Diagnosis and Treatment of Latent Tuberculosis Infection due to Initiation of Anti-TNF Therapy

  • Patients with immune-mediated inflammatory diseases (IMIDs) are increasingly being treated with anti-tumor necrosis factor (TNF) agents and are at increased risk of developing tuberculosis (TB). Therefore, diagnosis and treatment of latent TB infection (LTBI) is recommended in these patients due to the initiation of anti-TNF therapy. Traditionally, LTBI has been diagnosed on the basis of clinical factors and a tuberculin skin test. Recently, interferon-gamma releasing assays (IGRAs) that can detect TB infection have become available. Considering the high-risk of developing TB in patients on anti-TNF therapy, the use of both a tuberculin skin test and an IGRA should be considered to detect and treat LTBI in patients with IMIDs. The traditional LTBI treatment regimen consisted of isoniazid monotherapy for 9 months. However, shorter regimens such as 4 months of rifampicin or 3 months of isoniazid/rifampicin are increasingly being used to improve treatment completion rates. In this review, the screening methods for diagnosing latent and active TB before anti-TNF therapy in patients with IMIDs will be briefly described, as well as the current LTBI treatment regimens, the recommendations for managing TB that develops during anti-TNF therapy, the necessity of regular monitoring to detect new TB infection, and the re-initiation of anti-TNF therapy in patients who develop TB.

  • Preloading Strut applied during installation of the wall jack, but additionally serves to minimize the displacement of soil pressure acting upon dissolution due to the difficulty. In this study, we developed an index of support for the release of stress to facilitate the dismantling of the strut uninstall. The refractive support the axial force acting on the strut are supportable, is refracted at minimum load, disassembly should be easy. In order to find the optimal shape and structural stability of the refractive support We have performed the numerical analysis and performance test to determine the final model. We carried out model tests and UTM test in order to understand the refractive performance and durability of the refractive support for optimal model. Results of the test UTM is refracted all shot 5 times within a target hit number, it was found that there is no problem of the refractive performance. Further, the results of model experiments, it was found that to ensure sufficient durability more than the performance target value of the pin joint support structure.

Analysis of gonadotropin-releasing hormone (GnRH) test results in girls with precocious puberty (성조숙증 여아에서 생식샘자극호르몬분비호르몬 검사 결과의 분석)

  • Purpose:The gonadotropin-releasing hormone (GnRH) test results of girls with precocious puberty were analyzed to determine whether this test can efficiently and clearly differentiate between central precocious puberty (CPP) and other disorders. Methods:Clinical and laboratory data of 54 girls with precocious pubertal signs were reviewed. Intravenous GnRH test was performed with blood samples obtained at 0, 30, 60, and 90 minutes. A peak luteinizing hormone (LH) level of ${\geq}5.0IU/L$ was indicative of CPP. Results:Of the 40 girls with CPP, 36 (90.0%), 3 (7.5%), and 1 (2.5%) showed peak LH levels at 30, 60, and 90 minutes, respectively. A percentage of girls whose peak LH ${\geq}5.0IU/L$ up to 30, 60, and 90 minutes was 92.5%, 100%, and 100%, respectively. The peak LH/follicle stimulating hormone (FSH) ratio of girls with CPP was 0.89${\pm}$0.49 and was <1 in 16 of the 40 girls (40.0%). Girls with peak LH/FSH ratio of >1.0 showed higher chronological age (CA) ($8.3{\pm}0.6$ vs. $7.7{\pm}1.0$ years, P=0.033), bone age (BA) ($10.9{\pm}0.8$ vs. $9.7{\pm}1.1$ years, P=0.001), and BA-CA difference ($2.6{\pm}0.7$ vs. $2.0{\pm}0.7$ years, P=0.009) than those of girls with peak LH/FSH ratio of ${\leq}1.0$. Higher percentage of girls with peak LH/FSH ratio of >1.0 showed advanced breast development (${\geq}TannerIII$) (93.7% vs. 41.7%, P=0.001). Conclusion:LH levels after 30 and 60 minutes of intravenous GnRH administration are the most useful for diagnosing CPP in girls.

Experimental Study for Flushing of Sediment Bypass Pipe underneath Rubber Weir (고무보 저층수 배출관의 유사 배제 성능 실험 연구)

  • Most small weir installed in Korea is concrete solidated weir. Fixed weir causes stagnant flow, which leads to deposit sediment just upstream of weir. As time goes on, it would induce reduction of water storage capacity and invoke the serious water quality issues. Therefore, there has been a growing interest in movable weir. Especially, the flexible rubber weir is easy to install and possible to operate in extreme environments. However, even though this type can be flatable, it is also not free from sediment deposition problem. Thus, to enhance the ability of releasing deposition the bypass pipe was constructed underneath it. In this study the performance of its ability was examined with hydraulic model test. This bypass pipe was designed with 3 different dimensions to connect between each bottom of upstream and downstream of a weir, such as Type A, B, and C. The efficiency of drainage of deposition upstream was studied under two water of upstream and sediment heights. In addition, the ability of sediment emission through the bypass pipe after the pipe was blocked by debris like soil, vegetation et al. was examined by video monitoring. From this study, it was suggested a dimensionless equation which show the relationship of variable parameters and amount of emission sediment through bypass pipe. And it was found that the most significant factors on efficiency of releasing were elbow angle and discharge, and the ability of emission when the pipe was blocked was most highly influenced in tilting length.

Functional Electric Stimulation-assisted Biofeedback Therapy System for Chronic Hemiplegic Upper Extremity Function

  • Purpose: Rehabilitative devices are used to enhance sensorimotor training protocols, for improvement of motor function in the hemiplegic limb of patients who have suffered a stroke. Sensorimotor integration feedback systems, included with these devices, are very good therapeutic frameworks. We applied this approach using electrical stimulation in stroke patients and examined whether a functional electric stimulation-assisted biofeedback therapy system could improve function of the upper extremity in chronic hemiplegia. Methods: A prototype biofeedback system was used by six subjects to perform a set of tasks with their affected upper extremity during a 30-minute session for 20 consecutive working days. When needed for a grasping or releasing movement of objects, the functional electrical stimulation (FES) stimulated the wrist and finger flexor or extensor and assisted the patients in grasping or releasing the objects. Kinematic data provided by the biofeedback system were acquired. In addition, clinical performance scales and activity of daily living skills were evaluated before and after application of a prototype biofeedback system. Results: Our findings revealed statistically significant gradual improvement in patients with stroke, in terms of kinematic and clinical performance during the treatment sessions, in terms of manual function test and the Purdue pegboard. However, no significant difference of the motor activity log was found. Conclusion: Hemiplegic upper extremity function of a small group of patients with chronic hemiparesis was improved through two weeks of training using the FES-assisted biofeedback system. Further research into the use of biofeedback systems for long-term clinical improvement will be needed.

Wild Ginseng Attenuates Anxiety- and Depression-Like Behaviors During Morphine Withdrawal

  • The purpose of this study was to evaluate whether wild ginseng (WG) administration could attenuate anxiety- and depression-like behaviors and expression of corticotrophin-releasing factor (CRF) and neuropeptide Y (NPY) following withdrawal from repeated morphine administration in rats. Male rats were administered daily doses of WG (50, 100, or 200 mg/kg, i.p.) for 5 days, 30 min before morphine injection (40 mg/kg, s.c). The anxiety- and depression-like behavioral responses were measured 72 h after the last morphine injection using an elevated plus maze (EPM) and forced swimming test (FST), respectively. Changes in hypothalamic CRF and NPY expressions were also examined by analyzing their immunoreactivities in the hypothalamus. Daily administration of WG significantly reduced anxiety- and depression-like behavior, and elicited the suppression of CRF expression and the stimulation of NPY expression in the hypothalamus. Our results demonstrated that WG extract might be effective at inhibiting the anxiety and depression responses due to morphine withdrawal by possibly modulating the hypothalamus CRF and NPY systems. Furthermore, these findings imply that WG extract can be used for developing new medication to cure or alleviate morphine withdrawal symptoms and to prevent relapses of morphine use.

  • Catamenial hemoptysis is syndrome characterized by bleeding from the bronchial trees and lungs that occurs synchronously with the female menstrual cycle. Etiologic mechanism of pulmonary endometriosis is still controversial, and the diagnosis is usually made on the basis of the clinical history and exclusion of other causes of recurrent hemoptysis. Serial computed tomograms of the chest during and in the interval between menstruations have been proved to be a useful confirmatory test. We experienced a 33-year-old female patient who had been previously diagnosed as pelvic endometriosis pathologically, experienced cyclic hemoptysis during menstruations. The diagnosis of pulmonary endometriosis was made based on her history and changes in the character of the lesions as documented on radiologic studies of the chest. She was treated successfully with GnRH analogue and there is no evidence of recurrence.

An Experimental Study on the Shear Bond Strength of Fluoride-Containing Sealant and Non-Fluoride Containing Sealant (불소함유여부에 따른 치면열구전색제의 전단결합강도에 관한 실험적 연구)

  • This study was to evaluate shear bond strength of fluoride-releasing sealant and nonfluoride releasing sealant to enamel surface of bovine tooth. 80 extracted bovine teeth were randomly assigned to four groups, and four kinds of sealants including Teethmate-A(Kuraray Co.), Teethmate-F(Kuraray Co.), Helioseal(Vivadent Co.), Helioseal-F(Vivadent Co.) were bonded to exposed enamel surfaces using silicon plate. Shear bond strength was determined in an instron universal testing machine at a crosshead speed of 1mm/min. Then, the fracture surfaces of test specimens were investigated with scanning electron microscope. The obtained results were as follows; 1. The shear bond strength decreased in the following order : Teethmate-A(18.31MPa), Teethmate-F(11.90MPa), Helioseal (11.74 MPa), Helioseal-F(10.64MPa). 2. The shear bond strength of Teethmate-A showed significantly higher than that of Teethmate-F(P<0.05), but Helioseal and Helioseal-F didn't showed statistically different(P<0.05). 3. According to the SEM, Teethmate-A group showed cohesive failure, and Teethmate-A group & Helioseal group showed mixed pattern of cohesive and adhesive failure and Helioseal-F group showed adhesive failure.

Role of Glucocorticoids in Fasting-induced Changes in Hypothalamic and Pituitary Components of the Growth Hormone (GH)-axis

  • To directly test if elevated glucocorticoids are required for fasting-induced regulation of growth hormone (GH)-releasing hormone (GHRH), GHRH receptors (GHRH-R) and ghrelin receptors (GHS-R) expression, male rats were bilaterally adrenalectomized or sham operated. After 7 days, animals were fed ad libitum or fasted for 48 h. Bilateral adrenalectomy increased hypothalamic GHRH to 146% and decreased neuropeptide Y (NPY) mRNA to 54% of SHAM controls. Pituitary GHRH-R and GHS-R mRNA levels were decreased by adrenalectomy to 30% and 80% of shamoperated controls. In shamoperated rats, fasting suppressed hypothalamic GHRH (49%) and stimulated NPY (166%) mRNA levels, while fasting increased pituitary GHRH-R (391%) and GHS-R (218%) mRNA levels. However, in adrenalectomized rats, fasting failed to alter pituitary GHRH-R mRNA levels, while the fasting-induced suppression of GHRH and elevation of NPY and GHS-R mRNA levels remained intact. In fasted adrenalectomized rats, corticosterone replacement increased GHRH-R mRNA levels and intensified the fasting-induced decrease in GHRH, but did not alter NPY or GHS-R response. These data suggest that elevated glucocorticoids mediate the effects of fasting on hypothalamic GHRH and pituitary GHRH-R expression, while glucocorticoids are likely not the major determinant in fasting-induced increases in hypothalamic NPY and pituitary GHS-R expression.

Analysis of Organic Matter and Nutrient Leaching Characteristics of Agricultural Land Soils in Reservoir Area (저수구역 경작지 토양의 유기물 및 영양염류 용출특성 분석)

  • Soils in agricultural lands contain large amount of organic matter and nutrients due to the injected fertilizers and manure. During heavy rain, surface water and base runoff pollutants flows into a nearby stream or lake with eroded soil from agricultural lands. On the other hands, agricultural lands near the lake are inundated due to the increase of the water level in the lake, leading to organic matter and nutrient release from the inundated soil. In this study, releasing rates of nutrient salts and organic substances were analyzed for the soil in the agricultural land, where cultivation activities has been carried out and periodically flooded, to account for the possibility of contamination from the inundated agricultural land in reservoir areas The experiment results have shown that COD was released from the soil in anaerobic conditions, and T-P was released in both anaerobic and aerobic conditions. However, in the case of T-N, it was found that the runoff by soil was not made before the rainfall occurred, and when the soil was impound due to rainfall, the elution occurred under the aerobic conditions. Through the results of this study, it was possible to account for the effect of flooded agricultural lands on the water quality in the lake, and this could be reflected in an efficient agricultural non-point pollution management policy. In order to determine the precise releasing rate for each agricultural land, it is believed that the leaching experiment for paddy fields and grasslands are needed.
