Antibiotic-Induced Increase in Inflammatory Markers in Cured Infectious Spondylitis : Two Case Reports

  • Conservative therapy with appropriate antibiotics is essential for most patients with infectious spondylitis. Although most antibiotics do not cause problems if it used properly and serious side effects are rare, side effects can occur with any class of drugs and adverse reactions of antibiotics can range from mild allergic reactions to serious and fulminant adverse events. These side effects are also extremely variable from patient to patient and from antibiotic to antibiotic. A side effect of antibiotics may paradoxically increase inflammatory marker levels. Here, the author presents two cases of antibiotic-induced increase in inflammatory markers in cured infectious spondylitis. The patients were successfully treated after stopping the antibiotic therapy. The differential diagnosis between antibiotic side effects and infection should be considered very carefully because the treatment is completely different. Although the exact mechanisms underlying successful treatment without antibiotics are unclear, we should consider the side effects of antibiotics when following inflammatory markers during treatment of infectious spondylitis.

  • Ginseng has been used as a popular herbal medicine in East Asia for at least two millennia. However, 20(R)-ginseng saponins, one class of important rare ginsenosides, are rare in natural products. 20(R)-ginseng saponins are generally prepared by chemical epimerization and microbial transformation from 20(S)-isomers. The C20 configuration of 20(R)-ginseng saponins are usually determined by 13C NMR and X-ray single-crystal diffraction. 20(R)-ginseng saponins have antitumor, antioxidative, antifatigue, neuroprotective, and osteoclastogenesis inhibitory effects, among others. Owing to the chemical structure and pharmacological and stereoselective properties, 20(R)-ginseng saponins have attracted a great deal of attention in recent years. In this study, the discovery, identification, chemical epimerization, microbial transformation, pharmacological activities, and metabolism of 20(R)-ginseng saponins are summarized.

A Study on the Spatial and Environmental Characteristics of Forest Biology using GIS: A Case Study of Baekdudaegan area, Gyeongsangbuk-do and Chungcheongbuk-do (GIS를 이용한 산림 생물의 공간적·환경적 특성 분석 - 백두대간(경북·충북)을 대상으로 -)

  • The purpose of this study was to understand the geographical and environmental distribution of animals and plants in Baekdudaegan region using field survey and GIS data. Crucial factors were selected and analyzed to understand the distributional characteristics of wild animals (16 species in 5 orders) and rare endemic plants (20 species in 12 orders). These crucial factors include stand factor (forest type, DBH class, and crown density), soil factor (bed rock, soil texture, and organic matter), geographical factor (elevation, slope, aspect) and climatic factor (temperature, rain fall, humidity). Finally, ten crucial factors were selected by statistical analysis and categorized for analyzing geographical and environmental features. Three orders such as Rodentia, Carnivora, and Artiodactula in wild animal showed the similar habitat characteristics with the small diameter and the elevation range from 801 to 1,000m. The Hydropotes inermis of Artiodactyla and Rattus norvegicus of Rodentia were different in the type of orders, but they had the similar habitat characteristics with the coniferous forest and loam. On the other hand, four orders such as Tubiflorales, Liliales, Ericales, and Rhamnales in the rare and endemic plants were showed high occurrence rate in the organic matter between 4 and 6%. The Rodgersia podophylla of Rosales and Gastrodia elata Blume of Microspermae were different in the type of orders, but they had the similar habitat characteristics with the stand factor and soil factor.

Qualitative Analysis of Research Papers of KIGAM World Class Laboratories (WCL) Candidates (논문 질적평가를 통한 KIGAM 세계수준 후보연구실 기술수준 평가)

  • For technology level assessment of KIGAM World Class Laboratories (WCL) candidates, bibliometric and qualitative analysis was conducted on their research papers listed on the SCIE database during 2009-2012. For the six research areas of geoscience and mineral resources, a research excellence indicator was applied using a Modified Rank Normalized Impact Factor (mrnIF), which was introduced by Heo et al. (2008) and Cho (2013). The KIGAM research department in rare metals utilization had the highest score for Impact Factor (IF) per paper in 2012 but the groundwater department or the exploration geophysics department came first based on the position and the mrnIF. Applying the mrnIF, the KIGAM research department in groundwater achieved excellent results in 2009 and 2011 and the urban mine department or exploration geophysics department came first place in other years. In the groundwater area, the percentage of research papers over 80 or 90 mrnIF, using Cho (2013)'s research excellence index, was the highest in 2011. The Cho (2013)'s excellent research indicator, 20%, the ratio of over 90 mrnIF was matched in the urban mining area for the whole research period, 2009-2012, and in the groundwater area for several years except 2010. Qualitative analysis of research papers can show the technology level of research departments. KIGAM World Class Laboratories (WCL) candidates should focus on increasing the quality and the quantity of their research papers.

A Study on the Evaluation of Biotope Preservation Value in District Unit - Case Study in Sinseo-Dong, Daegu - (지구단위 차원에서의 비오톱 보전가치평가 연구 - 대구광역시 신서동 택지개발 사업지구를 사례로 -)

  • This research has a meaning to provide basic data for eco-friendly way of district unit plans and ecological landscape planning by evaluation of biotope preservation value at the level of district unit and designating land development of the site, the whole area of Sinseo-dong (Dong-gu, Daegu metropolitan city) for research site. The summary of analysis result is as follows. As a result of classification of biotope types on the research site, it is divided into 11 biotope groups such as a residential biotope group and 51 specific biotope types which is subordinate to the groups. As a result of the first value assessment on classified biotope types, there are 16 types of natural rivers which is full of vegetation as a I class. Also it is analysed as 9 types of IIclass, 14 of IIIclass, 8 of IVclass, and 4 of Vclass. In particular, in light of a wildlife habitat, EB, in case of broad-leaved tree of mixed forest assessed as a II class, was classified into Iclass which is one-step upgraded as a final class with the analysis as there is a structural characteristic (more than 71% of low density, 50 years of age-class). As a result of second assessment, it is analysed that there are 17 special sites (1a,1b) and 33 special sites (2a, 2b, 2c) respectively for preservation of species and biotope. Particularly, in case of the No. 27 space, it was assessed that it has the value of about medium (IIIclass) level, but its value was upgraded with the on-spot detailed investigation that most of Aristolochia contorta, designated as a rare plant by Ministry of Environment, is growing. It is regarded that the above-mentioned research result on evaluation of biotope preservation value is expected to provide very important basic materials for future district unit plans and smooth integration with landscape ecology plans and eco-friendly space development.

A Case of Hepatopulmonary Syndrome in a Patient with Child-Pugh Class A Liver Cirrhosis (Child-Pugh 분류 A군 간경화에 수반된 간폐증후군 1예)

  • Hepatopulmonary syndrome (HPS) is characterized by a defect in arterial oxygenation that's induced by pulmonary vascular dilatation in the setting of liver disease. Some studies have shown the relationship between the presence of the HPS and the severity of liver disease, but there are only rare cases of HPS inpatient with Child-Pugh class A liver cirrhosis. We report here on a case of a 58 years-old male who suffered from progressive dyspnea for the previous few years. He was diagnosed with alcoholic liver cirrhosis 5 years previously. There was no significant abnormality on the chest radiograph and transthoracic echocardiography, but the arterial blood gas analysis revealed severe hypoxemia. Contrast-enhanced transesophageal echocardiograpy with agitated saline demonstrated a delayed appearance of microbubbles in the left cardiac chambers. Thus, he was finally diagnosed with HPS. This case suggests that we should consider HPS when a patient with compensated liver cirrhosis has unexplained dyspnea.

  • The new class of $Gd_5(Si_xGe_{1-x})_4$ compounds undergoes a simultaneous magnetic/structural phase transition giving a high level of strain that can be induced either by change in temperature or by application of a magnetic field. Profound changes of structural, magnetic, and electronic changes occur in the $Gd_5(Si_xGe_{1-x})_4$ system lead to extreme behavior of the material such as the giant magnetocaloric effect, colossal magnetostriction, and giant magnetoresistance. These unique material characters can be utilized for various applications including magnetic solid refrigerants, sensors, and actuators.

A R&D Model for Korean SI Companies' International Competitiveness (World Class SI 기업으로의 도약 : 핵심 경쟁력 확보를 위한 R&D 모델 개발)

  • This thesis suggests a R&D model for Korean SI companies who aggressively drive oversea business to overcome domestic market limitation and to become world class SI players. Even though there have been a lot of discussions on IT technologies and new business models, scientific approach to a R&D model for SI companies is rare due to its natural distinctiveness from manufacturing and general service domain. Therefore, this will be a flagship thesis to keep up our study. As business market environment gets more competitive, major Korean SI players will be confronted with much more keen competition among themselves. And as they are willing to expand their oversea market presence, they can not help struggling against global SI players. So, here we are presenting SI players the reinforcement of R&D as one of the key components to build up their own core competencies. That is, SI players have to adopt and evaluate fast changing and complicated IT technologies at the right time, and to apply them into a real business with cost-effective service delivery and operation processes internally. The ultimate purpose of all of these activities is to offer a SI company the value added business model based on its innovative technical capabilities and well refined service deliver model. For this purpose, we suggests, SI company's R&D has to play an important role of emerging technology verification/evaluation/utilization, value added business model creation, future innovation lead, standard body participation, and effective service delivery and quality system development. To become a more effective R&D organization, hybrid R&D, in which central R&D and divisional sub R&D work together, is considered as an idea model. The reliability of the R&D model for Korean SI companies', here we suggest, has been checked by SI companies R&D specialists and on-site business people.

Latent Class Analysis of Gambling Activities among Korean Adolescents (한국 청소년 도박유형 특성의 잠재계층분석)

  • Purpose: The aim of this study is to identify the types of gambling among adolescents and provide basic prevention information regarding adolescents' gambling problems. Methods: Secondary data from representative national survey on 2015 Youth Gambling Problems of Korea Center on Gambling Problems were used. Using latent class analysis (LCA), 13 gambling types such as offline and online games of 14,011 adolescents were classified, and gambling experiences and characteristics were analyzed. Results: The subgroups of adolescent gambling were identified as four latent classes: a rare group (84.5% of the sample), a risk group (1.0%), an offline group (11.9%), and an expanded group (2.6%). The types and characteristics of gambling among the latent classes differed. In the risk group, adolescents participated in online illegal sports betting and internet casino, and gambling time, gambling expenses, and the number of gambling types were higher than other groups. Conclusion: Gambling frequently occur among adolescent, and the subtypes of gambling did not reveal homogeneous characteristics. In order to prevent adolescent gambling problems, it is a necessary to develop tailored prevention intervention in the nursing field, which is appropriate to the characteristics of adolescent gambling group and can help with early identification.

The Effects of both Social Class of Adolescents Formed in Online Game World and Gaming Time on the Acceptance of Moral Issues (온라인 게임세계에서 형성된 청소년의 사회적 지위와 게임 시간이 도덕적 문제 수용에 미치는 효과)

  • This study was carried out to verify whether there were any differences in accepting moral issues between adolescents who played using characters of high status (either objectively or subjectively highly ranked within a game) in the online game world and those who played using characters of low status (either objectively or subjectively lowly ranked). It also investigated whether there were moderating interactional effects for the amount of time spent on playing games and the influence of adolescents' social statuses in games over accepting moral issues. Results showed that, overall, adolescents who played using a lower-ranking character in the game world displayed a higher acceptance of moral issues than those who played using characters of the two other categories (equal to average level or higher). Furthermore, interactional effects were observed for those who played using a low-ranking character and spent longer hours playing games, as they were more likely to accept moral issues. This study is significant in that it offers many implications, as a rare study that verified the effects of cyberspace status on moral judgements made in the real world.
