항공사고조사위원회의 업무개선에 대한 제언 (Suggestions for the Improvement of Aviation and Railway Accident Investigation Board in Korean)

  • ICAO Universal Safety Oversight Audit Programme is a comprehensive evaluation for aviation safety matters, including aircraft operation, licensing and aircraft incident/accident investigation. As this programme is measures aviation safety level of States, there is a need to meet international standards, especially in the field of aircraft incident/accident investigation. This research discusses points required to satisfy international standards, which are independence and autonomy of an accident investigation board, the lack of the enough number of investigators and efficient management of those personnel, the effective composition and use of an investigation report, an accident investigation itself to build big data and the creation of a cooperation system among States. This paper suggests various opinions in order to enhance aviation safety especially through aircraft accident investigations, using case studies from the US, UK and Japan.

  • As a public transportation possible to convey a large quantity, the railway is safe and keeps time, but it has hazards to cause a disaster if the accidents such as collision, derailment, and fire occur. So advanced countries carry out System Safety Plan with various program activities which have connected orders to maintain or improve safety level by finding hazards, evaluation, taking measures and practice, and improving problems. Especially they systematically manage hazards to cause railway accidents and the factors which possibly threat safety, using national classification of risk and causes with analysis of the related data such as establishing accident/incident data and safety regulations/standards. As executing railway safety regulations, domestic railway is currently trying to improve railway safety management system. The research of classification system of accidents/incidents is one thing to make railway safety management systems better. In this research, we reviewed hazardous factors of railway systems and classification of the causes as the beginning of system safety management, and we conducted study on development of railway accident classification based on findings of this research. The results are able to be used in identifying hazards and activities of systemic safety management at the step of railway accident report and investigation.

철도사고 대응매뉴얼 과부하에 대한 진단과 개선방안 연구 (Diagnosing Railway Incident Response Manuals and Their Improvement)

  • 비상대응계획은 사고의 예방보다는 신속한 대응을 통하여 인적 물적 피해를 최소화 하기 위한 목적으로 설계된다. 그러나 매뉴얼 작성 및 운용을 위하여 많은 노력과 비용을 지출함에도 불구하고 이의 효용성에 대한 논란은 계속되고 있다. 매뉴얼의 다양한 종류, 복잡함으로 인한 활용도 저하, 이해의 어려움, 비 현실성, 의사 판단의 제약, 상시 협력 부족 등과 관련된 많은 논란의 시작은 법에서 정하는 다양한 매뉴얼의 종류와 그 구성 내용의 상이함에서 비롯된다. 국내외 철도와 관련된 법률에서 철도운영기관 입장에서 마련하여야 하는 매뉴얼의 종류와 그 내용을 분석한 결과 2개의 법률인 '재난 및 안전관리 기본법'과 '철도안전법'에 따라 서로 다른 형태의 매뉴얼을 각각 마련해야 하는 문제가 있다. 이러한 법에 따라 철도운영기관이 모든 매뉴얼을 각각 작성 운영하는 것보다 한 개의 법(철도안전법) 적용으로만 한정하여 중복된 매뉴얼 생성 및 관리의 책임을 경감시킬 필요가 있다. 내용의 구성에서도 사고유형별, 위기수준별, 위치 별로 구분하여 비상 대응 내용에서의 불필요한 중복을 줄일 필요가 있고, 실제 사고에서 우선적으로 활용되는 내용들을 토대로 시간 순서에 따라 재구성 하는 방안을 제시함으로써 매뉴얼의 활용 성을 높일 수 있을 것이다.

철도사고가 철도경영에 미치는 영향 (Effects of Accidents on Railroad Operations in Korea)

  • Korean railway has run about 110 years since 1989 and played a great role of industrialization in Korea. It is known that rail transport systems have many advantages of being more safe, energy-efficient, and environment-friendly, as compared to other transportation systems. However, railway incidents are often attributed to the failure of safety management and critical to the efficiency of railroad industry. This study reviewed economic, financial, and general statistical information on Korea Railroad. Based on the statistical data, the effect of accidents on railroad management was analyzed. Correlation analysis revealed that railway accident had a negative effect on the gross profit of Korea Railroad. In order to reduce potential risks and incident rate, some recommendations are proposed. Actual or potential applications of this research include safety guidelines for improving efficiency of railroad industry.

철도 위험도평가 전산시스템(RAIMS)의 설계 및 개발에 관한 연구 (A Study on the Development of Railway Risk Assessment Information Management System)

  • Risk assessment of a railway system should be periodically conducted managing a large amount of accumulating accident/incident data and scenarios, which generally requires enormous time and efforts. Therefore, special information management system is essential for railway risk assessment, where data needed for decisions on managing the railway safety could be promptly supported. In this study, we develop the framework of a railway risk assessment information management system (RAIMS). The RAIMS is composed of two main modules: 1) hazard data processing module; 2) and risk assessment module. Hazard data could be turned into risk information using these two modules. The RAIMS will be useful in finding hazard conditions, quantitatively assessing the risk, and providing pertinent risk measures, eventually serving to prevent railway accidents and reduce severities of railway accidents.

국내 헬리콥터 조종사 인적오류 사고 분류 및 분석 (Classification and Analysis of Human Error Accidents of Helicopter Pilots in Korea)

  • There are two to three helicopter accidents every year in Korea, representing 5.7 deaths per 100,000 flights. In this study, an analysis was conducted on helicopter accidents that occurred in Korea from 2005 to 2017. The accident analysis was based on the aircraft accident and incident report published by the Aircraft and Railway Accident Investigation Board. This Research analyzed the characteristics of accidents occurring in Korea caused by human error by pilots. Accident analysis was done by classifying the organization, flight mission, aircraft class, flight stage, accident cause, etc. Pilot's huan error was classified as Skill-based error, decision error and perceptual error in accordance with the HFACS taxonomy. The accidents caused by pilot's human error were classified into five categories: powerlines collision, loss of control, fuel exhaustion, unstable approach to reservoir, and elimination of tail rotor.

철도 사고 및 장애의 인적오류 유형 분석 (An Analysis of Human Error Mode and Type in the Railway Accidents and Incidents)

  • Human error is one of the major contributors to the railway accidents or incidents. In order to develop an effective countermeasure to remove or reduce human errors, a systematic analysis should be preferentially performed to identify their causes, characteristics, and types of human error induced in accidents or incidents. This paper introduces a case study for human error analysis of the railway accidents and incidents. For the case study, more than 1,000 domestic railway accidents or incidents that happened during the year of 2004 have been investigated and a detailed error analysis was performed on the selected 90 cases, which were obviously caused by human error. This paper presents a classification structure for human error analysis, and summarizes the analysis results such as causes of the events, error modes and types, related worker, and task type.

  • Currently, approx. 10,532 turnouts were installed on the KR line, of which 3,644 turnouts were installed in the main line where high speed operation is performed. This shows that it is necessary to improve the performance of turnout as one of vulnerable areas for the safe operation of a train. Even though the number of railway accidents is decreasing every year due to the renewal of railway facilities, there are still many factors that cause the occurrence of an accident. In particular, an incident in turnout area does not only have a high risk of a serious accident but also affect the operation of a train on the adjacent track in most cases, and consequently a big social loss is expected due to the delay in train operation as well as the loss of life and property. The objective of this study is to examine the accidents occurred in turnout area that is one of typical vulnerable areas over the past ten years, on the basis of the results, and to take an appropriate measure by finding out the major cause of accidents in turnout area so that a systematic safety system can be established to prevent or reduce accidents in turnout area.

국내 철도 탈선사고에 대한 조사분석 연구 (A study on the analysis of derailment accidents in Korea)

  • According to ' Monthly report of train accidents and incidents ' of KNR 184 numbers of derailment accidents/incidents had happened during last five years(from 1999 to 2003). Among them, there have happened only four injured accidents such as West Daejeon derailment accident, Jungang line, Pyegje station, and Daejeon station one. But it is necessary to consider a countermeasure for derailment, as its accident/incident have big risk which might produce a large scale damage and lots of injured persons. This paper analyzes the derailment accidents/incidents which have happened for last five years in KNR, and suggests their countermeasures.
