Factors Affecting Regular Medical Services Utilization of Chronic Disease Patients - Focusing on the Hypertension, Diabetes Mellitus, Hyperlipidemia - (만성질환자의 정기적 의료이용에 영향을 미치는 요인 - 고혈압, 당뇨병, 고지혈증을 중심으로 -)

  • Objectives: This study aims to identify the factors associated with regular medical services utilization of chronic disease patients. Methods: The research selected 4,489 adults aged over 30, diagnosed with hypertension, diabetes, hyperlipidemia, hypercholesterolemia, from the Korea health panel. We analyzed states of regular medical service utilization using descriptive statistics. Multiple regression analysis was used to examine the main factors associated with regular medical services utilization in chronic disease patients. Results: In terms of socio-demographic factors, gender, age, marital status, education level, employment, household income and disability were significantly different between hypertension, diabetes, hyperlipidemia and hypercholesterolemia. Among health status and behavioral factors, number of chronic diseases, subjective health status, smoking, high risk drinking, regular meals, physical activity, obesity were significantly different. From the multiple logistic regression analysis, age, number of chronic diseases, obesity, type of chronic diseases were associated with regular medical services utilization. Conclusions: It is necessary to develop effective health education programs and individualized approach to improve continuous management in chronic diseases patients.

  • This study aims to identify factors to affect regular utilization status of medical care in cardio-cerebrovascular patients. The research selected 770 cardio-cerebrovascular patients among surveyees from the Korea Health Panel 2010. We analyzed states of medical care utilization using descriptive statistics. Logistic regression analysis was used to examine the main factors associated with regular utilization status of medical care in cardio-cerebrovascular patients. In result, the significant factors associated with regular utilization status of medical care in cardio-cerebrovascular patients were age, education level, household income level. CCI, presence or absence of high risk drinking, and presence or absence of obesity. There's a high probability that patients aged between 60 and 69, equal to and higher than those of high school graduate in education level, upper middle class in household income, the higher CCI, absence of high risk drinking, presence of obesity utilize medical care services more regularly. Therefore, it is necessary to develop effective program and individualized approach for patients using lesser periodical medical care and patients with high risk drinking problem. In the future, these findings can be used an important data for healthcare policy and assessment.

Relationship between scaling, regular check-up and dental telephone services in dental clinic patients (치과 내원환자들의 스켈링과 정기검진 및 전화서비스와의 관련성)

  • Objectives : The purpose of the study is to investigate satisfaction, awareness and utilization of dental telephone services in dental clinic patients. Methods : A self-reported questionnaire was filled out by 220 dental clinic patients in G metropolitan city from March 11 to September 27, 2013. The data were analyzed by a descriptive analyses, ${\chi}^2$-test and relevant factors were determined using logistic regression analysis by SPSS 12.0. Results : The experience rate of scaling was 68.0% and the average number was $1.04{\pm}1.13$. The experience rate of regular check-up was 41.0% and the average number was $1.01{\pm}1.29$. Satisfaction and utilization rate of scaling and regular checkup was higher than that in those who did not receive the services. The major variables influencing the experience of scaling were woman, utilization of dental telephone service and regular check-up were awareness, utilization of dental telephone service and reason of utilization of dental telephone service(preventive). Conclusions : The dental telephone service in dental patients improved oral health. Accordingly, it is necessary to develop the professional dental hygiene program for oral health in dental patients.

Analytical Studies on Medical Utilization Behaviors in Rural Areas (농촌지역주민의 의료이용행위에 영향 주는 자극요인분석)

  • This study was conducted for the purpose of fin-ding out the variance explaining the medical facilities utilization behavior, which is defined adaptation behavior Process by focal, contextual, residual stimuli in Roy's Adaptation Model. What kinds of characteristics can explain adaptation behavior in Roy's Model? And which is the relative importance of input variables? For this analysis, stepwise multiple regression and path analysis was used. The data come from the 1981 Baseline Household Interview Survey in remote rural area. The findings of the analysis can be summarized as follows: First, Total variance of independant variables for adaptation behavior, that is medical facilities utilization including clinic, drug store, health center, herb medicine was shown 16.2 percent. The most important variable which explain the dependent variable was the occurance of illness with the Ra of value 0.112. The illness symptom, living level, regular care source was shown important variables with relatively high the R²value and significant beta coefficient. Second, in the path analysis of variables which is selected important variables, the occurance of illness was shown variable which has the highest direct effect which 0.297 path coefficient. Also the education level of household was shown variable which has the highest indirect effect through living level and the occurance of illness in causal model. Third, This analysis suggests that the occurance of illness belonging focal stimuli are more influenced than others. To sum up, It is seem to the occurance of illness, illness symptom belonging focal stimuli have high explanation ability through direct effect, education level of household among contextual stimuli have explanation ability through indirect effect.

Factors associated with dental service utilization among older adults (한국 노인의 치과의료이용 영향 요인)

  • Objectives: It is crucial to detect and prevent oral problems early. Older adults, in particular, should go for regular dental checkups to maintain a healthy oral environment. This study identified the factors affecting regular dental examinations and unmet dental needs in older adults. Methods: This study analyzed 3,362 older adults aged 65 or over using the data from the 7th Korean National Health and Nutrition Examination Survey (KNHANES). Frequency analysis was performed on general characteristics. In addition, chi-square tests were done to determine differences in regular dental checkups and unmet dental needs according to general characteristics. Logistic regression analysis was performed on factors influencing regular dental checkups and unmet dental needs. Results: Factors influencing regular dental checkups were statistically significant according to residential areas, level of education, household income, and levels of physical activity (p<0.05). Factors influencing unmet dental needs were statistically significant according to residential areas, level of education, private medical insurance, and levels of dental pain (p<0.05). Conclusions: National level policy strategies should be implemented to improve the oral health of older adults. These strategies should encourage regular dental checkups to effectively lower the rate of unmet dental needs.

Study of maintenance behavior based on the experience of the tooth brushing instruction for implant patient (임플란트 환자의 잇솔질 교육경험에 따른 유지관리행태 연구)

  • Objectives : This study aims to provide the basic data for the maintenance and management of healthy implants through finding out the educational experience of toothbrushing and the oral hygiene management for the implant patients and to determine the maintenance measure. Methods : From April 1 to october 1, 2012, 250 patients in the dental clinic in Daejeon completed the self-reported questionnaire. Results : As for the educational experience of toothbrushing, 66.4 percent received the training for toothbrushing and 33.6 percent had not received the training for toothbrushing. Those who brushed three times a day accounted for 74.1 percent. Those who received the training for toothbrushing comprised 56.0 percent and took regular check up of every 6 months. Approximately 50.0% received regular scaling service every 6 months. Periodical medical check up comprised 55.6% among those who received toothbrushing education. Approximately 63.9% thought that periodical medical check up is the most important to maintain oral health. Conclusions : There were important factors for oral health. They were tooth-brushing, the utilization of various oral hygienic products, and the awareness of importance of oral hygiene management. The customized education program for the general public should be implemented as regular as possible.

Establishment and Management of an Educational Outcome Cohort at the Keimyung University School of Medicine (계명대학교 의과대학 교육성과 코호트의 구축과 운영 사례 )

  • An educational outcome cohort has been established at Keimyung University School of Medicine to help make educational policy decisions and improve educational programs based on data. The purpose of the educational outcome cohort is to support educational policy decisions for achieving graduation outcomes smoothly and to accomplish the intended human resources development of the university through objective analyses and regular monitoring, providing continuous feedback. The data collected for the educational outcome cohort include the student identifications of freshmen, entrance exam scores, premedical and medical school grades, titles and forms of student academic research, the results of psychological testing, scholarship recipient lists, volunteer clubs, and so forth. The data are collected using an information utilization agreement approved by the Institutional Review Board, and the collected data are encrypted and stored on a dedicated computer for enhanced personal information security. Proposals to access and utilize the educational outcome cohort data must be discussed and approved by the Educational Outcome Cohort Committee, which decides on the scope and method of utilization. The collected and managed educational outcome cohort data have been used to develop comparative programs to improve students' competency and to support admission policy decisions through an analysis of the characteristics and performance of medical school students. The establishment and utilization of the educational outcome cohort will play an important role in determining the School of Medicine's educational policies and suggesting new directions for educational policies in the future.

The Utilization of Medical Care Benefits of School Personnel on Duty (교직원의 공무상 요양급여제도 이용 현황)

  • Purpose: To investigate the utilization of medical care benefits of school personnel on duty in Seoul. Method: The subjects were 551 workers under Seoul Metropolitan Office of Education(male 301, female 250) who got medical care benefits from March 2000 to February 2005. To analyze the data, frequency, chi-square test, and t-test by SAS package 9.12 were used. Result: The results show that elementary school personnel got the highest ranking, 314(57.0%). Among subjects, 57.2% personnel got accident when they were working on their own duty, 21% during school events and 13.5% were during commutes. They got medical care from orthopedics 75.9%, neurosurgery 7.6%, dental 4%, and 8 male and 2 female died during these days. The most frequent diseases of males and female were fracture and joints related disease. The reason of most health problem was due to injury(92.3%). Conclusion: The results of this study suggest that the continuous health care services like regular health check for school personnel, or social safety network to prevent injury are needed.

Influencing Factors on Influenza Vaccination among Korean 50~64 Age Adults Living Alone (1인가구 장년 성인(50~64세)의 인플루엔자 백신 접종률에 영향을 미치는 요인)

  • Purpose: Based on Anderson's model of health service utilization, we investigated the factors that affect the influenza vaccination status in Korean adults aged 50-64 years, who live alone. Methods: Data of 194 Korean adults aged 50-64 years, who live alone were obtained from the Korea National Health and Nutrition Examination Survey 7th Edition (2016-2018). Descriptive statistics and a multiple logistic regression model were used for statistical analysis. Results: The influenza vaccination rate in the study population was 39.3%. Educational status and health behaviors such as smoking habits and regular medical check-up were associated with the influenza vaccination rate in Korean adults aged 50-64 years, who live alone. Multiple logistic regression analysis showed that educational level lower than middle school graduation (odds ratio [OR] 2.02), non-smoking status (OR 1.98), and lack of regular medical check-up (OR 0.62) were significantly associated with the influenza vaccination rate in Korean adults aged 50-64 years, who live alone. Conclusion: The influenza vaccination rate in Korean adults aged 50-64 years, who live alone, is relatively low; therefore, policies should be implemented to improve the influenza vaccination rate in this population. Influenza vaccination in single households was affected by health promotion activities; therefore, interventions are warranted to encourage overall health promotion activities.

Correlations Among Body Weight, Life-Style and Health Status in Korean Adults (생활양식, 체중과 건강수준의 상관성분석)

  • Correlations among body weight and sociodemographic factors, including life - style were tested as social determinants of health in a sample of 5,201 adults in Korea. The aim of this study was to determine the extent to which sociodemographic variables and life-style associated health behaviors explain body weight distribution. A second aim was to explain the relation of body weight and health status to stress the importance of body weight as an early risk indicator of health status. The canonical correlation between the weight distribution(underweight and overweight) and the independent variables was 0.29, 17% of the total variance was explained. Perceived health level represented the highest contribution(canonical coefficient 0.82) to body weight. Sociodemographic factors such as sex, economic status, and life-style factors such as smoking, exercise, regular meais and sleep showed comparatively high contributions to body weight. The relevance of body weight for health status including the rate of chronic disease and the rate of medical utilization was significant. Especially, underweight was clarified as being mere important than overweight to morbidity level and medical utilization. These findings suggest that perceived body weight is an important indicator of health status and is thus a valuable variable to be considered for nursing intervention and health education related to the promotion of health.
