BIOADHESIVE GEL PREPARATIONS FOR RECTAL DRUG DELIVERY

  • Many attempts have been made to use hydrogel as del ivory systems for various drug and vioactive materials to prolong and control their pharmacological activities. Rectal administration of drugs by using hydrogel bases, such as poloxamer ABA block copolymer (Pluronic F-127) gels, polyacrylic acid (Carbomer 934, 940, or 941) aqueous gels, and polyvilyl alcohol gels, have been reported on the preparation and potential suppository use of new gels(Eudragit L, Eudragit S, and Eudispert) that are block copolymers of methacrylic acid and methyl methacrylate. If) These hydrogel and xerogel preparations, especially Eudispert hv gels, show excellent staying and bioadhesive effects in the lower part of the rectum in rats and rabbits compared with those of polyethylene glycol (PEG)2000 and Witepsol H-15(or S-55) suppositories. Visual and optical microscopic observation of rectal membrances indicated no irritation or abnormality after administration of Eudispert hv tydrogel and xerogel.

  • Gastrointestinal stromal tumors (GISTS) are rare, but potentially aggressive tumors. GISTS are generally found in the stomach or small intestine and less commonly in the colon, rectum, or an intra-abdominal sites but have rarely been documented in the esophagus. GISTS were definded as the most common mesenchymal tumors of the gastrointestinal tract for which there is incomplete understanding of their lineage, while their relationship with differenciated. We reported a very rare case of GISTS of lower esophagus in a 60-year-old woman with relevant literature review.

  • A 10-day old male calf exhibited multiple congenital anomalies of the urinary and gastrointestinal tracts, including renal fusion (horseshoe kidney), ureteral fusion, rectovesicular fistula, and atresia ani. The single kidney was fused at the caudal poles. The left kidney and cranial half of right kidney were shrunken, while the remaining lobules were hypertrophic. Ureters were fused cranially and bifurcated caudally. The terminal rectum was narrowed and connected with the bladder. The anus was imperforate. The cause of these anomalies could not be determined. This is the first report of this constellation of congenital anomalies in a calf.

Group B Streptococcal Disease in Korean Neonates (한국 신생아에서 B군 사슬알균 감염증)

  • Group B streptococcus (GBS) is the leading cause of neonatal sepsis and meningitis in developed countries. This article reviews the neonatal invasive GBS disease, maternal GBS colonization, and prevention strategies in the context of recent epidemiological changes in Korea. Although Korean neonates had been supposed to have low incidence of invasive GBS disease, GBS has been recently reported to be the most common cause of invasive neonatal infection after 1990s. Among Korean pregnant women, GBS carriage rate in the vagina and rectum has been reported to be much lower than that in Western countries. However, it has increased in recent studies. For decision making about preventive strategy for neonatal GBS disease in Korea, further studies are required in terms of the incidence of neonatal GBS infection and serotype distribution. In addition, studies about maternal carriage rate and serotype distribution have to be continued.

Inferior Mesenteric Plexus Block Performed in the Lateral Position (측와위에서 시행한 하 장간막 신경총차단)

  • Inferior mesenteric plexus block(IMPB) sa useful nerve block for the relief of intractable lower abdominal and pelvic pain caused by a lower abdominal visceral or a pelvic malignancy. IMPB has been performed in the prone position. But there are many patients who can't lie in the prone position, because ascites is frequently noticed in cancer patients and they also frequently received abdominal operations. We performed IMPB in the lateral position on two patients with lower abdominal pain, Case 1: A 77 year old female who had a right ovarian cancer with metastatic cancer of descending colon and rectum, experienced complete pain relief. Case 2: A 72 year old female who had a far advance pancreatic cancer with intestinal obstruction due to carcinomatosis received right and left celiac plexus block and right and left IMPB. The patient was satisfied with the result of these pain blocks. Conclusion; IMAPB performed in the lateral position on two patients with lower abdominal pain and their results were excellent for pain relief.

Origin and Meanings of Shape of Vital Gate(命門) in Yixuerunmen(醫學入門) (『의학입문(醫學入門)』 명문(命門) 형상(形狀)의 유래와 의의)

  • Objectives : To research origin of the shape of vital gate(命門) in Yixuerunmen makes us understand more exactly what the author, Li Chan(李梴) thought vital gate shaped. Methods : It's shape described in his book was compared with pictures portraying it in Hua Tuo Xuan Men Nei Zhao Tu(華陀玄門內照圖), one of references of his book. Results : He reasonably modified its passing track, while explaining it according to the paintings in Hua Tuo Xuan Men Nei Zhao Tu. Vital gate, as he thought, was not an real organ like the other five viscera, but a cord or a tube similar to blood vessels. He believed its cord had long connections from pericardium to terminal of urethra, which went through pericardium upward, right kidney downward, right around terminal rectum down-frontward, and urethra in parallel outward. Conclusions : He had consistent understandings for vital gate to penetrate several different viewpoints, as based on pictures in Hua Tuo Xuan Men Nei Zhao Tu.

  • The regional distribution and relative frequency of endocrine cells were studied immunohistochemically in the gastrointestinal tract (GIT) of the Manchurian chipmunk, Tamias sibiricus asiaticus. Six kinds of endocrine cells were identified in this study. 5-hydroxytryptamine (5-HT)-immunoreactive cells were detected throughout the GIT. These cells were observed in moderate numbers in the pylorus, duodenum, jejunum, ileum, fundus, colon, and rectum. Somatostatin- and bovine pancreatic polypeptide (BPP)-immunoreactive cells were also identified throughout the GIT. The former were abundant in the pylorus region while the latter were scattered In ileum and colon. Motilin-immunoreactive cells were rarely detected in the small intestine. A few neurotensin-immunoreactive cells were detected in jejunum, ileum and colon. Also, a few substance P-immunoreactive cells were observed to be restricted to duodenum and jejunum.

Management for locally advanced cervical cancer: new trends and controversial issues

  • This article reviewed new trends and controversial issues, including the intensification of chemotherapy and recent brachytherapy (BT) advances, and also reviewed recent consensuses from different societies on the management of locally advanced cervical cancer (LACC). Intensive chemotherapy during and after radiation therapy (RT) was not recommended as a standard treatment due to severe toxicities reported by several studies. The use of positron emission tomography-computed tomography (PET-CT) and magnetic resonance imaging (MRI) for pelvic RT planning has increased the clinical utilization of intensity-modulated radiation therapy (IMRT) for the evaluation of pelvic lymph node metastasis and pelvic bone marrow. Recent RT techniques for LACC patients mainly aim to minimize toxicities by sparing the normal bladder and rectum tissues and shortening the overall treatment time by administering a simultaneous integrated boost for metastatic pelvic lymph node in pelvic IMRT followed by MRI-based image guided adaptive BT.

  • At the Seoul first slaughter house, 2,964 heads of swine were examined for the infection and distribution of Cysticercus tenuicollis. The results were summarized as follows. 1. The Cysticercus tenuicollis were detected from 106 heads of swine and the infection rate were 3.58 percent. 2. Incidence at each province were 5.7% in Jeon-ra-puk Do, 4.4% in Chung-cheong-nam Do, 3.83% in Kyong-sang-nam Do, 3.57% in Jeon-ra-nam Do, 2.52% in Kang-won Do, 1.75% in Kyong-sang-puk Do, and Kyong-gi Do and Seoul City were not recognized. 3. The monthly variation of the infection of Cysticercus tenuicollis were not found nearly through a year. 4. The usual habitat of Cysticercus tenuicollis were recognized the omentum and liver, but unusually one case was found around the rectum.

Study on the Jangwi of Young Chu (령추(靈樞)${\cdot}$장위(腸胃)에 대한 연구)

  • Jangwi(腸胃) was recorded to Young Chu(靈樞)'s volume 33. Also, This Was recorded in Tae So(太素)‘s volume 10 Sindo Jangdo(身度 腸道), Gap Eul(甲乙)’s volume 2 Goldojangdojangwisosujechil(骨度腸道腸胃所受第七) and You Gyung(類經)‘s volume 4 Jangsnagyou jangwidaesojisu(臟象類 腸胃大小之數). Reason which call that it is Jangwi(腸胃) is that is describing length, width, diameter, weight, capacity, bends state of digestive organ from mouth to rectum. This chapter can not be understood in view of yin and yang five elements with Makedo(맥도), Goldo(骨度). This chapter can approach from specially viewpoint that is anatomy. Name of This chapter's terminology and position and types are very realistic. I feel pity that study does not exist about this. Therefore, I compared with comments of several doctors and modern anatomy. I believe that will be beaconed to understand This chapter.
