미세혈관감압술에도 지속된 삼차신경통의 고주파 열응고술을 이용한 치험 -증례 보고- (Radiofrequency Thermocoagulation for Trigeminal Neuralgia Sustained Following Microvascular Decompression -A case report-)

  • The authors report the result achieved in the treatment of trigeminal neuralgia patient, especially V2 involved patient, using radiofrequency (RF) thermocoagulation of Gasserian ganglion. A 62-year old female patient had severe burning pain on right cheek usually initiating from upper molar teeth area for 10 years. She was treated with microvascular decompression operation 10 years ago. However, there was no pain relief by operation. She wanted non-surgical treatment. Therefore, we recommended RF thermocoagulation therapy. After 2 times of RF thermocoagulation, there was excellent pain relief without complications. And, for 6months follow-up, there were no pain, and no evidences of complication and recurrence.

  • Primary hyperhidrosis, a disorder of unknown etiology, is characterized by excessive uncontrollable sweating, most often of the palm surface of the hands, armpits, groin and feet. To decrease the symptoms of hyperhidrosis, drug therapy, iontophoresis, excision of axillary sweat glands and thoracoscopic sympathectomy have been attempted. A lumbar sympathectomy is one of the available choices for the treatment hyperhidrosis of the lower extremities. A 28-year old female patient presented with excessive sweating of her hands and feet. For the treatment of her foot hyperhidrosis, a bipolar radiofrequency ablation system was used to ablate the lumbar sympathetic ganglion, with a successful result. This modality will receive greater attention as an available alternative to lumbar sympathetic neurolysis.

  • Although antiarrhythmic medication has been the main treatment modality for arrhythmia in children, in recent decades technological development and computerization have made great advances in nonpharmacological therapy. This article reviews the transcatheter radiofrequency ablation for tachycardia in children, recent advances of device therapy for bradycardia, antitachycardia pacing, implantable cardioverter defibrillator. As a new field of device therapy, cardiac resynchronization therapy for congestive heart failure is also mentioned.

정위적 삼차신경절 고주파열응고술 (Stereotactic Radiofrequency Gasserian Ganglionotomy)

  • When medical therapy fail to relieve pain at tolerable level for patients confirmed with trigeminal neuralgia, presence of mass lesion excluded, surgery is indicated. Innumerable surgical strategies have been attempted for the treatment of trigeminal neuralgia but only four have proven appropriate: (1)stereotactic radiofrequency gasserian ganglionotomy, (2) percutaneous glycerol gangliolysis, (3) percutaneous microcompression, (4) microvascular decompression. Radiofrequency thermocoagulation of the gasserian ganglion stems from the efforts of Sweet. This technique is the surgical treatment of choice around the world for surgical treatment for trigeminal neuralgia. Since 1986, over 14,000 cases have been reported utilizing this technique. To improve the treatment method further, an electrode with a flexible curved tip has been developed for easier and more precise electrode placement and lesion production during the thermocoagulation of gasserian ganglion. This operation was performed recently on three patients at Hallym University Hospital. using a curved tip electrode. Complete relief of pain was achieved for all patient. However, some complications were noted.

Radiofrequency Ablation and Excision of Multiple Cutaneous Lesions in Neurofibromatosis Type 1

  • Background Von Recklinghausen disease or neurofibromatosis type 1 is an autosomal dominant genetic disorder of chromosome 17q11.2. The most common characteristic findings of NF 1 include multiple and recurrent cutaneous neurofibromas associated with psychosocial distress. Methods Sixteen patients (9 female, 7 male; average age, 31 years; range, 16 to 67 years) with multiple cutaneous neurofibromas between March 2010 and February 2012 were included in the study. All patients were treated with radiosurgical ablation and excision under general anesthesia. Results All 16 patients were satisfied with the results, when questioned directly during the outpatient department follow-up. The only complaint from a few patients was minimal scarring, but acceptable results were obtained in the end. Conclusions The radiofrequency procedure is almost bloodless and quick, creating a smaller necrotizing zone. Therefore, instead of employing the time consuming traditional surgery, such as laser therapy and electrosurgical excision, that produces uncertain results and can affect normal adjacent tissue, treatment of neurofibromas with radiofrequency ablation and excision can be an alternative choice of treatment for patients with a large number of neurofibromas.

In recent years, cooled radiofrequency ablation (CRFA) has become a viable treatment option for knee OA. This review thoroughly evaluated the existing literature on CRFA therapy for knee OA. It delved into the mechanisms behind CRFA, evaluated its clinical efficacy, and investigated potential avenues for future research and application. The insights gained from this review are crucial for healthcare professionals, researchers, and policymakers, offering an updated perspective on CRFA's role as a viable therapeutic option for knee OA.

갑상선암에 대한 고주파 절제 적용의 과거, 현재, 그리고 미래 (Application of Radiofrequency Ablation to Thyroid Cancer: Past, Present, and Future)

  • 발생률이 높고, 예후는 좋지만 재발을 잘하는 경향이 있는 갑상선암에서도 전통적인 치료법은 수술이다. 하지만, 2000년대 초반부터, 간암, 폐암, 신장암에서와 마찬가지로 고주파 절제술이 원발 갑상선암 및 재발 갑상선암에 적용되기 시작했다. 원발암에서는 1 cm 이하 크기의 미세유두암 저위험군에서 수술 불가능한 경우에 대한 많은 연구가 이뤄져 왔고, 1 cm보다 더 크고 4 cm 이하의 다양한 군에 대해서도 일부 연구가 시행되었으며, 전반적으로 모두 양호한 결과를 보고하였다. 재발암에 대해서도 주로는 국소 경부 재발암에 고주파 절제술이 시행되었으며, 일부 폐, 뼈 등의 원격 전이에 대해서도 시도되었고, 전반적으로 양호한 결과가 보고되었다. 최근 들어, 한국, 미국, 유럽 등 각지에서 갑상선암의 고주파 절제술에 대해 옹호하는 움직임이 있다. 하지만, 미국 국립 종합 암 네트워크(National Comprehensive Cancer Network) 등의 중립적인 임상지침에는 고주파 절제술이 아직 인정되지는 않은 상태이다. 지금까지의 노력들과 미래의 연구를 바탕으로 향후 가까운 미래에 갑상선암의 치료에 있어 고주파 절제술이 적절한 자리매김을 할 것으로 전망한다.
