서론 : 본 연구는 무작위 대조군 실험(Randomized control trial: 이하 RCT) 연구에서 치료의향분석(Intention-to-treat analysis)의 올바른 활용에 대해서 고찰하고자 한다. 본론 : 치료의향분석은 RCT 연구에서 무작위 분배 이후에 모든 대상자의 데이터를 결과에 포함하는 분석방법이다. RCT 연구의 결과는 일반화에 있어서 매우 높은 타당성을 갖는다. 이런 측면에서 중도 탈락을 포함한 모든 대상자의 데이터를 결과 분석에 포함하여야 그 결과를 실제 임상에 적용했을 때 중재의 효과가 예측 가능하기 때문이다. 이런 이유로 RCT 연구에 대한 질평가 도구들은 치료의향분석 실시여부를 확인하고 있다. 그럼에도 많은 연구자들은 치료의향분석의 이해도가 낮아 잘못 활용하고 있거나 활용하지 않는 경우가 많다. 결론 : 본 고찰 연구를 통해 국내 많은 연구자들이 치료의향분석방법을 정확하게 이해하고 올바르게 활용하여 RCT 연구에 반영할 것을 기대한다.

This study aimed to examine the clinical efficacy of electroacupuncture treatment for carpal tunnel syndrome by reviewing published randomized controlled trials. Among the 186 studies retrieved from 7 online databases (PubMed, Cochrane library, CNKI, NDSL, RISS, OASIS, KMbase) on October 29, 2021, 4 studies were selected according to the inclusion, exclusion criteria, and were evaluated using risk of bias. Control groups for electroacupuncture were wearing a splint at night, traditional acupuncture, and medication. Methods such as total effective, functional status scale, symptom severity scale, electromyography, tip pinch strength, visual analogue scale, numeric rating scale, and ultrasound were used to evaluate the therapeutic effect. Electroacupuncture was reported to have significant treatment results compared with the control group in methods such as total effectiveness, electromyography, and tip pinch strength. However, the quality of the studies (using risk of bias) does not allow reliable conclusions to be made. Many high quality (low risk of bias) randomized controlled trials are needed to examine the efficacy of electroacupuncture treatment for carpal tunnel syndrome.
Objectives The purpose of this study is to analyze clinical studies on effectiveness of herbal medicine in rhinosinusitis. Methods We searched the randomized controlled trials (RCTs) with herbal medicine treatment on rhinosinusitis from the Pubmed in recent 10 years (from 2008 to 2018). Results 11 RCTs were reviewed. In 10 out of 11 studies showed that the herbal medicine may be effective in the treatment of chronic rhinosinusitis without nasal polyp and uncomplicated acute rhinosinusitis. However, inclusion and exclusion criteria, and outcome measures were varied among different studies. No serious adverse reactions were reported from the herbal medicine treatment. Conclusions The results of these trials showed that herbal medicine may be effective in the treatment of rhinosinusitis. Well-designed RCTs for domestic herbal medicine treatment on rhinosinusitis are needed to prove its efficacy clearly.
Objectives : The purpose of this study is to analyze research trends on the effects of catgut-embedding therapy on allergic rhinitis. Methods : We searched potentially relevant randomized controlled trials using various electronic databases and extracted data using a standardized form. Results : Finally, 11 studies were selected and analyzed. All studies were conducted in China and the number of studies has increased especially in recent 3 years. 11 studies compared catgut-embedding therapy with usual care, acupuncture, sham catgut-embedding therapy. Most studies showed that catgut-embedding therapy was effective and had long lasting effect compared with comparison group. Conclusions : All studies were conducted in China and it is necessary to review its evidence for clinical application in Korea. Further studies are needed on this topic in order to demonstrate the effectiveness and safety clearly.
Objectives: The purpose of this systematic review is to overview and evaluate the effect on postpartum hypogalactia after acupuncture treatment. Methods: Relevant randomized controlled trials (RCTs) were identified by database searches in Embase, Pubmed, CENTRAL, CNKI, KISS. Searching keywords were "Hypogalactia" or "Hypolactaion" and "Acupuncture". After searching qualified articles, we independently assessed the risk of bias according to the Cochrane Rob criteria. Results: Among the 81 articles five RCTs were finally selected. Four of the selected were statistically significant. One of them was not statistically significant because it was comparative study of chinese medicine and acupuncture treatment. The quality of the evidence for specific comparisons ranged from low to high. Conclusions: Our systematic review found encouraging. Acupuncture treatment would be effective way to increase insufficient milk supply on postpartum hypogalactia.
Objectives The purpose of this study is to analyze clinical studies on the effectiveness of TCM (traditional Chinese medicine) for pompholyx treatment and provide evidence for Korean medicine use. Methods Randomized controlled trials (RCT) which used TCM as a treatment for pompholyx were searched in China National Infrastructure (CNKI) platform. The risk of bias of studies was assessed by using Cochrane risk of bias (RoB) 2.0 tool. Results The selected 10 RCTs showed that the experimental group treated with either TCM alone or with a combination of TCM and Western medicine showed more significant therapeutic effect compared to the control group which Western medicine alone. Conclusions Based on the studies from China, TCM may be an effective treatment for pompholyx. This provides evidence for Korean medicine use, and its possible effectiveness in treating pompholyx
Objectives The purpose of this study is to analyze clinical studies on effectiveness of herbal medicine in influenza compared with Oseltamivir. Methods We searched the randomized controlled trials (RCTs) with herbal medicine treatment on influenza compared to Oseltamivir from the Pubmed, CNKI, OASIS, NDSL, J-stage, and CiNii. Results 14 out of 717 studies were selected and analyzed. The herbal medicine treatment had a significant effects on the alleviation of fever, cough, sore throat than the Oseltamivir control treatment. Herbal medicine had same therapeutic effectiveness like Oseltamivir on duration of influenza-like symptoms and viral shedding. No serious adverse reactions were reported from the herbal medicine treatment. Conclusions The results of these trials showed that the herbal medicine may be more effective than the Oseltamivir in the treatment of influenza. Well-designed RCTs for domestic herbal medicine treatment on influenza are needed to prove its efficacy clearly.
Objectives The objective of this study was to investigate the impact of applying acupuncture as intervention of peripheral facial paralysis in children. Methods We conducted searches on China National Knowledge Infrastructure and Wanfang Data using the keywords "facial paralysis", "children", and "acupuncture". Results Six randomized controlled trials were included in the analysis. The acupoints most frequently utilized were Hapgok (LI4), Jichang (ST4), Yangbaek (GB14), Sabaek (ST2), Hyeopgeo (ST6), Yeonghyang (LI20), Yepung (TE17), and Taeyang (EX-HN5). The Stomach Meridian (ST) was the most targeted. Acupuncture treatment was found to be as effective as or more effective than Western medicine treatment in certain cases. Conclusions Acupuncture therapy shows promise for treating pediatric peripheral facial paralysis. However, further clinical randomized controlled trials are necessary to validate the efficacy and safety of acupuncture treatment.
Objectives : To evaluate the effectiveness of Chuna Manual Therapy for adhesive capsulitis by a systematic review manner. Methods : Five foreign electronic databases (Pubmed, Ovid-medline, Embase, Cochrane library, Chinese Academic Journals (CAJ)) and two Korean medical electronic databases (Oriental Medicine Advanced Searching Integrated System (OASIS) and Research Information Sharing Service (RISS)) were searched to find all randomized controlled trials (RCTs) using Chuna Manual Therapy as a treatment for adhesive capsulitis. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. Results : Twenty-one RCTs met our inclusion criteria. The meta-analysis showed positive results on the use of Chuna Manual Therapy combined with acupuncture or exercise therapy in terms of the efficacy rate and pain using the Visual Analogue Scale (VAS), compared to acupuncture or exercise alone [SMD-1.81 (95% CI & -2.54, -1.08), P<0.001, RR 1.19 (95% CI 1.14, 1.23), P<0.001)]. Positive results in terms of efficacy rate were also obtained comparing Chuna Manual Therapy to acupuncture alone [RR 1.10 (95% CI 1.02, 1.18), P=0.01]. Conclusions : Our systematic review found favorable results on the effectiveness of Chuna Manual Therapy for pain and efficacy rate of adhesive capsulitis. However, evidence was limited due to the lack of well-designed RCTs. More qualified clinical trials are needed to obtain stronger evidence.
Objectives : The purpose of this study is to assess the effect of using Boyanghwano-tang(BYHWT) for diabetic retinopathy. Methods : We searched randomized controlled trials(RCTs) that used BYHWT for diabetic retinopathy in 10 databases(RISS, KISS, ScienceON, KCI, OASIS, DBpia, CNKI, Wanfang Database, PubMed, Cochrane Library) on August 12, 2023. Outcome measurements included total effective rate(TER) and visual acuity as the primary outcome measurements, and the secondary outcome measurements included fasting blood glucose(FBG), 2hPG, HbA1c, ET-1, NO, etc. The indicator of stability included adverse reactions. A meta-analysis was conducted according to TER, visual acuity, FBG. Results : A total of 8 randomized control trials were selected. The treatment group which used BYHWT showed significant improvement effects in TER and visual acuity compared to the control group which used western medicine or chinese patent medicine alone. TER: (RR: 1.22, CI 1.14 to 1.31, P=<0.00001), visual acuity: (RR: 2.06, CI 1.40 to 3.03, P=0.0002). Secondary outcome measurements including FBG also showed improvement in the treatment group compared to the control group. FBG: (MD: -0.15, CI -0.43 to 0.31, P=0.28). Adverse reactions were only mentioned in one study, which showed that no adverse reactions occurred in both the treatment and control groups. Conclusions : This study suggests that BYHWT is effective for diabetic retinopathy. However these findings should be interpreted cautiously due to the insufficient number of studies and unknown or high risk of bias in the included trials.
