상대 순위가 진로 결정에 미치는 영향 (Impact of Ordinal Rank on Career Choice)

  • 본 연구는 동료(peer)에 대비한 학생의 상대적인 학업성취가 진로 결정에 어떠한 영향을 주는지를 살펴본다. 구체적으로 학교 내에서 학생들의 상대적인 수학 순위가 이과 선택에 미치는 영향을 분석하였다. 분석을 위해 서울시 소재 학생들의 패널데이터를 이용하였으며, 수학 성적을 통제하더라도 상대 순위가 낮은 경우 이과에 진학할 확률이 11%포인트 이상 감소함을 확인하였다. 높은 상대 순위는 여학생에게 추가적인 양의 이과 진학 효과를 나타내 상대 순위가 성별로 다르게 작용함을 보였다. 분석 결과에 따르면 상대 순위효과 중 약 12%만이 자신감과 같은 주관적 평가에 의해 설명되며, 이로 미루어보아 내신 성적을 통한 경로가 더 지배적일 것으로 추측할 수 있다.

랭킹 SVM과 RDBMS의 밀결합 통합 (Tightly Coupled Integration of Ranking SVM and RDBMS)

  • 지난 십 년간 랭킹은 데이터 마이닝 분야의 활발한 연구분야였다. 그러나 랭킹은 다른 데이터 마이닝 기법들과 비슷하게 RDBMS와는 독립적으로 개발되었고, 그로 인해 기존에 널리 사용되고 있는 RDBMS들과의 연동성이 떨어진다는 단점이 존재하게 되었다. 다른 데이터 마이닝 기법들은 소결합이나 밀결합 접근법을 이용하여 RDBMS와 연동하기 위한 연구가 활발하게 진행되어 왔고, 그 결과 실제로 사용 가능한 응용시스템들이 나오게 되었다. 그러나 랭킹에서는 이와 같은 노력들이 잘 이루어지지 않고 있다. 본 논문에서는 랭킹 작업을 RDBMS에 연동하여 효율적으로 수행하기 위하여 MySQL에 Ranking SVM을 통합하는 작업을 진행하였다. 밀결합 접근법을 기반으로 하는 우리의 구현은 MySQL에 랭킹을 위한 새로운 SQL 명령어를 추가하였고 랭킹 작업의 효율성을 확인하기 위해서 소결합 접근법을 기반으로 하는 Ranking SVM과 성능을 비교 평가하여 훈련단계에서 $10{\sim}40%$, 예측단계에서 평균 60%의 성능향상을 확인할 수 있었다.

  • Backgraounds : Recent studies provide the evidences that the efficacy of acupuncture may be no better than placebo or inconclusive. These results are very different from those of the actual clinical situations in many acupuncture medical institutions. Objectives : The present study was designed to evaluate the influencing factors which affect the efficacy of acupuncture scale(FEAS) as the methodological assessment tool of acupuncture for examining acupuncture interventions and to demonstrate the importance of it in randomized controlled trials of acupuncture. Data sources : Electronic data were retrieved from NDSL, Pubmed, sciencedirect, LWW, OVID, Black-Well Synergy, Wiley Interscience, EBSCO HOST, springer, PML, and Kluwer. No electronic data were collected from MEDLIS and MEDLAS. Study selection : The inclusion criteria were five systematic reviews included in Alberta study and all randomized controlled trials obtained from their references. Study analysis : The acupuncture rationale, methods of stimulation, treatment regimen, and the practitioner's background were rated by FEAS, and the scores were compared with those by other methodological assessment tools. Results : The number of positive conclusions of high-rank RCTs by FEAS was the same as or higher than that of high-rank RCTs by other methodological assessment tools. Conclusions : We have analysed 5 systematic reviews and their objectives 58 RCTs using FEAS. Practitioner's background has been described slightly in some reviews and studies. It may directly influence the effectiveness of acupuncture negatively in the systematic reviews.

기계적 견인이 슬관절 골관절염 환자의 통증과 신체 기능에 미치는 효과 (The Effect of Mechanical Traction on Pain and Physical Function in Patients With Knee Osteoarthritis)

  • The purpose of this study was to identify the effects of mechanical traction applied to the knee joint on pain, knee range of motion (ROM), timed up and go (TUG) and Western Ontario and MacMaster Universities Osteoarthritis (WOMAC) in patients with knee osteoarthritis (KOA) of Kellgren-Lawrence radiological rating scale II or III. Twenty three patients participated in the experiment for a period of four weeks. After baseline assessment, the patients with KOA were randomized into two groups: the traction group ($n_1=12$), which received traction with general physical therapy; and the control group ($n_2=11$), which received general physical therapy only on unilateral knee joints. Patients received interventions once a day, three times a week, for four weeks. Wilcoxon signed-rank test was used to analyze the change of dependent variances within the group during pre and post intervention. Mann-Whitney U test was used to analyze the change of dependent variances as TUG and passive ROM between the two groups. Analysis of covariance was used to analyze the change of dependent variances as numeric pain rating scale (NPRS) and WOMAC score between the two groups. In Wilcoxon signed-rank test, the traction group improved significantly with regard to NPRS (p<.01), passive knee flexion ROM (p<.01), passive knee extension (p<.05), TUG (p<.01) and WOMAC scores (p<.01) after intervention for four weeks, but not for the control group. In the Mann-Whitney U test and analysis of covariance, no significant difference was seen among all the dependent variances after intervention for four weeks between the two groups. These outcomes suggest that further studies should be carried out to determine the effects of mechanical traction prior to using it for the treatment of patients with knee osteoarthritis.

수간호사가 활용하는 근무성적 평정에 관한 조사연구 (A Survey on Head Nurses' Perceptions and Performances on Staff Evaluation)

  • This study was designed and carried out to identify the methodologies of the staff evaluation used by the headnurses and their perception on the purpose of the evaluations, and to measure the degree and type of perceived purpose and the degree of implementation of staff evaluation principles into preactice. A hundred and eleven from four general hospitals with 600 beds and over were conveniently sampled. Data were gathered from August 20, through September 10, 1994 ; demographic data of respondents and 'methodologies used for staff evaluation' by questionaires, level of 'perceived purposes of staff evaluation' and level of implementation of the 'principles of staff evaluation into practice' by Likert-type rating scales developed by the researcher. Raw and percentile scores and means were calculated and data were processed further by t-tests and ANOVA. Results are as follows ; 1. Traditional methodlogies : checklists(90.1%) and ranking methods(45.9%) revealed to be the two mostly prefered and used methods. 2. Of the 20 item-descriptions of the 'purpose of staff evaluation': 10 'control, evaluative' and 10 'non-control, developmental' the first(mean=3.19) to the ninth(mean=2.74) in rank order revealed to be the 'control, evaluative purpose', while the tenth in rank order revealed to be the first among the 'non-control, evaluative' item(mean-1.57). 3. Level of purpose of staff evaluation perceived as 'control ; evaluative'(mean=43.76, S.D.=5.01) revealed to be significantly(t=-3.86, p<.05) highter than 'non-control ; developmental'(mean=40.65, S.D.=3.41). 4. Level of implementation of 'staff evaluation principle into practice' revealed to be moderately high(mean=2.40).

Dietary patterns based on carbohydrate nutrition are associated with the risk for diabetes and dyslipidemia

  • Several studies have been conducted on dietary patterns based on carbohydrate nutrition in Asian populations. We examined the cross-sectional associations in dietary patterns based on carbohydrate nutrition, including the glycemic index (GI) with dyslipidemia and diabetes among the Korean adult population. We analyzed 9,725 subjects (3,795 men and 5,930 women, ${\geq}$ 20 years) from the Fourth Korea National Health and Nutrition Examination Survey. Dietary information was collected using single 24-hour recall. Reduced rank regression was used to derive dietary patterns from 22 food groups as predictor variables and four dietary factors related to the quantity and quality of carbohydrates as response variables. Two dietary patterns were identified: 1) the balanced pattern was characterized by high intake of various kinds of foods including white rice, and 2) the rice-oriented pattern was characterized by a high intake of white rice but low intake of vegetables, fruits, meat, and dairy products. Both patterns had considerable amounts of total carbohydrate, but GI values differed. The rice-oriented pattern was positively associated with hypertriglyceridemia in men and low high density lipoprotein-cholesterol in both men and women. The balanced pattern had no overall significant association with the prevalence of dyslipidemia or diabetes, however, men with energy intake above the median showed a reduced prevalence of diabetes across quintiles of balanced pattern scores. The results show that dietary patterns based on carbohydrate nutrition are associated with prevalence of dyslipidemia and diabetes in the Korean adult population.

The Attributable Risk of Smoking on All-Cause Mortality in Korean: A Study Using KNHANES IV-VI (2007-2015) with Mortality Data

  • Background: It is not evident that the attributable risk of smoking on mortality in Korea has decreased. We investigated the impact of smoking on all-cause mortality and estimated the attributable risk of smoking in Korean adults. Methods: Those aged ≥20 years with smoking history in the Korean National Health and Nutrition Examination Surveys (KNHANES) 2007-2015 were enrolled. We categorized the participants into three groups as follows: never smoker, <20 pack-years (PY) smokers, and ≥20 PY smokers. We applied inverse probability weighting using propensity scores to control various confounders between the groups. All-cause mortality risks were compared between the groups using the Kaplan-Meier log-rank test. The effects of smoking-attributable risks (ARs) on mortality were also calculated. Results: A total of 50,458 participants were included. Among them, 19,334 (38.3%) were smokers and 31,124 (61.7%) were never smokers. Those with a smoking history of 20 PY or more (≥20 PY smokers), those with a smoking history of less than 20 PY (<20 PY smokers), and never smokers were 18.1%, 20.2%, and 61.7%, respectively, of the study population. Smokers had a higher risk of all-cause mortality compared to never smokers (log-rank test p<0.01). The ARs of smoking were 21.8% (95% confidence interval [CI], 5.7%-37.9%) and 9.0% (95% CI, 6.1%-12.0%) in males and females, respectively. ARs decreased from 24.2% to 19.5% in males and from 9.5% to 4.1% in females between 2007-2010 and 2011-2015. Conclusion: Our study using KNHANES IV-VI data demonstrated that smoking increased the risk of all-cause mortality in a dose-response manner and the ARs of smoking on mortality were 21.8% in males and 9.0% in females during 2007-2015. This suggests that the ARs of smoking on mortality have decreased since around 2010.

건강증진 프로그램이 고지혈증 근로자의 자기효능감, 건강한 생활양식 및 혈중지질에 미치는 효과 (Effect of Health Promotion Program on Self-efficacy, Healthy Lifestyle and Serum Lipid Level in Employees with Hyperlipidemia)

  • Purpose: This quasi-experimental study was performed to investigate the effects of a health promotion program(HPP) on self-efficacy, healthy lifestyle and blood lipid profile in employees with hyperlipidemia. Methods: The subjects were forty-three employees who were diagnosed as having hyperlipidemia during routine health examination at two worksites in Seoul in 2001. Of the forty-three subjects, thirty were allocated to the experimental group and the remaining thirteen were allocated to the control group. Data were collected from September 24, 2001 to March 16, 2002. The HPP was applied to the experimental group for 11 weeks and included health education, diet counseling, watching videos, conference, and so on. The health education included information about exercise, smoking cessation, and abstinence from drinking alcohol and seven sessions of hyperlipidemia education. The experimental group was requested to keep a daily health promotion lifestyle diary. This diary was analyzed by a dietician and subjects were instructed based on the results. As for data analyses, wilcoxon signed rank test and wilcoxon rank sum test and x2-test were carried out using SAS program. Results: 1. Self-efficacy scores of the experimental group were significantly more increased than those of the control group (experimental: 5.86 10.80, control: -4.04 11.91, p=0.018). 2. Healthy lifestyle scores of the experimental group were significantly more increased than those of the control group (experimental: 0.19 0.26, control: -0.05 0.29, p=0.024). 3. Blood total cholesterol values of the experimental group were significantly more decreased than those of the control group (experimental: -13.07 30.10mg/dl, control: 10.00 26.57mg/dl, p=0.033). 4. Blood triglyceride values of the experimental group were significantly more decreased than those of the control group (experimental: -29.17 192.40mg/dl, control: 63.31 107.53mg/dl, p=0.050). Conclusion: These findings indicate that the HHP could be effective in improving self-efficacy, healthy lifestyle and blood HDL cholesterol and decreasing blood total cholesterol in employees with hyperlipidemia. Therefore, the HHP could be suggested as an effective nursing intervention for employees in the worksite by ultimately preventing cerebral and cardiac vessel complications related to hyperlipidemia.

시간열 마이크로어레이 데이터를 이용한 질병 관련 유의한 패스웨이 유전자 집합의 검출 (A Method of Identifying Disease-related Significant Pathways Using Time-Series Microarray Data)

  • 최근 특정 질병의 진단이나 예후 예측을 위해 마이크로어레이 실험 데이터를 이용한 질병 관련 바이오마커 검출 연구가 활발히 진행되고 있다. 특히 정상인에 비해 질병 환자군에서 특이하게 발현되는 개별 유전자를 바이오 마커로 이용하는 기존의 방식과는 달리 동일한 생물학적 패스웨이에 관여하는 유전자 집합의 변화를 분석하여 특이하게 발현되는 패스웨이 유전자 집합을 바이오 마커로 사용하는 유전자 집합 분석(Gene-set analysis) 연구가 주목받고 있다. 본 논문에서는 다양한 실험 조건 요인을 가지는 시간열 마이크로어레이 실험 데이터를 이용한 유의한 패스웨이 유전자 집합을 검출하는 방법에 대해 제안한다. 시간열 마이크로어레이 데이터을 이용하여 유전자 집합 분석을 수행하기 위해서는 시간에 따른 유전자 발현값의 변화에 따라 개별 유전자의 유의성을 나타내는 스코어를 maSigPro (microarray Significant Profiles)를 이용하여 계산한 후, 이를 기반으로 전체 유전자의 순위를 결정하여 후보 유전자 집합에 대한 유의성 검증을 윌콕슨 순위합 검증을 통해 수행한다. 후보 유전자 집합의 생성을 위해서는 MSigDB (Molecular Signatures Database)의 패스웨이 정보를 이용하였으며, 본 논문에서 제안한 방법의 검증을 위해 공개된 전립선 암 관련 시간열 마이크로어레이 실험 데이터에 적용한 결과 실제로 전립선암과 관련된 것으로 밝혀진 7개의 패스웨이 중 6개의 패스웨이를 정확하게 검출할 수 있었다.

Comparison of the Quality of Various Polychromatic and Monochromatic Dual-Energy CT Images with or without a Metal Artifact Reduction Algorithm to Evaluate Total Knee Arthroplasty

  • Objective: To compare the quality of various polychromatic and monochromatic images with or without using an iterative metal artifact reduction algorithm (iMAR) obtained from a dual-energy computed tomography (CT) to evaluate total knee arthroplasty. Materials and Methods: We included 58 patients (28 male and 30 female; mean age [range], 71.4 [61-83] years) who underwent 74 knee examinations after total knee arthroplasty using dual-energy CT. CT image sets consisted of polychromatic image sets that linearly blended 80 kVp and tin-filtered 140 kVp using weighting factors of 0.4, 0, and -0.3, and monochromatic images at 130, 150, 170, and 190 keV. These image sets were obtained with and without applying iMAR, creating a total of 14 image sets. Two readers qualitatively ranked the image quality (1 [lowest quality] through 14 [highest quality]). Volumes of high- and low-density artifacts and contrast-to-noise ratios (CNRs) between the bone and fat tissue were quantitatively measured in a subset of 25 knees unaffected by metal artifacts. Results: iMAR-applied, polychromatic images using weighting factors of -0.3 and 0.0 (P-0.3i and P0.0i, respectively) showed the highest image-quality rank scores (median of 14 for both by one reader and 13 and 14, respectively, by the other reader; p < 0.001). All iMAR-applied image series showed higher rank scores than the iMAR-unapplied ones. The smallest volumes of low-density artifacts were found in P-0.3i, P0.0i, and iMAR-applied monochromatic images at 130 keV. The smallest volumes of high-density artifacts were noted in P-0.3i. The CNRs were best in polychromatic images using a weighting factor of 0.4 with or without iMAR application, followed by polychromatic images using a weighting factor of 0.0 with or without iMAR application. Conclusion: Polychromatic images combined with iMAR application, P-0.3i and P0.0i, provided better image qualities and substantial metal artifact reduction compared with other image sets.
