Bayesian Survival Analysis of High-Dimensional Microarray Data for Mantle Cell Lymphoma Patients

  • Background: Survival time of lymphoma patients can be estimated with the help of microarray technology. In this study, with the use of iterative Bayesian Model Averaging (BMA) method, survival time of Mantle Cell Lymphoma patients (MCL) was estimated and in reference to the findings, patients were divided into two high-risk and low-risk groups. Materials and Methods: In this study, gene expression data of MCL patients were used in order to select a subset of genes for survival analysis with microarray data, using the iterative BMA method. To evaluate the performance of the method, patients were divided into high-risk and low-risk based on their scores. Performance prediction was investigated using the log-rank test. The bioconductor package "iterativeBMAsurv" was applied with R statistical software for classification and survival analysis. Results: In this study, 25 genes associated with survival for MCL patients were identified across 132 selected models. The maximum likelihood estimate coefficients of the selected genes and the posterior probabilities of the selected models were obtained from training data. Using this method, patients could be separated into high-risk and low-risk groups with high significance (p<0.001). Conclusions: The iterative BMA algorithm has high precision and ability for survival analysis. This method is capable of identifying a few predictive variables associated with survival, among many variables in a set of microarray data. Therefore, it can be used as a low-cost diagnostic tool in clinical research.

모-영아 상호작용 증진을 위한 간호중재가 수유시 모-영아 상호작용에 미치는 효과 (Effects of the Nursing Intervention on Mother-Infant Interaction)

  • Mother and infant relationship has a great influence on child's developments. In this study, nursing intervention to increase maternal sensitivity to the infant's cues was applied to 25 primiparas (Experimental Group). Mother and infant interacations of these primiparas were compared with those of 25 primiparas (Control Group) who did not receive the nursing intervention. Fifty primiparas and infants were recruited from a university hospital, a general hospital, and an OBGY clinic located in Taegu city. Mother and infant interactions were assessed at 6 weeks after birth using videotapes. Feeding situations were videotaped and two trained observers analyzed the tapes. Data were collected from March 23rd to July 27th of 1998. Mother and infant interactions during feeding were assessed by the response rating scale which was modified by the author based on NCAST feeding scale (Barnard, 1978a) and AMIS scale (Price, 1983). The validity of the modified rating scale was verified by faculty members and researchers who previously had research experience in the area. Cronbach's Alpha of the modified scale for this study was .90. The data was analyzed by SAS program, using wilcoxon rank sums test, chi square test, Fisher's exact test, and ANOVA. Findings were as follows: 1. Mothers in the experimental group were more likely to have higher scores in mother and infant interactions during feeding than mothers in the control group. 2. Mothers in the experimental group showed better sensitivity to infant's signals or cues, provided growth fostering, and had higher responsibility to the infant's distress than mothers in the control group. 3. Infants in the experimental group showed higher clarity of cues and responsibility to the mother's behaviors than infants in the control group. 4. Mothers and infants in the experimental group showed higher synchronic responses than mothers and infants in the control group. In conclusion, this study has shown that the applied nursing intervention promoted mother and infant interaction among primiparas. Therefore, this study suggests that the nursing interventions to increase maternal sensitivity to the infant's cues should be broadly applied to primiparas, which can be beneficial to the social, affective, and cognitive developments of their children.

탄성밴드를 이용한 저항운동이 뇌졸중 환자의 근력, 근긴장도, 균형 및 보행에 미치는 영향: 예비 (The Effect of Resistance Exercise Using Elastic Band on Muscle Strength, Muscle Tone, Balance and Gait in Stroke Patients : A Pilot Study)

  • Purpose : This study aimed to determine whether resistance exercise using elastic bands for 6 weeks could improve the muscle strength, muscle tone, balance and gait among stroke patients. Methods : A total of 11 stroke patients who had agreed to participate in the study were randomly divided into 3 groups receiving elastic band resistance training on their less affected side (experimental group 1, n=4), affected side (experimental group 2, n=3), and both the sides (experimental group 3, n=4). Muscle strength, muscle tone, balance and gait were assessed using a hand-held dynamometer, the modified ashworth scale (MAS), the berg balance scale (BBS), and G-walk respectively, before and after training. Wilcoxon signed-rank test was used to analyze intergroup changes after the intervention, whereas the Kruskal Wallis H test, and Mann-Whitney U test were used to analyze intergroup changes in all variables. Results : Strength in all the muscle, except for the flexor and extensor muscles in the affected knees, was increased in all the groups, however, no significant difference was observed. No change in the muscle tone (MAS) was found in any group. All the groups showed increased BBS scores walking speed, and walking symmetry following training, although, the differences were not significant. Changes in the affected step length of experimental group 1 were significantly greater than those of experimental group 3, whereas changes in the affected step length in experimental group 3 were significantly greater than those of experimental group 2. Conclusion : Our results suggest that resistance exercise using elastic bands are a potential intervention for improving lower extremity muscle strength, balance, and gait among stoke patients.

  • Understanding how and why people differ is a fundamental, if distant, goal of research efforts to bridge psychological and biological levels of analysis. General fluid intelligence (gF) is a major dimension of individual differences and refers to reasoning and novel problemsolving ability. A conceptual integration of evidence from cognitive (behavioral) and anatomical studies suggeststhat gF should covary with both task performance and neural activity in specific brain systems when specific cognitive demands are present, with the neural activity mediating the relation between gF and performance. Direct investigation of this possibility will be a critical step toward a mechanistic model of human intelligence. In turn, a mechanistic model might suggest ways to enhance gF through targeted behavioral or neurobiological intervent ions, We formed two different groups as subjects based on their scholarly attainments. Each group consists of 20 volunteers(aged 16-17 years, right-handed males) from the National Gifted School and a local high school respectively. To test whether individual differences in general intelligence are mediated at a neural level, we first assessed intellectual characteristics in 40 subjects using standard intelligence tests (Raven's Advanced Progressive Matrices, Wechsler Adult Intelligence Scale, Torrance Tests of Creative Thinking) administered outside of the MR scanner. We then used functional magnetic resonance imaging (fMRl) to measure task-related brain activity as participants performed three different kinds of computerized reasoning tasks that were intended to activate the relevant neural systems. To examine the difference of neural activity according to discrepancy in general intelligence, we compared the brain activity of both extreme groups (each, n=10) of the participants based on the standard intelligence test scores. In contrast to the common expectation, there was no significant difference of brain region involved in high-g tasks between both groups. Random effect analysis exhibited that lateral prefrontal, anterior cingulate and parietal cortex are associated with gF. Despite very different task contents in the three high-g-low-g contrasts, recruitment of multiple regions is markedly similar in each case, However, on the task with high 9F correlations, the Prodigy group, (intelligence rank: >99%) showed higher task-related neural activity in several brain regions. These results suggest that the relationship between gF and brain activity should be stronger under high-g conditions than low-g conditions.

Clinical outcome of ultrasound-guided atelocollagen injection for patients with partial rotator cuff tear in an outpatient clinic: a preliminary study

  • Background: Atelocollagen has been studied for restoration of rotator cuff tendon. In this study, we attempted to evaluate the clinical outcome of ultrasound-guided atelocollagen injection in an outpatient clinic for patients with partial rotator cuff tear. Methods: We recruited 42 outpatients who visited our hospital from May 2019 to September 2019. Atelocollagen injection was performed in patients with partial rotator cuff tear diagnosed by magnetic resonance imaging and ultrasound. American Shoulder and Elbow Surgeons (ASES), Constant, Korean Shoulder Score (KSS) and Simple Shoulder Test (SST) scores, and range of motion were assessed before injection and after 2 months. Statistically, we analyzed the clinical results using the Wilcoxon signed-rank test. Results: Finally, 15 patients were enrolled for analysis. There was no significant difference between pre- and post-injection in terms of range of motion, ASES (57.0 vs. 60.4), Constant (56.4 vs. 58.9), KSS (64.6 vs. 68.5), and pain-visual analog scale (4.2 vs. 3.7), except function-visual analog scale (F-VAS; 6.3 vs. 7.1) and SST (6.6 vs. 6.9). A significant difference was found in SST (P=0.046) and F-VAS (P=0.009). According to the ultrasound results at 2 months, we found hyperechoic materials in three of seven patients. The most common complication of atelocollagen injection was post-injection pain (53%, 8/15). Conclusions: Ultrasound-guided atelocollagen injection for partial rotator cuff tear showed no significant change in terms of clinical outcomes, except for F-vas and SST score. Tendon regeneration was not clear due to the remnants of atelocollagen present at 2-month follow-up ultrasound. There seems to be alarming post-injection pain for 2 to 3 days in the patients who received atelocollagen injection in an outpatient clinic.

승모근과 전거근 강화운동이 기능적 어깨충돌증후군을 가진 척수손상 환자의 통증과 근 활성도에 미치는 영향 (The Effects of Trapezius and Serratus Anterior Strengthening Exercise on Pain and Muscle Activation in Spinal Cord Injury Patients with Functional Shoulder Impingement Syndrome)

  • Purpose: The aim of this study was to investigate the effects of trapezius and serratus anterior strengthening exercise on the shoulder pain and muscle activation of patients with spinal cord injury and functional shoulder impingement syndrome. Methods: The study consisted of 10 patients with spinal cord injury who were hospitalized in Rehabilitation Hospital U, Uijeongbu, South Korea. The exercise was implemented three times a week for 10 weeks. In each session, the subjects performed one of a total of five types of exercise at mid-level intensity. The shoulder pain and disability index (SPADI) was used to evaluate the patients before and after the intervention. The muscle activation of the upper trapezius, middle trapezius, lower trapezius, and anterior serratus muscle was assessed by surface electromyography (EMG) at the beginning of the experiment and 10 weeks later. Wilcoxon's singed-rank test was conducted to determine differences in the pain index and muscle activation before and after the exercise. The level of statistical significance was set at ${\alpha}=0.05$. Results: SPADI scores significantly decreased after the exercise (p<0.05). In comparisons of muscle activation, there was a significant improvement in the upper trapezius at $60^{\circ}$ shoulder joint flexion (p<0.05). There was no significant improvement at $90^{\circ}$ shoulder joint flexion. The middle trapezius showed a significant improvement at $120^{\circ}$ shoulder joint flexion (p<0.05). Conclusion: Trapezius and serratus anterior strengthening exercise reduced pain in spinal cord injury patients with functional shoulder impingement syndrome. The decreased muscle activation of upper trapezius and increased muscle activation of the anterior serratus muscle at $60^{\circ}$ shoulder joint flexion point to positive effects of the exercise on supraduction of the scapula.

척추세움근 매선침치료가 만성요통환자에 미치는 효과 (Effects of the Embedding Acupuncture Treatments for Chronic Low Back Pain Patients)

  • Objectives To investigate clinical effects of needle embedding acupuncture treatments for chronic low back pain patients. Methods 30 patients with chronic low back pain were recruited and randomized into two groups-the embedding acupuncture group or the placebo. At baseline, the age, height, weight, visual analogue scale (VAS), Oswestry disability index (ODI) scores were measured. And surface electromyography (SEMG) data of both erector spinae at L2, L4 level were also measured on both groups and asymmetry index (AI) were calculated. The embedding or placebo acupuncture treatment was performed on the erector spinae according to SEMG values; immediately after the first evaluation and 48 hours after the first visit. After 96 hours of intervention, the VAS, ODI score and SEMG of both erector spinae were measured again. Statistical significance was determined using the Wilcoxon signed ranks test or the Wilcoxon rank sum test. Results The mean VAS, ODI score after treatment was decreased significantly compared with baseline on both groups. And the VAS, ODI score and AI of the embedding acupuncture group was more decreased significantly than the placebo (p<0.05). Conclusions The results suggest that embedding acupuncture for chronic low back pain patients was effective on the VAS pain score, ODI score and AI of the erector spinae.

ADHD 위험 아동에 대한 반응성 부모교육이 아동의 문제행동과 중심축 발달행동에 미치는 효과 (Effects of a Responsive Parenting Education Program on Child's Behavioral Problems and Pivotal Developmental Behaviors in Children at Risk for Attention Deficit Hyperactivity Disorder)

  • Purpose: This study was done to determine whether a parenting education program using responsive teaching strategies is effective for parenting stress, maternal interactional behavior and behavioral problems in children at risk for Attention Deficit Hyperactivity Disorder (ADHD). Methods: Participants in this study were 17 elementary school children and their parents. The ADHD risk group was determined by scores on the Korean-ADHD Rating Scale. The parenting education program was developed based on Dr. Mahoney's responsive teaching curriculum. Mothers participated in the program once a week for 8 sessions. Treatment outcome was evaluated using the Korean version of the Child Behavior CheckList (K-CBCL), parenting stress, maternal interactional behavior, and child pivotal developmental behavior. Data were analyzed using Wilcoxon signed rank test, Mann-Whitney test, and regression analysis. Results: After the parenting education program, no significant difference in parenting stress (Z=-1.00, p=.320) was found, but there was a significant decrease in the child's internal behavior problems (Z=-2.05, p=.040), and also a significant improvement in maternal interactional behavior and a significant difference in child pivotal developmental behavior (Z=-2.67, p=.008). Conclusion: The results indicate that parenting education programs based on responsive teaching strategies are effective and that application of a program is recommended to prevent behavioral problems and improve maternal child interaction for children at risk for ADHD.

지체장애주부의 조직적인 행동과 가족구성원의 참여도 조사연구 (Physically Disabled Homemaker's Organizational Activities and Family Participation)

  • The objectives of this study were as follows: 1) To describe the group of selected disabled homemakers in terms of demographic and medical variables related to organizational activities in the home and family participation in household activities. 2) To identify those demograhpic and medical variables that were related to organizational activities in the home and family participation in household activities. 3) To identify relationship between organizational activities in the home and family participation in household activities. Questionnaires were given to the selected disabled homemakers living in Seoul. The sample was small(N=35) and it was not random; therefore generalizations could not be made to the population as a whole. Data were analyzed by mean, Kruskal-Wallis one-way analysis of variance by ranks, and Spearman rank correlation coefficients. Homemaker's organizational activities were measured by 16 items about physical and mental activities from Mumaw's Organizational Activities Index. Family participation in household activities was measured by 8 items about how often family members participated in two areas of household activities: house chores and extra-activities. The results were as follows: 1) The homemaker's higher organizational activities scores were associated with younger homemaker, higher the level of education, small households, and higher the economic status. The homemaker without children performed better organizational household activities. The homemaker with shorter the duration of disability performed better organizational activities. The homemaker who needed crutches for mobility performed better organizational household activities. 2) The homemaker who was younger, higher the level of education, with smaller households, and lower the economic level was helped by husband. Husband helped better the homemaker with shorter the duration of disability in household activities. 3) Factors affecting children's participation in disabled homemaker's house chores and extra-activities were the age of homemaker and the marital status. The homemaker was single and older, the children were more helpful. 4) Task standardization score was the highest among the factors of homemaker's organizational activities and families with a disabled homemaker participated more extensively than families with abled homemaker. 5) Significant intercorrelation was found between the dependent variables.

A Comparative Analysis of Editorial Leaders' Profiles of Major and Non-Western Library and Information Science Journals

  • Due to the competitive nature of journal publishing, editorial leadership has become an increasingly important issue on many editorial teams. This study aimed to compare the major and non-Western international journals in library and information science and reveal the differences between them. To conduct this study, journals indexed by Scopus and Web of Science were analyzed in terms of gender, professional position and rank, institutions, and the iSchool status of the editorial leaders' institutions. The most notable results were the following: a) As a whole, both types of journals lacked true internationalization. Editorial leaders of major journals tended to be from Western countries, whereas editorial leaders of non-Western journals tended to be from non-Western countries; b) Most non-Western journals tended to appoint editorial leaders from the same country as the publisher's country; and c) Almost all editorial leaders of non-Western journals were from various non-Western countries and tended to have lower h-index scores, and their institutions were not part of the iSchool. Future research should assess editorial leadership, compare the results of this study to other disciplines, and find effective ways to collect data on editorial leaders while minimizing ethical concerns in order to meet future research needs.
