Comparison of the ability of Reciproc and Reciproc Blue instruments to reach the full working length with or without glide path preparation

  • Objectives: The purpose of the present study was to compare the mean preparation times and frequency with which Reciproc and Reciproc Blue instruments reached the full working length in mandibular molars, with or without glide path preparation. Materials and Methods: Previously untreated mesiobuccal and mesiolingual canals with completely formed apices were randomly divided into 6 groups (n = 50) depending on the usage of Reciproc (RC; VDW), Reciproc Blue (RC Blue; VDW), C-Pilot (CP; VDW), and R-Pilot (RP; VDW) files: RC, RC Blue, RC + C-Pilot (RC-CP), RC-Blue + C-Pilot (RC Blue-CP), RC+R-Pilot (RC-RP), and RC Blue + R-Pilot (RC Blue-RP). A glide path was prepared using the hand-operated C-Pilot or the machine-operated R-Pilot instruments, respectively. The ${\chi}^2$ test, analysis of variance, and the Tukey post hoc test were used for statistical comparisons. Results: No statistically significant differences were observed in the distribution of the frequency of reaching the full working length in the RC (94%), RC Blue (88%), RC-CP (94%), RC Blue-CP (90%), RC-RP (96%), and RC Blue-RP (92%) groups (p > 0.05). Conclusions: Preparation of a glide path did not have a significant effect on reaching the full working length using these systems.

Comparison of the shaping ability of novel thermally treated reciprocating instruments

  • Objectives: The present study aimed to evaluate the shaping ability of 2 thermally treated nickel-titanium reciprocating systems in simulated curved canals. Materials and Methods: Forty simulated canals were prepared to apical size 25 using Reciproc Blue R25 (VDW) and WaveOne Gold Primary (Dentsply Sirona) instruments. Standard pre- and post-preparation images were taken and superimposed. The removal of resin material was measured at 5 standard points: the canal orifice, halfway between the canal orifice and the beginning of the curve, the beginning of the curve, the apex of the curve, and the end-point of the simulated canal. The data were analysed using the independent sample t-test with a 5% significance threshold. Results: The canals in which Reciproc Blue R25 was used showed a significantly greater widening than those in which WaveOne Gold was used at 4 of the 5 measurement points (p < 0.05). The Reciproc Blue R25 instrument removed significantly more resin from the inner aspect of the curve at 2 of the 5 points and similar amounts at the remaining 3 points. At the 2 apical points, there was no significant difference between the Reciproc Blue R25 and WaveOne Gold Primary instruments. Conclusion: Both instruments respected the original canal anatomy; however, WaveOne Gold resulted in a more conservative shape with less transportation.

Effect of number of uses and sterilization on the instrumented area and resistance of reciprocating instruments

  • Objectives: This study evaluated the effect of repeated uses and autoclaving in the instrumented area, fracture resistance, and time of instrumentation of thermally treated nickel-titanium reciprocating systems. Materials and Methods: Two hundred simulated canals were instrumented using Reciproc Blue and WaveOne Gold. Each file was used up to 10 times or until fracture. The instrumented area was measured in pre- and post-operative images, using ImageJ software. Kaplan-Meier survival analysis evaluated the number of uses of instruments before fracture. Instrumented area and time of instrumentation were analyzed by Mann-Whitney U test and Kruskal-Wallis. Correlations among the number of uses and instrumented area were measured. The level of statistical significance was set at p < 0.05. Results: Reciproc Blue presented a higher estimated number of uses in comparison with WaveOne Gold (p = 0.026), but autoclaving did not affect the resistance to fracture of instruments (p > 0.05). The instrumented area was different among the evaluated groups (p = 0.039), and the instrumented area along the uses of both tested instruments was reduced. With the time of instrumentation, there was also a significant difference among the evaluated groups; the groups without sterilization cycles were faster, in comparison to those submitted to autoclaving (p = 0.010). Conclusions: Reciproc Blue was more resistant than WaveOne Gold, suffering later fracture. Additionally, the sterilization cycles did not influence the estimated number of uses of thermally treated reciprocating instruments, but the instrumented area of root canals was reduced along with the repeated uses of both instruments.

Shaping ability and apical debris extrusion after root canal preparation with rotary or reciprocating instruments: a micro-CT study

  • Objectives: The aim of this study was to evaluate the shaping ability of the TruShape and Reciproc Blue systems and the apical extrusion of debris after root canal instrumentation. The ProTaper Universal system was used as a reference for comparison. Materials and Methods: Thirty-three mandibular premolars with a single canal were scanned using micro-computed tomography and were matched into 3 groups (n = 11) according to the instrumentation system: TruShape, Reciproc Blue and ProTaper Universal. The teeth were accessed and mounted in an apparatus with agarose gel, which simulated apical resistance provided by the periapical tissue and enabled the collection of apically extruded debris. During root canal preparation, 2.5% sodium hypochlorite was used as an irrigant. The samples were scanned again after instrumentation. The percentage of unprepared area, removed dentin, and volume of apically extruded debris were analyzed. The data were analyzed using 1-way analysis of variance and the Tukey test for multiple comparisons at a 5% significance level. Results: No significant differences in the percentage of unprepared area were observed among the systems (p > 0.05). ProTaper Universal presented a higher percentage of dentin removal than the TruShape and Reciproc Blue systems (p < 0.05). The systems produced similar volumes of apically extruded debris (p > 0.05). Conclusions: All systems caused apically extruded debris, without any significant differences among them. TruShape, Reciproc Blue, and ProTaper Universal presented similar percentages of unprepared area after root canal instrumentation; however, ProTaper Universal was associated with higher dentin removal than the other systems.

Reciprocating 파일 시스템에서 Glide Path가 근관만곡도 유지에 미치는 영향 (The Effect of Glide Path on Canal Centering Ability in Reciprocating File System)

  • 본 실험은 왕복 회전 운동을 하는 단일 파일 시스템으로서 Reciproc과 WaveOne을 이용하였으며 glide path를 형성했을 때와 안 했을 때의 근관의 중심 변위율과 만곡도의 감소를 비교하여 새로운 단일 파일 시스템에서 glide path가 근관 성형 후 근관의 형태에 미치는 영향을 연구하였다. 총 40개의 모형 근관에 메틸렌블루 용액을 주입한 후 근관성형 전 이미지를 스캔 하였다. 네 개의 그룹으로 나누어졌으며 그룹 1은 PathFile (Dentsply Maillefer) #1, 2, 3 사용 후 R25 크기의 Reciproc 파일(VDW)을 사용하여 근관 성형하였다. 그룹 2는 PathFile 사용 없이 R25 크기의 Reciproc 파일을 사용하여 근관 성형하였다. 그룹 3은 PathFile #1, 2, 3 사용 후 Primary 크기의 WaveOne 파일 (Dentsply Maillerfer)을 이용하여 근관 성형하였다. 그룹 4는 PathFile 사용 없이 WaveOne 파일을 이용하여 근관 성형하였다. 파일은 모두 21 mm 길이를 사용했다. 각각의 파일을 위한 전기 모터로 Reciproc 파일은 VDW.SILVER motor (VDW), WaveOne 파일은 Waveone endo motor (Dentsply Maillefer)와 PathFile은 X-SMART (Dentsply Maillefer)를 사용하였다. 근관 성형을 마친 모형 근관을 다시 이미지 스캔 하였다. 성형 전, 후의 이미지를 Adobe Photoshop CS 3 (Adobe Systems Incorporated)을 이용하여 중첩하여 평가하였다. 중심 변위율은 Calhoun과 Montgomery에 의해 제안된 다음 공식을 사용하여 계산되었다:CR=|X1-X2|/Y. 실험 결과 근관의 만곡도가 감소하는 치근단 1, 2 mm level에서는 glide path의 형성에 따른 근관의 중심 변화율의 통계학적 유의성이 보이지 않았다. 근관 만곡 부위인 치근단 3, 4, 5 mm level에서는 glide path를 부여한 것이 통계학적으로 유의성 있게 낮은 중심 변위율을 보였다. 치근단 1 mm level에서는 Reciproc 파일이 유의성 있게 낮은 중심 변위율을 보였고, 치근단 1, 6 mm level을 제외한 모든 level에서 WaveOne 파일이 유의성 있게 낮은 중심 변위율을 보였다. 치근단 6 mm level에서는 Reciproc과 WaveOne의 차이는 관찰할 수 없었고 glide path를 형성한 그룹이 형성하지 않은 그룹에 비해 유의성 있게 낮은 중심 변위율을 보였다. 본 실험을 통해 만곡된 근관에서 근관 성형 전 glide path의 형성은 근관의 원래 형태를 유지하는데 필수적이며, 만곡된 근관에서는 WaveOne 파일 시스템의 선택이 근관의 원래 형태를 유지하는데 더 적절할 것으로 생각된다.

Effects of the endodontic access cavity on apical debris extrusion during root canal preparation using different single-file systems

  • Objectives: This study was conducted to evaluate the effects of traditional and contracted endodontic cavity (TEC and CEC) preparation with the use of Reciproc Blue (RPC B) and One Curve (OC) single-file systems on the amount of apical debris extrusion in mandibular first molar root canals. Materials and Methods: Eighty extracted mandibular first molar teeth were randomly assigned to 4 groups (n = 20) according to the endodontic access cavity shape and the single file system used for root canal preparation (reciprocating motion with the RCP B and rotary motion with the OC): TEC-RPC B, TEC-OC, CEC-RPC B, and CEC-OC. The apically extruded debris during preparation was collected in Eppendorf tubes. The amount of extruded debris was quantified by subtracting the weight of the empty tubes from the weight of the Eppendorf tubes containing the debris. Data were analyzed using 1-way analysis of variance with the Tukey post hoc test. The level of significance was set at p < 0.05. Results: The CEC-RPC B group showed more apical debris extrusion than the TEC-OC and CEC-OC groups (p < 0.05). There were no statistically significant differences in the amount of apical debris extrusion among the TEC-OC, CEC-OC, and TEC-RPC B groups. Conclusions: RPC B caused more apical debris extrusion in the CEC groups than did the OC single-file system. Therefore, it is suggested that the RPC B file should be used carefully in teeth with a CEC.

Efficacy of reciprocating instruments and final irrigant activation protocols on retreatment of mesiobuccal roots of maxillary molars: a micro-CT analysis

  • Objectives: This study evaluated the efficacy of 3 reciprocating systems and the effects of 2 instruments for irrigant activation on filling material removal. Materials and Methods: Forty mesiobuccal roots of maxillary molars were prepared up to size 25.06 and obturated. Micro-computed tomography (micro-CT) examination #1 was performed. Teeth were then divided into 4 groups (n = 10), according to the retreatment protocol: (1) manual, (2) Reciproc Blue, (3) WaveOne Gold, and (4) X1 Blue. Micro-CT examinations #2 and #3 were performed after filling removal and repreparation, respectively. Next, all teeth were divided into 2 new groups (n = 20) according to the irrigant activation protocol: XP Clean (XP Clean size 25.02) and Flatsonic (Flatsonic ultrasonic tip). Micro-CT examination #4 was performed after irrigant activation. Statistical analysis was performed with a significance level set at 5%. Results: WaveOne Gold removed a significantly greater amount of filling material than the manual group (p < 0.05). The time to reach the WL was similar for all reciprocating systems (p > 0.05). X1 Blue was faster than the manual group (p < 0.05). Only manual group improved the filling material removal after the repreparation stage (p < 0.05). Both activation protocols significantly improved the filling material removal (p < 0.05), without differences between them (p > 0.05). Conclusions: None of the tested instruments completely removed the filling material. X1 Blue size 25.06 reached the working length in the shortest time. XP Clean and Flatsonic improved the filling material removal.

Comparison of shaping ability of the Reciproc Blue and One Curve with or without glide path in simulated S-shaped root canals

  • Objectives: This study aimed to assess the impact of a glide-path on the shaping ability of 2 single-file instruments and to compare the centering ability, maintenance of original canal curvatures and area of instrumentation in simulated S-shaped root canals. Materials and Methods: Forty simulated S-shaped root canals were used and were prepared with One Curve (group OC), One G and OC (group GOC), Reciproc Blue (group RB) and R-Pilot and RB (group PRB) and scanned before and after instrumentation. The images were analyzed using AutoCAD. After superimposing the samples, 4 levels (D1, D2, D3, and D4) and 2 angles (Δ1 and Δ2) were established to evaluate the centering ability and modification of the canal curvatures. Then, the area of instrumentation (ΔA) was measured. The data were analyzed using 2-way analysis of variance and Tukey's test for multiple comparisons (p < 0.05). Results: Regarding the centering ability in the apical part (D3, D4), the use of the glide-path yielded better results than the single-file groups. Among the groups at D4, OC showed the worst results (p < 0.05). The OC system removed less material (ΔA) than the RB system, and for Δ1, OC yielded a worse result than RB (p < 0.05). Conclusions: The glide-path improved the centering ability in the apical part of the simulated S-shaped canals. The RB system showed a better centering ability in the apical part and major respect of the canal curvatures compared with OC system.

Successful nonsurgical treatment of type II dens invaginatus with 5 root canals using a self-adjusting file: a case report

  • The present report describes the endodontic treatment of an Oehlers type II dens invaginatus in a maxillary lateral incisor with 5 root canals, an extremely rare condition. Apical periodontitis and related symptoms were noted. Cone-beam computed tomography was used to aid the diagnosis, reveal tooth morphology, and assist in canal location. The pulp chamber was carefully accessed, and the root canals were explored under magnification. All root canals were prepared with an R25 Reciproc Blue system and sodium hypochlorite (NaOCl) irrigation. After initial preparation, a self-adjusting file (SAF) with NaOCl and ethylenediaminetetraacetic acid was used to complement the disinfection. Additionally, calcium hydroxide medication was applied. Vertical compaction was used to fill the canals with a calcium silicate-based endodontic sealer and gutta-percha. After 12 months, the patient exhibited healing of the periapical region, absence of symptoms, and normal dental function. In conclusion, this nonsurgical treatment protocol was successful in promoting the cure of apical periodontitis. Both complementary disinfection with an SAF and use of calcium hydroxide medication should be considered when choosing the best treatment approach for dens invaginatus with very complex anatomy.
