Longitudinal management of recurrent temporomandibular joint ankylosis from infancy to adulthood in perspective of surgical and orthodontic treatment

  • This study was performed to describe the longitudinal management of recurrent temporomandibular joint (TMJ) ankylosis from infancy to adulthood in perspective of surgical and orthodontic treatment. A 2-year-old girl was referred with chief complaints of restricted mouth opening and micrognathia due to bilateral TMJ ankylosis. For stage I treatment during early childhood (6 years old), high condylectomy and interpositional arthroplasty were performed. However, TMJ ankylosis recurred and symptoms of obstructive sleep apnea (OSA) developed. For stage II treatment during early adolescence (12 years old), gap arthroplasty, coronoidectomy, bilateral mandibular distraction osteogenesis, and orthodontic treatment with extraction of the four first premolars were performed. However, TMJ ankylosis recurred. Because the OSA symptoms reappeared, she began to use a continuous positive airway pressure device. For stage III treatment after completion of growth (20 years old), low condylectomy, coronoidectomy, reconstruction of the bilateral TMJs with artificial prostheses along with counterclockwise rotational advancement of the mandible, genioglossus advancement, and orthodontic treatment were performed. After stage III treatment, the amount of mouth opening exhibited a significant increase. Mandibular advancement and ramus lengthening resulted in significant improvement in the facial profile, Class I relationships, and normal overbite/overjet. The OSA symptoms were also relieved. These outcomes were stable at the one-year follow-up visit. Since the treatment modalities for TMJ ankylosis differ according to the duration of ankylosis, patient age, and degree of deformity, the treatment flowchart suggested in this report could be used as an effective guideline for determining the appropriate timing and methods for the treatment of TMJ ankylosis.

대량 객혈 환자에서 동맥 색전술의 치료 효과 (Transcatheter Arterial Embolization in the Treatment of Massive Hemoptysis)

  • 연구배경 : 대량 객혈은 내과적 혹은 외과적 치료로도 사망율이 매우 높은 임상증상이다. 대량 객혈은 높은 기관지 동맥 혹은 비기관지성 전신동맥에서 출혈된다고 알려져 있으므로, 최근 상기 혈관들을 색전시키는 것이 치료방법으로 되고 있으나 초기 색전후 일부에서 객혈이 재발되는 것이 문제로 되어 있다. 이에 저자들은 대량출혈환자에서 동맥색전을 하여 즉각적인 지혈정도를 알아보고, 즉시 지혈된 후 객혈이 재발되는 환자들의 임상소견 및 초기 동맥촬영소견을 조사하여 보았다. 방법 : 1988년 1월부터 1991년 7월까지 전부 21명의 환자에서 대량 객혈로 동맥색전술을 시행하였다. 환자 모두에서 대퇴통맥으로 경피하 삽관술을 하여 기관지동맥외에도 비기관지 전신동맥에 혈관촬영을 시행한 후, 병변부위의 혈관에 gelfoam pieces로 색전을 실시하였다. 이후 환자들을 추적관찰하면서 동맥색전술의 효과를 평가하였다. 결과 : 처음 동맥색전후 21명의 전환자에서 즉시 지혈은 성공하였다. 9명의 환자에서 출혈이 재발되었으며, 12예의 환자들에서는 객혈의 재발은 없었다. 합병증으로는 발열, 흉통, 기침, 배뇨 장애, 장마비, 하지마비 및 비장경색증등이 있었으며, 9명의 환자에서는 별 증상을 호소하지 않았다. 출혈이 재발된 환자중, 2명은 대량객혈로 사망하였고, 2명은 폐절제술을 시행받았으며, 4명은 매번 객혈이 재발시마다 반복적인 동맥색전술 혹은 내과적 보존치료를 받고 있다. 결론 : 재출혈의 가능성이 있기는 하지만, 동맥색전술은 대량 객혈환자에서 출혈을 비교적 빠르고 안전하게 멈출 수 있는 치료법이다. 또 수술적 치료가 불가능한 폐질환 환자에서 객혈이 재발시마다, 반복 시행하는 동맥색전술은 일단 급한 출혈을 막고 생명을 연장시키는 데에 도움을 주리라 사료된다.

측두하악장애 재발환자의 임상양태에 관한 연구 (Clinical Features of the Recurred Patients with Temporomandibular Disorders)

  • A patient with TMJ osteoarthritis and anterior open bite was treated with an intermaxillary traction device. Pretreatment examination revelaed a pain in both TMJ during mouth opening, moderate tendernesso f left sternocleidomastoid and right trapezius muscles. Anterior open Bite was aobserved with interincisal distance of 2mm. Tomograms and MRI showed anterior disc displacement withouit reductoin of both temporomandibular joints, and the condyles were flattened and slightly eroded. A pair of full-coverage occlusal appliances was made on both maxillary and mandibular dentition, with pivoting fulcrum on the site of the second moalr. Traction force was gained by the intermaxillary orthodontic elastics which were hooked by orthodontic brackets on the labial surfaces of the upper and lower anterior and premolar teeth. After 8 weeks of traction treatment, the joint pain was subsided completely and the anterior open bite was closed to get an edge to edge relationship of anterior teeth.

간이정신진단검사를 이용한 구취환자들의 인성평가에 대한 연구 (A Study of Personality Profile of Halitosis Patients by Symptom Checklist-90-Revision(SCL-90-R))

  • In order to evaluate the clinical features of the rcurred patients with temporomandibular disorders, 52 patients were subjected at the Dept. of Oral Medicine, PNUH from 1985 through 1996. As related to gender, duraton and diagnosis, Ll the relate factors-multiple diagnosis, emotinal problem, history of treatment, parafunctional habit, life distrubance, and trauma were analyzed. The obtained results were as follows : 1. The patients with multiple diagnosis were 83 percent and more often seen in women, chronic group and muscle-joint disorder group. 2. Duration of retreatment in chronic group and muscle-joint disorder was longer. 3. The patients with oral parafunctional habits were 89 percent and more often seen in women, chronic gorup, muscle-joint disorder group. 4. the Recurred patients showed 46 percent in history treatment, 42 percent in life distrubances, 39 percent in macrotrauma and 12 percent in emotional problem.

Extensive and aggressive growth of adenoid cystic carcinoma in the lacrimal gland

  • Adenoid cystic carcinoma (ACC) in the lacrimal gland is a very rare disease with poor overall prognosis. Its primary treatment is surgical excision, including orbital exenteration and radical orbitectomy, which is combined with radiotherapy and chemotherapy. Age, histopathologic type, bone invasion, and tumor extent are known factors that affect the prognosis of ACC. Furthermore, perineural invasion is highly associated with local tumor recurrence and tumor base invasion. Here, we report a rare case of ACC in the lacrimal gland with superior sagittal sinus invasion that repeatedly recurred after the surgical excision.

  • The neuroma is a tumor of nerve tissue that partially or completely severed through incomplete regeneration process. Neuromas that formed in the stump of a limb following amputation is a cause of the stump pain and can make intractable pain. The authors report a rare case of 36-year-old man with neuroma at stump, which has been recurred three times. This patient was treated with end-to-end neurorrhaphy after resecting neuroma. Follow-up at out-patient clinic showed satisfied result.

  • A 25-year-old man presented with a history of pain and crepitus in the right shoulder; he had been previously treated with arthroscopic anterior stabilization using four metallic suture anchors for recurrent traumatic anterior instability 1 year earlier. In this report, we present a patient with recurrent glenohumeral instability combined with anchor-induced arthropathy who was managed with modified arthroscopic transglenoid reconstruction following arthroscopic suture anchor retrieval.

소의 제4위전위증 교정을 위한 봉봉합법 (Bar Suture for the Treatment of Bovine Abomasal Displacement)

  • Surgical treatment was performed in 507 heads out of 1078 dairly cows which were diagnosed as abomasal displacement. Out of 507 heads. bar suture was performed in 280 heads. Twelve heads out of 280 heads(4.9%) recurred. Incorrective suture of the reticulum or pylorus instead of abomasum occurred in 3 cows. The cows in which abdominal wall was sutured but the suture was not inserted into the abomasum were 4 heads. The cows in which bar was broken are 4 heads. A cow suffered peritonitis after bar suture. Bar suture technique is thought to be a good method as a first choice for the correction of abomasal displacement.

자연기흉의 임상적 고찰 -336례 보고- (A Study on the Results of Surgical Treatment of Spontaneous Pneumothorax - 336 Cases in 269 Patients -)

  • Spontaneous pneumothorax is a common surgical problem and occurs most frequently in male patients under 40 years of age. The records of 336 cases in 269 patients, diagnosed and treated as spontaneous pneumothorax during the period of 1981, Jan.- 1987, Dec., at the department of thoracic and cardiovascular surgery, college of medicine, Hallym university, were reviewed retrospectively. The results were as follows; 1. Spontaneous pneumothorax was commonly found between the age of 20-40[55%]. 2. Sexual ratio of male to female was 4.4:1. 3. The most common symptoms were dyspnea and chest discomfort or chest pain. 4. Pulmonary tuberculosis was the most common cause of the disease[30.8%]. 5. Recurrent rate after closed thoracotomy drainage was 28.2%, and not recurred after operation. 6. Bullae or blebs were found most commonly at the apical segment of both upper lobes.

  • We report a case of recurred Takayasu,s arteritis.The patient was 28-year-old female underwent aorto-left common carotid and aorto-left subclavian bypass graft replacement 1 year ago.Unfortunately, she was readmitted because of newly developing angina and both eye claudication severe headache. Aorto-coronary angiogram showed complete obstruction of left common carotid artery ,stenosis of right carotid artery bifurcation and ostial stenosis of right coronary artery.Bilateral carotid arteries bypass graft with great saphenous vein and right coronary artery bypass graft with right internal mammary artery were done at same the time and she discharged after 21 days without any problem.
