  • It is well known that the lifetime incidence of low back pain is extraordinarily high, but those who incur the majority of the cost, both personally and financially, are the chronic pain. Stabilization programmers attracted our interest, with their aims of using the muscle system to protect spinal joint structures from further repetitive microtrauma, recurrent pain and degerative change. In overviewing the stabilizing role of the trunk and back mucles our attention became focused on muscles which controlled the lumbar and lumbosacral joints rather than on muscles which span the spine from the thorax to pelvis. It was considered that muscles such as the lumbar multifidus, transversus abdominis, and possibly also parts of the obliquus internus abdominis, would most likely function to stabilize the segments of the lumbar spine. In order to check if these muscles were functioning in low back pain patients, it was necessary to devise specific muscle tests. The new concept involves exercises using only relatively low activity levels in the muscles. More emphasis is placed on a motor skill which has to be relearned, practised and then gradully incorporated back into functional movement.

Improvement in shear strength characteristics of desert sand using shredded plastic waste

  • In the Kingdom of Saudi Arabia, the shallower depth of the earth's crust is composed of loose dune or beach sand with soluble salts. The expansive behavior of salt bearing soil, fluctuation of ground water table and extreme environmental conditions offer a variety of geotechnical problems affecting safety and serviceability of the infrastructure built on it. Despite spending money, time and other resources on repair and rehabilitation, no significant attention is paid to explore the root causes of excessive differential settlement and cracking to these facilities. The scientific solution required to ensure safety and serviceability of the constructed infrastructure is to improve the strength and durability properties of the supporting ground. In this study, shredded plastic is employed as a low cost and locally available additive to improve strength characteristics of the desert sand. The study shows a remarkable increase in the shear strength and normal settlement of the soil. A seven (07) degree increase in angle of internal friction is achieved by adding 0.4 percent of the shredded plastic additive. The effect of different proportions and sizes of the plastic strips is also investigated to obtain optimum values. Such a long-lived solution will seek to reduce maintenance and repair costs of the infrastructure facilities laid on problematic soil along with reduction of environmental pollutants.

  • With the rise in popularity of photographic and video cameras, an increasing number of fields are now using thermal imaging cameras. One such application is in the diagnosis of breast cancer, as thermal imaging provides a low-cost and noninvasive method. Thermal imaging is particularly safe for pregnant women, and those with large, dense, or sensitive breasts. In addition, excessive doses of radiation, which may be used in traditional methods of breast cancer detection, can increase the risk of cancer. This paper presents one method of breast cancer detection. Breast images were taken using a thermal camera, with preliminary experiments conducted on Cambodian women. Then the experimental results were analyzed and compared using Shannon entropy and logistic regression.

  • A fiber-reinforced composite (FRC) fixed prosthesis is an innovative alternative to a traditional metal restoration, as it is a conservative treatment method. This case report demonstrates a detailed procedure for restoring a missing anterior tooth with an FRC. A 44-year-old woman visited our department with an avulsed tooth that had fallen out on the previous day and was completely dry. This tooth was replanted, but it failed after one year. A semi-direct technique was used to fabricate a FRC fixed partial prosthesis for its replacement. The FRC framework and the pontic were fabricated using a duplicated cast model and nanofilled composite resin. Later on, interproximal contact, tooth shape, and shade were adjusted at chairside. This technique not only enables the clinician to replace a missing tooth immediately after extraction for minimizing esthetic problems, but it also decreases both tooth reduction and cost.

  • The bridges, which were built between 20 and 30 years ago in rural area, have to support unexpected overload caused by excessive amount of transportation. For these rural bridges, repairs and replacements are needed. To avoid the high cost of rehabilitation, the bridge rating must correctly report the present load-carrying capacity. Rating engineers use Allowable Stress Design (ASD), Load Factor Design (LFD), and Load Resistance Factor Design (LRFD) to evaluate the bridge load carrying capacity. In this paper, the load rating methods are introduced, and it is illustrated how to use the load test data from literature survey. Load test is conducted to the bridge that was built 30 years ago in rural area. From load test results, new maintenance strategy is suggested instead of the bridge replacement.

Effects of Inspiration and Expiration Exercise Combined with Upper Extremity Proprioceptive Neuromuscular Facilitation on Forced Volume Vital and Peak Expiratory Flow

  • Purpose: The purpose of this study was to examine the effects of inhalation and exhalation exercise combined with upper extremity proprioceptive neuromuscular facilitation pattern on two spirometry values: forced volume vital (FVC) and peak expiratory flow (PEF). Methods: Thirty-two healthy adults were divided into two groups: 1) a combined group, which performed upper extremity D2 flexion pattern (shoulder flexed/abducted/external rotated, forearm supinated, wrist radial deviated, and finger extended) during exhalation and D2 extension pattern (shoulder extended/adducted/internal rotated, forearm pronated, wrist ulnar deviated, and finger flexed) during inhalation; and 2) reverse combined group, which performed the D2 flexion pattern during inhalation and the D2 extension pattern during exhalation. The inverse application of upper extremity movements during inhalation and exhalation induced selective resistance or assistance on respiration. FVC and PEF were measured at two time points, before and after four weeks. Results: In both groups, the pre-post intervention comparison showed significant increases in FVC and PEF (p < .05). In the between-groups comparison, the reverse combined group showed a significantly higher PEF than the combined group at four weeks post intervention (p < 0.05). Conclusion: The combined respiration exercise with reverse PNF upper extremity patterns using selective resistance showed an effective increase in PEF in healthy adults. Clinicians and researchers might consider using selective resistance as a widely applicable and cost-effective option for respiratory rehabilitation planning.

Implant Supported Overdenture using Locator® System on Edentulous Patient (완전 무치악 환자에서 Locator® system을 이용한 임플란트 피개의치 수복 증례)

  • It is difficult to provide prosthodontic treatment to fully edentulous patients with severe alveolar bone resorption, because this makes patents hard to endure functional load, and to adapt to the dentures, which result in emotional stress to patients. Implant supported overdenture can be chosen to solve these problems. Among several types of them, the implant retained and tissue supported overdenture, is available to the patients of low masticatory force with the reduced cost. Attachments also can be used for increasing retention and esthetics in dentures. Especially, $Locator^{(R)}$ system needs a small vertical interarch space for restoration and is able to compensate the difference of angle between fixtures. In this study, we restored maxilla with conventional complete denture, and mandible with implant and tissue supported overdenture and $Locator^{(R)}$ system.

Investigation into the Development of Technology for Orthopeadic Surgery Utilizing Reverse Engineering and Rapid Prototyping Technology (역공학과 쾌속조형공정을 이용한 정형외과수술기법 개발에 관한 연구)

  • The objective of this paper is to propose a new technology of the orthopaedic surgery using the combination of reverse engineering (RE) based on CT data and rapid prototyping (RP). The proposed technology utilizes symmetrical characteristics of the human body and capability of the combination of RE and RP, which rapidly manufactures three-dimensional parts from CT data. The original .stl data of injured extents are generated from the mirror transformation of .stl file fur uninjured extents. The physical shape before injuring is manufactured from RP using the original .stl data. Subsequently, pre-operative planning, such as a selection of proper implants, preforming of the implant, a decision of fixation locations and an insert position for the implant, an estimation of the invasive size, and pre-education of operators are performed using the physical shape. In order to examine the applicability and the efficiency of the proposed surgical technology, various case studies, such as a distal tibia commented fracture, a proximal tibia plateau fracture and an iliac wing fracture of pelvis, are carried out. From the results of case studies, it has been shown that the proposed technology is an effective surgical tool of the orthopaedic surgery reducing the operational time, the operational cost, the radiation exposure of the patient and operators, and morbidity. In addition, the proposed technology could improve the accuracy of operation and the speed of rehabilitation.

  • Active and therapeutic sport recovery is becoming a popular and important component in approving performance for pro and recreational athletes alike. It is also helping in the realm of injury prevention. In the search of finding modalities that are widely effective, natural, and safe, acupuncture is a viable and cost-effective treatment for helping athletes achieve this goal. More direct related research is needed, but testimonials from pro athletes and the body of research that currently exists provides powerful evidence on acupunctures ability to help with enhancing recovery. Specializing in acupuncture and exercise science, Chris integrates acupuncture into musculoskeletal rehabilitation therapy or fitness training for pain modulation, speedy recovery, and enhanced performance. Clients can choose to focus on one-on-one corrective exercise therapy, manual and massage therapy, or acupuncture. However, for best results, Chris recommends all three. Other modalities that he uses in therapy are acu-taping, herbal therapy, nutrition supplementation, cupping, guasha, and stretching techniques. The corrective exercise component is one-on-one body balancing management, focusing on strength and conditioning, post physical rehab - exercise therapy, integrative sport specific exercise, weight loss, core strengthening, dynamic lumbar stabilization, active recovery techniques, and myo-fascial release techniques. The acupuncture component focuses on sport injuries, myofascial pain, peripheral neuropathy, arthritis, facial rejuvenation, stress, smoking cessation, addiction detoxification program, weight management, sport recovery and performance.

Case Analysis of Visiting Nursing Center for Improving Efficiencies: Based on Business Management Consulting (방문간호센터 경영효율성 개선 사례 분석: 경영 컨설팅 적용을 중심으로)

  • Purpose: This study aimed to present the management strategies necessary to improve the operational efficiency of visiting nurse centers and evaluate their effectiveness. Methods: The subjects of this study were visiting nurse centers registered as long-term care centers. Based on value chain analysis, cost information analysis, and data envelope analysis, the study was carried out according to the Magerison's management consulting procedure, for six months. This procedure comprised eight sub-steps of approach and application. Results: The following management strategies were agreed upon: establishment of a cooperative network with other visiting care centers, creation of high satisfaction of external customers by providing practical training to care workers, and making rehabilitation and exercise services as the core nursing activities to be focused on. Conclusion: The management consulting process and analysis method applied in this study can referred to as a useful methodological framework for revitalizing visiting nursing centers in the future.
