Analysis of Priority Factors and Relative Importance about the Components of Rehabilitation Physical Activity Program (재활체육프로그램 구성요인의 상대적 중요도 및 우선순위 분석)

  • The purpose of this study is to develop systematical components and implementation of rehabilitation physical activity by analyzing the relative importance among each factor. To achieve the purpose of this study, 20 participants were selected, and they consisted of instructors and adapted physical activity teachers and professional who have experiences in institutes and hospital where have operate program related to rehabilitation physical activity and adapted physical activity program. Delphi technique was conducted for three times by the participants. For the decision of priorities about improvement factors, AHP was performed. The results were as follows. As the result of this study, the factors of the rehabilitation physical activity consisted of six type of exercise; fitness, psychological exercise, aquatic exercise, sense & perception exercise, ambulatory exercise, and motor skills. Also, 21 sub-factors were also extracted. The result from analyzing the priorities of factors was fitness 33.2%, aquatic exercise 20.6%, psychological exercise 16.3%, sense & perception exercise 11.7%, ambulatory exercise 10.8% and motor skills 8.6%. Also, among results from the weighted state of priority factors, muscular strength and endurance were the most importance factor which had 23.4% weighting, and meditation was the least importance factor which had 0.5% weighting.

Literature Review of Exercise Therapy Effects in Patients with Breast Cancer (유방암 환자의 운동치료 효과에 대한 문헌고찰)

  • Purpose : This study was to evaluate the effectiveness of exercise therapies for breast cancer patients through review analysis of the rehabilitation they had usually received to increase the quality of their lives. Exercise therapy for breast cancer patients has proven to be highly effective, so it is strongly recommended. Method : Focusing on breast cancer-related journal articles published since 1990, an analysis using the Spolit and Medline search engines was undertaken of breast krebs, tumors, cancer, sportaktivitaet, and exercise in patients with breast cancer. Exercise for breast cancer patients has been considered to have positive effects on fatigue, muscle weakness, and the range of motion of upper extremities, as well as on depression. Breast cancer patients have to be divided according to their identifying clinical characteristics, and then have to be prescribed a suitable exercise program. In accordance with previous articles, a varied exercise program was organized in terms of frequency, duration, and intensity. The frequency was more than three times per week. Its duration was about 30 minutes per session. Result : The intensity of exercise was moderate. The types of exercises included recommended strengthening exercises, as well as various aerobic exercises. Based on previous data, a new exercise method for breast cancer patients was introduced in this study. Conclusion : Above all, exercise programs should be planned according to the different characteristics of breast cancer patients, in order to reduce physical impairments and psychological problems.

The Effect of Home stayed Stroke Patients' gait, Valance, Activities of Daily Living, Depression in the Aerobic Walking Exercise Program. (유산소 걷기운동 프로그램이 재가 뇌졸중 환자의 보행, 균형, 일상활동 수행능력, 우울에 미치는 효과)

  • This study was a quasi-experimental study of nonequivalent control group pretest- posttest design to investigate the effect of aerobic walking exercise program on the physical & psychological functions of home stayed stroke patients. The data were collected during the period of May 20th to August 15th, 2001. The subjects for this study were 40 hemiplegic stroke patients with the experimental group consisting of 19 patients and the control group being composed of 21 patients. The patients selected for this study were: (a)living in J city who had been diagnosed with stroke and at home after being discharged from the hospital, (b)suffering from stroke for 6 months to 5 years, (c)without recognition disorder with the MMSE-K score above 25, (d)below 2 on the modified Ashworth scale, (e)free from heart and pulmonary disease (f)able to walk beyond 15 minutes for themselves. The aerobic walking exercise program for the experimental group was aerobic exercise and education and supportive care. The aerobic exercise was 8 weeks' period, three times a week, 35 to 50 minutes a day. And the education and supportive care was consisted of one home visiting and 2 times telephoning a week. The data were analysed by $X^2$-test, paired t-test and unpaired t-test and ANCOVA through SAS/PC program. The results of the study were as follows: 1. There was insignificant difference in the gait length experimental and control group. There was significant difference in the gait speed between the two groups. 2. There was significant difference in the dynamic valance between the two groups. 3. There was significant difference in ADL score between the two groups. 4. There was no significant difference in the depression between the two groups. As shown above, the results of 8 weeks' the aerobic walking exercise program for home stayed stroke patients produced positive effects on gait speed, dynamic valance, ADL score. And this program was expected that it was more effective in different intervention period, verified program. Also it was needed follow study.

Study on the Satisfaction Use of Rehabilitation Exercise Room (일부 보건소의 재활운동실 이용 만족도에 관한 연구)

  • The purpose of this study was to investigate the degree of satisfaction in using rehabilitation exercise rooms in public health centers. A total of 100 subjects were participated in the study who were clients of rehabilitation exercise rooms in two public health centers in one city of Kyunggi Do, Korea. Data were collected from March 1 to 30, 2003 using questionnaire. The data were analyzed using SPSS/PC computer program. The result are as follows : 1. The mean age of subjects was 62.6 years old, and men constituted $67.0\%$ of the participants. Most participants $(64\%)$ were in the middle socioeconomic status, and $(65\%)$ of subjects was diagnosed as stroke. 2. Regarding the source of information about rehabilitation exercise room in public health center, $56.0\%$ obtain information from colleagues or neighbors, and for most participants the time required to travel to rehabilitation exercise room was less than 30min on foot. Among rehabilitation services, Taping therapy $(53.0\%)$ was most frequently used among participants, 3. Sum of all satisfaction score was $71.10(\pm6.60,\;range:\;1-90)$ showed higher level of satisfaction. Cost, reliability, and kindness were main factors that affect the degree of satisfaction among participants. 4. Twenty-four percent of the respondents utilized clinic services in the health centers, and $27.4\%$ stoped rehabilitation services from hospitals and/or clinics due to high cost. 5. In the use of rehabilitation exercise room was positively correlated to age (r=.214, p<0.05) and disease duration(r=.241. p<0.05).

Effects of Rhythmic Exercise Program on the Perceived Health Status, Depression, Life Satisfaction and Physical Strength in Elderly (율동운동 프로그램이 노인의 지각된 건강상태, 우울, 생활 만족도, 체력에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of rhythmic exercise program on perceived health status, depression, life satisfaction and physical strength in elderly. Method: A convenience sample of 20 elderly, participated in the study from a community health center located in an urban city, South Korea. SPSS/WIN 17.0 was used for descriptive analysis, paired t-test and Pearson correlation. Results: After 8 weeks exercise program, perceived health status (t=0.48, p<.001), depression (t=-0.93, p=.001), life satisfaction (t=1.01, p=.002 and physical strength (t=1.70, 0.70, 0.72, all p<.001) in elderly were significantly improved. Three were significant correlations between depression and perceived health status (r=-.63, p=.004) and life satisfaction (r=-.70, p=.001). Conclusion: Further are warranted to validate the effects of a rhythmic exercise program for the elderly in a larger sample.

Comparison of the Rehabilitation Program after Rotator Cuff Repair by Time Closed Chain Exercise (회전근개봉합술 후 닫힌사슬운동 적용 시점에 따른 효과 비교)

  • PURPOSE: This study aimed to examine the pain, range of motion (ROM), upper extremity task performance, and functional levels of patients after rotator cuff repair according to the timing of a closed chain exercise thereby presenting basic data for an effective rehabilitation program. METHODS: The intervention was applied three times per week, one hour per day, for four weeks to 40 participants, 78 of whom had undergone rotator cuff repair. The participants were divided into four groups and assigned to usual general physical therapy and an open chain exercise. Group I consisted of the open chain exercise only. The closed chain exercise was applied to group II after the 4 times, group III after the 7 times, group IV after the 10 times. Measurement were used ROM, visual analogue scale (VAS), box and block test (BBT), and shoulder pain and disability index (SPADI). A one-way analysis of variance was conducted to test differences. RESULTS: There were significant differences in the internal/external rotation between group I and group II. The VAS significantly differed between group II and group I, group III, and group IV. The BBT results of group II and group I were significantly different compared to those of group IV. The SPADI significantly differed between group II and group I and between group II and group IV. CONCLUSION: The closed chain exercise was effective for patients following rotator cuff repair from the second week after active exercise was prescribed, verifying its applicability in rehabilitation programs.

Comparison of Supraspinatus Muscle Architecture During Three Different Shoulder Strengthening Exercises Using Ultrasonography

  • Background: Strengthening the supraspinatus is an important aspect of a rehabilitation program for subacromial impingement and tendinopathy. Many authors recommended empty-can (EC), full-can (FC), and prone full-can (PFC) exercises to strengthen the supraspinatus. However, no ultrasonography study has yet investigated supraspinatus muscle architecture (muscle thickness; MT, pennation angle; PA, fiber bundle length; FBL) in relation to supraspinatus strengthening exercises. Objects: The purpose of this study was to compare the architecture (MT, PA, and FBL) of the supraspinatus muscle during three different types of exercises (EC, FC, and PFC) using diagnostic ultrasound. Methods: Participants performed three different exercises: (A) EC; the arm was maintained at $60^{\circ}$ abduction with full internal rotation in the sitting position, (B) FC; the arm was maintained at $60^{\circ}$ abduction with full external rotation in the sitting position, and (C) PFC; the arm was maintained at $60^{\circ}$ abduction with full external rotation in the prone position. Ultrasonography was used to measure the MT, PA and FBL of the supraspinatus. One-way repeated analysis of variance with Bonferroni's post-hoc test was used to compare between the three exercises and the initial position of each exercise. Results: Compared with each initial position, the FC exercise showed the greatest mean difference in muscle architecture properties and the PFC exercise showed the least mean difference. Conclusion: The findings suggest that the FC exercise position may have an advantage in increasing the amount of contractile tissue or producing muscle power and the PFC exercise position may be useful in a rehabilitation program because it offers the advantage of maintaining the muscle architecture properties.

Effects of Rehabilitation Programs with Couching Exercise or Home Exercise on Shoulder Pain, Muscle Strength, and Daily Living Functions in Patients with Shoulder Impingement Syndrome (재활 프로그램을 적용한 코칭 운동과 자가 운동이 견관절 충돌증후군 환자의 견부통증, 근력 및 일상생활기능에 미치는 효과)

  • Purpose: This study aimed to investigate the effects of rehabilitation programs with coaching exercise (CE) or home exercise (HE) on shoulder pain, muscle strength, and daily living functions (DLF) in patients with shoulder impingement syndrome. Methods: With quasi-experimental design, thirty patients (15 CE and 15 HE) participated in rehabilitation programs for 12 weeks. At first, data were collected for pain and DLF using questionnaires with testing muscle strength and range of motion. Then, subjects were allowed to receive training on rehabilitation with structured training materials. CE group visited sports center three times a week at P hospital and HE group did self-exercise at home. Data were analyzed using SPSS program. Results: After 12 weeks, both groups showed statistically significant changes in improving pain, muscle strength, and DLF. Compare to HE group, CE group showed a statistically significant improvement in shoulder pain (p=.021), muscle strength in internal rotation (p=.001) and abduction (p=.013) and DLF (p=.012). Conclusion: Rehabilitation programs for patients with shoulder impingement syndrome were effective to improve pain, muscle strength, and DLF. Although CE would bring the better effects, it is possible to apply HE through structured education to patients who are difficult to visit hospitals.

Effects of a Complex Exercise Program using Dynamic Taping on Patients with Tension-Type Headache and Chronic Neck Pain with Forward Head Posture (다이나믹 테이핑을 적용한 복합 운동프로그램이 긴장성 두통과 전방머리자세를 동반한 만성 목 통증 환자에게 미치는 효과)

  • Purpose : This study examined the effects of pain, neck dysfunction, psychosocial level, headache impact test (HIT), postural alignment, and trapezius muscle tone of the complex exercise program using dynamic taping in patients with tension headache and chronic neck pain with forward head posture. Methods : The design of this is a randomized controlled trial (RCT). Thirty-four patients with chronic neck pain were screened using a randomized assignment program and assigned to experimental group (n=17) and control group (n=17). Both groups underwent a complex exercise program. In addition, the experimental group dynamic taping was applied to the upper trapezius muscle. All interventions were applied three times per week for four weeks. The visual analogue scale (VAS), the neck disability index (NDI), short form-12 health survey questionnaire (SF-12), Headache impact test-6 (HIT-6), Craniovertebral angle (CVA), Cranial rotation angle (CRA), upper trapezius muscle tone were compared to evaluate the effect on intervention. Results : Both groups showed significant differences before and after the intervention in VAS, NDI, SF-12, HIT-6, and CVA, CRA (p<.05). In addition, significant differences in NDI and upper trapezius muscle tone were observed between the experimental group and control group (p<.05). Conclusion : A complex exercise program using dynamic taping for patients with tension headache and chronic neck pain with forward head posture are effective method with clinical significance in improving the function and reducing upper trapezius muscle tone.

The effects of virtual reality-based physical therapy in stroke patients

  • Objective: Final goal of nerve advancement therapy is to provide maximum ability to function independently in life to patients. This paper appraises and describes basic concepts of the virtual reality (VR) based exercise program to improve functional movement for neurologically impaired patients. Design: Review article. Methods: Stroke patients from the physical therapy department while wearing comfortable clothing receive therapy and also VR based motion therapy administered by the therapist in charge. After evaluation of stroke patients, therapy includes an exercise program that is suitable for use with stroke patients; stroke patients wear head-mounted display while in front of the computer, where the camera is located; they follow the action on the screen and the computer perceives the operation of the stroke patients according to subject accomplishment. Results: According to obstacle condition of stroke patients using the method, which is various environments after setting, in stroke patients, there is a possibility of presenting suitable therapeutic environments. The display presentation of the method, which is identical, causes difficulty for all stroke patients. According to subject accomplishment; stroke patients result in execution of repetition training and deepening study, which leads to mobility. Conclusions: The VR based rehabilitation training programs is a difference of the existing video training program, is immediate feedback and compensation method. It will provide rehabilitation training services for the family of the patient whose condition could be improved with rehabilitative therapy where it is a continuous circumstance as a matter of the social welfare facility therapy.
