scale (GOS) score, and we divided outcomes into good (GOS 4-5) or poor (GOS 1-3). We identified the risk factors for post-traumatic seizure (PTS) and outcomes using univariate and multivariate analyses. Results : The total number of patients was 60, 39 males and 21 females. Most common age group was between 0 to 5 months, and the median age was 6 months. Falls were the most common cause of injury (n=29, 48.3%); among them, 15 were falls from household furniture such as beds and chairs. Ten patients (16.7%) developed PTS, nine in one week; thirty-seven patients (61.7%) had skull fractures. Forty-eight patients had initial GCS scores of 13-15, 8 had scores of 12-8, and 4 had scored 3-7. The diagnoses were as follows : 26 acute subdural hematomas, 8 acute epidural hematomas, 7 focal contusional hemorrhages, 13 subdural hygromas, and 4 traumatic intracerebral hematomas larger than 2 cm in diameter. Among them, two patients underwent craniotomy for hematoma removal. Four patients were victims of child abuse, and all of them had PTS. Fifty-five patients improved to good-to-moderate disability. Child abuse, acute subdural hematoma, and subdural hygroma were risk factors for PTS in univariate analyses. Multivariate analysis found that the salient risk factor for a poor outcome was initial GCS on admission. Conclusion : The most common cause of traumatic head injury in individuals aged less than 24 months was falls, especially from household furniture. Child abuse, moderate to severe TBI, acute subdural hematoma, and subdural hygroma were risk factors for PTS. Most of the patients recovered with good outcomes, and the risk factor for a poor outcome was initial mental status.

Risk factors for canine magnesium ammonium phosphate urolithiasis associated with bacterial infection

  • Background: With limited information available, the association among urinary tract infections, urease-producing bacteria and the presence of magnesium ammonium phosphate (MAP) urolithiasis in canines in Thailand requires more study. Objectives: This study aimed to investigate the association between demographic characteristics of canines and the presence of MAP urolithiasis in canines, and to evaluate antimicrobial susceptibility patterns of bacteria isolated from canine uroliths. Methods: A total of 56 canines admitted for treatment with surgical removal of uroliths were recruited. Demographic characteristics and clinical chemistry data were recorded. Bacteria isolated from the removed uroliths were identified. Chemical compositions of the uroliths were analyzed by Fourier transform infrared spectrometer. Potential risk factors were determined with univariable and multivariable logistic regression analyses. Results: Of 56 canine urolithiasis, bacteria were isolated from uroliths of 38 canines (27 MAP and 11 non-MAP) but not from uroliths of 18 canines (5 MAP and 13 non-MAP). The most common bacteria found in nidus of MAP uroliths was Staphylococcus pseudintermedius (approximately 51%). An antimicrobial resistance was frequently found in Staphylococci isolates (42.86%). Multivariate logistic regression analysis showed that the predictors of MAP urolith in canine urolithiasis were being female (p = 0.044; adjusted odds ratio [OR], 10.22; 95% confidence interval [CI], 1.06-98.24) and the positive urolith culture (p = 0.012; adjusted OR, 8.60; 95% CI, 1.60-46.30). Conclusions: Our results indicate that S. pseudintermedius (a urease-producing bacterium) is the major causative bacteria of MAP uroliths. A positive urolith culture and being female are risk factors of MAP urolithiasis in canines.

소아에서 발생한 갑상설관낭종의 재발에 영향을 미치는 위험인자 (Risk Factors Affecting Recurrence of Thyroglossal Duct Cyst in Children)

  • Thyroglossal duct cysts (TGDC) are the most common type of congenital developmental anomaly encountered in the anterior midline of the neck in childhood. The aim of the study was to evaluate the clinical characteristics of TGDC and identify any factors that could be related to recurrence after surgery. This study consisted of a retrospective chart review of 45 patients treated at Kyungpook National University Hospital for TGDC between 1990 and 2008. All records were reviewed for age and sex, length of history, presentation, diagnostic methods, sizes and locations of cyst, surgical management, histopathology of the lesion and recurrences. The statistical analysis of risk factors for recurrence was made using the Fisher's exact test with a significance level of p (0.05. The male to female ratio was 2.2:1 with a male preponderance. The mean age at operation was 5 years and 2 months (4 months - 17 years). The most common presenting symptom was a nontender cervical mass (78 %). Most TGDC were found in the midline position. Twenty four were infrahyold, 17 were hyoid, and 4 were suprahyoid level. Forty one (91 %) patients received the Sistrunk operation, and 4(9 %) patients received Cyst excision. Postoperative a seroma developed in six patients in the early postoperative days. There were a total of 3(6.6 %) recurrences, 2 in patients who had excision only and in one patient who had the Sistrunk operation. Univariate analysis for risk factors with recurrence showed that there was no statistical relationship between the presence of preoperative infection and the development of recurrence. The removal of hyoid bone along with TGDC was a statistically significant risk factor for recurrent disease. This study suggests that the Sistrunk operation Is the treatment of choice for TGDC in order to reduce recurrence.
