Recurrent T-cell lymphoma occurred in the vocal fold: a case report (성대에서 발견된 재발성 T세포 림프종에 대한 증례 보고)

  • Background. Lymphoma of the vocal fold is extremely rare due to low lymphoid content in the larynx. To date, fewer than 100 cases of laryngeal lymphoma have been reported; however, none of these literatures are concerned about exclusive laryngeal involvement of recurred lymphoma which initially appeared in other body sites. Specific consensus about management for these patients yet to exist, due to its rare occurrence; however, the main modality of treatment is chemotherapy alone or in combination with radiation therapy. Case. Herein, we report a case of a 51-year-old female patient who had recurrent T-cell lymphoma developing in bilateral vocal folds. The patient was originally diagnosed of T-cell lymphoma in right colic flexure 10 years ago, and was cured by chemotherapy. Immunohistochemical stain revealed the histologic type of recurred tumor in the vocal folds that are identical to the previously cured lymphoma. Conclusion. To the best of our knowledge, this was the first case that recurrent lymphoma occurred solely in the vocal folds. Despite its rarity, lymphoma should be put in the index of suspicion among those patients with decreased vocal fold mucosal wave without definite vocal fold mass who had a history of cured lymphoma.

A Case of Uterine Adenocarcinoma in A Spayed Female Dog (중성화된 암캐에서 발생한 자궁 선암종 증례)

  • A 10-year-old spayed female Pekinese dog was referred to veterinary medical teaching hospital of Chonbuk National University with signs of dysuria, abdominal distension, and hematuria. Abdominal radiography revealed a large oval soft tissue mass in cranial abdomen, which was a fluid filled cyst containing a round mass by ultrasonography. Exploratory laparatomy found a 4 cm cyst containing reddish brown fluid and a 1.5 cm small round mass attached to the wall. Cytological impression of the imprint smears of the resected mass were highly malignant adenocarcinoma, which was confirmed by histopathology of the mass and the cyst. After the surgery the patient recovered uneventfully and discharged. The tumor recurred about one year after surgery, and second surgery performed at the owner's request. However the tumor recurred in two months with metastatic disease in the lung and the dog survived three more months after surgery.

  • Atrial fibrillation is characterized by beat to beat irregularity of shape, size, conduction time and polarity. The mechanism of atrial fibrillation can be explained by so called "Multiple wavelet theory". The adverse effect of atrial fibrillation is the decrease of cardiac output by absence of atrial kick[10 ~ 15%] and the possibility of thrombosis in the left atrium which is dangerous to develop the thromboembolism is increased. The present study was designed to assess the effect of overdrive pacing on the suppression of recurring of atrial fibrillation after open heart surgery and the results were summarized as follows: 1. There were no significant differences of factors between converting patients and non-converting patients to normal sinus rhythm by electric cardioversion after open heart surgery. 2. Among converting patients to normal sinus rhythm, there were no significant differences of factors between study group and control group. 3. Cardiothoracic Ratio on the preoperative chest film was significantly larger in the patients of recurring atrial fibrillation within 72hrs than in the patients of maintaining normal sinus rhythm. [61.7$\pm$ 1.4% vs 67.7$\pm$2.4%, p=0.03] 4. There was a significant difference of suppressive effects between overdrive pacing group and control group among recurred cases until 24, 48, and 72hours [Fisher`s exact test ; p=0.037, p=0.076, p=0.53, respectively] 5. There was a difference of the delay of recurring of atrial fibrillation between study group and control group among recurred cases within 72 hours.[53.4$\pm$6.9hr vs. 19.3$\pm$3.8 hr, p<0.01] We think that the overdrive pacing may suppress the natural pacemaker and the converted normal sinus rhythm is maintained longer than control group during critical immediate postoperative period.ve period.

The Effect of Fibrin Glue as a Prevention Against Spontaneous Pneumothorax (Fibrin Glue가 자연기흉의 재발에 미치는 영향)

  • The spontaneous pneumothorax is the sudden collapse of the lung usually by air leakage from the ruptured sub-pleural bleb and has high recurrence rate. For prevention against recurrence, many drugs such as tetracycline, talcum powder, quinacrine, etc. have been used but the effects are not satisfactory. We reduced the recurrence rate successfully by the fibrin glue instillation through the chest tube. From the January 1989 to September 1990, we have managed 65 patients of spontaneous pneumothorax with closed thoracostomy and fibrin glue[fibrinogen 1gm/50ml with approtinin 3, 000kIU /ml, thrombin 5, 000IU /ml in 3% each 10ml] instillation through the chest tube. And we compared the results with those of 106 patients of spontaneous pneumothorax who were managed only by the closed thoracostomy from January 1985 to December 1988. Only the patients who visited our hospital with recurrence were considered as the recurred cases but the others were considered as not recurred. And the removal of chest tubes usually done 3 days after cessation of air leakage or 2 days after fibrin glue instillation Statistical analysis was done by X2-test. The results were as followings: 1. The recurrence rate of fibrin glue instillation group was lower than that of non-instillation group[1st attack: 15.1% versus 27.6% p<0, 05, the 2nd attack: 33.3% versus 73.7% p<0.01, the total 18.5% versus 35.8% p<0.01]. 2. The mean duration of chest tube drainage in the fibrin glue instillation group was shorter than non-instillation group[4.24$\pm$1.36 days versus 4.48$\pm$1.73 days p<0.05]. 3. The mean duration of hospitalization was shorter in the instillation group [8.12$\pm$3.5 days versus 10.8$\pm$3.8 days p<0.05] The complications were transient mild fever, chest pain, pleural effusion in 46 cases of 65 patients, but those didn`t make any problem. We concluded that the fibrin glue is effective in the reduction of recurrence rate, obliteration of air leakage and duration of hospitalization.

  • In the benign bone tumor such a fibrous dysplasia, destructive lesion is generally treated by curettage and simple bone graft. Such lesions are unlikely to recur if treated local curettage with bone graft or simple excision of the lesion. When it is impossible to cure only with simple bone graft due to wide extent of tumor, vascularized fibular graft have been introduced for functional loss and appearance. The recurrence of the primary tumor in the grafted fibula is rare in benign bone lesion. We experienced a case of fibrous dysplasia which was recurred in the grafted fibula following the initial treatment with vascularized fibular graft. So we report a case of our experience.

Gamma Knife Radiosurgery for Cancer Metastasized to the Ocular Choroid

  • Objective : Choroidal metastases (CMs) are the most common intraocular tumor. Management is mainly radiation therapy with goals of pain control and visual improvement. However, many radiation-related complications are reported. Since gamma knife radiosurgery (GKS) for CM was first reported in 1995, few cases have been reported. We report 7 cases of CMs treated with GKS. Methods : From April 2011 to November 2014, 7 patients with CM underwent GKS. Their median age at treatment was 64 years (range, 51-71 years). Four males and three females were treated. Lung cancer was the most common primary pathology, followed by renal cell carcinoma and stomach cancer. Four patients had multiple cerebral lesions and were treated simultaneously for choroidal lesions. The median marginal dose of 20 Gy (range, 15-25 Gy) was administered at the 50% isodose line. Results : Median follow-up period after GKS was 8 months (range, 2-38.3 months). Four patients expired due to underlying malignancy progression. Except for two patients who were not followed with magnetic resonance image after GKS, all patients showed size reduction in the treated lesions, but a new choroidal lesion appeared in one patient and one recurred. Six of seven patients reported subjectively improved visual symptoms. Visual acuity improved in 2 patients, and 2 were stable upon objective examination. One patient showed no improvement in visual acuity, but ocular pain was relieved; another patient showed improved vision and tumor remission, but visual deterioration recurred. Conclusion : GKS was shown to be safe and effective and should be considered for CM treatment.

  • Primary leiomyosarcoma of the mediastinum is an extremely rare tumor that develops either in the mesenchymal cells of the visceral mediastinum or in the smooth muscle of the great vessels. In the literature, the most significant factor for determining survival is the ability to completely resect the tumor. But there is 30~64% recurrence rate and there is also a lack of data for the role of adjuvant therapy after complete resection. We report here on a case of recurred leiomyosarcoma of the mediastinum that was surgically removed via left thoracotomy 2 years previously and this was misdiagmosed as benign disease.

Clinical Comparison of Posterolateral Fusion with Posterior Lumbar Interbody Fusion

  • Objective : The purpose of this study is to compare the outcomes of two methods for stabilization and fusion : Postero-Lateral Fusion [PLF, pedicle screw fixation with bone graft] and Posterior Lumbar Interbody Fusion [PLIF, cage insertion] for spinal stenosis and recurred disc herniation except degenerative spondylolisthesis. Methods : Seventy one patients who underwent PLF [n=36] or PLIF [n=35] between 1997 and 2001 were evaluated prospectively. These two groups were compared for the change of interbody space, the range of segmental angle, the angle of lumbar motion, and clinical outcomes by Prolo scale. Results : The mean follow-up period was 32.6 months. The PLIF group showed statistically significant increase of the interbody space after surgery. However, the difference in the change of interbody space between two groups was insignificant [P value=0.05]. The range of segmental angle was better in the PLIF group, but the difference in the change of segmental angle was not statistically significant [P value=0.0l7]. Angle of lumbar motion was similar in the two groups. Changes of Prolo economic scale were not statistically significant [P value=0.193]. The PLIF group showed statistically significant improvement in Prolo functional scale [P value=0.003]. In Prolo economic and functional scale, there were statistically significant relationships between follow-up duration [P value<0.001]. change of interbody space [P value<0.001], and range of segmental angle [P value<0.001]. Conclusion : Results of this study indicate that PLIF is superior to PLF in interbody space augmentation and clinical outcomes by Prolo functional scale. Analysis of clinical outcomes showed significant relationships among various factors [fusion type, follow-up duration, change of interbody space, and range of segmental angle]. Therefore, the authors recommend instrumented PLIF to offer better clinical outcomes in patients who needed instrumented lumbar fusion for spinal stenosis and recurred disc herniation.

Trichilemmal Carcinoma from Proliferating Trichilemmal Cyst on the Posterior Neck

  • Trichilemmal cysts are common fluid-filled growths that arise from the isthmus of the hair follicle. They can form rapidly multiplying trichilemmal tumors-, also called proliferating trichilemmal cysts, which are typically benign. Rarely, proliferating trichilemmal cysts can become cancerous. Here we report the case of a patient who experienced this series of changes. The 27-year-old male patient had been observed to have a $1{\times}1cm$ cyst 7 years ago. Eight months prior to presentation at our institution, incision and drainage was performed at his local clinic. However, the size of the mass had gradually increased. At our clinic, he presented with a $5{\times}4cm$ hard mass that had recurred on the posterior side of his neck. The tumor was removed without safety margin, and the skin defect was covered with a split-thickness skin graft. The pathologic diagnosis was a benign proliferating trichilemmal cyst. The mass recurred after 4months, at which point, a wide excision (1.3-cm safety margin) and split-thickness skin graft were performed. The biopsy revealed a trichilemmal carcinoma arising from a proliferating trichilemmal cyst. This clinical experience suggests that clinicians should consider the possibility of malignant changes when diagnosing and treating trichilemmal cysts.

Juxtacortical Osteogenic Sarcoma of Chondroblastic Type on the Maxilla (상악골에 발생된 연골형성현 측피질골육종)

  • 39-year-old female had been treated for the exophytic mass on buccal aspect of the left, maxillary posterior area 2 years and 8 months ago. Tentative diagnosis was obtained as fibrous dysplasia on clinical and radiographic examinations and histopathologic findings revealed as osteochondroma after bone trimming at that time. She revisited for the treatment of recurred lesions. We reviewed this case with clinical. radiologic and histopathologic standpoints retrospectively, and came to a conclusion that the tumor primarily occurred was juxtacortical osteogenic sarcoma and recurred due to inadequate treatment and then expanded over intramedullary. This case shows that the diagnosis of osteosarcoma should take account of the patient history, clinical. radiographic and histopathologic findings and it requires attentive follow up check. Retrospectively reviewed results were as follows ; At first visit, oral examination revealed a bony hard swelling on the buccal aspect of the left maxillary posterior area. Radiographically, a dense radiopaque mass was noted on the site. The lesion showed hot uptake of /sup 99m/Tc-MDP. Histopathologic diagnosis was done as osteochondroma, but it was considered as osteogenic sarcoma when compared with the recurrent lesion. When she revisited for the treatment of multiple bony swelling on the left maxilla, radiograms showed typical features of malignancy such as widening of periodontal ligament space and sunray appearace, and coincided with benign characters as follows; relatively well circumscribed lesion and expansion and displacement of the adjacent structures. Finally, histopathologic findings of the lesion was well differentiated chondroblastic osteogenic sarcoma.
