Initial and Recurrent Anterior Dislocation in Shoulder (견관절 초기 전방 탈구와 재발성 전방 탈구)

  • Purpose: To assess the usefulness of early stabilization for initial shoulder dislocation which is indicated in some patients by comparing the arthroscopic findings and the outcomes of Bankart repair in the initial and recurrent dislocation. Materials and Methods: The study was performed on 16 cases of initial dislocation and 44 cases of recurrent dislocation. The follow-up period was 25 months in the initial dislocation group and 28 months in the recurrent group. Bankart lesion and the adjacent tissues were compared, and the results were evaluated by the Rowe rating scale. Results: Detached labrum and capsular ligament of the initial dislocation group were elastic and unretracted. While in the recurrent dislocation those were inelastic, and displaced and adhesive in many cases. The outcomes were mean 95 points in the initial group and 91 points in the recurrent group. In the initial group, apprehension was detected in only 1 case(6%). In the recurrent group, instability was detected in 4 cases(9%). Conclusion: Our data suggest that Bankart lesion in the initial dislocation can be repaired readily by surgery, the recovery is efficient, and the recurrence would be reduced. Thus, in active young sports lovers as well as athletes, early stabilization surgery for initial shoulder dislocation may be more helpful.

Change-point Approach for Analyzing Failure Trend in Repairable Generating Systems (수리 가능 발전기 시스템의 고장추세 분석을 위한 변화점 접근방법)

  • A number of trend test methods, i.e., Military Handbook test and Laplace test etc., have been applied to investigate recurrent failures trend in repairable systems. Existing methods provide information about only existence of trend in the system. In this paper, we propose a new change-point test based on the Schwarz Information Criterion(SIC). The change-point approach is more informative than other trend test methods in that it provides the number of change-points and the location of change-points if it exists, as well as the existence of change-point for recurrent failures. The change-point test is applied to nine 300MW generating units operated in East China. We confirm that the change-point test has a potential for establishing optimal preventive maintenance policy by detecting change-point of failure rate.

Self-Recurrent Neural Network Based Sliding Mode Control of Biped Robot (이족 로봇을 위한 자기 회귀 신경 회로망 기반 슬라이딩 모드 제어)

  • In this paper, we design a robust controller of biped robot system with uncertainties, using recurrent neural network. In our proposed control system, we use the self-recurrent wavelet neural network (SRWNN). The SRWNN makes up for the weak points in wavelet neural network(WNN). While the WNN has fast convergence ability, it dose not have a memory. So the WNN cannot confront unexpected change of the system. However, the SRWNN, having advantage of WNN such as fast convergence, can easily encounter the unexpected change of the system. For stable walking control of biped robot, we use sliding mode control (SMC). Here, uncertainties are predicted by SRWNN. The weights of SRWNN are trained by adaptive laws based on Lyapunov stability theorem. Finally, we carry out computer simulations with a biped robot model to verify the effectiveness of the proposed control system,.

Treatment of Patella Recurrent Dislocation Through Percutaneous Lateral Release and Arthroscopic Medial Reefing (슬개골 재발성 탈구에 대한 외측부 유리술 및 내측 중첩술을 이용한 치료)

  • Purpose: To evaluate the results of percutaneous lateral release and arthroscopic medial plication for patients with recurrent patella dislocation. Materials and Methods: This study includes the thirty-one cases of patients who have had a surgical operation from March 2001 to March 2008. All the patients had recurrent patella dislocation after trauma history. The average age was 24.2 years old and the average follow up was 47.4 months (minimum 24 months). Results: The preoperative congruence angle, lateral patellofemoral angle, Lysholm score was $24.8^{\circ}$, $-6.2^{\circ}$, 75 points on average respectively. However, the postoperative congruence angle, lateral patellofemoral angle, Lysholm score was improved to normal range: $-2.8^{\circ}$, $10.2^{\circ}$, 95 points on average. Five cases showed the recurrent dislocation after operation. Among them, three cases showed femoral trochlear dysplasia. Conclusion: Percutaneous lateral release and arthroscopic medial plication showed satisfactory results for recurrent patella dislocation with low complication rate. However, in cases with femoral trochlear dysplasia, another surgical treatment may be considered.

Open Versus Arthroscopic Technique in the Traumatic Recurrent Anterior Dislocation of the Shoulder (견관절 전방 재발성 탈구에 대한 관혈적 술식과 관절경적 술식의 결과 비교)

  • Purpose: We compared the results of open and arthroscopic Bankart repair in traumatic recurrent anterior dislocation ,3f the shoulder. Materials and methods: We analysed 7 cases underwent open Bankart repair (group I) and 13 cases underwent arthroscopic Bankart repair (group Ⅱ). The average follow-up period was 68.1 months (51-113 months) in group I and 41.1 months (16~57 months) in group Ⅱ. All patients in group I and Ⅱ were non-athletes. We analyzed statistically objective evaluation, such as the stability of shoulder joint, the range of motion, pain, impaired throwing, Bankart rating system by Rowe and subjective evaluation, visual analog scale (VAS) between two groups. Results: In terms of dominant and non-dominant shoulders, the age at initial episode of dislocation, the elapsed time from injury to surgery, the number of preoperative dislocations associated with susceptibility to apprehension. respectively, there was no statistically significant differences between two groups. In group I the average Rowe's scortls was 84.3 and 3 cases (43%) had excellent results,4 cases (S7cfo), good ones. In group H the average Rowe's scores was 87.3 and 7 cases (54%) had excellent results,6 cases, good ones. There was tendency to show more excellent results in group ll, but there was no statistically significant differences. The average VAS were 90.3 points in group I and 88 points in group Ⅱ, which showed also no statistically significant differences. Conclusion: Open and arthroscopic Bankart repairs had no significant difference and showed also good results in travinatic recurrent anterior dislocation of shoulder.

Additional Thermal Shrinkage in Treatment of Recurrent Traumatic Anterior Shoulder Instability (만성 외상성 견관절 전방 불안정성의 치료에서 병행한 관절낭 열 수축술)

  • Purpose: In the traumatic anterior shoulder instability, the laxity of joint capsule and ligament is frequently demonstrated. Although a arthroscopic procedure to address anterior instability with joint capsular redundancy have generally provided good results, its recurrence rate is higher than open procedure. By reducing the capsular redundancy, thermal shrinkage is likely to improve the outcome of arthroscopic anterior stabilization. The objective of this study was to evaluate additional thermal capsular shrinkage as a treatment of joint capsular redundancy in anterior shoulder instability. Materials and Methods: From March 1999 to June 2000, 25 shoulders of 23 patients of recurrent anterior shoulder dislocation underwent arthroscopic Bankart repair with shrinkage procedure. The mean follow up was 29 months and average age at the time of operation was 26 years. Of these patients, 20 were male and 3 were female who had been experienced the average 8 times of dislocation before operation. Thermal shrinkage alone without Bankart repair was performed in two cases who did not have Bankart lesion. The clinical result was evaluated in according to Modified Rowe Score. Results: The Modified Rowe Score was improved from preoperative 35 points to postoperative 88 points. None of cases showed recurrence of dislocation. But, in two cases, temporary sensory hypesthesia of the axillary nerve was developed and in two cases of postoperative stiffness, arthroscopic capsular release and brisement were performed. Conclusion: Additional capsular shrinkage in arthroscopic technique to address recurrent anterior shoulder instability could treat effectively the capsular redundancy.

  • Prosthetic valves are being widely used in the treatment of heart valve disease. Prosthetic valve endocarditis (PVE) is one of the most catastrophic complications seen in these patients. In particular, prosthetic valve dehiscence can lead to acute decompensation, pulmonary edema, and cardiogenic shock. Here, we discuss the medical management of late PVE in a patient with a prior history of late and redo early PVE and recurrent dehiscence. According to the present case, we can summarize the learning points as follows. A prior history of infective endocarditis increases the risk of relapse or recurrence, and these patients should be evaluated very cautiously to prevent late complications. Adequate debridement of infected material is of paramount importance to prevent relapse. A history of dehiscence is associated with increased risk of relapse and recurrent dehiscence.

Anterior capsular reconstruction with acellular dermal allograft for subscapularis deficiency: a report of two cases

  • Anterior glenohumeral instability with an irreparable subscapularis tear is a challenging problem for the orthopedic shoulder surgeon. Current techniques, including tendon transfers, yield inconsistent results with high rates of recurrent instability. Acellular dermal allografting has been used in young patients with massive superior rotator cuff tears with early success, but acellular dermal allografting is comparatively unstudied in anterior deficiency. We present two cases of anterior capsular reconstruction with an acellular dermal allograft in patients ages 66 and 58 years with irreparable subscapularis tendon tears. Follow-up for both patients exceeded 4 years, with forward flexion >140°, external rotation exceeding 60°, a Single Assessment Numeric Evaluation score >90 points, a visual analog scale score of 0 points, and an American Shoulder and Elbow Score of 98 points. In conclusion, acellular dermal allografting can be used to reconstruct the anterior capsule in patients with massive irreparable subscapularis tears, similar to its use in superior capsular reconstruction in patients with massive posterosuperior rotator cuff tears.

  • In this paper, we discuss a continuous self-map of an interval and the existence of an uncountable strongly chaotic set. It is proved that if a continuous self-map of an interval has positive topological entropy, then it has an uncountable strongly chaotic set in which each point is recurrent, but is not almost periodic.

Clinical Evaluation of Recurrent Intussusception (재발성 장중첩증에 대한 임상적 분석)

  • To evaluate the clinical findings of the recurrent intussusception, 351 patients with 445 intussusceptions were reviewed. Recurrence rate, pattern of recurrence, reducibility, pathologic lead points (PLP), and operative findings and long term follow up of the multiple recurrences were analyzed. Of 351 patients, 303 had no recurrence, 26 had one recurrence, and 22 had multiple recurrences. Over all recurrence rate was 16.4% ; 18.5% were managed by air reduction, 16.2% by barium reduction and 5.9% by operation. Eleven PLPs were proved operatively and an additional 6 suspected PLPs were depicted radiologically. The most frequent PLP was ileal lymphoid hyperplasia. Intervals between reduction and recurrence were less than 2 weeks in 31 cases, between 2 weeks and 1 year in 55, and more than 1 year in 8. The longest interval was 2 years and 4 months.
