Purpose: To compare volumetric modulated arc therapy of RapidArc with robotic stereotactic body radiation therapy (SBRT) of CyberKnife in the planning and delivery of SBRT for hepatocellular carcinoma (HCC) treatment by analyzing dosimetric parameters. Materials and Methods: Two radiation treatment plans were generated for 29 HCC patients, one using Eclipse for the RapidArc plan and the other using Multiplan for the CyberKnife plan. The prescription dose was 60 Gy in 3 fractions. The dosimetric parameters of planning target volume (PTV) coverage and normal tissue sparing in the RapidArc and the CyberKnife plans were analyzed. Results: The conformity index was $1.05{\pm}0.02$ for the CyberKnife plan, and $1.13{\pm}0.10$ for the RapidArc plan. The homogeneity index was $1.23{\pm}0.01$ for the CyberKnife plan, and $1.10{\pm}0.03$ for the RapidArc plan. For the normal liver, there were significant differences between the two plans in the low-dose regions of $V_1$ and $V_3$. The normalized volumes of $V_{60}$ for the normal liver in the RapidArc plan were drastically increased when the mean dose of the PTVs in RapidArc plan is equivalent to the mean dose of the PTVs in the CyberKnife plan. Conclusion: CyberKnife plans show greater dose conformity, especially in small-sized tumors, while RapidArc plans show good dosimetric distribution of low dose sparing in the normal liver and body.
The conventional delivery quality assurance (DQA) process for RapidArc (Varian Medical Systems, Palo Alto, USA), has the limitation that it measures and analyzes the dose in a phantom material and cannot analyze the dosimetric changes under the motional organ condition. In this study, a DQA method was designed to overcome the limitations of the conventional DQA process for internal target volume (ITV) based RapidArc. The dynamic DQA measurement device was designed with a moving phantom that can simulate variable target motions. The dose distribution in the real volume of the target and organ-at-risk (OAR)s were reconstructed using 3DVH with the ArcCHECK (SunNuclear, Melbourne, USA) measurement data under the dynamic condition. A total of 10 ITV-based RapidArc plans for liver-cancer patients were analyzed with the designed dynamic DQA process. The average pass rate of gamma evaluation was $81.55{\pm}9.48%$ when the DQA dose was measured in the respiratory moving condition of the patient. Appropriate method was applied to correct the effect of moving phantom structures in the dose calculation, and DVH data of the real volume of target and OARs were created with the recalculated dose by the 3DVH program. We confirmed the valid dose coverage of a real target volume in the ITV-based RapidArc. The variable difference of the DVH of the OARs showed that dose variation can occur differently according to the location, shape, size and motion range of the target. The DQA process devised in this study can effectively evaluate the DVH of the real volume of the target and OARs in a respiratory moving condition in addition to the simple verification of the accuracy of the treatment machine. This can be helpful to predict the prognosis of treatment by the accurate dose analysis in the real target and OARs.
Purpose: To determine feasibility of RapidArc in sequential or simultaneous integrated tumor boost in whole brain radiation therapy (WBRT) for poor prognostic patients with four or more brain metastases. Materials and Methods: Nine patients with multiple (${\geq}4$) brain metastases were analyzed. Three patients were classified as class II in recursive partitioning analysis and 6 were class III. The class III patients presented with hemiparesis, cognitive deficit, or apraxia. The ratio of tumor to whole brain volume was 0.8-7.9%. Six patients received 2-dimensional bilateral WBRT, (30 Gy/10-12 fractions), followed by sequential RapidArc tumor boost (15-30 Gy/4-10 fractions). Three patients received RapidArc WBRT with simultaneous integrated boost to tumors (48-50 Gy) in 10-20 fractions. Results: The median biologically effective dose to metastatic tumors was 68.1 $Gy_{10}$ and 67.2 $Gy_{10}$ and the median brain volume irradiated more than 100 $Gy_3$ were 1.9% (24 $cm^3$) and 0.8% (13 $cm^3$) for each group. With less than 3 minutes of treatment time, RapidArc was easily applied to the patients with poor performance status. The follow-up period was 0.3-16.5 months. Tumor responses among the 6 patients who underwent follow-up magnetic resonance imaging were partial and stable in 3 and 3, respectively. Overall survival at 6 and 12 months were 66.7% and 41.7%, respectively. The local progression-free survival at 6 and 12 months were 100% and 62.5%, respectively. Conclusion: RapidArc as a component in whole brain radiation therapy for poor prognostic, multiple brain metastases is an effective and safe modality with easy application.

The dust generated from the electric arc furnace steel making process is classified as hazardous material by Korean Environmental Protection Acts, mainly because of the existence of water teachable Pb, Zn and Cd. Thus the treatment of EAF dust is being carried out to fulfill both the environmental aspect and recovery of valuable metals. To establish the proper process for recovering the valuable metals (Fe, Zn, Pb and Cd) and producing the non-toxic slag from EAF dust, using RAPID-10 system, feasibility study have been carried out. To find out the scale-up factor for designing the commercial scale EAF dust treatment process(capacity 50,000 ton EAF dust per year) entitled RAPID-50 system. The design and construction of RAPID-50 (RIST Arc Plasma Industrial Device) system for treating 50,000 ton of EAF dust per year is now undergoing. Overall plan for treating EAF dust generated in KOREA will be setup after successful operation (December, 2002) of RAPID-50 system.
호흡연동 래피드아크 치료는 무건운 하중의 선형가속기 갠트리의 회전과 정지의 반복과정에서 갠트리 회전 재시작점의 오차와 다엽조리개의 정확한 움직임 및 갠트리 속도와 같은 래피드아크의 선량정확도를 결정하는 요소들의 오차 가능성으로 인한 선량 오류가 발생할 수 있다. 본 연구에서는 이러한 갠트리의 회전과 정지의 반복적인 동작이 호흡연동 래피드아크 치료의 정확도에 어떠한 영향을 끼치는 지 치료 시 총 회전과 정지 동작 수를 결정하는 빔조사 구간 길이의 변화를 통해 분석하였다. 총 10명의 간암 환자를 대상으로 래피드아크 치료계획을 수립하였고, RPM 호흡연동 장치와 정확한 빔조사 구간 길이를 설정하기 위해 동적 팬텀을 사용하였다. 각 래피드아크 치료계획 당 EPID를 사용한 portal dosimetry delivery quality assurance (DQA) 계획과 이차원 다이오드 검출기배열장치인 MapCHECK2를 사용한 DQA 계획을 수립하여 호흡연동 방사선 치료과정에서 누적된 선량분포의 정확도를 분석하였다. 모든 환자의 호흡주기는 4초로 설정하였고, 수립한 DQA 계획들을 호흡연동 없이 연속적으로 조사하는 것과, 1초의 빔조사 구간과 2초의 빔조사 구간, 총 3가지의 경우에 대해 실제 방사선량 측정과 감마평가를 통해 선량의 정확도를 분석하였다. Portal dosimetry DQA 경우 평균 감마평가의 합격률은 호흡연동 없이 연속적일 때 $98.72{\pm}0.82%$ 였고, 1초의 빔조사 구간의 경우 $94.91{\pm}1.64%$, 2초의 빔조사 구간의 경우 $98.23{\pm}0.97%$이었다. MapCHECK2 DQA경우 평균 감마평가의 합격률은 호흡연동 없이 연속적일 때 $97.80{\pm}0.91%$였고, 1초의 빔조사 구간의 경우 $95.38{\pm}1.31%$, 2초의 빔조사 구간의 경우 $97.50{\pm}0.96%$였다. 본 연구 결과를 통해 빔조사 구간의 길이가 증가하여 갠트리 정지 동작 수가 감소할수록 호흡연동 래피드아크의 선량 정확도가 증가함을 확인할 수 있었으며, 이러한 특성을 호흡연동 방사선치료 대상 환자의 선정 및 호흡방식에 대한 교육과정에 고려되어야 할 것으로 판단되었다.
목 적 : 본 연구는 이미 상용화가 시작된 호흡 동조 체적 세기조절 회전 방사선 치료(Gated RapidArc) 이전의 자동화가 되지 않는 장비들에서 호흡 동조 방사선 치료(Gated radiation therapy)와 체적 세기조절 회전 방사선 치료(VMAT)를 동시에 시행할 수 있게 Gated RapidArc의 정확성을 분석하여 유용성을 평가하고, 진폭모드(Amplitude mode)를 이용하여 Gated RapidArc가 자동으로 되지 않는 장비에 환자를 적용하여 보고자 하였다. 대상 및 방법 : 방사선량 분포의 분석은 물 등가물질 고체 팬톰과 GafChromic 필름(EBT2 QD+, USA)을 이용하였으며, Film QA (ver. 2.2, USA) 필름 분석 프로그램을 이용하여 Gamma 인자(3%, 3 mm)를 분석하였다. 또한, 삼차원 선량 분포의 정확도를 확인하기 위해서 Matrixx(IBA Dosimery, Germany) 선량 측정 장비와 Compass(IBA Dosimetry, Germany) 선량 분석 프로그램을 이용하였다. 고체 팬톰을 이용한 호흡 동조 주기 신호는 4D 팬톰(Dynamic Thorax Phantom, CIRS, USA)과 Varian RPM(Real-Time Position Monitor) 호흡 동조 시스템을 이용하여 만들었으며, 자유호흡(free breathing)과 호흡정지(breath holding) 시에 따른 방사선량 분포를 각각에 대하여 분석 평가하였다. 환자에게 적용하기 위하여 2013년 2월부터 2013년 8월까지 간암환자 4명을 대상으로 4DCT의 영상을 얻기 위하여 충분한 호흡주기 연습후에 환자의 호흡주기에 맞게 위상모드(Phase mode)를 이용하여 환자가 고글의 호흡주기 패턴을 눈으로 보고 정확하게 따라할 수 있도록 하면서 4DCT의 영상을 획득하였다. Gated RapidArc 치료를 위하여 진폭모드(Amplitude mode)의 호흡주기를 만들어 3회 호흡을 시행한 후 40%~60%의 구간에서 5~6초 호흡을 참을 수 있도록 연습을 하고, 치료 시 40%~60%의 구간에서 환자가 숨을 참을 때 Beam On 버튼을 눌러주는 방식의 반자동으로 치료를 시행하였다. 결 과 : 비 호흡 및 호흡 동조 체적 세기조절 회전 방사선 치료기법 간의 절대선량은 전산화 치료 계획을 이용한 계산값과 1% 이내의 차이를 보였으며, 치료 기법 간의 차이 또한 1% 이내의 차이를 보였다. Gamma 인자(3%, 3 mm)는 99% 이상의 일치함을 보였으며, 각 장기별 선량 차이는 대체로 95% 이상의 일치함을 보였다. 또한 호흡 동조 체적 세기조절 회전 방사선 치료를 위하여 만든 진폭모드(Amplitude mode)의 호흡주기와 실제 환자의 호흡주기가 잘 일치하는 것을 볼 수 있었다. 결 론 : 비 호흡 동조와 호흡 동조 시 체적 세기조절 회전 방사선 치료간의 절대 선량 및 방사선량의 분포가 매우 잘 일치함을 보였다. 이는 호흡 동조 체적 세기조절 회전 방사선 치료 기법을 이용하여 호흡에 따라 움직이는 흉부나 복부의 종양 치료에 적용이 가능한 것으로 사료된다. 또한 실제 치료환자를 대상으로 고글을 통하여 진폭모드(Amplitude mode)의 호흡주기를 만들어 Gated RapidArc가 자동으로 되지 않는 장비에 치료를 적용한 결과, 5~6초정도 정지된 호흡에서 호흡 동조 체적 세기조절 회전 방사선 치료가 원활히 이루어짐을 알 수 있었다.
This paper applies the arc detection algorithm to prevent the false detection in photo voltaic series arc detection circuit, which is required not only to detect the series arc quickly, but also not falsely detect the arc for the non-arc noise. For this purpose, this study proposes a rapid and preventive false detection method of single peak noise and short noise signals. First, to prevent false detection by single peak noise, Discrete wavelet transform (DWT)-based characteristic parameters are applied to determine the shape and the amplitude of the noise. In addition, arc fault detection within a few milliseconds is performed with the DWT iterative algorithm to quickly prevent false detection for short noise signals, considering the continuity of serial arc noise. Thus, the method operates not only to detect series arc, but also to avoid false arc detection for peak and short noises. The proposed algorithm is applied to real-time serial arc detection circuit based on the TMS320F28335 DSP. The serial arc detection and peak noise filtering performances are verified in the built simulated arc test facility. Furthermore, the filtering performance of short noise generated through DC switch operation is confirmed.
The feasibility for a practical use of underwater wet arc welding process is experimentally investigated by using low hydrogen and high oxide type electrodes and TMCP steel plates. Main results are summarized as follows: 1)The absorption speed of the coated low hydrogen and high titanium oxide type eletrodes becomes constant after about 30 minutes in water, and more steeping time in water does not influnce welding arc behavior. 2) By bead appearance and X-ray inspection, the high titanium oxide type electrode is better than the low hydrogen type in underwater arc welding process. 3) The mechanical properties of underwater wet arc welds depend upon welding conditions more than those of in-air welds, and the optimum welding condition can be obtained. 4) Because of quenching effect by rapid cooling rate in underwater wet welding, the maximum hardness of HAZ is increased relatively higher in underwater wet welding, process.
환자 호흡할 때 흉 복부 내부에 있는 장기의 위치는 주기적으로 변한다. 이에 따라 방사선치료 동안 종양에 대한 선량불확도가 발생하게 되며, 불확도를 줄이기 위한 여러 방사선치료방법이 제시되고 있다. 호흡연동방사선치료는 특정 위상 또는 진폭에만 방사선을 조사하는 방법으로 불필요한 피폭선량은 줄일 수 있는 장점이 있지만 긴 치료 시간과 노력이 필요하다는 단점이 있다. 호흡연동방사선치료 중 회전세기조절방사선치료(respiratory gated Volumetric Modulated Arc Therapy, VMAT)는 다른 호흡연동치료시스템에 비해 치료 시간이 짧다는 장점이 있기 때문에 본 연구는 respiratory gated VMAT 치료 선량의 정확성을 검증하여 임상 적용의 적절성을 평가하고자 한다. 본 연구는 총 6개의 VMAT 치료계획(Eclipse, ver. 8.6, Palo Alto, USA)을 토대로 수행되었으며, 각각의 치료계획은 AAA 알고리즘을 이용해서 선량이 계산되었다. 환자의 호흡운동을 구현하기 위해 환자 테이블 위에 1차원운동팬텀이 사용되었으며, 영상 기반 추적 시스템(Real-time Position Management, RPM, Varian Medical Systems, Palo Alto, USA)을 통해 운동 주기 신호를 획득하였다. 또한, 2차원-이온함-배열(MatriXX, IBA, Germany) 측정기를 이용하여 특정 호흡 신호 위상에 따른 전달 선량을 측정하였다. 측정된 선량과 치료 계획된 선량을 정성적인 분석을 위해 상용화되어 있는 선량분석용 프로그램(I'mRT, IBA, Germany)을 통해 2차원 선량분포를 0에서 1사이의 감마지표(Gamma index) 비교 결과 모든 케이스에서 97% 이상의 일치함을 확인하였다. 따라서 호흡연동 회전세기조절 방사선치료는 호흡연동방사선치료의 단점인 시간적인 제약을 일정 부분 해소할 수 있었으며 2차원 선량분포 비교 결과 오차값 3%이내의 정확도에서 환자정도관리 수준을 만족하였고 임상적용이 가능함을 확인하였다.
One of the methods to consider the effect of respiratory motion of a tumor target in radiotherapy is to establish a treatment plan with the internal target volume (ITV) created based on an accurate analysis of the target motion displacement. When this method is applied to intensity modulated radiotherapy (IMRT), it is expected to yield a different treatment dose distribution under the motion condition according to the IMRT method. In this study, we prepared ITV-based IMRT plans with conventional IMRT using fixed gantry angle beams, RapidArc using volumetric modulated arc therapy, and tomotherapy using helical therapy. Then, the variation in dose distribution caused by the target motion was analyzed by the dose measurement in the actual motion condition. A delivery quality assurance plan was prepared for the established IMRT plan and the dose distribution in the actual motion condition was measured and analyzed using a two-dimensional diode detector placed on a moving phantom capable of simulating breathing movements. The dose measurement was performed considering only a uniform target shape and motion in the superior-inferior (SI) direction. In this condition, it was confirmed that the error of the dose distribution due to the target motion is minimum in tomotherapy. This is thought to be due to the characteristic of tomotherapy that treats the target sequentially by dividing it into several slices. When the target shape is uniform and the main target motion direction is SI, it is considered that tomotherapy for the ITV-based IMRT method has a characteristic which can reduce the dose difference compared with the plan dose under the target motion condition.

제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
