Tramadol as a Voltage-Gated Sodium Channel Blocker of Peripheral Sodium Channels Nav1.7 and Nav1.5

  • Tramadol is an opioid analog used to treat chronic and acute pain. Intradermal injections of tramadol at hundreds of millimoles have been shown to produce a local anesthetic effect. We used the whole-cell patch-clamp technique in this study to investigate whether tramadol blocks the sodium current in HEK293 cells, which stably express the pain threshold sodium channel Nav1.7 or the cardiac sodium channel Nav1.5. The half-maximal inhibitory concentration of tramadol was 0.73 mM for Nav1.7 and 0.43 mM for Nav1.5 at a holding potential of -100 mV. The blocking effects of tramadol were completely reversible. Tramadol shifted the steady-state inactivation curves of Nav1.7 and Nav1.5 toward hyperpolarization. Tramadol also slowed the recovery rate from the inactivation of Nav1.7 and Nav1.5 and induced stronger use-dependent inhibition. Because the mean plasma concentration of tramadol upon oral administration is lower than its mean blocking concentration of sodium channels in this study, it is unlikely that tramadol in plasma will have an analgesic effect by blocking Nav1.7 or show cardiotoxicity by blocking Nav1.5. However, tramadol could act as a local anesthetic when used at a concentration of several hundred millimoles by intradermal injection and as an antiarrhythmic when injected intravenously at a similar dose, as does lidocaine.

Residue Monitoring and Dietary Risk Evaluation of Fungicide Propiconazole in Leafy Vegetables under Greenhouse Conditions

  • Residue monitoring of propiconazole (PCZ) in cabbage, shallot, and spinach was conducted under multi-trial greenhouse conditions. This study aimed to understand the fate of the applied fungicide in these vegetables. Furthermore, the associated health risk of PCZ in leafy vegetables was assessed through dietary risk assessment. Commercially available PCZ (22% suspension concentrate) was administered thrice according to the OECD fungicide application interval guideline. The plant samples were extracted using a slightly modified QuEChERS technique and analyzed using gas chromatography-tandem mass spectrometry. The average PCZ recovery was between 84.5% and 117.6%, with a <5% coefficient of variance. The dissipation of PCZ residue in cabbage, shallot, and spinach after 14 days was 96%, 90%, and 99%, respectively, with half-lives of <5 days. Meanwhile, dietary risk assessments of PCZ residues in the studied vegetables using the risk quotient (RQ) were significant < 100 (RQ < 100). Thus, the population groups considered in this study were not at substantial risk from consuming leafy vegetables sprayed with PCZ following critical, good agricultural practices.

Implant placement with inferior alveolar nerve repositioning in the posterior mandible

  • This case report presents inferior alveolar nerve (IAN) repositioning as a viable approach for implant placement in the mandibular molar region, where challenges of severe alveolar bone width and height deficiencies can exist. Two patients requiring implant placement in the right mandibular molar region underwent nerve transposition and lateralization. In both cases, inadequate alveolar bone height above the IAN precluded the use of short implants. The first patient exhibited an overall low alveolar ridge from the anterior to posterior regions, with a complex relationship with adjacent implant bone level and the mental nerve, complicating vertical augmentation. In the second case, although vertical bone resorption was not severe, the high positioning of the IAN within the alveolar bone due to orthognathic surgery raised concerns regarding adequate height of the implant prosthesis. Therefore, instead of onlay bone grafting, nerve transposition and lateralization were employed for implant placement. In both cases, the follow-up results demonstrated successful osseointegration of all implants and complete recovery of postoperative numbness in the lower lip and mentum area. IAN repositioning is a valuable surgical technique that allows implant placement in severely compromised posterior mandibular regions, promoting patient comfort and successful implant placement without permanent IAN damage.

A rapid separation of Cs, Sr and Ba using gas pressurized extraction chromatography with inductively coupled plasma-mass spectrometry

  • We present a rapid method for the determination of Cs, Sr, and Ba, heat generators found in highly active liquid wastes, by gas-pressurized extraction chromatography (GPEC) using a column containing a cation-exchange resin. GPEC is a microscale column chromatographic technique that uses a constant flow rate of solvent (0.07 mL/min) with pressurized nitrogen gas supplied through a valve. In particular, because this method uses a small sample volume (a few hundred microliters), it produces less chemical waste and allows for faster separation compared to traditional column chromatography. In this study, we evaluated the separation of Cs, Sr, and Ba using GPEC. The eluate from the column (GPEC or conventional column chromatography) was quantitatively analyzed using inductively coupled plasma-mass spectrometry to measure the column recovery and precision. The column reproducibility of the proposed GPEC system (RSDs of recoveries) ranged from 2.7 to 4.1 %, and the column recoveries for the three elements ranged from 72 to 98% when aqueous HCl was used as the eluent. The GPEC results are slightly different in efficiency and separation resolution compared to those of conventional column chromatography because of the differences in the eluent flow rate as well as the internal diameter and length of the column. However, the two methods had similar recoveries for Cs and Sr, and the precision of GPEC was improved by two-fold. Remarkably, the solvent volume required for GPEC analysis was five times lower than that of the conventional method, and the total analysis time was 11 times shorter.

  • In this paper, we propose an effective speech reinforcement technique under the near-end and the far-end noise environments. In general, since the intelligibility of the far-end speech for the near-end listener is significantly reduced under near-end noise environments, we require a far-end speech reinforcement approach to avoid this phenomena. Specifically, based on the estimated background noise spectrum of the near-end, we reinforce the far-end speech spectrum by incorporating the more general cases under the near-end with background noise. Also, we propose the novel approach to reinforce the actual speech signal except for the noise signal in the far-end noisy speech signal. The performance of the proposed algorithm is evaluated by the CCR (Comparison Category Rating) test of the method for subjective determination of transmission quality in ITU-T P.800 under various noise environments and shows better performances compared with the conventional method.

Role of MR Neurography for Evaluation of the Lumbosacral Plexus: A Scoping Review (요천추 신경총에 대한 자기공명신경조영술의 역할: 주제 범위 문헌고찰)

  • Purpose MR neurography (MRN) is an imaging technique optimized to visualize the peripheral nerves. This review aimed to discover an optimized protocol for MRN of the lumbosacral plexus (LSP) and identify evidence for the clinical benefit of lumbosacral plexopathies. Materials and Methods We performed a systematic search of the two medical databases until September 2021. 'Magnetic resonance imaging', 'lumbosacral plexus', 'neurologic disease', or equivalent terms were used to search the literature. We extracted information on indications, MRN protocols for LSP, and clinical efficacy from 55 studies among those searched. Results MRN of the LSP is useful for displaying the distribution of peripheral nerve disease, guiding perineural injections, and assessing extraspinal causes of sciatica. Three-dimensional short-tau inversion recovery turbo spin-echo combined with vascular suppression is the mainstay of MRN. Conclusion Future work on the MRN of LSP should be directed to technical maturation and clinical validation of efficacy.

Enhanced extraction of copper and nickel based on the Egyptian Abu Swayeil copper ore

  • The continuous increasing of the global demand of copper and nickel metals raises the interest in developing alternative technologies to produce them from copper sulfide ore. Also, in line with Egypt's vision 2030 for achieving the sustainable socioeconomic development which aims at developing alternative and eco-friendly technologies for processing the Egyptian ores to produce these strategic products instead of its importing. These metals enhance the advanced electrical and electronic industries. The current work aims at investigating the recovery of copper and nickel from Abu Swayeil copper ore using pug leaching technique by sulfuric acid. The factors affecting the pug leaching process including the sulfuric acid concentration, leaching time and temperature have been investigated. The copper ore sample was characterized chemically using X-ray fluorescence (XRF) and scanning electron microscope (SEM-EDX). A response surface methodology develops a quadratic model that expects the nickel and copper leaching effectiveness as a function of three controlling factors involved in the procedure of leaching was also investigated. The obtained results showed that the maximum dissolution efficiency of Ni and Cu are 99.06 % and 95.30%, respectively which was obtained at the following conditions: 15 % H2SO4 acid concentration for 6 hr. at 250 ℃. The dissolution kinetics of nickel and copper that were examined according to heterogeneous model, indicated that the dissolution rates were controlled by surface chemical process during the pug leaching. The activation energy of copper and nickel dissolution were 26.79 kJ.mol-1 and 38.078 kJ.mol-1 respectively; and the surface chemical was proposed as the leaching rate-controlling step.

A CLINICAL STUDY ON SUPERIORLY BASED PLATYSMA MYOCUTANEOUS CERVICAL FLAP FOR RECONSTRUCTION FOLLOWING INTRAORAL SOFT TISSUE CANCER SURGERY (구강내 연조직 암 절제후 상부기조 광경근 근피부 경부 피판을 이용한 구강내 재건에 관한 임상적 연구)

  • The goal of reconstruction following ablative therapy for intraoral cancer is the restoration of form and function to permit a return to activities of daily life. Traditional reconstruction includes split thickness skin grafts, myocutaneous flaps and, more recently, various free flaps. Free flaps provide higher level of functional recovery relative to that seen with other techniques but require the complexity of the technique and microvascular anastomosis and thus, extended surgical time and occasionally a second team for harvesting. The platysma myocutaneous cervical flap is a possible alternative for intraoral reconstruction. It is thin and pliable like the tissue provided by the radial forearm free flap. It can be harvested with enough tissue to close most head and neck ablative defects. There is virtually no donor site morbidity involved. This study evaluated 7 patients affected by intraoral squamous cell carcinoma (SCC). All patients underwent the resection of intraoral SCC with neck dissection and subsequent intraoral reconstruction with the superiorly based platysma myocutaneous cervical flap. Flap-related complications occurred in 3 patients. Adjuvant radiation therapy was performed in 3 patients. Average follow-up was 24.1 months after surgery, with a range of 8 to 42 months. All patients presented self assessment of discomfort associated with intraoral recipient sites and cervical donor sites. However, the neck function measured by two-inclinometer technique was within the normal range during relatively long term follow-up period. Our study concluded that superiorly based platysma myocutaneous cervical flap is good alternative to free flaps, especially for relatively smaller defects and for the defects appropriate for the rotation arc of the flap.

Gas Hydrate Exploration by using PCS(Pressre Core Sampler): ODP Leg 204 (압력코어를 이용한 가스 하이드레이트 탐사: ODP Leg 204)

  • Natural gas in deep sediment may occur in three phases based on the physical and chemical conditions. If the concentration of gas in pore water is less than the solubility, gas is dissolved. If the concentration of gas is greater than its solubility (water is saturated or supersaturated with gas), gas occurs as a fee gas below the gas hydrate stability Lone (GHSZ) and is present as solid hydrate within the GHSZ. The knowledge of gas concentration in deep sediment appears critical to determine the phase of natural gases and to understand the formation and distribution of gas hydrate. However, reliable data on gas concentration are usually available only from the upper section of marine sediment by the headspace gas technique, which is widely used for sampling of gases from the sediments. The headspace gas technique represents only a fraction of gases present in situ because sediments release most of the gases during recovery and sampling. The PCS (Pressure Core Sampler) is a downhole tool developed to recover a nominal $1{\cal}m$ long, $4.32{\cal}cm$ diameter core containing $1,465cm^3$ of sediment, pore water and gas at in situ pressure up to 68.9 MPa. During Leg 204, the PCS was deployed at 6 Sites. In situ methane gas concentration and distribution of gas hydrate was measured by using PCS tool. Characteristics of methane concentration and distribution is different from site to site. Distribution of gas hydrate in the study area is closely related to characteristics of in situ gas concentration measured by PCS.

Pallidotomy Guided by MRI and Microrecording for Parkinson's Disease (파킨슨환자의 자기공명영상과 미세전극기록을 이용한 담창구 파괴술)

  • Objective : The exact position of the lesion during the pallidotomy is critical to obtain the clinical improvement of parkinson's disease without damage to surrounding structure. Ventriculogrphy, CT(computed tomograpy) or MRI(magnetic resonance imaging) have been used to determine the initial coordinates of stereotactic target for pallidotomy. The goal of this study was to determine whether microelectrode recording significantly improves the neurophysiologic localization of the target obtained from MRI. Methods : Twenty patients were studied. They underwent a unilateral pallidotomy. Leksell frame was applied and T1 axial images parallel to the AC-PC(anterior commissure-posterior commissure) plane using a 1.5 Tesla MRI with 3mm slice thickness were obtained. Anteroposterior coordinate of target was chosen at 2mm in front of the midcommissural point and lateral coordinate between 19 and 22mm from the midline. The vertical coordinate was calculated on coronal slice using a fast spin echo inversion recovery sequence(FSEIR) related to the position of the choroidal fissure and ranged over 4-5mm below the AC-PC plane. Confirmation of the anatomical target was done on axial slices using the same FSEIR sequence . Microrecording was done at the pallidum contralateral to the symptomatic side using an electrode with a tip diameter of $1{{\mu}m}$ diameter tip and 1.1-1.4 mOhm impedance at 1000Hz. Electrophysiologic localization of the target was also confirmed intraoperatively by macrostimulation. Results : Microrecording techniques were reliable to define the transition from the base of the pallidum which was characterized by the disappearance of spike activity and by the change of the audible background activity. Signals from high amplitude neurons firing at 200-400Hz were recorded in the pallidal base. X, Y and Z coordinates of target obtained from the MRI were within 1mm from the X, Y, Z coordinates obtained with microrecording in 16 patients (80%), 15 patients(75%), 10 patients(50%) respectively. The difference of Y coordinate between on MRI and on microrecording was 4mm in only one patient. Conclusion : The MRI was accurate to localize the target within 1mm of the error from microrecording target in 70% of the patients. 4mm discrepancy was observed only once. We conclude that MRI alone can be used to determine the target for pallidotomy in most patients. However, microrecording technique can still be extremely valuable in patents with aberrant anatomy or unusual MRI coordinates. We also consider physiologic confirmation of the target using macrostimulation to be mandatory in all cases.
