Intracranial Atherosclerotic Disease; Current Options for Surgical or Medical Treatment

  • Recently, intracranial atherosclerosis has become a major cause of ischemic stroke, appearing more frequently in Koreans than Caucasians. Symptomatic or asymptomatic intracranial atherosclerosis is a disease that could recur readily even during the treatment with anti-platelet agents. When the symptoms develop, ischemic stroke can not be recovered readily. Therefore, aggressive treatments such as endovascular therapy and bypass surgery are required in addition to medical treatment for the intracranial artery stenosis. Recent intracranial stenting and drug eluting stenting have shown as very advanced effective therapeutic modalities. Nevertheless, until now, a randomized controlled study has not been conducted. Regarding bypass surgery, since the failed EC-IC bypass surgery study performed 20 years ago, extensive studies on its efficacy has not been conducted yet, and thus it has to be performed strictly only in hemodynamically compromised patients. Unless breakthrough drugs that suppress the progression of intracranial atherosclerosis and the formation of thrombi, and facilitate the regression of the arterial stenosis, the treatment concept of the recovery of the blood flow of stenotic arterial territory by mechanical recanalization or bypass surgery would be remained for the prevention as well as treatment of ischemic stroke caused by intracranial atherosclerosis.

Chondrosarcoma of the Calcaneus in 13 year old Aged Patient - A Case Report - (13세 환자의 종골에 발생한 연골육종 - 증례 보고 -)

  • Chondosarcoma of the calcaneus in young child is extremely rare. A thirteen-year old male patient has suffered from right heel pain for several months before. Dominant gross feature was hard mass and swelling on one entire right heel, and increased density and calcification on entire calcaneus with cortical destruction, and calcific densities on soft tissues around calcaneus were seen on roentgenogram of right ankle. It was diagnosed to be chondrosarcoma by bone biopsy. It was treated by below knee amputation and chemotherapy, and no recur and distant metastsis during 12 months after operation.

  • A naval shipboard inevitably movies in a pitch and roll direction under the influence of wave and wind in the sea. As a result, the shipboard gets in a continuous turning motion back/front and right/left. And the shipboard is also constantly exposed to many different kinds of disturbance signals including the vibrations of various frequencies from the internal equipments and their vibrations, strong waves, and impact from explosion. This paper formulates multi-body dynamic models similar to an actual system and simulates the pitch/roll positions of a 2-axes gimbals with PI controller for consecutive behavior of a naval shipboard including disturbance.

  • Temporomandibular joint (TMJ) dislocation is defined that the disc-condyle complex is positioned anterior to the articular eminence in the open mouth position, and is unable to return to a normal closed mouth position without a manipulative maneuver. TMJ dislocation can recur habitually and result several problems to patients such as discomfort, pain, fear, and anxiety. The only definitive treatment for TMJ dislocation is surgical alteration of the joint itself. In most cases, however, a surgical procedure is far too aggressive for the symptoms experienced by the patient. In addition, the effect of surgical treatment may be insufficient, and the recurrence have been reported. It is also possible to develop several complications after surgical treatment. Therefore much effort should be directed at supportive therapy in an attempt to eliminate the disorder or at least reduce the symptom to tolerable levels. Through this cases the authors present favorable treatment outcome using occlusal splint with the patient of TMJ dislocation. Occlusal splint therapy can be considered as easy, safe, and useful non-invasive modality to treatment of TMJ dislocation.

Reconsideration of urine culture for the diagnosis of acute pyelonephritis in children: a new challenging method for diagnosing acute pyelonephritis

  • Acute pyelonephritis (APN) should be detected and treated as soon as possible to reduce the risk of the development of acquired renal scarring. However, in the medical field, urine culture results are not available or considered when the prompt discrimination of APN is necessary and empirical treatment is started. Furthermore, urine culture cannot discriminate APN among children with febrile urinary tract infection (UTI) (pyelitis, lower UTI with other fever focus). Therefore, the usefulness of urine culture for diagnostic purposes is small and the sampling procedure is invasive. Congenital hypoplastic kidney is the most common cause of chronic kidney injury in children. Thus, it is desirable that a main target be detected as early as possible when imaging studies are performed in children with APN. However, if APN does not recur, no medical or surgical treatment or imaging studies would be needed because the acquired renal scar would not progress further. Therefore, the long-term prognosis of APN in young children, particularly infants, depends on the number of recurrent APN, not other febrile UTI. New methods that enable prompt, practical, and comfortable APN diagnosis in children are needed as alternatives to urinary catheterization for urine culture sampling.

  • Most of the works in Time Series Analysis are based on the Auto Regressive Integrated Moving Average (ARIMA) models presented by Box and Jeckins(1976). If the data exhibits no ap-parent deviation from stationarity and if it has rapidly decreasing autocorrelation function then a suitable ARIMA(p,q) model is fit to the given data. Selection of the orders of p and q is one of the crucial steps in Time Series Analysis. Most of the methods to determine p and q are based on the autocorrelation function and partial autocor-relation function as suggested by Box and Jenkins (1976). many new techniques have emerged in the literature and it is found that most of them are over very little use in determining the orders of p and q when both of them are non-zero. The Durbin-Levinson algorithm and Innovation algorithm (Brockwell and Davis 1987) are used as recur-sive methods for computing best linear predictors in an ARMA(p,q)model. These algorithms are modified to yield an effective method for ARMA model identification so that the values of order p and q can be determined from them. The new method is developed and its validity and usefulness is illustrated by many theoretical examples. This method can also be applied to an real world data.

A Newborn with Gastric Hemangioma Treated Using Propranolol

  • Gastric hemangiomas are rare benign vascular tumors that can cause severe gastrointestinal system bleeding. We presented the case of a neonate with fresh bleeding and melena from the orogastric tube and detected gastric hemangioma in esophagogastroduodenoscopic examination. Propranolol is widely used in treatment of cutaneous hemangiomas and non-gastric gastrointestinal system hemangiomas. However, the surgical approach is preferred for treating gastric hemangiomas, and there are few reports of gastric hemangiomas associated with non-surgical treatment. Gastric hemorrhage decreased with antacid and somatostatin treatment. Propranolol treatment was initiated before the surgery decision. After three weeks of treatment, we observed regression in the hemangioma with endoscopic evaluation. During the course of treatment, the patient's gastrointestinal system bleeding did not recur, and there were no side effects associated with propranolol.

  • Most of large scale solar panel handling robots adopt the timing-belt drive system for its driveline because of the simplicity and the easiness of implementation. The vibration caused by the flexure of the timing belt would increase as the size and the weight of the panel that the robot handles increase and the vibration would deteriorate the precision and/or productivity of the whole robot system. For the development of a proper control system and for the improvement of the design of the robot it is important to estimate the oscillatory response of the robot system including the flexible drive system properly. In this paper a flexible multi-body dynamics model of a large-scale solar-panel-handling robot with the flexible timing-belt drive system is developed using a generic multi-body dynamics analysis program, RecurDyn.

Dynamic Analysis of a Linear Feeder for Uniform Transformation of Grains (곡물의 균일한 이송을 위한 리니어 피더의 동특성 해석)

  • The purpose of this study is to improve the performance of a linear feeder that can transport grains uniformly. In order to analyze the dynamic behaviors of a linear feeder, the displacements of the feeder are measured by several accelerometers when it is in an operating condition. After the signal data from the accelerometers are captured in the time domain, the feeder motion in the space is visualized by using graphic computer software. In addition, a dynamic model of the feeder is established for a multi-body dynamics simulation. For the dynamic simulation, RecurDyn, which is a commercial multi-body dynamic package, is used. From the experimental and the computational approaches, an optimal dynamic motion is obtained for uniform transportation of grains. Furthermore, we also consider the determination of design parameters for optimal dynamic motion such as centroid, stiffness, and damping coefficient of the feeder system.

  • The malignant small round cell tumor of the thoracopulmonary region was described by Askin in 1979 and called as Askin Tumor. The Askin tumor is a rare, arises from the soft tissues of the chest wall or peripheral lung which is predominantly in younger generation. Clinical and pathologic appearance were very similar to Ewing's sarcoma and rhabdomyosarcoma but when examined it by electron microscopy, there was some different in morphology. The tumor tended to recur locally and did not seem to disseminated widely but the median survival was only 8 months.
