급속압축팽창장치에서의 글로우 플러그 충돌분무의 연소 특성 (Combustion Characteristics of Diesel Spray Impinging on a Glow Plug in RCEM)

  • Circumstances require improving diesel engine, and many studies have been done in constant volume chamber(CVC). Because the combustion mechanism of a diesel engine has many difficulties with non-homogeneous nature, there has been a limitation to analyzing the combustion mechanism with CVC. Studies are often given in a real engine, but also it has difficulties in modifying configuration of combustion chamber etc. To get more easy way for mote engine-like test, a rapid compression mechanism has been introduced. This study addresses to designing a rapid compression expansion machine(RCEM) driven by compressed air, and to applying it on IDI diesel combustion chamber which has a glow plug. RCEM is introduced first and its characteristics are tested, then spray/combustion characteristics of diesel spray impinging on a glow plug in RCEM combustion chamber are investigated. The results show active combustion in the system employing spray impinging on a glow plug so as to improve combustion efficiency.

외과적 급속상악확장술 후 악골 및 치아의 위치 변화에 대한 연구 (A STUDY OF SKELETAL AND DENTAL CHANGES AFTER SURGICALLYASSISTED RAPID MAXILLARY EXPANSION)

  • Purpose: The aim of this study was to evaluate the skeletal and dentoalveolar dimensional changes following surgically-assisted rapid maxillary expansion (SARME). Patients & methods: Thirteen adults who had been treated by SARME for transverse maxillary deficiency from May 2000 to December 2003 were evaluated. The SARME procedure was the subtotal Le Fort I osteotomy combined with pterygomaxillary separation and anterior midpalatal osteotomy. Dental study casts and posteroanterior cephalometric radiographs were taken before operation, after removal of expansion device, and follow up period. Nasal cavity width, skeletal and dentoalveolar parameters were measured pre- and post-operatively. Results: 1. Mean nasal cavity width was increased 12%$(0{\sim}21%)$ of total expansion after retention. 2. Mean maxillary interdental width was increased 70%$(47{\sim}99%)$, 95%$(84{\sim}115%)$, and 77%$(57{\sim}94%)$ of total expansion in the canine, the first premolar, and the first molar region, respectively after retention. 3. Mean maxillary alveolar bone width was increased 66%$(42{\sim}84%)$, 74%$(42{\sim}94%)$, and 57%$(31{\sim}78%)$ of total expansion in the canine, the first premolar, and the first molar region, respectively after retention. 4. Mean palatal vault depth was decreased 1.3 mm ($0.5{\sim}2.0$ mm) after retention. 5. Mean interdental and alveolar bone width of the mandibular canine and intermolar width of mandible were slight increased as maxilla was expanded after retention. 6. There were statistical differences between preoperative and postoperative values of nasal cavity, all maxillary interdental and interalveolar widths, palatal vault depth, mandibular interdental and interalveolar width of canine(paired t-test, p<0.05). 7. The maxillary interdental and alveolar bone width were decreased approximately 25% of total expansion by relapse at follow up period. Conclusion: In conclusion, most amounts of maxillary interdental expansions were acquired with the expansion of the maxilla by SARME. For preventing the relapse, approximately 25% of the overexpansion was needed.

축소모형실험을 통한 터널 내 급속침수 차폐자동화 시스템 작동형태에 대한 연구 (An experimental study on the operation mode of rapid flooding protection system in tunnel)

  • 본 연구는 팽창구조체를 이용한 급속차폐 자동화 시스템 검증에 중점을 두었다. 팽창구조체는 해저터널의 침수사고 발생시 막대한 인적 및 물적 피해가 발생하는데 이를 대비한 자동 급속차폐시스템이다. 특히 해저터널의 시공 및 운영에 있어 필수적이라고 할 수 있다. 본 연구는 이러한 문제에 대하여 고안된 팽창구조체를 이용한 급속차폐 자동화 시스템을 실험적으로 검증하였다. 이러한 검증을 하기 위해 모형터널을 40:1 축소율을 적용한 실내모형을 제작하여 0.1 bar, 0.15 bar의 공기압을 주입하여 10 sec, 20 sec로 팽창속도에 따라 나누어 4 case로 실험을 진행하여 누수량, 수압 등을 분석하였다. 연구 결과에 의하면 팽창구조체의 팽창 시 0.1 bar 보다 0.15 bar일 때 차폐효율이 좋았으며 팽창구조체의 공기압 주입 시간에 따른 차폐효율 및 유입수 제어효율의 차이도 나타났다. 본 연구 진행 결과 팽창구조체가 완벽하게 팽창한다는 가정조건에서는 팽창구조체의 내부공기압이 높고 팽창속도가 빠를수록 더욱 효과적으로 나타났다. 본 연구 결과로 향후 추가적인 팽창구조체의 보관방법, 형상과 팽창유도체 및 차폐시 팽창구조체의 이동억제와 유입수 제어에 도움이 되는 쐐기형 구조물에 대한 추가적인 연구가 필요하다.

CLINICAL CASES OF NON-SURGICAL PALATAL EXPANSION ON ADULT PATIENTS

  • In narrow maxillary arch, midpalatal suture can be readily opened in growing children with maxillary palatal expansion. In adult patients, narrow maxilla is generally treated surgically because their growth are deemed completed due to their age. However, in patients under 25, midpalatal suture may not be closed. In addition, maxillary expansion may depend upon the closure of other maxillary sutures, which generally remain open at this stage. The present study attempted suture openings with palatal expansion on 5 female patients in their early 20's. The opening was successful in 4 patients, while only one patient showed no suture opening. In all 4 subjects, no discomfort or pain was present during s\expansion, and the successful suture opening was confirmed on occlusal x-rays. Therefore, for those patients with narrow maxilla in their early 20's, rapid palatal expansion or slow palatal expansion may offer a simple and less complicated option which, if successful, may preclude the need for surgery and thereby circumvent the psychological and financial burdens for the patients.

Long-term stability of maxillary and mandibular arch dimensions when using rapid palatal expansion and edgewise mechanotherapy in growing patients

  • Objective: The purpose of this study was to assess the long-term stability of rapid palatal expansion (RPE) followed by full fixed edgewise appliances. Methods: This study included 67 patients treated using Haas-type RPE and non-extraction edgewise appliance therapy at a single orthodontic practice. Serial dental casts were obtained at three different time points: pretreatment ($T_1$), after expansion and fixed appliance therapy ($T_2$), and at long-term recall ($T_3$). The mean duration of the $T_1-T_2$ and $T_2-T_3$ periods was $4.8{\pm}3.5years$ and $11.0{\pm}5.4years$, respectively. The dental casts were digitized, and the computed measurements were compared with untreated reference data. Results: The majority of treatment-related increases in the maxillary and mandibular arch measurements were statistically significant (p < 0.05) and greater than expected for the untreated controls. Although many measurements decreased postretention ($T_2-T_3$), the net gains persisted for all of the measurements evaluated. Conclusions: The use of RPE therapy followed by full fixed edgewise appliances is an effective method for increasing maxillary and mandibular arch width dimensions in growing patients.

급속 구개확장 후 하악골 폭경변화에 대한 콘빔씨티를 이용한 평가 (Changes in mandibular transversal arch dimensions after rapid maxillary expansion procedure assessed through cone-beam computed tomography)

  • Objective: This study aimed at evaluating the changes in mandibular arch widths and buccolingual inclinations of mandibular posterior teeth after rapid maxillary expansion (RME). Methods: Baseline and post-expansion cone-beam computed tomographic (CBCT) images of patients who initially had bilateral posterior cross-bite and underwent RME with a banded-type expander were assessed in this study. The patients included 9 boys (mean age: $13.97{\pm}1.17$ years) and 11 girls (mean age: $13.53{\pm}2.12$ years). Images obtained 6 months after retention were available for 10 of these patients. Eighteen angular and 43 linear measurements were performed for the maxilla and mandible. The measurements were performed on frontally clipped images at the following time points; before expansion (T1), after expansion (T2), and after retention (T3). Statistical significance was assessed with paired sample $t$-test at $p$ < 0.05. Results: T1-T2 comparisons showed statistically significant post-RME increases for all measurements; similarly, T2-T1 and T3-T1 comparisons showed statistically significant changes. The maxillary linear and angular measurements showed decreases after expansion, and mandibular linear and angular measurements increased after retention. Conclusion: All mandibular arch widths increased and mandibular posterior teeth were uprighted after RME procedure.

Effect and stability of miniscrew-assisted rapid palatal expansion: A systematic review and meta-analysis

  • Objective: This study aimed to systematically analyze the effect and stability of miniscrew-assisted rapid palatal expansion (MARPE) to provide a reference for the clinical treatment of patients with maxillary transverse deficiency (MTD). Methods: We searched PubMed, Science Direct, Web of Science, Embase, Cochrane Library, CNKI, and Wanfang Database for relevant studies published before February 18, 2021 and selected them according to the eligibility criteria. The Cochrane Handbook for Systematic Reviews (version 5.1.0) criteria were used for the quality assessment of randomized controlled trials, while the scoring protocol of the methodological index for non-randomized studies was used for non-randomized controlled trials. Statistical analysis was performed using the RevMan5.3 software. Results: All the included studies showed a relatively high success rate of expansion. The changes in both the intermolar and alveolar widths after MARPE were statistically significant. MARPE exhibited greater skeletal expansion effects than did conventional RPE. The midpalatal suture was opened in parallel after MARPE. A small amount of relapse was observed 1 year after expansion. MARPE caused tooth inclination and a decrease in alveolar height, but it was less significant than in conventional RPE. Conclusions: MARPE may be an effective treatment modality for patients with MTD. It causes great transverse skeletal expansion in late adolescence. In comparison to conventional RPE, MARPE has lower detrimental periodontal effects and has certain clinical advantages.

Evaluation of alveolar bone loss following rapid maxillary expansion using cone-beam computed tomography

  • Objective: To evaluate the changes in cortical bone thickness, alveolar bone height, and the incidence of dehiscence and fenestration in the surrounding alveolar bone of posterior teeth after rapid maxillary expansion (RME) treatment using cone-beam computed tomography (CBCT). Methods: The CBCT records of 20 subjects (9 boys, mean age: $13.97{\pm}1.17$ years; 11 girls, mean age: $13.53{\pm}2.12$ year) that underwent RME were selected from the archives. CBCT scans had been taken before (T1) and after (T2) the RME. Moreover, 10 of the subjects had 6-month retention (T3) records. We used the CBCT data to evaluate the buccal and palatal aspects of the canines, first and second premolars, and the first molars at 3 vertical levels. The cortical bone thickness and alveolar bone height at T1 and T2 were evaluated with the paired-samples t-test or the Wilcoxon signed-rank test. Repeated measure ANOVA or the Friedman test was used to evaluate the statistical significance at T1, T2, and T3. Statistical significance was set at p < 0.05. Results: The buccal cortical bone thickness decreased gradually from baseline to the end of the retention period. After expansion, the buccal alveolar bone height was reduced significantly; however, this change was not statistically significant after the 6-month retention period. During the course of the treatment, the incidence of dehiscence and fenestration increased and decreased, respectively. Conclusions: RME may have detrimental effects on the supporting alveolar bone, since the thickness and height of the buccal alveolar bone decreased during the retention period.

유성견의 정중구개봉합 급속확대시 투여된 불화나트륨의 초기 효과에 관한 연구 (THE EARLY EFFECTS OF SODIUM FLUORIDE ON THE RAPID PALATAL EXPANSION IN GROWING DOGS)

  • 이 연구는 성장중인 유성견의 정중구개봉합 급속확대후 봉합부 골조직의 치유 및 골개조과정에 미치는 불화나트륨의 효과를 관찰하기 위하여 시행되었다. 생후 6개월된 유성견 18마리를 이용하여 10일간($180^{\circ}$ turn/day), 5mm의 정중구개봉합 급속확대를 시행하였고, 한군당6마리씩 배정하여 각각0, 15, 45일간 보정하였다. 각 6마리중3 마리는 실험군으로, 확대 시작일로부터 희생 직전까지 불화나트륨(1mg sodium fluoride(NaF)/Kg of body weight/day)을 경구 투여하였다. 불화나트륨을 투여하지 않은 나머지 3마리는 대조군으로 사용하였다. 혈청내 fluoride, calcium, phosphate 그리고 alkaline phosphatase 농도 변화에 대한 biochemical analysis를 시행하였고, 희생후 적출한 상악골은 비탈회 조직편을 제작한 후, $10{\mu}m$의 두께로 coronal section하여 Goldner's modified Masson trichrome법으로 염색하고 광학 현미경하에서 검경하였다. 확대 직후, 실험군과 대조군 토두에서 벌어진 봉합 간극은 염증 세포가 침윤된 fibrous connective tissue로 채워져 있었다. 신생골의 형성이 대조군에 비하여 실험군에서 현저하였으며 봉합부 양측 골단은 활성화된 조골세포와 신생골양조직으로 피개되어 있었다. 15일 보정군의 경우, 실험군에서는 계속적으로 봉합부 양 골단에서 골양조직과 활성화된 조골세포로 피복된 신생골 형성이 활발하였다. 그러나 대조군의 경우 신생골의 형성이 일부 관찰되었으나 실험군에 비하여 매우 저조하였다. 45일 보정군에서,대조군의 경우는 활성화된 조골세포를 거의 찾을 수 없었고, 봉합부 주위에서 다수의 파골세포가 관찰된 반면, 실험군에서는 계속적으로 조골세포의 활성이 유지되었고 골양조직의 형성도 활발하였다. 혈청내 alkaline phosphatase농도는 대조군의 경우 시간 경과에 따라 급속하게 저하된 반면, 실험군에서는 45일 보정군에서까지 계속적으로 확대 후에 높은 농도를 유지하였다. 이상의 결과에서, 불화나트륨은 정중구개봉합 급속확대 후 치유 과정에 있어서 조골세포의 활성과 골양조직의 형성을 보다 지속적으로 촉진시킴으로써 봉합부 골조직의 치유 및 재생과정에 유효한 효과가 있는 것으로 사료되었다.

유성견 급속정중이개시 정중구개봉합부위의 조직학적 변화에 대한 연구 (A LIGHT AND ELECTRON MICROSCOPIC STUDY OF CHANCES IN INTERMAXILLARY SUTURE DURING THE RAPID MAXILLARY EXPANSION OF YOUNG ADULT DOGS)

  • 정중구개봉합의 급속확장은 상악골의 악궁의 폭을 증가시키고, 전방이동을 용이하게하므로 전후방적인 악골의 부조화나 상악치열궁 협착 등을 치료하는데 널리 사용되고 있다. 이 연구는 정중구개봉합의 급속이개시 봉합부위에서 나타나는 골개조 과정 및 교원섬유의 변화를 관찰하고자 시행되었다. 생후 6-8개월된 유성견 4마리를 실험동물로 사용하여 인상을 채득한 후 주조된 급속정중이개장치에 Hyrax screw를 납착하여 장착시키고 1회에 $90^{\circ}$씩, 1일 2회전시켜 10일간 지속하였다. X-ray상에서 봉합의 이개를 확인한 후 이개직후, 15일, 30일 그리고 60일간의 보정기간을 거쳐 희생시키고 정중구개봉합부를 절취하여 봉합부위의 변화를 광학 및 주사전자현미경으로 관찰하여 다음과 같은 결과를 얻었다. 1. 정중구개봉합 확장 직후 양 봉합골단의 일부에서는 신생골 형성이 관찰되었으며, 양 골단 사이의 교원 섬유속이 중앙에서 늘어나 골간거리가 확장되었다. 2. 정중구개봉합 확장 15일 후, 봉합면에서는 많은 파골세포를 가진 골흡수면이 증가되었고 양 골단 사이에 서는 교원섬유속의 주행방향이 불규칙하고, 조섬유세포 수가 줄어든 반면 많은 염증세포가 침윤되어 있었다. 3. 정중구개봉합 확장 30일 후 양 봉합골단에서 많은 조골세포를 가진 신생골이 성장하여 해면골이 형성되었으며, 양골단 사이에서는 교원섬유속이 풍부해지고 조섬유세포 수가 증가되었다. 4. 정중구개봉합 확장 60일 후, 양봉합골단은 해면골이 치밀골로 대치되어 골간거리가 좁아졌지만 계속적인 골개조현상이 관찰되었고, 교원섬유속이 재생된 봉합면에 매입되었으며, 연결 중앙부에서는 서로 교차되고 있는 양상이 관찰되었다. 이상의 결과에 의하면 급속정중이개시 정중구개봉합 부위의 유주성 세포의 침윤상태가 유도되는 염증반응이 일어난 후 골개조현상이 일어났으며, 골개조 현상은 급속확장 2 개월 후에도 계속 진행되고 있었다.
