소수 잔존 치근에 Locator Root Attachment를 활용한 상악 가철성 국소의치 증례 (Maxillary removable partial overdenture within Locator Root Attachment in a retained root: a case report)

  • 최종 수복물을 가철성 국소의치로 계획하였을 때 구강내 소수 잔존 치근을 유지하고 활용한다면, 기능적으로 좀더 나은 결과를 얻을 수 있다. 이번 증례는 소수 치아와 잔존 치근을 갖고 있는 환자에서 'Locator Root Attachment'를 활용하여 상악 가철성 피개의치로 최종 수복한 증례이다. 잔존 치근에 대한 정확한 임상적, 방사선학적 평가 후 어태치먼트를 적용하여 기능적으로 우수한 결과를 얻을 수 있었기에 이에 보고하고자 한다.

  • 'Crossed occlusion' is the condition in which occlusal intercuspation is lost when several teeth on upper and lower jaw remain. This report describes a clinical case in which a patient had two upper-left posterior teeth and two lower-right posterior teeth; typically known as left-right crossed occlusion. Considering the patient's general condition and financial situation, the treatment plan included placement of two implants on each jaw against the remaining teeth using surgical guide. To find out the ideal position of implants, digital diagnostic wax-up was preceded by superimposing the cast and cone beam computed tomography image, which was aided with radiographic stents. The consequent surveyed implant bridge provided stable vertical stop for fabrication of the implant assisted removable partial dentures. The patient was satisfied with the functionality and esthetics of definitive prosthesis.

  • Crown-type implant-assisted removable partial dentures (CIRPDs) can be a feasible treatment option for partially edentulous patients. Here we report a case with remaining unilateral mandibular teeth. Two implants were placed in the posterior portion of the mandible using a surgical guide, and a distal-extension removable partial denture with implant-supported surveyed crowns was fabricated. After 12 months, both the abutment teeth and implants were in good condition. The treatment outcomes were satisfactory in terms of masticatory function and esthetics. The advantages of CIRPDs and considerations for obtaining successful clinical outcomes with these dentures are also discussed.

  • A major cause of missing permanent incisors is congenital abscence and extraction because of trauma and pathologic condition. The request for restoration of missing or spaced anterior teeth is common in dental practice. Problems, such as the tilting, drifting, and rotation of teeth adjacent to the space, complicate the restoration of apperance, and a normally simple restorative dental procedure may become difficult. There are two primary treatment alternatives to improving a dentition's irregular and spaced apperance-closing the space by orthodontic means or providing a prosthesis to disguise the space. The treatment choice depends on many variables, but, as a general rule, patients with a normal overbite, overjet, and buccal relationship are better treated by maintaining the sapce and providing a prosthesis, either fixed or removable. This case report presents two cases : Traumatic loss of maxillary right and left central incisors, Extraction of malformed mandibular right central inciosr. The loss of central incisor space was regained by the fixed-removable and fixed orthodontic appliance, and then Maryland bridge was cemented.

  • 하악 편측에 소수의 잔존치만 있는 증례에서 통상적인 국소의치로 수복하는 경우에 충분한 유지 및 저항을 얻기 어렵고, 무치악 치조제의 흡수에 따른 의치의 안정성 저하로 인해 잔존지대치에 유해한 힘이 가해질 것으로 예상된다. 이러한 경우에 다수의 임플란트를 이용한 고정성 보철로 수복하게 되면 여러 가지 장점이 있을 수 있으나, 해부학적, 경제적 요인에 의해 치료 방법으로 선택이 제한될 수 있다. 이에, 적은 수의 임플란트를 전략적 위치에 식립하여 임플란트 보조 국소의치(Implant-assisted removable partial denture, IARPD)의 제작을 대안으로 고려해 볼 수 있다. 본 증례보고에서는 전통적인 가철성 국소의치만으로는 적절한 지지, 유지, 안정이 어려운 두 임상증례에서 소수의 임플란트를 전략적 위치에 식립하고 로케이터(Locator) 부착장치를 적용하여 3년간 경과관찰을 통해 좋은 임상 결과를 확인하였기에 이를 보고하고자 한다.

치과보철기공물 제작실태에 관한 조사연구 (A SURVER OF THE ACTUAL CONDITIONS ON THE PRODUCTION OF DENTAL PROSTHESIS)

  • The author studied the actual conditions on the production of dental prosthesis made in laboratories, and also studied interrelationships between dentists and laboratory technicians in both personal and technical aspects. Two hundred-eighty four technicians, work in dental laboratories presently, were surveyed via mail and direct contact during the period from June 1 to June 30 and August 27 to August 28 in 1994 respectively. The obtained results were as follows : 1. Among the respondents, 90.5% we re working in commercial dental laboratories and their laboratories were mainly located in the Seoul area(40.9%, P<0.05). The numbers of employees in these laboratories were less than 10 persons(70.0%, P<0.01), and 75.9% of these laboratories have been in operation less than 15 years. 2. Most laboratory procedures were accomplished according to established disciplines. However, procedures such as die trimming in fixed restorations and the qualifications of the people designing removable partial dentures were not. Other problem areas were boxing of the working cast, the person determining the posterior palatal seal area, selection and arrangement of artificial teeth, occlusion rim correction and laboratory remounting of the processed denture in complete denture restorations. 3. Only half of the requesting dentists could send work authorizations to the laboratories with their work and even so, its contents were quite lacking. Consequently, there must be some standards in writing work authorization. 4. Technicians most desired clean and accurate impressions in fixed and removable dentures, and enough tooth reduction in porcelain fused to metal restorations. 5. For the establishment of better relationships between dentist and dental technician, the respondents desired the establishment of equal footing first(33.5%), and frequent conversations and muture understanding second(25.9%).

상악 고정성 임플란트 보철물 실패 환자에서 임플란트 피개의치로 전환한 치료 증례 (A conversion to implant overdenture in failed fixed implant prosthesis of edentulous maxilla: A case report)

  • 고정성 임플란트 보철물과 가철성 임플란트 피개의치는 상악 완전 무치악 환자를 임플란트를 이용하여 치료하는 대표적인 두 가지 방법이다. 임플란트 보철물 선택 시 정확한 진단과 평가가 이루어 지지 않고 치료방향을 선택하게 되면 생역학적, 기능적 및 심미적으로 불리한 치료결과를 낳고 장기적인 예후 또한 보장할 수 없다. 본 증례의 환자는 개인치과의원에서 상악 완전 무치악에 8개의 임플란트를 식립하여 고정성 보철물로 치료하였으나 상악 보철물의 실패로 인해 재치료를 위해 내원하였다. 환자의 요청으로 추가적인 임플란트를 식립 할 수 없었고 구강위생관리 문제 해결, 기존 임플란트의 활용 및 연결 고정을 통한 안정화, 심미성 등을 고려하여 바를 이용한 가철성 피개의치로 치료를 결정하였다. 심미성과 저작기능, 구강위생관리에 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.

엇갈린 교합 환자의 임플란트 지지 고정성 보철물과 Kennedy class IV 가철성 국소의치를 이용한 수복 증례 (Rehabilitation of a patient with crossed occlusion using mandibular implant-supported fixed and maxillary Kennedy class IV removable dental prostheses: A case report)

  • The term, 'crossed occlusion' implies clinical situation in which the residual teeth in one arch have no contact with those in the antagonistic arch, resulting in the collapse of occlusal vertical dimension. The treatment goal of this pathologic condition is restoration of the collapsed vertical dimension and stabilization of abnormal mandibular position. Previously, konus removable prostheses or tooth supported overdentures were suggested to solve crossed occlusion. Nowadays, dental implants have been used for definitive support to solve this problem. In this case report, a 65 years old female patient had a crossed occlusion, in which the maxillary posterior residual teeth and mandibular anterior residual teeth cross. Interim removable and fixed dental prostheses were used to confirm the proper vertical and horizontal jaw relation. After that, the mandibular posterior edentulous region was restored with implant-supported fixed dental prostheses. Computer tomography guided implant surgery was performed according to the concept of the restoration-driven implant placement. The maxillary anterior edentulous region was restored with Kennedy class IV removable prosthesis, considering the patient's economic status. The patient's jaw position and prostheses have been well maintained at the follow-up after 6 months of definitive restoration. The antero-posterior crossed occlusion problems appeared to be effectively solved with the combination of removable in one arch and implant-supported fixed prostheses in the other.

  • CAD/CAM (computer-aided design/computer-aided manufacturing) 기술을 이용한 인레이 및 여러 고정성 보철물의 제작이 보편화되고 구강내 스캐너가 널리 보급되면서 디지털 치의학의 개념이 치과 보철학 분야 전반에 적용되고 있다. 그러나 가철성 보철 부분에서는 디지털 개념의 적용이 쉽지 않으며 기존의 주조 방식에 많이 의존하고 있다. 따라서 가철성 보철물의 제작 과정에 이런 개념을 적용해보려는 시도는 중요한 의미가 있다. 본 증례에서는 하악 Kennedy class III 무치악 환자에서 보철적, 심미적 요구에 의한 교합고경의 증가와 함께 CAD 및 쾌속조형술(RP, rapid prototyping)을 이용한 가철성 국소의치를 제작하였다. CAD를 이용하여 서베잉 및 국소의치 금속 구조물을 설계하였으며, 쾌속조형술은 금속 구조물을 주조하기 위한 레진 패턴 제작에 이용하였다. 9개월 간의 임상적 관찰 기간 동안 만족스러운 심미적, 기능적 결과를 얻어 이를 보고하고자 한다.
