Mirror Neuron System and Stroke Rehabilitation (미러뉴런시스템과 뇌졸중 재활)

  • Purpose : The purpose of this article was to review the literature on mirror neuron system with reference to its functional diversity in stroke rehabilitation.. Method : This review outlines scientific findings regarding different neurophysiological properties in mirror neurons, and discusses their involvement in process of stroke rehabilitation. Result & Conclusions : Mirror neurons were first discovered in macaque monkey. These neurons, like most neurons in F5 areas in premotor cortex, fired when an individual performs an action, as well as when he/she observes a similar action done by another individual, although originally fired only during action execution. Mirror neurons form a network for motor planning and initiating of motor action. Thus, in stroke rehabilitation based on the mirror neuron-action observation, motor imagery, observation with intent to imitate and imitation-may help activate mirror neuron system for improved outcome of physical therapy. These studies provide a scientific theoretical basis and discuss for the use of mirror neuron system as a complement to clinical physical therapy in stroke rehabilitation.

A Study on Disability Database and Applicable System to Provide Continuous and Comprehensive Rehabilitation Service (지속적·포괄적 재활서비스 제공을 위한 장애인 통합 데이터베이스 및 활용체계 연구)

  • Background : Demands have increased for a variety of welfare services and customized services for persons with disabilities(PWD). A management System focused on PWD was needed to provide for comprehensive services. The purpose of this study was to design a disability database and an application system in order to provide continuous and comprehensive rehabilitation service for PWD. Methods : We analyzed local and abroad disability-related policies and systems and derived the contents that should be included in the integrated database for PWD through a survey among rehabilitation specialists. Result : The integrated database for PWD was composed of 7 categories including General Characteristics, Health & Medicine, Assistance, Education, Employment, Economics and Daily & Social Life. The applicable system of integrated database for PWD was proposed to help conducting policies in such areas as follows ; 'welfare', 'education and culture', 'economic activity', 'social participation' and 'Health'. Conclusion : The main goal of disability policy and strategy should be established by systematically analyzing disability-related data integrating database of PWD. Accordingly, specific objectives and directions for disability policies should be set and efficiently managed and operated. The integrated database for PWD may be utilized for disability-related policies and service monitering, sustainable and integrated management and community participation and integration based on the rights of the disabled.

  • Objectives : The purpose of this study is to find correlation about 7-Zone-Diagnostic System and Clinical Parameters using Oriental Health Examination. Methods : Subjects of our study were 325 cases who took Oriental Health Examination. We collected the data of body composition analysis, 7-Zone-Diagnostic System, blood analysis and devided by 7-Zone-Diagnostic system to acceleration, normalcy and depression. Results and Conclusions : The values of muscle mass, body fat mass, abdomen obesity rate, right arm water index, left arm water index, trunk water index, right leg water index, left leg water index, BMI, BMR, GOT, GPT, ${\gamma}$-GTP, BUN, TC, triglyceride and hemoglobin were continuous with 7-Zone-Diagnostic system.

  • In this study, we wolud be developed the fuzzy controlled PGO that controlled the flexion and the extension of each PGO's joint using the bio-signal and FSR sensor. The PGO driving system is to couple the right and left sides of the orthosis by specially designed hip joints and pelvic section. This driving system consists of the orthosis, sensor, control system. An air supply system of muscle is composed of an air compressor, 2-way solenoid valve(MAC, USA), accumulator, pressure sensor. Role of this system provide air muscle with the compressed air at hip joint constantly. According to output signal of EMG sensor and foot sensor, air muscles and assists the flexion of hip joint during PGO gait.

  • Assessment and rehabilitation of patients with unilateral visual neglect has been one of the fields that require assistive technology. Paper-and-pencil tests, including the LBT (Line Bisection Test), have been one of the most commonly used visual neglect assessment methods used in a clinical setting. The key motivation of this study was to establish a computer-based real-time assessment system for the hemi-neglect patients without altering the conventional paper-and-pencil based user tools. A digital penbased assessment and rehabilitation system, the ePen System, could eliminate the manual assessment time while maintaining measurement accuracy. As a result, the proposed system may assist rehabilitation specialists to assess and diagnose patients with unilateral visual neglect. This system can be applied to a range assessment and rehabilitation modalities based on a pen and paper. It can also be applied to various patients such as those with Parkinson's disease, stroke sufferers, or those who have experienced different forms of brain lesions.

  • In this study, active and recently in the field of vocational rehabilitation of persons with disabilities in the industry engaged in the primary mission of assisting you want to promote activation of devices that can perform the work of the harvest cart or unattended devices such as tool boxes and tracking system developed. This system is manually moved from a remote user is able to control, and also, the ability to track users unattended mounted. The system developed in this study in the field of vocational rehabilitation in order to allow all sides to assess more than three meters in open space system for the Y-and W-course driving range of the error from the final destination 5 times the period analyzed. Analysis of the user's tracking system developed without any problem and that could know.

The Development of Korean Rehabilitation Patient Group Version 1.0 (한국형 재활환자분류체계 버전 1.0 개발)

  • Background: Rehabilitations in subacute phase are different from acute treatments regarding the characteristics and required resource consumption of the treatments. Lack of accuracy and validity of the Korean Diagnosis Related Group and Korean Out-Patient Group for the acute patients as the case-mix and payment tool for rehabilitation inpatients have been problematic issues. The objective of the study was to develop the Korean Rehabilitation Patient Group (KRPG) reflecting the characteristics of rehabilitation inpatients. Methods: As a retrospective medical record survey regarding rehabilitation inpatients, 4,207 episodes were collected through 42 hospitals. Considering the opinions of clinical experts and the decision-tree analysis, the variables for the KRPG system demonstrating the characteristics of rehabilitation inpatients were derived, and the splitting standards of the relevant variables were also set. Using the derived variables, we have drawn the rehabilitation inpatient classification model reflecting the clinical situation of Korea. The performance evaluation was conducted on the KRPG system. Results: The KRPG was targeted at the inpatients with brain or spinal cord injury. The etiologic disease, functional status (cognitive function, activity of daily living, muscle strength, spasticity, level and grade of spinal cord injury), and the patient's age were the variables in the rehabilitation patients. The algorithm of KRPG system after applying the derived variables and total 204 rehabilitation patient groups were developed. The KRPG explained 11.8% of variance in charge for rehabilitation inpatients. It also explained 13.8% of variance in length of stay for them. Conclusion: The KRPG version 1.0 reflecting the clinical characteristics of rehabilitation inpatients was classified as 204 groups.

  • Physical rehabilitation consists of exercises that help individuals who have suffered injury, disease, or nervous system disorders to recover lost or impaired functions. The exercises need to be tailored to the degree and type of the patient's condition. To this end, a comprehensive rehabilitation data delivery system that connects medical institutions and local communities should be established, and an intelligent rehabilitation service technology should be developed. An intelligent rehabilitation service integrates and manages the patient's health information provided by medical institutions and the overall function evaluation information measured in the community. Based on the collected data, personalized rehabilitation services can be provided. We examine supportive policies and technological trends for rehabilitation exercise and sports. Additionally, we investigate development trends and key technologies of intelligent rehabilitation services being developed at ETRI.

Quantitative Evaluation of Rehabilitation Therapy Based on a Two-Finger Force Measurement System

  • This paper describes the evaluation of the effectiveness of rehabilitation therapy for patients with finger paralysis based on a two-finger force measurement system (TFFMS). The paralyzed fingers can be recovered through rehabilitation therapies. The finger pressing force of the patients can be measured utilizing the TFFMS previously developed by the author [7]. The TFFMS, however, has not been fully adopted as a standard method for evaluating the therapy owing to the lack of a standard protocol. The pressing force of healthy volunteers and patients is analyzed with the TFFMS to explore the feasibility of the TFFMS as an evaluation device. The test confirms that the established standard protocol is useful to quantitatively assess the progress of finger rehabilitation therapy.

  • For rehabilitation of stroke patients the repeat training is needed. However it is difficult to keep up the training for the patients because it is boring. Even if the patients continue the training there is no objective way to evaluate the remedial value. Recently there is a new attempt to apply the haptic system to the rehabilitation of the stroke patients. In this study a haptic system is applied to the rehabilitation of the stroke patients. Through the comparative analysis of the experimental data for the normal people and patients, the validity of the proposed rehabilitation was verified. In conclusion, the patient's condition at 8 days after the experiment has demonstrated the level of ADL.
