A Case Report of HBsAg Seroclearance in Chronic Hepatitis B Patient

  • Hepatitis B surface antigen (HBsAg) seroclearance is a rare event in chronic hepatitis B virus (HBV) infection which acquires the disease early in life. A case study have examined with asymptomatic chronic hepatitis B carrier who exhibits HBsAg seroclearance in anti-HBe positive. We comprehensively studied the biochemical, virological and clinical aspects of a patient with HBsAg seroclearance. Liver biochemistry, serological markers, serum HBV DNA levels, and development of clinical complications were monitored. Mutation of hepatitis B virus is suspected serum HBsAg detected by the HBsAg assay systems of VITROS (OrthoClinical Diagnostics, USA), AxSYM (Abbott Laboratories, USA), Elecsys (Roche Diagnostics, Germany) and ADVIA Centaur (Bayer Diagnostics, USA). These four immunoassays showed negative results. Also, the patient had undetectable serum HBV DNA. Therefore, no mutation within the "a" determinant of HBsAg, which might escape detection from HBsAg immunoassay were found. Natural seroclearance was confirmed.

Space Missions to Asteroids

  • Asteroids represent a significant resource for space exploration and scientific research. Various scientific missions have already performed and planned to investigate and understand the characteristics of asteroids. This talk introduces many space missions to asteroids. Representing missions to asteroids are the NASA's NEAR, Deep Space-1, Dawn, OSIRIS-Rex, SCOUT, DART, and ESA's Rosetta, and JAXA's Hayabusa 1 and 2, and DESTINY+ missions, and others. Although it is a very rare event, the possibility of Earth-crossing asteroids (ECAs) colliding with the Earth can never also be ignored. Numerous mitigation concepts also have been proposed to deflect ECAs in preparing for the disasters which might occur in future days. In the early studies for mitigation schemes, most of analyses were centered on to deflect ECAs with impacting the energy to the object to change its orbit. This talk also introduces many methods to deflect the orbit of ECAs, and shows spacecraft trajectories to asteroids.

  • A postoperative contralateral supra- and infratentorial epidural hematoma after decompressive surgery is an extremely rare event. We describe a 38-year-old male with a contralateral supra- and infratentorial acute epidural hematoma just after decompressive surgery for an acute subdural hematoma. A contralateral skull fracture involving a lambdoidal suture and an intraoperative brain protrusion may be warning signs. The mechanisms, along with relevant literature, are discussed.

  • Rupture of the esophagus after blunt trauma is a rare event. But any type of esophageal rupture has the high morbidity and mortality rate. In these situations, the sign and symptom of the esophageal rupture is subtle and nonspecific; therefore, the physicians are usually not suspicious. Delaying in diagnosis prevents proper treatment (surgical or non-surgical) before significant complications occurred. We report a case of a cervical esophageal perforation with primary repair and drainage after blunt trauma.

  • We encountered a rare condition of rice body in subacromial and subdeltoid spaces in a patient with unremarkable medical history. Although it is uncommon, there have been continued reports on its formation in certain type of infective and inflammatory arthritis. However, except for a traumatic event, evaluation yielded no known and conceivable cause for his chronic inflammatory bursitis. Relatively typical findings for rice body on magnetic resonance imaging have been described, and in our case the imaging prompted us to schedule early removal, which is generally accepted as the management of choice to prevent further progression of symptoms. The symptoms of the shoulder showed significant improvement, and a close follow-up schedule has been recommended for observation of recurrence and development of any foreseeable underlying cause.

Traumatic Epidermal Inclusion Cyst under Anterolateral Thigh Free Flap on Great Toe

  • Epidermal inclusion cyst is a common mass in life. It is covered with a stratified squamous epithelium, thus, there is a granular cell layer adjacent to the keratin-containing cyst lumen. It can be caused by mechanical force, trauma, or a spontaneous event. It can rupture spontaneously or be ruptured by external mechanical forces. Epidermal inclusion cysts that exhibit inflammation or recur should be removed by simple excision. In this case, the patient showed an epidermal inclusion cyst under an anterolateral thigh free flap, which can cause the palpable mass to go unnoticed. First we thought he had neuroma formation after a surgical procedure on his foot. However it was an epidermal inclusion cyst, which was diagnosed by a special pathologist. It is a curious and rare case.

  • Damaging winds, associated with a variety of weather phenomena, are frequently experienced in New Zealand. Observations and modelling of two recent extreme wind events; the Taranaki tornado outbreak of July 2007, and the Greymouth down-slope easterly wind storm of July 2008 are described in detail here. Post-event engineering damage surveys, rare for New Zealand, were done for these storms and the results are summarized here. Finally, the issue of sampling extreme wind events is raised and the need to include detailed numerical modelling analysis to understand wind gust climatologies at observing sites and extending these to wider regions is discussed.

Intraventricular Hemorrhage Long after Successful Encephaloduroarterio Synangiosis in Moyamoya Patient

  • Intraventricular hemorrhage long after successful encephaloduroarterio synangiosis (EDAS) is very rare. The effect of revascularization surgery for preventing hemorrhagic event of moyamoya disease remains controversial. We report a 17-year-old female with intracerebral hemorrhage and intraventricular hemorrahge 10 years after successful EDAS. Even though cerebral vessels angiography showed good collateral circulations without specific weak points, a cerebral hemorrhage could occur in patient with ischemic type of moyamoya disease long after successful indirect bypass operations. Good collateralization of cerebral angiography or magnetic resonance perfusion image after indirect bypass surgery would ensure against ischemic symptoms, not a hemorrhage. And, thus a life-time follow-up strategy might be necessary even if a good collateral circulation has been established.

Mutations in the tyrosine kinase domain of the EGFR gene are rare in the Korean Oral Squamous Cell Carcinoma

  • The epidermal growth factor receptor(EGFR) protein kinase signaling is an important pathway in cancer development and recently reported that EGFR and its kinase domain molecules are mutated in various of cancers including head and neck cancer. Functional deregulation of EGFR due to mutations in coding exons and copy number amplification is the most common event in cancers, especially among receptor tyrosine kinases(TK). We have analyzed Korean oral squamous cell carcinomas (OSCC) cell lines for mutations in EGFRTK. Exons encoding the hot-spot regions in the TK domain of EGFR (exons 17 to 23) were amplified by using polymerase chain reaction(PCR) and sequenced directly. EGFR expression was also analyzed in 8 OSCC cell lines using western blotting. Data analysis of the EGFR exons 17 to 23 coding sequences did not show any mutations in the 8 OSCC cell lines that were analyzed. The absence of mutations indicate that protein overexpression might be responsible for activation rather than mutation.

Heterogeneous 입력원을 갖는 ATM 스위치의 셀 손실확률 추정을 위한 Hybrid 시뮬레이션 기법

  • 광대역 종합정보 통신망의 핵심요소라 할 수 있는 ATM 스위치의 성능척도 중 가장 중요하게 다루어지고 있는 것은 셀 손실확률과 셀 전달지연시간이다. 이 중에서도 샐 손실확률기 추정에 대한 연구가 활발히 진행되고 있는데, ATM 스위치는 손실에 민감한 트래픽까지도 제대로 다루기 위하여 정도까지의 샐 손실확률을 보장할 수 있어야 한다. 이와같은 희소사건(rare event)의 확률 추정에 있어 원하는 정도의 precision을 가능한한 적은비용으로 얻어내기 위한 분산축소기법은 필수적이라 할 수 있다. Homogeneous 입력원을 갖는 ATM 스위치의 셀 손실확률 추정에 관련된 이전의 연구결과는 시뮬레이션과 분석적기법을 혼합시켜 얻어지는 새로운 개념의 추정치, 즉 hybrid 시뮬레이션 추정치의 도입을 통하여 상당한 정도의 분산축소 효과를 거둘 수 있음을 나타내주고 있다. 본 연구는 이에 대한 확장으로, 각각의 도착 프로세스가 서로 다른heterogeneous 입력원을 갖는 ATM 스위치의 셀 손실화률 추정에 적용될 수 있는 hybrid 시뮬레이션 기법을 개발하고자 한다. 사용된 모델은 이산시간대기모델()로 각입력원의 도착 프로세스는 Interrupted Bernoulli Process로 가정하였으며, 분석적 기법의 적용을 위한 입력원 통합(aggregation) 알고리듬과 실제 시뮬레이션 방법 등을 제시하였다. 또한 제시된 기법의 성능은 기존의 일반적인 시뮬레이션 추정치를 이용하여 얻어진 결과와의 비교를 통하여 분석되었다.
