Radiation induced lung injury has long been considered a treatment limiting factor for patients requiring thoracic radiation. This radiation induced lung injury happens early as well as late. Radiation induced lung injury can occur in two phases viz. early (< 6 months) when it is called radiation pneumonitis and late (>6 months) when it is called radiation induced lung fibrosis. There are multiple factors that can be patient, disease or treatment related that predict the incidence and severity of radiation pneumonitis. Radiation induced damage to the type I pneumocytes is the triggering factor to initiate such reactions. Over the years, radiation therapy has witnessed a paradigm shift in radiation planning and delivery and successfully reduced the incidence of lung injury. Radiation pneumonitis is usually a diagnosis of exclusion. Steroids, ACE inhibitors and pentoxyphylline constitute the cornerstone of therapy. Radiation induced lung fibrosis is another challenging aspect. The pathophysiology of radiation fibrosis includes continuing inflammation and microvascular changes due to pro-angiogenic and profibrogenic stimuli resembling those in adult bronchiectasis. General supportive management, mobilization of airway secretions, anti-inflammatory therapy and management of acute exacerbations remains the treatment option. Radiation induced lung injury is an inevitable accompaniment of thoracic radiation.

Purpose: The degree of radiation-induced lung fibrosis (RILF) can be measured quantitatively by fibrosis volume (VF) on chest computed tomography (CT) scan. The purpose of this study was to investigate the interobserver and intraobserver variability in CT-based measurement of VF. Materials and Methods: We selected 10 non-small cell lung cancer patients developed with RILF after postoperative radiation therapy (PORT) and delineated VF on the follow-up chest CT scanned at more than 6 months after radiotherapy. Three radiation oncologists independently delineated VF to investigate the interobserver variability. Three times of delineation of VF was performed by two radiation oncologists for the analysis of intraobserver variability. We analysed the concordance index (CI) and inter/intra-class correlation coefficient (ICC). Results: The median CI was 0.61 (range, 0.44 to 0.68) for interobserver variability and the median CIs for intraobserver variability were 0.69 (range, 0.65 to 0.79) and 0.61(range, 0.55 to 0.65) by two observers. The ICC for interobserver variability was 0.974 (p < 0.001) and ICCs for intraobserver variability were 0.996 (p < 0.001) and 0.991 (p < 0.001), respectively. Conclusion: CT-based measurement of VF with patients who received PORT was a highly consistent and reproducible quantitative method between and within observers.

A beneficial radioprotective agent has been used to treat the radiation-induced lung injury. This study was performed to investigate whether curcumin, which is known to have anti-inflammatory and antioxidant properties, could ameliorate radiation-induced pulmonary inflammation and fibrosis in irradiated lungs. Rats were given daily doses of intragastric curcumin (200 mg/kg) prior to a single irradiation and for 8 weeks after radiation. Histopathologic findings demonstrated that macrophage accumulation, interstitial edema, alveolar septal thickness, perivascular fibrosis, and collapse in radiation-treated lungs were inhibited by curcumin administration. Radiation-induced transforming growth factor-${\beta}1$ (TGF-${\beta}1$), connective tissue growth factor (CTGF) expression, and collagen accumulation were also inhibited by curcumin. Moreover, western blot analysis revealed that curcumin lowered radiation-induced increases of tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$), TNF receptor 1 (TNFR1), and cyclooxygenase-2 (COX-2). Curcumin also inhibited the nuclear translocation of nuclear factor-${\kappa}B$ (NF-${\kappa}B$) p65 in radiation-treated lungs. These results indicate that long-term curcumin administration may reduce lung inflammation and fibrosis caused by radiation treatment.
Background: Radiation-induced pneumonitis and pulmonary fibrosis are common dose-limiting complications in patients receiving radiotherapy for lung, breast, and lymphoid cancers. In this study, we investigated the characteristics of effective immune cells related to pneumonitis and fibrosis after irradiation. Materials and Methods: After anesthesia, the whole thorax of C57BL/6 mice was irradiated at 14 Gy. The lung tissue and bronchoalveolar lavage fluid were collected at defined time points post-irradiation for the determination of histological and immunohistochemical analysis and inflammatory cell population infiltrated into the lung. Results and Discussion: Whole thoracic irradiation increased the deposition of extracellular matrix (ECM), lung weight, and pleural effusions, which started to die from 4 months later. At 4 months after irradiation, the numbers of macrophages and lymphocytes as well as neutrophils were increased dramatically in the lung. Interestingly, the macrophages that were recruited into the lung after irradiation had an enlarged foamy morphology. In addition, the expressions of chemokines (CCL-2, CCL-3, CXCL-10) for the attraction of macrophages and T cells were higher in the lung of irradiated mice. The high expressions of these chemokines were sustained up to 6 months following irradiation. In thoracic irradiated mice, infiltrated macrophages into the lung had the high levels of Mac-3 antigens on their surface and upregulated the hallmarks of alternatively activated macrophages such as arginase-1 and CD206. Furthermore, the levels of IL-4 and IL-13 were higher in a BAL fluid of irradiated mice. Conclusion: All results show that thoracic irradiation induces to infiltrate various inflammation-related immune cells, especially alternatively activated macrophages, through enhancing the expression of chemokines, suggesting that alternatively activated macrophages are most likely important for leading to pulmonary fibrosis.
BACKGROUND/OBJECTIVES: Exposure of the normal lung tissue around the cancerous tumor during radiotherapy causes serious side effects such as pneumonitis and pulmonary fibrosis. Radioprotectors used during cancer radiotherapy could protect the patient from side effects induced by radiation injury of the normal tissue. Delphinidin has strong antioxidant properties, and it works as the driving force of a radioprotective effect by scavenging radiation-induced reactive oxygen species (ROS). However, no studies have been conducted on the radioprotective effect of delphinidin against high linear energy transfer radiation. Therefore, this study was undertaken to evaluate the radioprotective effects of delphinidin on human lung cells against a proton beam. MATERIALS/METHODS: Normal human lung cells (HEL 299 cells) were used for in vitro experiments. The 3-[4,5-dimethylthiazol-2-yl]-2,5 diphenyl tetrazolium bromide (MTT) assay assessed the cytotoxicity of delphinidin and cell viability. The expression of radiation induced cellular ROS was measured by the 2'-7'-dicholordihydrofluorescein diacetate assay. Superoxide dismutase activity assay and catalase activity assay were used for evaluating the activity of corresponding enzymes. In addition, radioprotective effects on DNA damage-induced cellular apoptosis were evaluated by Western blot assay. RESULTS: Experimental analysis, including cell survival assay, MTT assay, and Western blot assay, revealed the radioprotective effects of delphinidin. These include restoring the activities of antioxidant enzymes of damaged cells, increase in the levels of pro-survival protein, and decrease of pro-apoptosis proteins. The results from different experiments were compatible with each to provide a substantial conclusion. CONCLUSION: Low concentration ($2.5{\mu}M/mL$) of delphinidin administration prior to radiation exposure was radioprotective against a low dose of proton beam exposure. Hence, delphinidin is a promising shielding agent against radiation, protecting the normal tissues around a cancerous tumor, which are unintentionally exposed to low doses of radiation during proton therapy.
연구배경: 방사선치료 효율의 증가와 방사선 피폭 환자의 치료에 있어 방사선으로 인한 폐 손상의 기전을 잘 파악하는 것이 무엇보다 중요한 일이다. 최근 염증성 cytokine의 활성화가 초기 방사선 폐렴뿐만 아니라 후기에 생기는 폐 섬유화에도 기여하고 있다는 보고들이 많이 나오고 있다. 저자들은 염증성 cytokine의 역할을 알아보기 위하여 방사선 후 흰 쥐의 폐와 혈청에서 이들의 변화를 관찰하였다. 방 법: 90마리의 흰 쥐 폐에 20 Gy의 방사선을 조사한 후 정해진 시간에 폐를 적출하여 병리학적 소견을 관찰하였다. 동시에 혈청과 lung homogenate에서 ELISA kit를 이용하여 염증성 cytokine의 변화를 조사하였다. 결 과: 방사선 조사 후 조직에서 염증세포의 침윤이 증가하고 시간이 지남에 따라 폐 섬유화가 생기는 것을 확인할 수 있었다. TNF-$\alpha$와 IL-1$\beta$는 4시간과 3주째 lung homogenate에서 증가하였는데 3주째 더 많이 증가하는 양상을 보여 주었다. 하지만 혈청에서의 변화는 뚜렷하지 않았다. MIP-2의 경우에는 4시간에 lung homogenate에서만 증가한 반면 HMGB1은 3주째 혈청에서만 증가하는 것을 알 수 있었다. 결 론: 방사선 조사 후 TNF-$\alpha$와 IL-1$\beta$ 등의 염증성 cytokine들이 biphasic expression하는 것을 보여 주었다. 후기 염증성 cytokine들의 증가를 효과적으로 억제할 수 있는 방법이 모색되어야 할 것으로 생각되고 이는 폐 섬유화로 진행하는 만성 합병증을 예방하는 데 기여할 것으로 판단된다.
목적 : 방사선조사 후 발생하는 폐손상을 형태학적 측면에서 평가하고 captopril의 방사선조사 후 폐손상의 경감효과가 있는지를 확인하고 captopril의 방사선에 의한 폐손상의 영향에서 TNF-${\alpha}$와 TGF-${\beta}$의 변화을 알아보고자 하였다. 재료 및 방법 : Sprague-Dawley 종 수컷 흰쥐 30마리를 골라 방사선조사만 한 군, 방사선조사 후 captopril을 투여한 군으로 나누어 실험하였다. 방사선조사는 10 Gy, 20 Gy, 30 Gy를 우측 폐에 조사하였다. 방사선 단독 조사군은 방사선조사 후 각각 12 시간, 11주 후에 도살하고 방사선조사 후 captopril을 투여한 군(captopril 500 mg/L를 증류수에 타서 먹임)은 11주(fibrotic period) 후에 도살하여 광학현미경과 전자현미경으로 관찰하였다. 결과 : 방사선조사 후 12 시간내의 실험군의 폐는 부분적으로 폐실질의 허탈과 경화가 방사선조사량이 많아질수록 그 정도와 범위가 증가하였다. 방사선조사 후 11주에는 방사선 단독 조사군에서 폐섬유화의 정도와 범위가 방사선조사량이 많아질수록 증가하였고 captopril을 함께 사용한 군에서 방사선조사 단독군에 비해 폐섬유화의 경감효과가 현저하였다. 방사선 단독군에서는 방사선량이 많아질수록 비만세포의 수는 급격히 증가하였으며 Captoprll을 사용한 군이 사용하지 않은 군과 비교하여 비만세포 수의 증가 정도는 현저히 낮았고 교원질 침착의 정도도 현저히 감소되었다. TNF-${\alpha}$, TGF-${\beta}$는 방사선조사 직후(12 시간) 군에서는 방사선량이 증가함에 따라 그 발현이 증가하였으나 방사선조사 후 11주군에서는 TGF-${\beta}$는 방사선량이 많아짐에 따라 그 발현이 증가하였으며 captopril 투여군에서는 그 발현이 다소 감소하였다. Captopril을 사용한 군에서는 교원질의 양은 증가하였으나 방사선 단독군에 비해 교원질의 양이 적었고 혈관주위 비후의 정도와, 모세혈관의 변화정도, mast Cell의 수와 탈과립, 섬유모세포의 수도 적었다. 결론 : 방사선조사후 방사선 폐렴에서 captopril 영향은 방사선에 의한 비만세포의 출현을 억제시키고 교원질의 침착을 감소시킴으로써 방사선에 의한 섬유화를 예방할 것으로 생각된다. TNF-${\alpha}$와 TGF-${\beta}$의 발현은 방사선조사 후 초기에 증가하며 TGF-${\beta}$는 방사선조사 후 만성기에 방사선량이 많아질수록 발현이 증가하는 것으로 판단되었다. 본 연구 결과는 Captopril이 방사선조사 후 발생하는 폐손상을 감소시키는 기전을 밝히는 향후 연구에 중요한 자료가 될 것으로 생각된다.
1998년 1월부터 1999년 3월 사이에 방사선치료를 시행한 폐암 환자 중 6개월 이상 추적검사를 시행한 69명을 대상으로 방사선 비 투과성 병변을 조사하였다. 대상 환자 69명 중 추적 검사에서 방사선 폐 섬유화로 확인되는 병변을 제외한 방사선 비 투과성 병변이 발생한 경우는 38명이었고, 이 중 방사선폐렴은 24명, 폐렴은 8명, 재발 6명이었다. 6개월 이상의 추적검사에도 폐 섬유화를 보이지 않은 증례도 5례나 있었다. 방사선폐렴을 보인 경우 발생기간은 최초 방사선 치료일로부터 50일에서 130일이었고, 이후 섬유화로의 진행기간은 21일에서 104일이었다. 방사선학적 소견으로 단순촬영(24례)에서는 결절을 동반한 선상음영(II형: 10례, 42%) 및 섬유화를 동반한 폐경결(IV형: 8폐, 33%) 이 많았고, 간유리 음영 혹은 폐경결(I형: 4례, 17%)과 불규칙한 결절성 음영(III형: 2례, 8%)은 드물게 보였다, 전산화 단층촬영(17례)에서는 간유리 음영 혹은 균질성 폐경결이 5례(29%), 방사선 조사범위의 불규칙한 폐경결이 3례(18%), 방사선 조사 범위를 넘어선 경계가 불분명한 폐경결이 3례(18%), 그리고 섬유화를 동반한 방사선 조사 범위의 폐경결이 6례(35%)였다. 단순촬영상 II형을 보였던 4증례는 단층촬영에서 각각 2례씩 I형 및 III형으로 보였고, 단순촬영에서는 병변의 유무를 잘 알 수 없이 임상 증상만을 보였던 2례는 전산화 단층촬영에서 I형의 소견을 보여 단순촬영에 비해 전산화 단층촬영이 방사선 폐렴의 병리학적 진행과정을 이해하는데 도움을 줄 것으로 생각한다.
목 적: 산소를 이용하는 세포들은 반응성 산소기(reactive oxygen species, ROS)로 부터 자신을 보호하는 기전을 가지고 있는데, thioredoxin system으로부터 전자를 받아 과산화물을 물로 환원시키는 생화학적 성질을 가진 효모 단백질을 발견하여 이를 thioredoxin peroxidase(TPX)로 명명하였다. 이는 효모 뿐 아니라 체내 여러 장기에 고루 분포되어 있으며, 특허 종양세포에서 유리되는 반응생 산소기를 제거함으로써 종양세포의 발생(initiation), 성장(promotion) 및 전이(metastasis)를 억제하는 역할을 할 것으로 알려져 있으며, 폐암, 유방암 등의 종양세포내에서의 발현이 증가되는 것으로 보고되었다. 한편, 폐암의 치료에 있어서 방사선 치료는 폐암의 국소적 치료 및 증상 경감을 위해 중요한 위치를 차지하고 있으나, 폐의 방사선 치료에 있어 가장 중요한 장애요인은 치료후 발생하는 폐실질의 섬유화로서 방사선 조사후의 산소유리가(free radical) 손상 등에 의한 것으로 알려져 있으며, 이런 섬유화에 관여하는 인자 및 방어기전에 대한 연구는 미흡한 실정이다. 이에 저자 등은 방사선 조사 후에 백서의 폐조직에서 thioredoxin peroxidase의 아형 및 catalase등이 방사선 조사로 인한 폐손상의 방어기전에 어떤 역할을 하는지 알아보기 위하여 다음과 같은 연구를 시행하였다. 대상 및 방법: 백서 18마리를 각각 3마리씩 6개 군으로 나누었고, 그 중 15마리에 900cGY와 방사선을 조사하였다. 방사선 조사를 받은 백서를 조사직후, 1주후, 2주후, 3주후 및 6주후에 희생시켜 폐조직을 얻었고, 이들 폐조직과 대조군 3마리의 폐조직을 H&E 염색하여 폐섬유화의 정도를 관찰함과 동시에 TPX 아형 및 catalase에 대한 항체로 Western blot analysis를 시행하여 각각의 단백질의 발현정도를 비교하였다. 결 과: 대조군과 비교하여 볼 때 폐조직에서 H&E 염색상 방사선 조사후 시기별로 폐섬유화의 정도가 점차로 진행됨을 볼 수 있었으나, thioredoxin peroxidase 5가지 아형(TPX-A & B, HS22, MER5, HORF-06) 모두 방사선 조사전과 조사후, 시간경과에 따른 면역반응대(immunoreactive band)의 차이를 보이지는 않았다. Catalase 역시 면역반응대의 차이를 보이지 않았다. 결 론: 체내 여러장기, 특히 폐장에 많이 분포하고, 폐암세포에 의한 반응성 산소기(reactive oxygen species)에 의하여 발현이 증가되는 것으로 알려진 thioredoxin peroxidase가 방사선 조사후에 발생한 백서의 폐섬유화에서는 그 발현의 정도가 변하지 않는 것으로 미루어 방사선으로 인한 폐손상의 복원과정에는 관여하지 않는 것으로 생각된다.

Purpose: To evaluate the treatment results in early stage non-small cell lung cancer patients who have undergone fiducial-less CyberKnife radiosurgery (CKRS). Materials and Methods: From June 2011 to November 2013, 58 patients underwent CKRS at Asan Medical Center for stage I lung cancer. After excluding 14 patients, we retrospectively reviewed the records of the remaining 44 patients. All analyses were performed using SPSS ver. 21. Results: The median age at diagnosis was 75 years. Most patients had inoperable primary lung cancer with a poor pulmonary function test with comorbidity or old age. The clinical stage was IA in 30 patients (68.2%), IB in 14 (31.8%). The mean tumor size was 2.6 cm (range, 1.2 to 4.8 cm), and the tumor was smaller than 2 cm in 12 patients (27.3%). The radiation dose given was 48-60 Gy in 3-4 fractions. In a median follow-up of 23.1 months, local recurrence occurred in three patients (2-year local recurrence-free survival rate, 90.4%) and distant metastasis occurred in 13 patients. All patients tolerated the radiosurgery well, only two patients developing grade 3 dyspnea. The most common complications were radiation-induced fibrosis and pneumonitis. Eight patients died due to cancer progression. Conclusion: The results showed that fiducial-less CKRS shows comparable local tumor control and survival rates to those of LINAC-based SABR or CKRS with a fiducial marker. Thus, fiducial-less CKRS using Xsight lung tracking system can be effectively and safely performed for patients with medically inoperable stage I non-small cell lung cancer without any risk of procedure-related complication.

ID의 비밀번호 관리소홀, 부정사용에 의하여 발생하는 모든 결과에 대한 책임은 회원에게 있습니다.
③ 회원은 당 사이트 및 제 3자의 지적 재산권을 침해해서는 안 됩니다.
제 4 장 서비스의 이용
제 12 조 (서비스 이용 시간)
① 서비스 이용은 당 사이트의 업무상 또는 기술상 특별한 지장이 없는 한 연중무휴, 1일 24시간 운영을
원칙으로 합니다. 단, 당 사이트는 시스템 정기점검, 증설 및 교체를 위해 당 사이트가 정한 날이나 시간에
서비스를 일시 중단할 수 있으며, 예정되어 있는 작업으로 인한 서비스 일시중단은 당 사이트 홈페이지를
통해 사전에 공지합니다.
② 당 사이트는 서비스를 특정범위로 분할하여 각 범위별로 이용가능시간을 별도로 지정할 수 있습니다. 다만
이 경우 그 내용을 공지합니다.
제 13 조 (홈페이지 저작권)
① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
