PURPOSE. To longitudinally assess the quality of life in maxillectomy patients rehabilitated with obturator prosthesis. MATERIALS AND METHODS. Thirty-six subjects were enrolled in the span of 16 months, out of which six were dropouts. Subjects (age group 20-60 years) with maxillary defects, irrespective of the cause, planned for definite obturator prosthesis, were recruited. The Hindi version of European Organization for Research and Treatment of Cancer, Head and Neck version 1 of Quality of Life Questionnaire was used before surgical intervention and one month after definitive obturator. Questionnaire includes 35 questions related to the patient's physical health, well being, psychological status, social relation and environmental conditions. The data were processed with statistical package for social science (SPSS). Probability level of P<.05 was considered statistically significant. RESULTS. The quality of life after rehabilitation with obturator prosthesis was 81.48% (${\pm}13.64$) on average. On item-level, maximum mean scores were obtained for items problem with teeth ($1.87{\pm}0.94$), pain in mouth ($1.80{\pm}0.92$), trouble in eating ($1.70{\pm}0.88$), trouble in talking to other people ($1.60{\pm}1.22$), problems in swallowing solid food ($1.57{\pm}1.22$) and bothering appearance ($1.53{\pm}1.04$); while minimum scores were obtained for the items coughing ($1.17{\pm}0.38$), hoarseness of voice ($1.17{\pm}0.53$), painful throat ($1.13{\pm}0.43$), trouble in having social contacts with friends ($1.10{\pm}0.40$) and trouble having physical contacts with family or friends ($1.10{\pm}0.31$). CONCLUSION. Obturator prosthesis is a highly positive and non-invasive approach to improve the quality of life of patients with maxillectomy defects.
환자의 교합에 맞춰 적절한 고정성 보철물을 성공적으로 제작 및 수복하기 위해서 필요한 다양한 고려사항이 있다. 그 중 환자가 갖고 있는 교합의 형태를 파악하여 고정성 보철물에 부여하는 것은 필수적이라 할 수 있다. 견치 유도 교합, 군 기능 교합 등 여러가지 교합 형태를 파악하는데 있어서 하악의 운동을 기록하는 것이 중요한데, 1933년 Meyers에 의해 소개된 Functionally generated path (FGP) technique은 구내에서 왁스 등의 재료를 이용하여 교두가 지나가는 하악 운동로를 기록한 후 보철물 제작에 적용하는 치료 술식이다. 본 증례에서는 두 명의 환자에 있어서 상악 구치부에 이차 우식 혹은 치주염으로부터 기원한 치수 유래 병소가 있어, 근관 치료 및 치주 치료 완료 후 고정성 보철물 수복이 필요하였다. 장기적인 예후가 좋지 않은 치아이기에 측방 간섭을 최소화하기 위해 FGP technique을 이용하여 하악 운동을 기록하였다. 통상적인 석고 인상 방법이 아닌 구내 디지털 스캔을 시행함으로써 더 효율적으로 보철물을 제작하였다. 치료 후 환자와 술자 모두 기능적으로 만족할 만한 결과를 얻었기에 이를 보고하는 바이다.
Provisional fixed partial dentures(FPDs) are an important part of many prosthodontic treatment procedures. These provisional fixed prostheses must fulfill biologic, mechanical, and esthetic requirements to be considered successful. Consideration of all these factors and requirements are important because provisional resin restorations may be worn over a long period to assess the results of periodontal and endodontics therapies, and also during the restorative phase of implant reconstructive procedures. This in vitro study examined flexual strength of four resins commonly used for fixed provisional prostheses. The effects of polymerization conditions were also evaluated. The four resins tested were : Caulk Temporary bridge resin(L.D. Caulk Co. Dentsply International Millford), Jet(Lang Dental Mfg. Co. Chicago. ILL. U.S.A), Alike (Coe Laboratories. Inc. Chicago. ILL. U.S.A) and Tokuso Curefast (Coe Laboratories. Inc. Chicago. ILL. U.S.A) The test specimens were 65mm long, 14mm wide, and 3.5mm thickness. 10 specimens of four resins were cured for 15 minutes at atmospheric pressure and 10 specimens of four resins were cured at an additional pressure of approximately 20 psi. A total of 80 specimens were prepared. The flexual strength was determined by three-point bending test. Data were analysed with the Paired samples T-test and Tukey student-range test Within the limitations imposed in this study, the following conclusions can be drawn : 1. Under the condition of bench curing, Caulk Temporary bridge resin showed the highest flexual strength. In decreasing order, the flexual strength of the other materials was as follows : Jet, Tokuso Curefast, Alike, and Caulk Temporary bridge resin demonstrated significantly higher strength than other resins. 2. Under the condition of pressure curing, Jet showed the highest flexual strength. In decreasing order, the flexual strength of the other materials was as follows : Caulk Temporary bridge resin, Tokuso Curefast, and Alike. There were all statistically significant differences among four resins 3. There was a statistically significant difference between bench- and pressure-cured specimens in all four materials.
본 연구의 목적은 다공성의 티타늄 산화막에 불소를 처리한 결과를 평가하는 것이다. 디스크에 양극산화법을 통하여 다공성의 티타늄산화막표면을 얻은 후 불소를 처리하고 일정시간동안 Hank액에 침전을 시켰다. 양극산화를 통해 일정거칠기의 표면을 얻었으며 SEM과 XRD를 통하여 표면의 형상과 성분을 분석하였다. 생성된 표면은 빠른 표면 활성도를 보였으며, 적절한 거칠기와 좋은 골반응으로 골유착에 기여할 수 있을 것으로 여겨진다.
목적: 3.6 mm 이하 좁은 직경 임플란트의 일정 기간 생존율을 초기고정 값과 방사선학적 계측을 통해 분석해보고자 하였다. 연구 재료 및 방법: 24명의 환자에 식립된 직경 3.6 mm 이하, 길이 7 mm 이상의 38개의 임플란트를 조사하였다. 식립된 임플란트의 platform 직경, body 직경, 길이, 식립 부위 등을 확인하였고 식립 시 골유도재생술의 동반 시행 여부, 완성된 보철의 형태를 확인하였다. 초기고정 값은 임플란트 식립 직후 측정한 implant stability quotient (ISQ) 값을 확인하였고, 방사선 사진을 통해 식립된 임플란트의 근 원심 marginal bone level (MBL)을 측정하였다. 결과: 전체 임플란트의 생존율은 92.11% 이었고 ISQ값은 평균 66.26으로 나타났다. 생존한 임플란트의 MBL 변화는 평균 $0.14{\pm}0.31mm$를 나타내었다. Platform 직경이 body 직경보다 큰 임플란트 중 실패한 임플란트는 없었다. 결론: 좁은 직경의 임플란트는 좁은 직경의 치조제에 안정적인 치료 결과를 나타낸다 사료된다.
현대 사회에는 도시 생활과 환경으로 인한 건강 문제가 증가하고 있는데 자연환경은 이러한 문제에 좋은 대안으로 대두되고 있다. 특히 숲과 나무는 예전부터 치유를 위한 공간으로 알려져 왔다. 주요우울장애는 매우 일반적이고 만성적인 질병으로 치료방법에는 약물치료와 다양한 심리치료가 포함되어 있다. 그러나 이러한 치료들은 환자들의 기능과 활동을 복귀시키는 것 보다는 기분 증상을 치료하는 것에 집중되어 있다. 본 연구에서는 우울증 환자들의 재활을 위한 산림활동 심리프로그램을 개발하였다. 본 연구에는 약물치료로 우울 증상은 개선되었으나 활동과 기능이 회복되지 못한 9명의 우울증 환자들이 참여하였다. 산림치유 프로그램은 4회기가 진행되었으며 매 회기마다 환자들의 우울증상과 심박변이도를 측정하였다. 4주간의 산림활동 심리프로그램 후 HRSD(Hamilton Rating Scale for Depression)로 측정된 우울감이 유의하게 감소하였으며, 건강관련 삶의 질을 측정하는 SF-36의 하위 척도의 점수가 상승하였고 심박통일성이 증가하였다. 본 예비적 연구 결과, 산림환경을 이용한 활동과 심리치료적 프로그램은 약물치료 중인 우울증 환자들의 기능회복과 재발방지에 긍정적인 역할을 할 가능성을 확인하였다.
본 연구는 칼텐본을 접목한 흉추가동운동이 폐기능에 미치는 영향을 알아보기 위하여 20대 대학생 20명을 대상으로 4주간 실시하였다. 실험대상자 20명을 대상으로 실험군, 대조군으로 무작위 동공선택방식으로 그룹 배정을 하였다. 실험군은 횡격막 호흡운동 15분과 칼텐본을 적용한 흉추가동운동 15분으로 구성되고, 대조군은 횡격막 호흡운동으로 30분간 훈련하였으며, 두 군의 운동프로그램은 주 3회 4주간 실시하였다. 측정은 폐기능을 측정하여 자료를 분석하였다. 실험 전·후 폐기능을 분석해 보면, 실험군은 TV, IRV에서 유의하게 증가하였으며, 실험후에 두 집단간의 변화검증에서도 TV와 IRV에서 유의한 차이가 나타났다. 본 연구를 통해 칼텐본을 이용한 흉추가동운동이 폐 기능에 더 긍정적인 효과를 주는 것을 알 수 있었다. 앞으로 임상에서 호흡훈련 시 일반적인 호흡훈련에 흉추가동운동을 함께 접목하는 치료방법이 적극적으로 중재되기를 기대하며 다양한 추가운동프로그램의 개발이 필요할 것으로 사료된다.
구치 결손 부위가 장기간 방치될 경우 후방 구치의 근심이동과 대합치 정출 등의 원치 않는 치아이동, 치조골의 소실, 치아치조 교합의 붕괴를 야기할 수 있다. 따라서 치아상실 후 가능한 빠른 시일 내에 결손 부위에 대한 보철 수복 치료를 권장하고 있지만, 제3대구치 등 잔존 치아를 결손 부위의 적절한 위치로 이동시키는 교정치료를 병행할 경우 최적의 크기와 형태로 보철 수복이 가능하고, 기능교합 시 힘의 분산을 고르게 할 수 있으며, 수복의 범위를 최소화하여 생역학적으로 보다 유리한 치주 환경을 조성할 수 있다. 본 증례는 다수의 구치부 치아를 상실한 두 환자를 비교하여 구치부 치아 결손 환자에서 제3대구치의 교정적 활용시 고려할 사항에 대해 고찰하고자 한다.
본 연구에서는 노후 콘크리트 포장의 효율적 유지보수를 위하여 국내 최초 적용된 연속철근 콘크리트 개념의 얇은 덧씌우기 공법(CRCO : Thin Bonded Continuously Reinforced Concrete Overlay)의 거동 특성을 살펴보았다. 서해안 고속도로의 폐도 구간에 CRCO 공법과 JCO 공법(Jointed Concrete Overlay)을 시험시공 하였으며, CRCO 공법에 삽입된 종방향 철근으로 효과를 검토하였다. 시험시공 후 균열 조사 결과, CRCO 구간의 기존 줄눈부에서부터 반사 균열이 발생하였으나 그 폭은 매우 좁았다. 철근의 구속역할로 인해 CRCO 공법에서 더 많은 균열이 발생하였으며, 그 간격은 CRCP 보다 더 좁았다. 부착강도 측정 결과, 표면 처리로 Cold Milling 후 이물질을 제거하여 초기에 층간 부착 문제가 발생하지 않았다. 기존 JCP의 줄눈부에 수평으로 매설한 계측기 데이터분석 결과, 초기 균열 터짐 시 일정 폭 이상으로 벌어지지 못하게 종방향 철근이 잡아주는 역할을 하였으며, 온도변화에 대한 수평 변위 발생 억제 효과도 있었다. 하지만 층간 부착 문제가 발생할 경우, 철근의 구속 효과는 저감되었다. 수직 계측기의 데이터를 통해서 철근이 없는 JCO 공법에 비해 CRCO 공법은 종방향 철근 위치에서 층간 부착 문제가 발생하지 않았으며, 현장 코어링 작업을 통해 이를 확인하였다. 슬래브 내의 수평 거동을 분석한 결과, 초기에 철근의 구속으로 인해 CRCO 공법에서는 인장 변형률이, 구속이 미미한 JCO 공법은 압축력이 발생하였다. FWD 데이터 분석 결과, CRCO 공법의 처짐량이 JCO 보다 더 적게 발생하였으며, 지지력 평가에서는 전반적으로 덧씌우기 후가 더 크게 나타났으며, 특히 CRCO 공법이 JCO 공법에 비해 지지력에 대한 덧씌우기 두께 효과가 더 컸다.

Aromatherapy is a branch of complementary or alternative therapy which is increasing in popularity, yet has scant scientific credibility. Aromatherapy should be defined as treatment using odors and practised as such. However, essential oils are usually used in conjunction with therapeutic massage and often combined with counselling of some kind. Aromatherapy complements and enhances the therapeutic powers of massage. Massage is one of the most wonderful ways to relax and is throughly beneficial to health. Massage can help unknot tense and aching muscles and other minor symptoms of stress, leaving patients fresh and energized. As the use of aromatherapy within a health care setting has grown so rapidly in recent years, and will continue to do so, the need for suitable training has become apparent. No health service can afford the risk of having staff who are inadequately trained in the practice of aromatherapy using essential oils incorrectly on those in a state of ill-health, especially if the essential oils used are not to a standard suitable for therapeutic use. Training to an acceptable level in aromatic therapy is essential for safety and effectiveness. Knowledge of the nature and make-up of essential oils, their effect on the body and the emotions, and how, when, and where to apply them is imperative in order for them to be beneficial to a patient's health. In order to achieve best practice, further research is necessary to explore the use of aromatherapy in the management of multiple disorder.
