Salvage rapid maxillary expansion for the relapse of maxillary transverse expansion after Le Fort I with parasagittal osteotomy

  • Maxillary transverse deficiency is one of the most common deformities among occlusal discrepancies. Typical surgical methods are segmental Le Fort I osteotomy and surgically-assisted rapid maxillary expansion (SARME). This patient underwent a parasagittal split with a Le Fort I osteotomy to correct transverse maxillary deficiency. During follow-up, early transverse relapse occurred and rapid maxillary expansion (RME) application with removal of the fixative plate on the constricted side was able to regain the dimension again. RME application may be appropriate salvage therapy for such a case.

  • Orthopedic rapid maxillary expansion(RME) has been a common treatment modality used to widen narrow maxillae in young children. However, since more skeletally matured adolescents or adults has closed midpalatal suture, the result of RME was undesirable because of dental tipping with little or no basal skeletal movement and resulted to many other complications. After such treatment, complications often occurred such as alveolar bending, compression of periodontal ligament, extrusion, buccal tipping, and severe relapse. Thus, surgically assisted rapid maxillary expansion(SA-RME) is required, especially for patients over 14 years old, to skeletally release maxillary expansion. We used two methods of maxillary expansion surgery. Surgically assisted rapid maxillary expansion(SA-RME) & surgically assisted posterior segmental expansion(SA-PSE) were used for narrow maxilla. The study was divided into two groups(SA-RME group and SA-PSE group). SA-RME group was consisted of 2 males and 4 females, and the ages of materials ranged from 15 years to 25 years with a mean of 20.2 years. SA-PSE group was consisted of 1 male and 5 females, and the ages ranged from 13 years to 23 years with a mean of 18.7 years. Dental study models were fabricated before starting the expansion and immediately after the expansion was completed. It was fabricated again 1 month later, 3 months later when the expansion device was removed, and 6 months later after the expansion was completed. A repeated measures analysis of variance(ANOVA) test was applied to assess changes between each groups over time. The amount of expansion and the amount of tipping movement each in both groups were compared by using paired t-test and it was also compared between each subjects within the group by using independent t-test. Both SA-RME and SA-PSE group showed stable results, but SA-PSE group showed statical significance in tipping movement of second premolar. We compared 6 patients who recieved SA-RME with 6 patients who received SA-PSE, and appraised the clinical usefulness.

Rapid full-scale expansion joint monitoring using wireless hybrid sensor

  • Condition assessment and monitoring of bridges is critical for safe passenger travel, public transportation, and efficient freight. In monitoring, displacement measurement capability is important to keep track of performance of bridge, in part or as whole. One of the most important parts of a bridge is the expansion joint, which accommodates continuous cyclic thermal expansion of the whole bridge. Though expansion joint is critical for bridge performance, its inspection and monitoring has not been considered significantly because the monitoring requires long-term data using cost intensive equipment. Recently, a wireless smart sensor network (WSSN) has drawn significant attention for transportation infrastructure monitoring because of its merits in low cost, easy installation, and versatile on-board computation capability. In this paper, a rapid wireless displacement monitoring system, wireless hybrid sensor (WHS), has been developed to monitor displacement of expansion joints of bridges. The WHS has been calibrated for both static and dynamic displacement measurement in laboratory environment, and deployed on an in-service highway bridge to demonstrate rapid expansion joint monitoring. The test-bed is a continuous steel girder bridge, the Founders Bridge, in East Hartford, Connecticut. Using the WHS system, the static and dynamic displacement of the expansion joint has been measured. The short-term displacement trend in terms of temperature is calculated. With the WHS system, approximately 6% of the time has been spent for installation, and 94% of time for the measurement showing strong potential of the developed system for rapid displacement monitoring.

Comparison of the effects of rapid maxillary expansion and alternate rapid maxillary expansion and constriction protocols followed by facemask therapy

  • Objective: The aim of this retrospective study was to evaluate and compare the changes in the pharyngeal airway (PA), maxillary sinus volume, and skeletal parameters after rapid maxillary expansion (RME) and alternate rapid maxillary expansion and constriction (Alt-RAMEC) followed by facemask (FM) therapy. Methods: The records of 40 patients with skeletal Class III malocclusion due to maxillary retrognathism were collected, and the patients were assigned into two groups. The first group comprised 8 male and 12 female patients (mean age, $10.0{\pm}1.1years$) treated using RME/FM for an average of 10 months. The second group comprised 10 male and 10 female patients (mean age, $9.64{\pm}1.3years$) treated using Alt-RAMEC/FM for an average of 12 months. Cone-beam computed tomography images acquired before (T0) and after treatment (T1) were evaluated. Results: Regarding the skeletal effects, significant differences between the groups were the increase in ANS-HRP (perpendicular distance of ANS to the horizontal reference plane, 0.99 mm, p <0.05) in the Alt-RAMEC/FM group and the decrease in PP-SN (palatal plane to Sella-Nasion plane, $0.93^{\circ}$, p < 0.05) in the RME/FM group. Maxillary sinus volumes increased significantly in both the groups, and the increase was statistically significantly higher in the Alt-RAMEC/FM group. Although no significant intergroup differences were observed in PA volumes, both lower ($1,011.19mm^3$) and total ($1,601.21mm^3$), PA volume increased significantly in the Alt-RAMEC/FM group. Conclusions: The different expansion devices and protocols used with FM therapy do not seem to affect the forward movement of the maxilla and PA volumes. In contrast, the increase in maxillary sinus volume was greater in the Alt-RAMEC/FM protocol.

  • It was said that early rapid palatal expansion was the treatment of choice in case of maxillary constriction with posterior crossbite. The author tried to assess dimensional changes of nasal and oral cavity before rapid palatal expansion and over 4 months of retention by use of three coronal tomograms through the incisal, molar, and maxillary tuberosity areas. The subjects of this study were consisted of 9 boys and 11 girls ranging from 11 to 13 years old. The results were as follows: 1. A mean change in maxillary first molar width was 3.68mm and a mean change in nasal cavity width was 2.26mm after 6.70mm expansion by Hyrax-type screw and over 4 months of retention tomographically in molar cut. 2. A change in nasal cavity width tomographically showed in order of molar cut, incisal cut, maxillary tuberosity cut respectively. 3. There was no correlation between changes in oral cavity width and nasal cavity width.

Does mini-implant-supported rapid maxillary expansion cause less root resorption than traditional approaches? A micro-computed tomography study

  • Objective: This study aimed to evaluate the volume, amount, and localization of root resorption in the maxillary first premolars using micro-computed tomography (micro-CT) after expansion with four different rapid maxillary expansion (RME) appliances. Methods: In total, 20 patients who required RME and extraction of the maxillary first premolars were recruited for this study. The patients were divided into four groups according to the appliance used: mini-implant-supported hybrid RME appliance, hyrax RME appliance, acrylic-bonded RME appliance, and full-coverage RME appliance. The same activation protocol (one activation daily) was implemented in all groups. For each group, the left and right maxillary first premolars were scanned using micro-CT, and each root were divided into six regions. Resorption craters in the six regions were analyzed using special CTAn software for direct volumetric measurements. Data were statistically analyzed using Kruskal-Wallis one-way analysis of variance and Mann-Whitney U test with Bonferroni adjustment. Results: The hybrid expansion appliance resulted in the lowest volume of root resorption and the smallest number of craters (p < 0.001). In terms of overall root resorption, no significant difference was found among the other groups (p > 0.05). Resorption was greater on the buccal surface than on the lingual surface in all groups except the hybrid appliance group (p < 0.05). Conclusions: The findings of this study suggest that all expansion appliances cause root resorption, with resorption craters generally concentrated on the buccal surface. However, the mini-implant-supported hybrid RME appliance causes lesser root resorption than do other conventional appliances.

Comparison of changes in the nasal cavity, pharyngeal airway, and maxillary sinus volumes after expansion and maxillary protraction with two protocols: Rapid palatal expansion versus alternate rapid maxillary expansion and constriction

  • Objective: To evaluate and compare a series of volume changes in the nasal cavity (NC), nasopharynx, oropharynx, and maxillary sinuses (MS) in growing Class III patients after either rapid palatal expansion (RPE) or alternate rapid maxillary expansion and constriction (Alt-RAMEC) followed by facemask (FM) therapy, by using cone-beam computed tomography (CBCT). Methods: Forty growing Class III patients were retrospectively selected and divided into two matched groups: RPE/FM (14 females, 6 males; mean age, 9.66 ± 1.23 years) and Alt-RAMEC/FM groups (14 females, 6 males; mean age, 10.28 ± 1.45 years). The anteroposterior and vertical displacements of Point A, the volumes of the NC, nasopharyngeal, oropharyngeal, and MS were measured at different time points: pretreatment (T1), postexpansion (T2), and postprotraction (T3). Results: Both groups demonstrated significant maxilla advancement (by 1.3 mm) during expansion, with a statistically significant intergroup difference during protraction (RPE/FM, 1.1 mm; Alt-RAMEC/FM, 2.4 mm; p < 0.05) and throughout the treatment (RPE/FM, 2.4 mm; Alt-RAMEC/FM, 3.7 mm; p < 0.05). NC and nasopharyngeal airway volumes increased significantly in both groups after expansion, protraction, and treatment. The oropharyngeal and MS volumes increased in both groups after protraction and post-treatment. However, no volumetric differences were observed between the two groups. Conclusions: There was no significant difference in airway volume changes, including NC, nasopharyngeal, oropharyngeal airway, and MS, between RPE/FM and Alt-RAMEC/FM groups at different time points. Although there was significantly more forward movement after protraction in the Alt-RAMEC/FM group, the difference was deemed too small to be clinically relevant.

Skeletal and dentoalveolar effects of different types of microimplant-assisted rapid palatal expansion

  • Objective: To evaluate the following null hypothesis: the skeletal and dentoalveolar expansion patterns in the coronal and axial planes are not different with two different types of microimplant-assisted rapid palatal expansion (MARPE) systems. Methods: Pretreatment (T0) and post-MARPE (T1) cone-beam computed tomography (CBCT) images of 32 patients (14 males and 18 females; mean age, 19.37) were analyzed. We compared two different MARPE systems. One MARPE system included the maxillary first premolars, maxillary first molars, and four microimplants as anchors (U46 type, n = 16), while the other included only the maxillary first molars and microimplants as anchors (U6 type, n = 16). Results: In the molar region of the U6 and U46 groups, the transverse expansion at the midnasal, basal, alveolar, and dental levels was 2.64, 3.52, 4.46, and 6.32 mm and 2.17, 2.56, 2.73, and 5.71 mm, respectively. A significant difference was observed in the posterior alveolar-level expansion (p = 0.036) and posterior basal-bone-level expansion (p = 0.043) between the groups, with greater posterior skeletal and alveolar expansion in the U6 group. Conclusions: Compared with the U46 group, the U6 group showed greater posterior expansion at the alveolar and basal-bone levels, with an almost parallel split. Both groups showed a pyramidal expansion pattern in the coronal view.

  • Recently, rapid palatal expansion technique is widely used for the correction of the skeletal imbalance in Cl III malocclusion patients. There were many studies about the cephalometric changes to rapid palatal expansion but quantitative analysis were small. The purpose of this study was to analysis the stresses and displacement of the maxilla in human dry skull to rapid palatal expansion. The results were as follows: 1. The anterior portion of palate show more lateral and inferior displacement than the posterior portion. But the posterior portion show more anterior displacement. 2. In transpalatal suture area, the medial portion show more anterior and inferior displacement than the lateral portion. But the lateral portion show more lateral displacement than the medial portion. 3. In mid-sagittal plane, the lower portion (palatal area) of maxilla show more anterior, lateral, inferior displacement than the upper portion (frontamaxillary stuture area). 4. In zygomatic arch, the adjacent area to maxilla show tonsil. stresses and the adjacent area to frontal bone show compressive stresses. 5. The sequence of stress bearing area to R.P.E. is upper retromolar area, upper 1st molar, 1st premolar, 2nd premolar, anterior segment of teeth.

급속 구개확장을 이용한 상악 견치의 이소맹출 치료: 증례보고 (RAPID PALATAL EXPANSION FOR THE TREATMENT OF AN ECTOPICALLY ERUPTING MAXILLARY CANINE: CASE REPORTS)

  • 상악 견치 매복은 소아 청소년기 환자들에게서 흔하게 발견되는 문제이다. 일반적으로 구개측 매복이 순측 매복 보다 더 흔한 것으로 알려져 있지만 서양에서는 오히려 순측 매복이 더 많은 것으로 보고되고 있다. 본 보고에서는 파노라마 방사선 사진상 상악 견치의 순측매복이 의심되는 8-10세의 혼합치열기 아동을 대상으로 급속 구개확장(rapid palatal expansion)을 시행하였으며, 이를 통하여 상악 측절치의 근원심 치근각도를 조절하여 매복 가능성이 있는 견치의 정상맹출을 유도할 수 있었기에 보고하는 바이다.
